Epilepsia partialis continua beyond status epilepticus: a generator-based neurophysiological framework for clinical decision-making.

Epilepsia Partialis Continua (EPC) is traditionally classified as a focal motor form of status epilepticus, yet its clinical course and therapeutic needs are remarkably heterogeneous. Current classification strategies, largely based on etiology, duration, or temporal evolution, do not adequately reflect the underlying neurophysiological organization of the epileptic system or explain the variability in systemic involvement and treatment response observed in practice. This review proposes a generator-based neurophysiological framework that reinterprets EPC according to the biological stability

Unraveling sleep dysfunction in progressive supranuclear palsy: a pilot study on the role of periodic limb movements in sleep and insomnia.

BACKGROUND: The association between progressive supranuclear palsy (PSP) and sleep disorders, particularly periodic limb movements in sleep (PLMS), remains inadequately studied, despite their impact on disease progression and quality of life. OBJECTIVES: This study aims to assess the prevalence and severity of sleep disorders in PSP, with a focus on PLMS, in comparison to Parkinson's disease (PD). METHODS: 11 patients with PSP and 10 patients with PD completed validated sleep questionnaires and underwent home-based polysomnography. RESULTS: Patients with PSP reported significantly poorer sl

Real-World Patterns of Botulinum Toxin Treatment in Hyperkinetic Movement Disorders: A 9-Year Nationwide Analysis in France.

BACKGROUND: Hyperkinetic movement disorders, including dystonia, tremor, and myoclonus, are disabling conditions often managed with botulinum toxin type A (BoNT-A). Real-world evidence on treatment patterns remains limited. OBJECTIVE: This nationwide, population-based study aimed to evaluate trends in BoNT-A use in France between 2015 and 2023, using the French National Hospital Discharge Database (Programme de Médicalisation des Systèmes d'Information, PMSI). METHODS: Patients were classified into four categories (dystonia, tremor, myoclonus, and other abnormal movements), based on Internat

Fractures beyond spasms: Progressive encephalomyelitis with rigidity and myoclonus associated with GABA-linked bone fragility.

Progressive encephalomyelitis with rigidity and myoclonus (PERM) is a rare and severe form of stiff-person spectrum disorders, typically associated with painful spasms, brainstem involvement, and autonomic dysfunction. Previous reports have described fractures as an uncommon but recognized complication of PERM, typically attributed to mechanical stress from recurrent spasms; however, disrupted γ- aminobutyric acid (GABA)ergic signaling may also impair bone metabolism. We report a 53-year-old man with seronegative PERM who developed vertebral and pelvic fractures without trauma during hospitali

Neuromyelitis optica spectrum disorder with painful tonic spasms as the first symptom: a case report.

BACKGROUND: Painful tonic spasms (PTS) are an underrecognized initial manifestation of neuromyelitis optica spectrum disorder (NMOSD), presenting before classic neurological deficits in rare cases. CASE PRESENTATION: A 51-year-old man presented with progressive right lower limb PTS lasting ≤10 s, spreading bilaterally and impairing mobility within 1 month. Neurological examination showed bilateral hyperreflexia and myoclonus. Spinal MRI revealed longitudinally extensive T2 hyperintensity and gadolinium enhancement at T1-T6 levels. Serum aquaporin-4 immunoglobulin G (AQP4-IgG) antibody was pos

Case Report: Recurrent pathogenic mutation c.110G>A in DHDDS gene.

Recent studies have demonstrated the close association of mutations in the dehydrodolichyl diphosphate synthase (DHDDS) gene with neurodevelopmental disorders and the onset of epilepsy. This report describes a female patient harboring a de novo heterozygous variant c.110G>A (p.Arg37His) in the DHDDS gene, characterized by childhood-onset myoclonus-like movement disorder (at age 6) and late-onset epilepsy (at age 17). The movement disorder was remarkably improved through the levetiracetam+ clonazepam+ haloperidol triple therapy, and epileptic seizures were also effectively controlled. A retrosp

Essential genetic testing in movement disorders - results from a Delphi study.

BACKGROUND: While genetic testing in Movement Disorders (MD) has expanded enormously, access to genetic testing and genetic counseling remains asymmetric at the global scale. Guidance on efficient testing strategies for clinicians, governments and stakeholders is crucial. OBJECTIVES: Establish a list of genetic movement disorders considered essential as determined by a group of MD experts. METHODS: All genes associated with MD were searched using the OMIM and MDS Gene database. We collected all additional tests available at 4 different laboratories from the EuroGentest database. The results

Pretreatment with different doses of oliceridine attenuates etomidate-induced myoclonus during painless gastroscopy: a randomized controlled trial.

BACKGROUND: Etomidate is commonly used for sedation during gastroscopy but is associated with a high incidence of myoclonus. This study evaluated the effectiveness of oliceridine pretreatment in reducing etomidate-induced myoclonus. METHODS: In this prospective, double-blind, randomized controlled trial, 150 patients undergoing painless gastroscopy were assigned to receive oliceridine at 0.01 mg/kg (Group A1), 0.02 mg/kg (Group A2), or normal saline (Group S) prior to etomidate administration. Remifentanil (0.2 μg/kg) and etomidate (0.3 mg/kg) were then administered. The incidence and severit

Unilateral inferior olivary hypertrophy in a patient with primary progressive apraxia of speech.

Hypertrophic olivary degeneration (HOD) is a rare neurological condition due to hypertrophy of the inferior olivary nucleus (ION), usually due to the disruption of the Guillain-Mollaret triangle (GMT). Here, we studied a patient with primary progressive apraxia of speech (PPAOS), a neurodegenerative disease condition of impaired motor speech production, who developed HOD and Parkinsonian features later in her disease course. We examined the patient's disease course and evaluated various clinical, pathological, and neuroimaging variables. The patient developed motor speech problems at the onset

Effect of ciprofol-etomidate mixtures for deep sedation during gastrointestinal endoscopy: Protocol for a three-arm, double-blind randomized controlled trial.

INTRODUCTION: Deep sedation for gastrointestinal endoscopy can be achieved using either ciprofol or etomidate, both associated with distinct adverse events such as cardiopulmonary depression, myoclonus, and postoperative nausea and vomiting. This study aims to evaluate the safety and efficacy of two ciprofol-etomidate mixtures at varying volume ratios compared with ciprofol alone in patients undergoing gastrointestinal endoscopy. METHODS: This three-arm prospective study will include 135 participants aged 18-65 years who are scheduled for gastrointestinal endoscopy under deep sedation. Patien

Polypharmacy's paradox: accelerated decline in a rare case of Lafora body disease.

PURPOSE: Lafora body disease (LBD) is a rare autosomal recessive progressive myoclonic epilepsy characterized by neurodegene-ration due to an intracellular accumulation of poorly branched polyglucosan inclusions. Effective pharmacological management remains challenging due to the risk of drug-induced exacerbation of symptoms and adverse interactions. CASE DESCRIPTION: We present the case of an 18-year-old male with intractable seizures, myoclonus, and cognitive regression. Electroencephalogram showed frequent generalized polyspike discharges, and histopathology confirmed Lafora bodies. The pa

Correlation Between Quantitative Pupillometry and Other Markers for Neuroprognostication of Comatose Patients After Cardiac Arrest: A Bicentric Study.

OBJECTIVES: The prognostic role of quantitative pupillometry in comatose patients after cardiac arrest (CA) has been recently described, but data on quantitative pupillary light reflex (qPLR) are limited, especially in a multimodal approach and regarding good outcome. We assessed qPLR correlations with clinical features, electroencephalogram (EEG), somatosensory evoked potentials (SSEPs), and neuron-specific enolase (NSE), as well as qPLR prognostic values toward poor and good outcomes.
