Dosimetric Comparison of Functional Radiosurgery Treatments for Trigeminal Neuralgia, Essential Tremor, and Obsessive-compulsive Disorder Using Gamma Knife, CyberKnife, and Linear Accelerators.

PURPOSE: This study aims to evaluate and compare dose distribution for a trigeminal neuralgia (TN), essential tremor (ET), and obsessive-compulsive disorder (OCD) plan, using Gamma Knife (GK) Perfexion, CyberKnife (CK), and linear accelerators (linac) for a single treatment case. MATERIALS AND METHODS: For all treatment platforms, plans utilized the same isocenter position and anatomical structures, with GK Perfexion plans as the reference. The prescribed doses were established at 90 Gy for TN, 140 Gy for ET, and 120 Gy for OCD. The irradiation geometry for each platform was defined to optimi

Neuromelanin-sensitive magnetic resonance imaging in the diagnosis and assessment of Parkinson's disease.

Parkinson's disease (PD) is a progressive neurodegenerative disorder characterized primarily by the progressive loss of dopaminergic neurons in the substantia nigra pars compacta (SNpc); noradrenergic neurons in the locus coeruleus (LC) are also frequently affected. Because clinical motor symptoms usually emerge only after substantial neurodegeneration has occurred, there is a compelling need for noninvasive imaging biomarkers capable of detecting early pathological changes. Neuromelanin-sensitive magnetic resonance imaging (NM-MRI) generates neuromelanin-related image contrast in the SNpc and

Is Unilateral Focused Ultrasound Thalamotomy as Effective and Safe as Unilateral Deep Brain Stimulation for Essential Tremor? - A critical evaluation.

BACKGROUND: Focused ultrasound (FUS) thalamotomy has become a common surgical treatment for essential tremor (ET), in some settings partially replacing deep brain stimulation (DBS). However, no randomized comparative studies have been performed. A review directly comparing unilateral FUS thalamotomy and unilateral DBS is therefore warranted. SUMMARY: A systematic literature search identified studies of thalamotomy FUS or thalamic DBS for ET. Eligible studies reported mean values for predefined items of the Clinical Rating Scale for Tremor (CRST) at 6-12 months follow-up. Studies with long-ter

Postoperative Changes in Line-Drawing Trajectories After Vim Thalamotomy for Essential Tremor: An Exploratory Analysis of Functional Correlates.

INTRODUCTION: Ventral intermediate nucleus (Vim) thalamotomy effectively suppresses essential tremor (ET); however, improvements in tremor severity do not necessarily translate into enhanced upper-limb dexterity or quality of life (QOL). In this study, we aimed to characterize postoperative changes in line-drawing trajectories and explore their functional correlates following Vim thalamotomy. METHODS: Thirty patients with ET who underwent unilateral Vim thalamotomy and 13 healthy controls (HC) were included. The ET group was assessed preoperatively and at 1 week and 1 month postoperatively us

Peripheral Nerve Stimulation for Essential Tremor: a Systematic Review and Meta-Analysis of Effectiveness and Safety.

BACKGROUND: Essential tremor (ET) is the most prevalent movement disorder worldwide. There are limited current pharmacological therapies, and their side effects often preclude adequate doses. Surgical therapies, such as Deep Brain Stimulation (DBS) and Focused Ultrasound (FUS), are reserved for patients with tremor that debilitates activities of daily living (ADLs). Peripheral nerve stimulation (PNS), including transcutaneous afferent patterned stimulation (TAPS) and transcutaneous peripheral nerve stimulation (TPNS), has emerged as a promising non-invasive neuromodulatory approach for many pa

EMG-Driven Phase-Locked Spinal Cord Stimulation Physiologically Modulates Essential Tremor.

Essential tremor (ET) is the most prevalent movement disorder, yet existing pharmacological and surgical treatments often lack efficacy or involve significant risks. Phase-locked neuromodulation offers a targeted approach to disrupt pathological oscillations, but its implementation through non-invasive spinal cord stimulation (tSCS) has remained largely unexplored. In this study, we present a novel closed-loop tSCS framework for ET based on real-time tremor phase estimation from surface electromyography (EMG). The system implements a Fourier Linear Combiner (FLC) algorithm to track the demodul

Tremor or myoclonus? A practical clinical approach to distinguishing two common hyperkinetic movement disorders.

Tremor and myoclonus are among the most common hyperkinetic movement disorders encountered in neurological practice, with tremor being considerably more common than myoclonus in everyday clinical practice, yet distinguishing between them remains a frequent diagnostic challenge. Although their definitions are conceptually distinct, several disorders - including cortical tremor, dystonic tremor, and generalized polymyoclonus - can mimic one another, leading to diagnostic uncertainty and inappropriate management. This practical review provides clinicians with a structured approach to recognizing

Bilateral multi-condition surface electromyographic characterization of tremor in Parkinson's disease and essential tremor.

OBJECTIVES: Clinical differentiation between Parkinson's disease (PD) and essential tremor (ET) remains challenging, particularly in early or overlapping cases. This study aimed to characterize bilateral tremor features using multi-condition surface electromyography (sEMG) and to evaluate task-dependent modulation patterns relevant to differential diagnosis. METHODS: Thirty patients with PD and forty-one with ET underwent bilateral sEMG recordings from agonist-antagonist muscle pairs. Tremor frequency, burst duration, amplitude, and activation patterns were systematically analyzed under four

Deep brain stimulation for arm tremor: one-year blinded follow-up of a randomized trial.

OBJECTIVE: To present the secondary endpoints at one-year blinded follow-up from our randomized controlled study, that compared the efficacy of DBS in the ventral-intermediate thalamic nucleus (VIM) with the posterior subthalamic area (PSA) three months each in crossover design, and showed significantly greater reduction of the Sum dominant arm tremor score (Fahn-Tolosa-Marin) during the PSA-DBS period. The outcomes at one year are now reported to verify target choice and sustained benefit with clinically optimized free programming. METHODS: Forty-five patients with isolated or combined arm a

Staged bilateral MR-guided focused ultrasound thalamotomy in a patient with cerebral cavernous malformations.

MRI-guided focused ultrasound (MRgFUS) thalamotomy is an effective ablative procedure for medication-refractory essential tremor (ET). However, limited data exist regarding its safety in patients with coexisting intracranial disease such as cerebral cavernous malformations. We present a man in his late 60s with medication-refractory ET who underwent staged bilateral MRgFUS thalamotomy. Preoperative MRI demonstrated heterogeneously T1-hyperintense and T2-hyperintense lesions in the left caudate head and basal ganglia consistent with cavernous malformations, with an associated developmental veno

The Role of Biologically Effective Dose in Clinical Improvement and Hyper-Response After Gamma Knife Radiosurgery for Essential Tremor.

BACKGROUND AND OBJECTIVES: Stereotactic radiosurgery (SRS) is a minimally invasive alternative for drug-resistant essential tremor (ET). The objective of this study was to evaluate whether biologically effective dose (BEDGy2.47) is relevant for tremor arrest after SRS for ET. METHODS: Two hundred sixty-one cases were treated in Marseille University Hospital (CHU Timone) between January 2009 and June 2021. The median age was 71.9 years (39.9-91.5). The treated side was the left ventrointermediate nucleus in 237 cases (90.8%). 130 Gy was prescribed in all cases. The median BEDGy2.47 was 4119.3

Modelling and Correcting the Effects of Tremor on Touchscreen Interaction.

Essential tremor and Parkinson's tremor are common movement disorders, affecting approximately 7% of adults over 65, increasing with age. Although numerous studies have used keyboard timing, stand-alone sensors, or smartphone sensors to identify and measure tremor severity, relatively little research has isolated and examined how tremor impacts overall mobile interaction and usability models. This study investigated controlled fixed levels of induced tremor in a standardised ISO pointing task to quantify the effect on usability models of different levels of tremor. Tremor was induced using a w

Cryoprobe to current practice: neurosurgical legacies of Dr. Irving S. Cooper in movement disorders.

Before the advent of levodopa therapy for Parkinson's disease, surgical lesioning of the motor thalamus or globus pallidus represented the primary therapeutic intervention for Parkinson's disease and essential tremor at a time when effective medical alternatives were severely limited. In this context, Irving S. Cooper directed one of the most prolific and controversial lesioning practices of the mid-20th century, performing thousands of procedures and documenting outcomes. Through chemopallidectomy, chemothalamectomy, and later cryosurgical techniques, Cooper investigated the relationship betw

The association between upper limb motor performances and gait dynamics in essential tremor: an emerging relationship.

Essential tremor (ET) is a complex disorder extending beyond upper limb action tremor. Both slowing in upper limb movements and impaired gait performances have been widely described. The present study aimed to assess clinical-instrumental relationship between wearable-sensor recorded upper limb motility, tremor and walking performances in ET and explore their role in disease characterization. A gyroscope taped over the index finger was used to quantify both tremor in three tasks (forward outstretched; lateral-wing; instrumental finger-nose, iFN) and upper limb performances in the iFN. Maximum

Safety of Staged Bilateral MRgFUS Thalamotomy: Associations Between Lesion Characteristics, Stereotactic Coordinates, and Adverse Events.

BACKGROUND AND OBJECTIVES: Staged bilateral magnetic resonance-guided focused ultrasound (MRgFUS) thalamotomy is increasingly performed for medication-refractory essential tremor, yet the relationship between lesion characteristics and profiles of adverse events (AE) across sessions remains poorly characterized. We sought to examine how lesion volume, stereotactic coordinates, and session order relate to AE profiles in staged bilateral MRgFUS thalamotomy. METHODS: Retrospective analysis of 50 patients who underwent staged bilateral MRgFUS thalamotomy at a single center. AE were assessed at po

Clinical Importance and Usefulness of Direct Targeting of the Ventral Intermediate Nucleus of the Thalamus Using Fluid and White Matter Suppression Imaging at 3T MRI.

BACKGROUND AND OBJECTIVES: Stereotactic targeting of the ventral intermediate nucleus of the thalamus (Vim) in tremor surgery traditionally relies on indirect, atlas-based coordinates, whereas direct visualization of the Vim using advanced MRI sequences has the potential to improve targeting precision and clinical outcomes. We evaluated the feasibility and clinical utility of fluid and white matter suppression (FLAWS) imaging for direct Vim targeting in the MRI-guided focused ultrasound (MRgFUS) procedure. METHODS: We retrospectively reviewed 30 MRgFUS procedures in 29 patients. Cases were di

Disorders of the Basal Ganglia and Cerebellum: A Comprehensive Review of Speech Sensorimotor Symptoms and Responses to Feedback Perturbations.

Situated within critical cortical-subcortical sensorimotor circuits, the basal ganglia and cerebellum are thought to play important roles in both feedforward and feedback mechanisms of speech sensorimotor control. Motor speech impairments and speech-related sensory deficits are indeed common in disorders involving these two structures such as Parkinson's disease, dystonia, and cerebellar degeneration. Recently, several studies have also investigated the impact of these disorders on the feedforward and feedback components of speech sensorimotor control by examining immediate compensation to une
