Genetic validation of ABI3 p.Ser209Phe variant and its effects on early brain pathology in asymptomatic elderly individuals.

BACKGROUND: Alzheimer's disease (AD) has a strong genetic component, with APOE ε4 being the most established risk factor through its effects on beta-amyloid (Aβ) metabolism and microglial function. Recent genetic studies have also implicated microglial genes, such as the ABI3S209F variant, to increased AD risk. As APOE ε4 and ABI3S209F influence microglial pathways through distinct mechanisms, their combined analysis may provide novel insights into AD pathophysiology. Therefore, we investigated ABI3S209F in the Finnish FinnGen cohort and in an imaging study of cognitively healthy older adults.

Decoding Alzheimer's disease through down syndrome: insights from a genetically defined population.

PURPOSE OF REVIEW: This review explores Alzheimer's disease (AD) in individuals with Down syndrome (DS), a genetically defined population with near-universal development of AD neuropathology by age 40. We examine the genetic basis of DS-AD, epidemiology, biomarker trajectories, and clinical trial innovations, highlighting how insights from DS research inform broader AD pathogenesis, early detection, and therapeutic strategies. RECENT FINDINGS: Advances in biomarker research, including longitudinal studies such as ABC-DS, have mapped predictable trajectories of amyloid, tau, and neurodegenerat

Treating Alzheimer's disease in a person living with HIV.

INTRODUCTION: Human immunodeficiency virus (HIV) therapies now support survival into advanced ages by suppressing HIV replication and preserving immune function. Alzheimer's disease (AD), the most common neurologic degenerative condition, may now be treated with anti-amyloid therapies (AAT). HIV was an exclusionary criterion in clinical trials for AAT, leaving the safety and efficacy of these medications unknown in this setting. METHODS: We report the development of AD supported by careful evaluations and biomarkers over a 10-year period in a person living with well-controlled HIV infection.

RBFOX1 association with age at onset of Alzheimer's disease.

INTRODUCTION: Genetic contributors to early onset Alzheimer's disease (AD) beyond APP and PSEN1/2 remain unknown. Identifying novel loci may reveal disease mechanisms and therapeutic targets. We investigated genetic variants influencing age at onset in early onset families from the Long-Life Family Study (LLFS). METHODS: Six families with at least two early onset cases (onset ≤ 65) were identified among 3476 LLFS participants. Genome-wide linkage analysis of age at onset was followed by single nucleotide polymorphism association. Validation analyses were performed in nine independent cohorts,

Association of Alzheimer's disease progression with YKL40 levels in peripheral blood and cerebrospinal fluid: a systematic review and meta-analysis.

INTRODUCTION: Chitinase 3-like protein 1 (CHI3L1 or YKL40) is a potential neuroinflammatory biomarker linked to the pathogenesis of Alzheimer's disease (AD). Previous studies have produced inconsistent results regarding YKL40 levels in various clinical stages of AD. This study aims to establish the correlation between YKL40 levels and AD progression through a meta-analysis of YKL40 levels in cerebrospinal fluid (CSF) and peripheral blood. METHODS: Comprehensive searches were conducted in PubMed, Medline, Web of Science, and the Cochrane Library to identify observational studies reporting CSF

Plasma miRNAs associated with cognitive impairment and brain hypometabolism in individuals with mild cognitive impairment.

Detecting practical biomarkers to facilitate early diagnosis of Alzheimer's disease (AD) before the onset of overt symptoms is crucial for clinicians to develop preventive and therapeutic strategies. There is substantial experimental evidence indicating the dysregulation of several microRNA (miRNA) expression levels in AD. They are known to play a crucial role in modulating critical intracellular signaling pathways associated with the core pathophysiological mechanisms of AD. In this cross-sectional study, we investigated the association of blood-based miRNAs as non-invasive, cost-effective bi

Phase II clinical trial of KCHO-1 for Alzheimer's disease: study protocol for a multicenter, randomized, double-blind, multi-dose, placebo trial.

BACKGROUND: Alzheimer's disease (AD) is the most common degenerative brain disorder leading to dementia, characterized by a slow onset and gradual progression. Early symptoms typically include difficulties in recalling recent events, which later extend to impairments in various cognitive functions such as language, judgment, and problem-solving, ultimately resulting in the complete loss of independent daily functioning. KCHO-1, a herbal extract with demonstrated anti-inflammatory and neuroprotective properties, has shown promise in mitigating the effects of neurodegenerative disorders. This st

Taurine as an Early-Phase Disease-Modifying Candidate for Alzheimer's Disease.

Alzheimer's disease is driven by converging pathological processes, including amyloid-β accumulation, tau dysfunction, synaptic failure, and chronic neuroinflammation, which emerge decades before clinical onset. Growing evidence supports the concept that early, upstream neuroprotective interventions may meaningfully alter disease trajectory in both sporadic and familial AD. Taurine, an endogenously abundant and clinically safe neuromodulator, has re-emerged as a promising multi-target regulator of AD-relevant pathways. Accumulating mechanistic data indicate that taurine modulates Aβ aggregatio

Inflammatory mechanisms underlying early Alzheimer's disease pathology: evidence from the aging rhesus macaque brain.

Inflammation plays a large role in the etiology of the late onset, sporadic form of Alzheimer's disease (AD), yet these critical factors are not adequately modeled in mice where inflammatory mechanisms often differ widely from primates. In contrast, aging rhesus macaques offer a powerful translational model for investigating how advancing age and inflammation initiate early-stage pathology in sporadic AD, and for evaluating preventive therapeutic strategies. Unlike rodents, macaques possess highly developed association cortices with magnified calcium signaling, human-like inflammatory response

CSF Proteomic Profiles Associated With White Matter Integrity in Cognitively Normal Older Adults With and Without Amyloid Pathology.

BACKGROUND AND OBJECTIVES: Increasing evidence indicates a potential role of white matter (WM) damage in the onset and progression of Alzheimer disease (AD). However, the biological processes underlying in vivo WM imaging biomarkers remain unclear. We sought to determine the molecular signatures associated with WM integrity in cognitively normal individuals with and without amyloid pathology. METHODS: We selected older individuals without dementia (Clinical Dementia Rating <1) from the Alzheimer Centrum Amsterdam when they had diffusion tensor imaging (DTI) and CSF proteomic (untargeted tande

Interaction between transient receptor potential vanilloid 4 and glutamate NMDA receptor subunit 1 mediates endoplasmic reticulum stress and neuroinflammation in postoperative delirium.

Postoperative delirium (POD) is a serious and prevalent neurocognitive complication that poses a major clinical challenge because its mechanism is unclear. This study identifies a pathogenic pathway centred on the direct interaction between transient receptor potential vanilloid 4 (TRPV4) and the essential N-methyl-D-aspartate receptor (NMDAR) subunit GluN1. Using a murine POD model, the neuron-centric glutamatergic dysfunction in the hippocampus was initially confirmed through ex vivo metabolic kinetic analysis. Transcriptomic analysis revealed upregulation of Trpv4, predominantly in neurons.

APP Deficiency Ameliorates FAD Presenilin 1 F105C and A246E Mutations-induced Mitochondrial Dysfunction in Human Cortical Neurons.

BACKGROUND: Mitochondrial dysfunction is widely regarded as a central and early feature of Alzheimer's disease (AD) pathology. Prior studies suggest that the accumulation of amyloid precursor protein (APP) within mitochondria contributes to this dysfunction. Mutations in presenilin-1 (PS1), which account for most cases of early-onset familial AD (FAD), have also been shown to impair mitochondrial function. In this study, we investigated how APP influences PS1 mutation-induced mitochondrial dysfunction in human cortical neurons derived from patient induced pluripotent stem cells (iPSCs). METHO

Apathy in Mild Behavioural Impairment: Associations with Cortical Thickness and Grey Matter Volume.

Mild Behavioural Impairment (MBI) is defined by later-life onset of persistent behavioural changes and is recognized as a risk marker for cognitive decline and dementia. Apathy, a core MBI domain characterized by diminished interest, initiative, and emotional reactivity, can emerge before dementia and is hypothesized to be associated with structural brain changes. While previous studies have explored Alzheimer disease (AD)-related neuroanatomical substrates of apathy in the dementia clinical stage, few have investigated these associations in cognitively normal (CN) or mild cognitive impairment
