Bone and Radiologic Findings in Congenital Generalized Lipodystrophy: A Systematic Review and Report of 60 Cases.

Congenital Generalized Lipodystrophy (CGL), usually caused by pathogenic variants in AGPAT2 (CGL1) and BSCL2 (CGL2), is characterized by near-total loss of subcutaneous adipose tissue, low leptin levels and severe metabolic and systemic comorbidities. Skeletal abnormalities including diffuse sclerosis, lytic-appearing bone lesions, and high bone mineral density have been recognized in CGL, but the prevalence and clinical and radiological features of these bone phenotypes remain ill-defined. The aim of this single-institution case series and systematic review was to evaluate bone manifestations

Osmotic demyelination without hyponatremia: the role of glycemic variability in a complex patient with diabetes.

Osmotic demyelination syndrome (ODS) is classically associated with rapid correction of hyponatremia; however, non-sodium osmotic shifts remain underrecognized. We report a young man with poorly controlled type 1 diabetes and extreme glycemic variability (13-486 mg/dL), admitted with cardiorenal syndrome requiring dialysis, who developed progressive neurological deficits. Brain MRI demonstrated findings consistent with extrapontine ODS despite stable sodium levels. Alternative etiologies were excluded, supporting glycemic variability and osmotic instability as key contributors, potentially exa

Hypernatremia as the Initial Presentation of Osmotic Demyelination Syndrome: A Rare Case with Favorable Neurological Outcome.

INTRODUCTION: Osmotic demyelination syndrome (ODS) occurs due to rapid shifts in serum sodium levels and osmolality, resulting in neuronal damage. While it is most often caused by the rapid correction of chronic hyponatremia, other electrolyte imbalances have also been identified as potential triggers. Risk factors include alcohol abuse, malnutrition, and liver disease, among others, with common symptoms including tetraparesis, dysphagia, and altered consciousness. CASE PRESENTATION: We report the case of a 45-year-old male with alcohol dependence who presented to the emergency department wit

Fatal neurological complications in anorexia nervosa: a case report of osmotic demyelination syndrome in the binge-eating/purging subtype.

BACKGROUND: Anorexia nervosa (AN) is associated with high mortality due to severe complications. Osmotic demyelination syndrome (ODS), although rare, is a severe neurological complication that has been associated with rapid correction of chronic hyponatremia. The occurrence of ODS in patients with AN, particularly the binge-eating/purging subtype (AN-BP), remains poorly recognized despite their increased susceptibility arising from chronic malnutrition and multiple electrolyte disturbances. CASE PRESENTATION: A 33-year-old woman was treated in the intensive care unit of an outside hospital on

Spleen-brain axis regulates myelin integrity via TGF-β1 signaling in cuprizone-induced demyelination and remyelination model.

Demyelination and impaired remyelination are hallmark features of many neurodegenerative and psychiatric disorders. Although central mechanisms have been widely investigated, the contribution of peripheral immune organs such as the spleen remains poorly understood. In this study, we examined the role of the spleen and transforming growth factor-β1 (TGF-β1) in cuprizone (CPZ)-induced demyelination and remyelination in mice. Splenectomy performed before CPZ exposure significantly worsened demyelination in the corpus callosum, as indicated by reduced myelinated area and myelin basic protein (MBP)

Clinical Spectrum, neuroimaging, treatment, and outcomes of dengue-associated spinal cord involvement.

Dengue-associated spinal cord involvement is an uncommon but clinically important neurological complication of dengue infection. Existing evidence is fragmented and largely limited to isolated case reports, case series, and small observational studies. This systematic review evaluated the clinical spectrum, neuroimaging findings, pathogenesis, treatment approaches, and outcomes of dengue-associated spinal cord involvement. A systematic review was conducted according to PRISMA 2020 guidelines. PubMed, Embase, Scopus, and Google Scholar were searched from inception to the final search date. Case

[Acute Flaccid Myelitis].

Acute flaccid myelitis (AFM) is a clinical syndrome characterized by acute flaccid paralysis that predominantly affects the spinal gray matter. It occurs mainly in children and is strongly associated with enterovirus D68. Magnetic resonance imaging typically demonstrates longitudinal lesions predominantly involving the gray matter, and the cerebrospinal fluid shows mild pleocytosis. The differential diagnosis from demyelinating transverse myelitis and Guillain-Barré syndrome is crucial for appropriate clinical management. AFM is included in acute flaccid paralysis (AFP) surveillance. Prompt no

MOGAD optic neuritis post enterovirus meningitis: a new association.

INTRODUCTION: Myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD) is a rare autoimmune demyelinating disorder of the central nervous system, typically presenting with optic neuritis, transverse myelitis or acute disseminated encephalomyelitis. While viral triggers such as Epstein-Barr virus and adenovirus have been reported, an association with enterovirus meningitis is not well documented. CASE PRESENTATION: We report a 17-year-old female who presented with enterovirus meningitis confirmed by cerebrospinal fluid PCR and leptomeningeal enhancement on MRI. One week later, s

Diquat exposure disrupts ferroportin-dependent iron homeostasis and drives ferroptosis-associated white matter injury.

Diquat (DQ) is one of the most widely used bipyridyl herbicides and has been identified as a potential neurotoxin; however, its primary central nervous system (CNS) targets and the underlying mechanisms remain unclear. In cases of acute human DQ poisoning, subcortical white matter lesions have been reported to emerge approximately two weeks after exposure, with demyelination representing the predominant pathological finding. We used an acute oral DQ exposure rat model that mimics human DQ poisoning. Magnetic resonance imaging (MRI) revealed prominent white matter abnormalities in the corpus ca

Immunotherapies for neuromyelitis optica spectrum disorder and myelin oligodendrocyte glycoprotein antibody-associated disease.

Neuromyelitis optica spectrum disorder (NMOSD) and myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD) are rare autoimmune neuroinflammatory disorders of the central nervous system (CNS) characterized by recurrent attacks of optic neuritis, transverse myelitis, brainstem, and brain syndromes. In contrast with multiple sclerosis (MS), the accumulation of disability in NMOSD and MOGAD is primarily related to relapses, while accumulation of disability between attacks is less common. Therefore, treatment strategies should mainly focus on effective treatment of acute relapses an

Consensus Recommendations for the Diagnosis and Treatment of Neuromyelitis Optica Spectrum Disorders (NMOSD): The MENACTRIMS Guidelines.

Neuromyelitis optica spectrum disorder (NMOSD) is a severe autoimmune disorder affecting the central nervous system, often misdiagnosed as multiple sclerosis. The identification of aquaporin-4-IgG (AQP4-IgG) has improved diagnostic precision and enabled targeted therapies. Given the unique regional challenges in healthcare delivery across the Middle East and North Africa (MENA) region, the Middle East and North Africa Committee for Treatment and Research in Multiple Sclerosis (MENACTRIMS) convened an expert panel to develop evidence-based, region-specific consensus recommendations for diagnosi

Diagnostic biomarkers for differentiating AQP4-IgG- negative NMOSD from other nervous system autoimmune disorders: a retrospective study.

BACKGROUND AND OBJECTIVE: The clinical manifestations of neuromyelitis optica spectrum disorders (NMOSD) overlap with those of other central nervous system (CNS) disorders, making differential diagnosis based on symptoms alone difficult. This study aimed to identify distinguishing indicators of aquaporin-4 (AQP4) IgG-negative NMOSD and assess their diagnostic value. METHODS: We analyzed four groups: 85 patients with AQP4-IgG-negative NMOSD, 192 with AQP4-IgG-positive NMOSD, 547 with other nervous system autoimmune disorders (e.g., multiple sclerosis, Guillain-Barré syndrome, and viral and aut

Natural killer cell reduction and dysfunction define a pathogenic and diagnostic axis in neuromyelitis optica spectrum disorder.

Neuromyelitis optica spectrum disorder (NMOSD) is a severe relapsing autoimmune disease of the central nervous system, where early diagnosis and monitoring are essential to prevent long-term disability. The cell-based assay recommended for aquaporin-4 immunoglobulin G (AQP4-IgG) serology offers well-established diagnostic accuracy, however, it is costly, time-consuming and not universally accessible in some hospitals. Here, we identify peripheral natural killer (NK) cell reduction as a readily accessible and quantifiable complementary diagnostic biomarker for NMOSD. Across two independent coho

Activation status of astrocytes drives the MS/NMOSD therapeutic paradox: Insights from IFNAR1 signaling.

Neuromyelitis optica spectrum disorder (NMOSD) is a rare autoimmune inflammatory disease of the central nervous system (CNS) that, despite overlapping phenotypic features with multiple sclerosis (MS), manifests with more severe clinical outcomes. The defining pathogenic driver of NMOSD is the aquaporin-4 (AQP4) autoantibody, which induces astrocytic injury via complement-dependent cytotoxicity (CDC). MS is predominantly managed with disease-modifying therapies (DMTs) such as interferon-beta (IFN-β) to reduce relapse rates. However, these therapies are often ineffective or even detrimental in N

Associations between personality traits and cognitive function in multiple sclerosis and neuromyelitis optica spectrum disorder: A cross-sectional study.

BACKGROUND: Multiple sclerosis (MS) and neuromyelitis optica spectrum disorder (NMOSD) are chronic demyelinating diseases of the central nervous system frequently associated with cognitive impairment (CI). Although both disorders share overlapping neurological manifestations, they differ substantially in underlying pathophysiology and disease course. Despite the clinical relevance of personality traits and cognition in chronic neurological disorders, the relationship between cognitive function and the five major personality domains remains insufficiently characterized in MS patients. Moreover,
