Congenital Generalized Lipodystrophy (CGL), usually caused by pathogenic variants in AGPAT2 (CGL1) and BSCL2 (CGL2), is characterized by near-total loss of subcutaneous adipose tissue, low leptin levels and severe metabolic and systemic comorbidities. Skeletal abnormalities including diffuse sclerosis, lytic-appearing bone lesions, and high bone mineral density have been recognized in CGL, but the prevalence and clinical and radiological features of these bone phenotypes remain ill-defined. The aim of this single-institution case series and systematic review was to evaluate bone manifestations

Synovitis, acne, pustulosis, hyperostosis, and osteitis (SAPHO) syndrome is a systemic inflammatory disease that involves mandibular osteomyelitis with diffuse sclerosis in 10% of cases. Drug therapy is the primary treatment choice; however, severe deformity resulting in restricted mouth opening requires mandibular reconstruction. We report a rare case of mandible reconstruction in a patient with SAPHO syndrome, using a vascularized fibular flap with long-term follow-up. One year after surgery, a fistula developed in the chin region following a rash on both lower extremities as a skin manifest

BACKGROUND: Osteoid osteoma is a benign bone tumor commonly affecting the lower extremities, typically the femur or tibia. However, its occurrence in the fifth metatarsal bone is rare, and atypical presentations can lead to delayed diagnosis and misinterpretation as other pathologies.
CASE PRESENTATION: We report the case of a 14-year-old Persian male athlete who presented with persistent lateral forefoot pain lasting 2 years, initially attributed to a traumatic injury. The pain was progressively worsening, nocturnal, and unresponsive to nonsteroidal anti-inflammatory drugs, deviating from th

A woman in her late 50s with a history of multiple prior orthopaedic surgeries presented with severe left hip pain. Initial pelvic radiographs showed diffuse sclerosis, cortical thickening and coarse trabeculae, suggesting polyostotic Paget's disease or another metabolic disorder. However, further imaging revealed widespread skeletal sclerosis and cortical thickening throughout the axial skeleton. In addition, a history of chronic inhalant abuse was reported in the medical record. In conjunction with the patient's history of computer duster abuse and potential difluoroethane exposure, a subseq

Osteoid osteoma is a benign osteoblastic tumour with a predilection for the lower extremity that rarely affects the forearm. It is commonly seen in adolescents and young adults, and is seldom diagnosed in the paediatric age group. We report a boy in his early childhood who presented with a swelling over the distal forearm, which was incidentally noted by the mother 3 months ago. Plain radiographs showed diffuse sclerosis of the dorsal cortex of the distal radius. CT scan showed a central lucent nidus in the intramedullary region and surrounding sclerosis in the radial metaphysis, confirming th

Osteopetrosis encompasses rare inherited metabolic bone disorders with defect in the osteoclast activity. Severe forms of presentation such as malignant infantile osteopetrosis are seen in infants and milder forms in older children. The clinical presentation includes failure to thrive, severe pallor, optic atrophy and hepatosplenomegaly. The disorder is characterised by dense bone on radiography, hence the name marble bone disease. A 10-month-old boy who presented with developmental delay, failure to thrive, nystagmus (which the mother described as wandering eye movements), splenomegaly of 16

The use of complementary and alternative medicine (CAM) has increased. Phytotherapy, also known as herbal medicine, is one of the CAM therapies that involves using medicinal plants and plant-derived substances. This study aims to evaluate the use of phytopharmaceuticals and the factors associated with their use in patients with Systemic sclerosis (SSc). Our study is designed as a survey and consists of two parts with 32 questions. The first part gathers demographic data with 14 questions, and the second part assesses participants' knowledge about phytotherapy with 18 questions. The questionnai

BACKGROUND: Diffuse sclerosing osteomyelitis (DSO) in mandible is a disease with unusual and undefined clinicoradiographic manifestations. Several medicines have been administrated for therapy, but the efficacy in bone remolding remains to be systematically evaluated.
OBSERVATION: A 37-year-old male and a 28-year-old female were diagnosed DSO after revealing diffuse sclerosis and osteolytic changes in the mandible, and treated by Janus-activated kinase signal and IL-6 inhibitor, respectively. Their symptoms were well controlled and the inflammatory indicators were decreased. During the mandib

INTRODUCTION: Systemic sclerosis (SSc) is an autoimmune disorder with fibrosis in multiple organs, autoantibodies, and microvascular abnormalities. Its origin is unclear, but it may result from circulatory damage, collagen metabolism disruption, and modifications in immunoregulation. The disease affects various organs and has high morbidity and mortality rates. SSc-related complications are managed using immunosuppressive medications that target autoantibodies. The main objective of this study was to assess the safety and efficacy of plasma therapy/plasmapheresis in managing SSc.
METHODS: Thi

BACKGROUND: Sclerodermiform lupus erythematosus (SDLE) is a rare Type 3 overlap syndrome of morphea and cutaneous lupus diagnosed with histopathologic features of both diseases present. It was first reported in 1976 by Umbert et al with a case series of four patients. SDLE is more common in young to middle-aged female patients.
METHODS: After IRB approval, we searched our internal pathology database for cases of SDLE, excluded any patients that did not fulfill the diagnostic criteria, and verified each case with a board-certified dermatopathologist.
RESULTS: Five patients with SDLE were iden

OBJECTIVES: Systemic sclerosis (SSc) is a disease characterized by diffuse sclerosis of skin and organs and small vessel vasculopathy. Despite it, large vessels can also be involved with ulnar artery vasculopathy, revealing as a more frequent feature of SSc. The aim of this paper is to assess the macrovascular involvement of SSc patients through an ultrasound (US) evaluation of radial and ulnar arteries.
METHODS: Radial and ulnar resistance indices (RIs) and peak systolic velocity (PV) (cm/s) together with clinical features of SSc patients were evaluated. Raynaud phenomenon (RP) and healthy c

Skeletal fluorosis is a metabolic bone disease caused by excessive consumption of fluoride from fluoride-contaminated water or foods. Such a condition often takes place in developing countries without proper handling of drinking water or food. However, in recent years, multiple cases of skeletal fluorosis have been observed in the United States due to the increasing frequency of recreational substance abuse. In this case report, a 26-year-old male with a history of polysubstance use disorder presented to the emergency department after being assaulted by store employees when attempting to steal

BACKGROUND: Osteosclerotic metaphyseal dysplasia (OSMD, OMIM 615198) is an extremely rare autosomal recessive osteopetrosis disorder resulting in a distinctive pattern of osteosclerosis of the metaphyseal margins of long tubular bones. To date, only thirteen cases have been reported (eight molecularly confirmed). Five homozygous sequence variants in the leucine-rich repeat kinase 1 (LRRK1) gene have been identified to cause OSMD. We present two male siblings with OSMD with a novel LRRK1 variant.

A 70-year-old man diagnosed with acinar adenocarcinoma prostate (Gleason score 4 + 4 = 8) underwent bone scintigraphy due to high serum prostate-specific antigen level (224 ng/mL; normal <4 ng/mL). Bone scan revealed moderately increased diffuse tracer uptake in the right hemipelvis, while the rest of the scan appeared normal, leading to suspicion of Paget's disease. Gallium-68-prostate-specific membrane antigen (PSMA) positron emission tomography-computed tomography scan (CT) showed a PSMA-avid lesion in the left lobe of the prostate, with no PSMA uptake in the areas of diffuse sclerosis in t

INTRODUCTION: Pentazocine, a synthetic opioid with partial agonist and antagonist activity administered by parenteral injection, was used clinically in the 1970s. Dermatologic complications at injection sites were reported soon after its introduction. These complications are thought to be underreported. Many well-documented cases describe the development of fibrosis and calcinosis at the site of parenteral pentazocine injections, but few reports document management or successful treatment of the long-term sequelae of these manifestations.
