Foot Drop as the Initial Manifestation of HIV Infection with Focal Brain Lesions: A Case Report and Diagnostic Challenge.

BACKGROUND: Neurological involvement in HIV infection encompasses a broad clinical spectrum, most commonly presenting as peripheral neuropathy. However, focal motor deficits such as isolated foot drop are rare, and cases accompanied by Central Nervous System (CNS) involvement pose significant diagnostic challenges. CASE: A 53-year-old male patient was referred to our clinic after testing positive for anti-HIV during the evaluation of a one-month history of foot drop and sore throat. On admission, neurological examination revealed isolated motor weakness without additional deficits. Brain Magn

Syphilis causing elevation of anti-CCP: a case report of medical mimicry.

INTRODUCTION: Infection with Treponema pallidum, or syphilis, is often called the "Great Imitator" because of its propensity for atypical presentations and non-specific symptoms. In addition, syphilis has been shown to create false-positive results for common tests, including Lyme disease, HIV, and multiple autoimmune conditions. One that has not been documented is anti-cyclic citrullinated peptide (anti-CCP). Anti-CCP is a highly specific test for Rheumatoid Arthritis. However, in this case, syphilis led to false-elevations in anti-CCP. CASE: A 29-year-old G5P3 female presented to the emerge

CSF peptidomics reveals an IGF2-derived peptide as a candidate biomarker for neurosyphilis diagnosis.

BACKGROUND: Neurosyphilis (NS) diagnosis is challenging due to the cerebrospinal fluid-Venereal Disease Research Laboratory (CSF-VDRL) assay has limited sensitivity, whereas CSF abnormalities lack sufficient specificity. METHODS: To identify novel CSF peptide biomarkers for NS, we performed comparative peptidomic analysis of CSF samples from 51 individuals across four groups: NS, syphilis/non-NS (NNS), infectious brain diseases without syphilis (IBD), and non-infectious brain disorders without syphilis (NIBD). Peptides were profiled by high-resolution liquid chromatography-tandem mass spectro

CV2/CRMP5 antibodies are specifically associated with parenchymal neurosyphilis: a retrospective cohort study.

BACKGROUND: Neurosyphilis can trigger neuro-specific immune responses, but evidence remains scarce. This study investigates the prevalence and clinical significance of neural antibodies in neurosyphilis, with a focus on CV2/CRMP5 antibodies. METHODS: We retrospectively analyzed 1149 patients with syphilitic neurological symptoms. Of 247 diagnosed with neurosyphilis, 215 met inclusion criteria. Using CBA and line blot, 97 patients suspected of autoimmune encephalitis were tested for neural antibodies, with positives confirmed by in-house CBA. Complete medical records were collected. Serum Toluidine Red Unheated Serum Test (TRUST) titer and cerebrospinal fluid parameters as predictors of neurosyphilis in HIV-negative people with late latent syphilis.

BackgroundThe diagnosis of neurosyphilis is challenging due to the limited sensitivity of cerebrospinal fluid Venereal Disease Research Laboratory (VDRL) test. Late latent syphilis is defined as positive treponemal serology without clinical manifestations and untreated infection for more than 1 year. Symptomatic or asymptomatic neurosyphilis is diagnosed based on cerebrospinal fluid findings and clinical symptoms. This cross-sectional study investigated serum Toluidine Red Unheated Serum Test (TRUST) titer and cerebrospinal fluid inflammatory markers as predictors of neurosyphilis in HIV-negat

Alterations of cognitive function and hippocampal metabolism in asymptomatic neurosyphilis patients: based on 1H-magnetic resonance spectroscopy.

OBJECTIVES: Neurosyphilis has been found to be associated with cognitive decline. However, as a common type of neurosyphilis, the change of cognitive function in asymptomatic neurosyphilis (ANS) has received little attention. This study aims to investigate the cognitive function and cellular metabolism changes in bilateral hippocampus in ANS. METHODS: Twenty-four ANS, 20 syphilis patients and 23 control individuals who were admitted in Hangzhou Third people's Hospital from June 2021 to July 2025 were selected. 1H-MRS (magnetic resonance spectroscopy) was used to detect the cellular metabolism

High Titers, Hidden Disease: Asymptomatic Neurosyphilis in Secondary Syphilis - A Case Series from Western Maharashtra.

Syphilis can involve the central nervous system at any stage; however, routine cerebrospinal fluid (CSF) examination is rarely performed in neurologically asymptomatic patients with secondary syphilis. We describe a case series of eight male patients with secondary syphilis and high serum venereal disease research laboratory (VDRL) titers (≥1:16) evaluated at a tertiary teaching hospital in western Maharashtra, India (2024-2025). Five patients were human immunodeficiency virus (HIV)-positive and on antiretroviral therapy. None had neurological symptoms or signs; however, all had CSF abnormalit

Cytokines in cerebrospinal fluid combined with machine learning improve the diagnostic accuracy and predict the progression of neurosyphilis.

BACKGROUND: Syphilis is a sexually transmitted disease (STD) caused by Treponema pallidum (TP) which could progress to neurosyphilis affecting the nervous system. However, the pathogenesis of neurosyphilis remain unknown. In addition, current diagnosis of neurosyphilis is mainly based on serological tests and cerebrospinal fluid (CSF) analysis, with limitations in terms of sensitivity and specificity. METHODS: From April 2021 to April 2023, 129 patients with syphilis were enrolled and divided into the non-neurosyphilis group (common syphilis, CS, n=19), asymptomatic neurosyphilis group (AN, n

Blood Culture-Negative Cardiovascular Syphilis in an Untreated HIV-Positive Man.

BACKGROUND Blood culture-negative endocarditis (BCNE) is an infection of the endothelium of the heart and valves with negative blood cultures. Cardiovascular syphilis is a late-stage presentation that typically involves the aorta and rarely affects the aortic valve. It is caused by the spirochete Treponema pallidum and is classified as a BCNE. The specific identification of T. pallidum as the causative pathogen in the heart requires next-generation sequencing on explanted tissue. Coinfection between individuals infected with human immunodeficiency virus (HIV) and syphilis is not uncommon. The

Eponymous neurologic signs of tabes dorsalis: A historical review (1846-1905).

This article examines the neurologic signs historically linked to tabes dorsalis and assesses their relevance in today's neurosyphilis diagnostics. Using PubMed/MEDLINE and major digitized archives, a structured historical review was conducted with searches in English, French, and German, combining terms related to syphilis and neurologic signs. Ten eponymous signs were identified between 1846 and 1905: Romberg, Remak, Westphal, Seeligmüller, Biernacki, Patrick, Sárbó, Bechterew (Bekhterev), Pitres, and Abadie. These signs point to a characteristic pattern of sensory impairment-patchy loss of

Ischemic Stroke Secondary to Meningovascular Syphilis.

Syphilis can present in a wide variety of ways, earning its reputation as "the great imitator". In a minority of cases, the bacteria can invade the central nervous system and produce meningovascular syphilis, an inflammatory vasculopathy that can mimic neurovascular disease by manifesting as an ischemic stroke. Consideration and timely recognition of this manifestation of syphilis infection is paramount because if left untreated treponemal induced arteritis can progress, keeping patients at increased risk for additional strokes. We report the case of a 36-year-old man whose untreated HIV place

Developing a multi-antigen fusion protein (Tp15-17-47) serological test to improve neurosyphilis diagnosis.

OBJECTIVES: Our previous research identified a luciferase immunosorbent assay (LISA) for detecting serum antibodies against Tp17 and Tp47 as a potential tool for diagnosing neurosyphilis (NS). In this study, we developed a serological test based on a multi-antigen fusion protein (Tp15-17-47) to simplify the detection process and improve the diagnostic accuracy for NS. METHODS: The Tp15, Tp17, and Tp47 genes were amplified separately by polymerase chain reaction (PCR) and assembled into a fusion gene coding for a tripartite fusion protein antigen using overlap PCR. This recombinant Tp15-17-47

Syphilis Co-Infection Among People Living with HIV in Romania: Epidemiological and Clinical Characteristics in a Single-Center Retrospective Study.

Syphilis and HIV are sexually transmitted disease (STDs) that interact synergistically. However, data on HIV-syphilis co-infection in Romania remain limited. We conducted a retrospective cohort study at a single Romanian HIV/AIDS Day Clinic, including 439 adult people living with HIV (PLWH) monitored between 2020 and 2025. Demographic, epidemiological, clinical, and laboratory data were collected, including HIV staging and syphilis history. Syphilis co-infection was identified in 81 patients (18.5%), and 61.5% met criteria for AIDS. Viral suppression was achieved in 82.2%, and 78.4% achieved C

Spinal H-Sign in Syphilitic Myelitis Mimicking MOGAD.

INTRODUCTION: Longitudinally extensive transverse myelitis (LETM) is classically associated with aquaporin-4 antibody-mediated neuromyelitis optica spectrum disorder and myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD). The "H-sign," reflecting gray matter involvement, is considered suggestive of MOGAD. However, infectious etiologies, including neurosyphilis, may rarely mimic autoimmune inflammatory myelopathies radiologically. CASE REPORT: A 50-year-old man presented with 6 months of erectile dysfunction and urinary retention, followed by progressive gait instability.

Effects of electrical stimulation on the cardiogenic differentiation of mesenchymal stem cells: A systematic review.

UNLABELLED: Electrical stimulation has emerged as a promising strategy for promoting cardiogenic differentiation of mesenchymal stem cells; however, the available evidence remains highly heterogeneous across stimulation parameters, experimental platforms, and outcome measures. This systematic review synthesized the available in vitro evidence regarding the effects of electrical stimulation on MSC cardiogenic differentiation, with particular emphasis on stimulation parameters, experimental platforms and biological outcomes related to early cardiogenic commitment and structural maturation. The great imitator behind an ischemic stroke: a case of meningovascular syphilis.

Neurosyphilis is a rare but reversible cause of ischemic stroke. We report a 41-year-old male with no cardiovascular risk factors initially diagnosed with Bell's palsy for left facial weakness. Four days later, he developed left-sided hemiparesis and dysarthria. Neuroimaging confirmed a right nucleocapsular ischemic stroke. Cerebrospinal fluid analysis revealed pleocytosis (39 cells/µL), elevated protein (191.9 mg/dL), and a reactive VDRL, confirming meningovascular neurosyphilis. Treatment with 14 days of intravenous Penicillin G led to complete clinical recovery after six months. This case h

Research progress on the mechanisms of neurosyphilis' impact on cognitive function, early identification, and prevention strategies.

Neurosyphilis, a severe and often neglected complication of Treponema pallidum infection, poses a significant global public health threat, particularly impacting marginalized populations such as men who have sex with men and people with human immunodeficiency virus. Despite global efforts to control syphilis, neurosyphilis is increasingly prevalent, with documented surges of >700% in some regions. Its role in driving cognitive decline often misdiagnosed as psychiatric or neurodegenerative disorders exemplifies "neglected" dimension of diseases. This narrative review synthesizes recent advances
