Orbitofrontal Epilepsies: Intracranial Electroencephalography and Surgical Aspects.

Orbitofrontal epilepsies (OFE) produce variable clinical semiologies and nonspecific electrographic patterns thereby being challenging to localize. Furthermore, systematic studies of the surgical management and outcomes in OFE are sparse. The authors review the current literature and discuss the intracranial electroencephalography, microsurgical techniques, and surgical outcomes of patients in the context of a 20-year surgical experience in treating 24 patients with OFE. The authors distinguish between purely orbitofrontal resections (OF-focal, n = 10) and those in whom additional brain region

Targeted nanopore long-read sequencing panel for the molecular diagnosis of intronic expansion in familial adult myoclonic epilepsy.

BACKGROUND: Familial adult myoclonic epilepsy (FAME), an autosomal dominant disorder, is characterized by cortical myoclonus and occasional generalized tonic-clonic seizures. To date, intronic pentanucleotide repeat expansions in at least seven genes, including SAMD12, TNRC6A, YEATS2, MARCHF6, STARD7, RAPGEF2, and RAI1, have been reported as causative. Detecting these repeat expansions using conventional sequencing techniques (Sanger or short-read next-generation sequencing) is not feasible as they cannot reliably span or characterize long repetitive elements. Although genetic testing has been

Systematic review of indirect costs to families of children with developmental epileptic encephalopathies.

BACKGROUND: Severe childhood epileptic encephalopathies have high burden on families and large indirect healthcare costs. Several studies have examined indirect costs primarily in Tuberous Sclerosis and Dravet syndrome, finding impacts in all major aspects of life. Indirect costs of these diseases are reported to be higher than other severe pediatric illnesses. To better understand the epilepsy care journey and possible barriers to optimal medical and surgical care, we sought to understand indirect costs of care and burden of illness for families. METHODS: A systematic search was conducted in

Natural History of Children and Adolescents With Dravet Syndrome: A 24-Month Follow-Up.

BACKGROUND AND OBJECTIVES: Dravet syndrome (DS) is an intractable developmental and epileptic encephalopathy caused primarily by pathogenic variants in the voltage-gated sodium channel α subunit 1 (SCN1A) gene. Patients with DS experience refractory seizures and significant cognitive and behavioral deficits. Longitudinal studies using standardized assessments are needed to systematically document patient outcomes over time. The BUTTERFLY study aimed to assess changes in adaptive functioning and neurodevelopment over 24 months in patients with DS. METHODS: BUTTERFLY was a US-based multicenter,

Compound heterozygous variants of CACNA1H change channel properties and contribute to intractable epilepsy with myoclonic-atonic seizures.

The CACNA1H gene, which encodes the T-type calcium channel Cav3.2, is known to confer susceptibility to childhood absence epilepsy (CAE) and has been implicated in various neurological disorders. However, its pathogenic significance, especially in childhood intractable epilepsies, has not been comprehensively explored. We performed whole-exome sequencing on a 4-year-old boy diagnosed with epilepsy with myoclonic-atonic seizures (EMAtS), and identified two missense variants in CACNA1H. One was a novel variant, p.D949H, inherited from the father, while the other was a known variant, p.R788C, inh

Long-term administration of EQU-001 (ivermectin) suppresses spasms in a rat model of infantile epileptic spasms syndrome.

We investigated effects of long-term and acute administration of EQU-001 (ivermectin) against NMDA-triggered spasms in immature rats prenatally exposed to betamethasone and in the model of infantile epilepsy spasms syndrome (IESS). Pregnant rats were injected with betamethasone on gestational day 15 (2 ×0.4 mg/kg, i.p.). Offspring were injected with ivermectin once daily (1-4 mg/kg, i.p.) from postnatal day (P) 10 through P15. Spasms were triggered on P15 with NMDA (17 mg/kg, i.p.). Other rats received a single dose of ivermectin (1-8 mg/kg, i.p.) on P15 prior to NMDA to test acute anticonvuls

Epidemiology and clinical characteristics of frontal absence seizures compared with primary absence seizures.

INTRODUCTION: Absence seizures due to secondary bilateral synchrony may be a manifestation of frontal seizures, indistinguishable from primary absence epilepsy (PAE), with a different prognosis and control. There are few epidemiological studies on frontal absences (FAE) in the group of absence epilepsy (AE) in children. OBJECTIVE: To describe the epidemiology of FAE in childhood and compare the characteristics, clinical evolution, and pharmacological response with PAE. PATIENTS AND METHOD: Retrospective study of cases with AE in children under 14 years of age from 2013 to 2022 in a tertiary

[Epileptic encephalopathy associated with a mutation in the KCNT1 gene].

According to the World Health Organization, more than 50 million people worldwide have epilepsy, and about 5 million new cases are reported annually. According to other data, the number of patients exceeds 70.000.000, highlighting the relevance of this condition. Studies of developmental epileptic encephalopathies leading to early disability are particularly relevant. About 900 gene mutations associated with different types of epilepsy have been identified so far. One of the prominent «channelopathy genes» representatives is KCNT1, coding the largest subunit of the sodium-activated potassium c

[Developmental epileptic encephalopathy caused by a de novo mutation in the SCN8A gene].

Developmental epileptic encephalopathy is characterized by a malignant course of refractory epilepsy and a growing defect in mental and motor development. To date, determinant genes have been identified for most of these conditions. This condition is also caused by mutations in the SCN8A gene of a voltage-gated sodium channel, which is detected in about 1% of children with early infantile epileptic encephalopathies. SCN8A gene mutations result in both an increase (gain-of-function, GOF mutation) and a decrease (loss-of-function, LOF mutation) in NaV1.6 sodium channel function, leading to a dec

New therapeutic strategies for Lafora disease: Evaluation of the safety, efficacy, pharmacokinetics and metabolomic profile of intravenous VAL-1221 treatment.

Lafora disease (LD) is a fatal progressive myoclonus epilepsy that affects previously healthy adolescents and lacks effective treatments. It is caused by pathogenic variants in EPM2A or NHLRC1, leading to the accumulation of polyglucosan in the brain and other tissues. This study is the first to evaluate the administration of a potentially disease-modifying drug - VAL-1221, a glycogen-degrading antibody-enzyme fusion - in LD patients through a 12-month compassionate use program. Five patients (aged 17-24 years; three females) with intermediate to advanced LD received VAL-1221 intravenous infus

Theory of mind in juvenile myoclonic epilepsy.

Juvenile myoclonic epilepsy (JME) is a common idiopathic generalized epilepsy often accompanied by executive dysfunction, affective symptoms, unfavorable behavior, and social outcomes, yet its impact on theory of mind (ToM) remains underexplored. We conducted an unmatched case-control study assessing 34 JME patients and 48 healthy controls, adjusted for age, sex, education, intelligence quotient, anxiety, and depression. Participants completed a brief version of the Faux Pas Recognition Test (FPRT) and the Reading the Mind in the Eyes Test, alongside measures of executive function, prospective

Disrupted Transcriptional Networks in Mammalian Cells Stably Over-Expressing Pathogenic Atrophin-1.

Dentatorubral-Pallidoluysian Atrophy (DRPLA) is a dominant neurodegenerative disease caused by CAG triplet repeat expansion in ATN1, which encodes the transcriptional co-repressor Atrophin-1. DRPLA features motor, cognitive, and epileptic symptoms and shares pathogenic mechanisms with other polyglutamine (polyQ) disorders, including protein misfolding, impaired autophagy, and transcriptional dysregulation. To understand disease mechanisms, we performed RNA-seq on HEK293T cells stably over-expressing wild-type or pathogenic ATN1. Cells expressing pathogenic ATN1 exhibited a distinct transcripto

Anesthetic Management Using Remimazolam for Dental Treatment in a Pediatric Patient With Dravet Syndrome.

Dravet syndrome (DS) is a rare, severe form of epilepsy characterized by seizures often triggered by fevers and other nonfebrile increases in body temperature. We report the case of a girl aged 3 years 5 months who underwent an intubated general anesthetic for dental treatment using remimazolam (RMZ). After a slow mask induction with sevoflurane, RMZ, remifentanil, and rocuronium were administered to facilitate oral intubation, after which general anesthesia was maintained with air, oxygen, and continuous infusions of RMZ and remifentanil. Intraoperative temperature and other vital signs were

Development and initial validation of the Communication Inventory Disability - Observer Reported (CID-OR): a measure of communication in CDKL5 deficiency disorder.

BACKGROUND: CDKL5 Deficiency Disorder (CDD) is a rare neurodevelopmental disorder characterised by early onset seizures combined with complex healthcare needs and developmental impairment that influence functional domains including communication. Communication is a high priority domain for families but currently used measures demonstrate floor effects. Emerging disease-modifying trials necessitate the precise and granular measurement of communication to determine drug efficacy. This study evaluated a new measure of communication for CDD called the Communication Inventory Disability – Observer

Neuronal hyperexcitability: A key to unraveling hippocampal synaptic dysfunction in Lafora disease.

BACKGROUND AND OBJECTIVE: Lafora disease (LD) is a rare progressive disorder caused by mutations in the EPM2A or EPM2B genes, characterized by the accumulation of Lafora bodies, drug-resistant epilepsy, and cognitive decline. To investigate the early molecular mechanisms of LD, we studied electrophysiological changes in the dentate gyrus (DG) of the Epm2aR240X knock-in mouse model at various ages. METHODS: Electrophysiological recordings measured neuronal membrane properties, epileptic-like activity, epileptic thresholds, and synaptic plasticity in Epm2aR240X mice at 1, 3, and 12 months. We a

The biochemical dynamics of the glycogen phosphatase laforin directly impact brain metabolism.

Laforin is the only known glycogen phosphatase. Mutations in the laforin gene lead to the fatal childhood dementia and progressive myoclonic epilepsy known as Lafora disease (LD). A hallmark of LD is aberrant, cytoplasmic, glycogen-like aggregates known as Lafora bodies. Surprisingly, recent reports indicate that overexpression of a phosphatase-deficient laforin mutant, with the catalytic cysteine mutated to serine (LCS), prevented the formation of Lafora bodies in a laforin KO mouse model. This finding led to questions regarding the biological relevance of laforin phosphatase activity and its
