Apigenin Regulating PI3K/AKT Pathway to Improve Depressive Behavior in Epileptic Rats.

INTRODUCTION: Depression is a common comorbidity in epilepsy, significantly impacting patients' quality of life. The hippocampus, linked to depression and neurodegeneration, is vulnerable in epilepsy. Epileptogenesis involves inflammation, oxidative stress, and neuronal damage, with the PI3K/AKT pathway playing a key role. Apigenin (API), a flavonoid in fruits and vegetables, shows neuroprotective, anti-inflammatory, and anti-apoptotic effects. This study investigates API's mechanisms in a LiCl-pilocarpine epileptic rat model, focusing on hippocampal neurogenesis and PI3K/AKT signaling as pote

Depressive symptoms as independent correlates of epilepsy-related cognitive burden.

OBJECTIVE: This study was undertaken to assess the relationship between the severity of depression and anxiety symptoms and epilepsy-related variables and cognitive burden in people with epilepsy (PwE), as assessed using EpiTrack. METHODS: We prospectively enrolled a cohort of PwE who underwent EpiTrack and evaluation by Generalized Anxiety Disorder-7 (GAD-7) and Beck Depression Inventory-II (BDI-II) scales. We assessed the correlation strength between EpiTrack, GAD-7, BDI-II, and the other clinical variables. Analysis of variance and covariance assessed the existence of GAD-7/BDI-II differen

Interictal epileptiform discharges are involved in momentary lapses of attention in children with epilepsy.

OBJECTIVE: Attention impairments are common in children with epilepsy and widely impact their quality of life. Interictal epileptiform discharges (IEDs) may induce subtle dysfunctions of various cognitive processes, but data regarding the impact of IEDs on sustained attention remain limited. The objective of the present study was to evaluate the impact of IEDs on continuous undivided attention in children with epilepsy, controlling for the number of treatments, type of epilepsy, frequency of seizures in the past year, age at onset, and comorbid attention disorder. METHODS: Using a computerize

Underutilization of syndrome-specific ICD-10 codes for genetic epilepsies: Implications for precision medicine.

Syndrome-specific International Classification of Diseases, 10th Revision (ICD-10) codes have the potential to improve identification of patients for precision therapies, clinical trials, and research, yet their real-world uptake is not well characterized. We evaluated the utilization of syndrome-specific ICD-10 codes at a large academic medical center among patients with pathogenic or likely pathogenic variants in 10 monogenic epilepsy genes with established codes (CDKL5, EHMT1, KCNQ2, MECP2, MED13L, SCN1A, SHANK3, SLC13A5, SLC2A1, SYNGAP1). Patients were identified from an institutional gene

Amygdala enlargement in pediatric epilepsy: A clinically meaningful finding?

RATIONALE: Amygdala enlargement (AE) is an increasingly recognized finding in temporal lobe epilepsy (TLE), particularly in patients without clear magnetic resonance imaging (MRI) lesions. While AE has been associated with later seizure onset, mood symptoms, and immune-mediated mechanisms, its role in epilepsy remains unclear. AE is not consistently epileptogenic and may be present in other epilepsy syndromes and even in healthy individuals. Most studies focus on adults; little is known about AE in pediatric TLE. We present a pediatric case series examining clinical, neuroimaging, and outcome

Cenobamate: A comprehensive review.

This article provides a review of cenobamate's (CNB's) pharmacology, mechanisms of action, efficacy, safety, and seizure/epilepsy types treated. CNB is an antiseizure medication approved in the U.S. in 2019 for treatment of adults with focal (partial-onset) seizures. Preclinical data indicated CNB had a broad spectrum of antiseizure activity in rodent models of generalized and focal seizures. CNB has two mechanisms of action that may contribute additively or synergistically to powerfully counter the development of the paroxysmal depolarizing shift in neurons: selective inhibition of persistent

High incidence of Y-chromosome mosaicism in male and female individuals with mild malformation of cortical development with oligodendroglial hyperplasia in epilepsy.

OBJECTIVE: Mild malformation of cortical development with oligodendroglial hyperplasia in epilepsy (MOGHE) is an underrecognized pediatric cortical lesion associated with somatic X-linked SLC35A2 variants in approximately 50% of individuals. The genetic etiology in individuals without detectable SLC35A2 mutations remains undefined, which limits our understanding of its pathomechanism and clinical characterization. The aim is to identify genetic correlates of MOGHE in individuals lacking pathogenic SLC35A2 variants and to investigate the clinicopathological and genomic correlations. METHODS: W

Comparative neuroprotective and cognitive effects of human umbilical cord mesenchymal stem cells, secretome, and exosomes in a rat model of pilocarpine-induced epilepsy.

BACKGROUND AND AIM: Epilepsy is a chronic neurological disorder characterized by recurrent seizures, neuronal degeneration, and cognitive impairment. Current antiepileptic therapies mainly control seizures and do not effectively prevent epileptogenesis or restore neurological function. Human umbilical cord mesenchymal stem cells (HuMSCs) and their cell-free derivatives, including secretome and exosomes, have emerged as promising regenerative therapies. This study aimed to compare the neuroprotective and cognitive effects of HuMSCs, secretome, and exosomes in a rat model of pilocarpine-induced

Epilepsy related mortality: more than sudden unexpected death in epilepsy (SUDEP).

BACKGROUND: The incidence of sudden unexpected death in epilepsy (SUDEP) in Ireland has previously been studied. The aim of this work is to examine how frequently epilepsy related deaths, other than SUDEP, occur in the Irish population. METHODS: All deaths referred to the coroner in 2019 were examined. Those with a history of possible epilepsy were noted and subjected to a more detailed assessment. Cases of SUDEP were excluded and other epilepsy related deaths were divided into those directly related to epilepsy and those indirectly related to epilepsy. RESULTS: We identified 54 non SUDEP ep

Cost-effectiveness of vagus nerve stimulation therapy for drug-resistant epilepsy across healthcare settings: a structured narrative review.

Drug-resistant epilepsy (DRE) is associated with substantial healthcare utilization, impaired quality of life (QOL), and considerable societal costs. Vagus nerve stimulation (VNS) has become an established adjunctive treatment for patients who are not suitable candidates for resective epilepsy surgery or who fail to achieve adequate seizure control with pharmacotherapy. This structured narrative review critically evaluates the current evidence on the cost-effectiveness of VNS in DRE. A comprehensive literature search was conducted across PubMed, Web of Science, Embase, Google Scholar, CNKI, an

Automated Classification of Sleep-Wake States and Seizures in Mice.

Sleep-wake states bi-directionally interact with epilepsy and seizures, but the mechanisms are unknown. A barrier to comprehensive characterization and the study of mechanisms has been the difficulty of annotating large chronic recording datasets. To overcome this barrier, we sought to develop an automated method of classifying sleep-wake states, seizures, and the post-ictal state in mice ranging from controls to mice with severe epilepsy with accompanying background EEG abnormalities. We utilized a large dataset of recordings, including EMG, EEG, and hippocampal local field potentials, from c

Cenobamate in developmental and epileptic encephalopathies and generalized epilepsies: A case report on epilepsy with myoclonic-atonic seizures and systematic review of current evidence.

BACKGROUND: Cenobamate (CNB) has demonstrated remarkable efficacy in the treatment of drug-resistant focal epilepsy (FE). However, its role in other epilepsy types - such as drug-resistant generalized epilepsies (GEs), combined generalized and focal epilepsies (CGFEs), and developmental and epileptic encephalopathies (DEEs) - remains poorly explored. This article assesses the current evidence of CNB efficacy in these often complex and challenging patient populations. METHODS: A case report is presented detailing a 22-year-old male with drug-resistant epilepsy with myoclonic-atonic seizures (E

Mesial-to-lateral gradients of epileptiform activity to localize mesial temporal lobe epilepsy.

OBJECTIVE: Mesial temporal lobe epilepsy is a common localization of drug-resistant epilepsy in adults. Patients often undergo intracranial electroencephalographic monitoring to confirm localization and determine candidacy for focal ablation or resection. Clinicians primarily base surgical decision-making on seizure onset patterns, with imaging abnormalities and information from interictal epileptiform discharge (spikes) used as ancillary data. How the morphology and timing of spikes within multielectrode sequences may inform surgical planning is unknown, in part due to the lack of measurement

Early age at seizure onset is a risk factor for attention deficit hyperactivity disorder in children with epilepsy.

Attention deficit hyperactivity disorder (ADHD) is one of the most common comorbidities in children with epilepsy. To determine whether those diagnosed with ADHD had an earlier seizure onset -independent of other epilepsy-related factors-, data from 203 children with epilepsy (6 to 17 years and 11 months; 53 % female) were analyzed. The Diagnostic and Statistical Manual of Mental Disorders fifth edition helped define two groups: epilepsy without (n = 96) or with (n = 107) ADHD. The academic repercussions of ADHD were assessed using a composite school impact index. A binary logistic regression

Enhancing sudden unexpected death in epilepsy (SUDEP) research through development of common data elements.

OBJECTIVE: Sudden Unexpected Death in Epilepsy (SUDEP) is a fatal complication for individuals living with epilepsy and is associated with significant personal and public burden. While certain neurotransmitters and neuronal pathways have been associated with SUDEP, the exact biological mechanisms are unknown. Preclinical research has been instrumental in providing clues to the underlying pathology but is limited by a lack of standardized methodologies for describing and collecting data. A key outcome of the Basic Science working group of the 2020 SUDEP Coalition Summit was the recognition that

miR-155-5p and miR-674-3p presence in peripheral blood and white blood cells as potential early biomarkers of temporal lobe epilepsy.

Epilepsy frequently develops as a result of a brain insult, for example, brain injury or stroke. Currently, no tools are allowing us to predict which trauma patients will eventually develop epilepsy. There is evidence that microRNA levels are altered in the blood, making them attractive candidates for peripheral biomarkers of epilepsy. We analyzed whole blood miRNA levels using a dual-platform approach and selected miR-155-5p and miR-674-3p as candidates for biomarkers of early changes in asymptomatic temporal lobe epilepsy. Additionally, we assessed white blood cell subpopulations containing
