Preconditioning induces ischemic tolerance by attenuating ferroptosis: involvement of the m6A-dependent FTO-IGF2BP1/3-ACSL4 regulatory Axis.

Cerebral ischemic preconditioning (CIP) confers robust protection against subsequent ischemic stroke by enhancing brain ischemic tolerance. Although ferroptosis is increasingly recognized as a critical pathogenic mechanism in stroke, the role of N6-methyladenosine (m6A) RNA modification in its regulation remains poorly understood. This study investigated the involvement of m6A signaling in the anti-ferroptotic mechanism of CIP using complementary in vivo mouse middle cerebral artery occlusion (MCAO) and in vitro oxygen-glucose deprivation (OGD) models. We found that CIP significantly attenuate

Diagnostic Impact of Intracranial Vessel Wall MRI on Ischemic Stroke Etiology in Young Adults.

BACKGROUND: Intracranial vessel wall imaging (VWI) enables visualization of arterial wall pathology and may improve etiologic classification of ischemic stroke, but existing evidence is largely based on retrospective studies. We aimed to prospectively evaluate the diagnostic impact of VWI in young stroke patients. METHODS: In this two-centre study, consecutive patients aged 18-59 years with acute ischemic stroke in the middle cerebral or anterior choroidal artery territory underwent contrast-enhanced VWI within 2 weeks of stroke onset. Stroke etiology was assessed using the Trial of ORG 10172

[Overview of MELT-Japan].

MELT-Japan was a multicenter randomized controlled trial (RCT) conducted in Japan between 2002 and 2005, in which the efficacy and safety of local fibrinolytic therapy using urokinase for acute ischemic stroke due to middle cerebral artery occlusion was compared with the best medical treatment available at the time. The results were reported in 2007. While there was no difference between the two groups in the primary endpoint (modified Rankin score[mRS], 0-2 at 90 days), the efficacy of endovascular treatment was confirmed in one of the secondary endpoints (mRS, 0-1 at 90 days), and there was

[The History of Evolution of Mechanical Thrombectomy Device Design and Introduction to Japanese Market].

Endovascular recanalization therapy for acute ischemic stroke (AIS) began with local fibrinolysis therapy using the development of microcatheters. Subsequently, mechanical thrombectomy therapy began with the development of the Merci retriever, which uses a shape-memory spiral wire to entangle thrombi in intracranial arteries. Then the stent retriever appeared and the effectiveness of endovascular recanalization therapy for occlusion of the internal carotid artery and proximal middle cerebral artery was confirmed by clinical trials. And the Penumbra system, which uses a large-diameter catheter

Design, Synthesis of 3-n-Butylphthalide-Hydrazine/Hydrazide Derivatives as Potential Anti-Ischemic Stroke Agents.

To identify novel therapeutic candidates for ischemic stroke, a series of hydrazine- and hydrazide-derived analogs based on 3-n-butylphthalide (NBP) were designed and synthesized in this study, and their anti-ischemic stroke activities were systematically assessed. Among all synthesized derivatives, the compound NBP-HY exerted potent antiplatelet aggregation and antioxidant activities in vitro. Consistent in vivo results revealed that NBP-HY significantly alleviated thrombus formation in both carrageenan-induced tail thrombosis and FeCl3-induced carotid artery thrombosis models. In a rat middl

Endovascular treatment versus best medical treatment for acute anterior cerebral artery occlusion: a single-center inverse probability of treatment-weighted cohort study.

INTRODUCTION: Evidence regarding the benefit of endovascular treatment (EVT) for isolated acute anterior cerebral artery occlusion remains limited. We aimed to compare the efficacy and safety of EVT versus best medical treatment (BMT) in this population. METHODS: We retrospectively screened consecutive patients with acute anterior cerebral artery occlusion between May 2020 and October 2025. The primary outcome was excellent functional outcome, defined as a 90-day modified Rankin Scale (mRS) score of 0-1. Secondary outcomes included 90-day mRS 0-2, mRS 0-3, mortality, symptomatic intracranial

Artificial intelligence for facial soft-tissue landmark localization, measurement, and classification in orthodontics: A systematic review.

Microglial polarization critically influences neuroinflammation and recovery after ischemic stroke. The role of C-type lectin domain family 7 member A (CLEC7A), a potential regulator in this process, remains unclear. Integrated bioinformatics analysis of Gene Expression Omnibus datasets identified CLEC7A as a hub gene. Its functional role was investigated using in vitro oxygen-glucose deprivation (OGD) models in BV2 microglia/HT22 neuron co-cultures and in vivo rat middle cerebral artery occlusion (MCAO) models. CLEC7A was knocked down in vitro and in vivo. Functional outcomes were assessed vi

Postoperative recruitment and remodeling of middle meningeal artery collaterals in adult moyamoya disease: implications for preservation during combined revascularization.

BACKGROUND: The middle meningeal artery (MMA) may provide pre-existing transdural flow and postoperative collateral recruitment in adult moyamoya disease (MMD). We evaluated MMA remodeling, recruitment, and the safety relevance of preserving visible branches during combined revascularization. METHODS: This retrospective hemisphere-level study included 110 operated hemispheres from 85 adults undergoing superficial temporal artery-middle cerebral artery bypass with encephaloduromyosynangiosis. Preoperative angiography showed visible MMA collaterals in 38 hemispheres and none in 72. Outcomes inc

The Mask of Delirium: Uncovering a Presumed Cardioembolic Stroke Through Comprehensive Clinical Evaluation.

Delirium and aphasia exhibit overlapping clinical features, complicating the diagnosis of acute neurological disturbances in older adults. Both conditions impair communication and orientation, leading to incoherence; however, they arise from distinct pathophysiological mechanisms and require different management strategies. This case concerns an 83-year-old woman with a 48-hour history of acute-onset confusion and abnormal speech. Magnetic resonance imaging (MRI) revealed an acute infarct in the inferior division of the left middle cerebral artery (MCA) territory, with cardioembolism considere

Basilar Artery Occlusion Associated With Paradoxical Embolism After Traumatic Spinal Cord Injury: A Case Report.

Basilar artery occlusion is a neurological emergency associated with high mortality and severe disability. In young patients without conventional vascular risk factors, identifying an uncommon embolic mechanism is essential for secondary stroke prevention. We report the case of a previously healthy 21-year-old man who sustained a 6-meter fall resulting in T9-T10 compression fractures with spinal canal compromise and complete traumatic spinal cord injury. He underwent posterior thoracic spinal instrumentation. During hospitalization, he developed sudden neurological deterioration characterized

Symptomatic meningioma in the third trimester of pregnancy: a case report from Kenya.

Meningiomas, the most common primary central nervous system (CNS) tumors, are typically benign and hormonally responsive. They have a higher incidence in women, particularly during the reproductive years. Pregnancy may trigger rapid tumor progression and symptom onset due to elevated hormone levels. Although rare in pregnancy, meningiomas may adversely affect maternal and fetal outcomes, increasing the risk of complications such as preterm labor and gestational hypertension. Management strategies range from conservative monitoring to surgical intervention depending on gestational age, tumor si

[Current Evidence of Mechanical Thrombectomy for Acute Cerebral Artery Occlusion].

Following previous negative randomized controlled trials (RCTs), the validity of mechanical thrombectomy (MT) for large-vessel occlusion (LVO) of the anterior circulation was established by multiple positive RCTs in 2015. The success of these RCTs was achieved by introducing a stent retriever, which enabled a safer procedure and higher revascularization rate. Since then, novel evidence has been accumulated by several emerging RCTs to expand the indication of MT. First, the time limit for MT was extended based on RCTs demonstrating the efficacy of MT in patients with a longer penumbra on radiol

Intranasal administration of enalapril confers acute neuroprotection by mitigating mitochondrial dysfunction and apoptosis in a rat model of ischemic stroke.

BACKGROUND: Stroke remains a leading cause of mortality and long-term disability worldwide, with effective treatments hindered by its complex pathophysiology. This study is the first to explore the neuroprotective potential of intranasal enalapril, in mitigating ischemic stroke outcomes through its influence on mitochondrial function. METHODS: Male Sprague-Dawley rats were subjected to middle cerebral artery occlusion (MCAO), followed by the intranasal administration of enalapril (0.03 mg/kg) post-ischemia. Neurological function was evaluated after 24 hours of reperfusion through the modified

[Emerging Therapeutic Strategies for Central Nervous System Regeneration].

Reperfusion therapy has markedly improved outcomes after acute ischemic stroke; however, many patients remain ineligible for treatment or sustain irreversible brain injury despite successful recanalization. Regenerative medicine therefore represents an important strategy for restoring damaged neural tissue and promoting functional recovery. Stem cell therapies may exert therapeutic effects through multiple mechanisms, including cell replacement, secretion of neurotrophic factors, modulation of inflammation and immune responses, angiogenesis, and intercellular transfer of mitochondria and other

Development of the LARyNGTH score to predict central neurogenic respiratory failure in lateral medullary infarction.

BACKGROUND: Central neurogenic respiratory failure (CNRF) is a fatal complication associated with lateral medullary infarcts (LMI). Its detection may be critical for timely intervention and to reduce the risk of mortality. This study aimed to identify the clinical predictors of CNRF in patients with LMI. METHODS: This study included patients with LMI who were admitted to a university hospital between 2010 and 2022. CNRF was defined as unexpected intubations due to hypercapnia, cardiopulmonary resuscitation owing to respiratory causes, or clinical detection of CNRF. Clinical, neurological, and

Stage-Associated Efficacy of Systemically Administered Neural Stem Cell-Derived Exosomes in Ischemic Stroke through the Prevention of Cx43 Degradation.

Ischemic stroke pathology is highly dynamic, making the optimal timing for biological interventions highly elusive. This study investigated the stage-associated therapeutic efficacy of neural stem cell-derived exosomes (NSC-Exos) for cerebral ischemia-reperfusion. The in vivo evaluations identified a restricted therapeutic window closely associated with the temporal evolution of the ischemic microenvironment. Early exosome intervention (4 or 24 h post-ischemia) significantly reduced infarct volume, restored blood-brain barrier integrity, and improved neurological functional recovery. In contra
