Surgical anatomy of the infratemporal fossa: the styloid diaphragm revisited.

INTRODUCTION: The infratemporal fossa (ITF) gives passage to most major cerebral vessels and cranial nerves. Dissection of the ITF is essential in many of the lateral cranial base approaches and in exposure of the high cervical internal carotid artery (ICA). We reviewed the surgical anatomy of this region. METHODS: Direct foraminal measurements were made in seven dry skulls (14 sides), and the relationship of these foramina to each other and various landmarks were determined. Ten ITF dissections were performed using a preauricular subtemporal-infratemporal approach. Preliminary dissections of

Basilar artery vasospasm after severe head injury: a preliminary transcranial Doppler ultrasound study.

BACKGROUND: Post-traumatic vasospasm after severe head injury is now a well known entity. However, all studies available in the literature have evaluated only the anterior cerebral circulation. We evaluated the incidence of basilar artery vasospasm in patients with severe head injury. METHODS: Basilar artery mean blood flow velocity was measured in 16 patients with severe closed head injury (Glasgow Coma Scale 8 or less) using transcranial Doppler ultrasonography. Ten normal subjects also underwent the same investigation. The patients' age ranged from 5 to 65 years. The study group included 1

Multiple cerebellar infarction due to vertebral artery obstruction and bulbar symptoms associated with vertical subluxation and atlanto-occipital subluxation in ankylosing spondylitis.

Ankylosing spondylitis (AS) results in disease-specific inflammation at the site of ligamentous insertion into the bone. Atlantoaxial joint subluxation and vertical subluxation of the axis may occur as a consequence of instability resulting from the inflammatory process. Spontaneous anterior atlantoaxial subluxation is a well recognized complication in about 2% of patients with AS, and presents with or without signs of spinal cord compression. Vertical subluxation may follow anterior or posterior subluxation. It was noted in 3-8% of patients with rheumatoid arthritis, but is an exceedingly rar

[A case of aortic coarctation presenting with Brown-Séquard syndrome due to radicular artery aneurysm].

We report a case of aortic coarctation who showed a Brown-Séquard syndrome due to the aneurysm of the anterior radicular artery. A 71-year-old right-handed hypertensive woman was admitted to the Research Institute for Brain and Blood Vessels because of subarachnoid hemorrhage in 1984. No aneurysm was detected even on the four-vessel cerebral angiography. However, the left subclavian artery was occluded associated with the coarctation. In 1997, she complained of numbness of the right lower extremity, which gradually expanded to the back and the right upper extremity. On neurological examination

Transcranial Doppler: state of the art.

INTRODUCTION: Transcranial color Doppler sonography permits the accurate assessment of intracranial arteries. The latest Doppler units, using the color and power techniques, can show even very small flow volumes (1 x 1 mm). Low frequency (2-2.5 MHz) and very focused transducers are used in transcranial color Doppler. The skull is a very strong barrier for ultrasounds, which requires the use of some acoustic windows like some thin portions of the skull bone or some natural skull foramina. The use of echocontrast agents in color Doppler seems to increase the applications of transcranial studies.

Bilateral medial frontal infarction in a case of azygous anterior cerebral artery stenosis.

We describe the unusual case of a 63-year-old woman with a history of arterial hypertension who presented a sudden weakness of the lower limbs followed by mutism, akinesia and dyspraxia. Magnetic resonance images showed a bilateral medial frontal infarction. Digital subtraction angiography documented a right azygous anterior cerebral artery with severe stenosis in its sub-callosal tract; the left anterior cerebral artery showed mild hypoplasia with only sub-frontal and fronto-polar branches. No embolic source was documented. Afterwards the patient presented a gradual and partial recovery of bo

Symptomatic haemodynamically significant carotid occlusion treated by posterior circulation revascularization.

Haemodynamic cerebral ischaemic symptoms secondary to occlusive atheromatous disease of the internal carotid artery may resolve with revascularization. We describe a case of symptomatic internal carotid artery occlusion and ipsilateral subclavian stenosis with duplex and angiographically demonstrated vertebrobasilar steal and a patent ipsilateral posterior communicating artery with flow towards the steal. The patient suffered frequent ipsilateral middle cerebral artery territory transient ischaemic attacks and one minor stroke. Transcranial Doppler demonstrated markedly impaired ipsilateral ca

Hemodynamic effects of middle cerebral artery stenosis and occlusion.

BACKGROUND AND PURPOSE: Middle cerebral artery (MCA) stenosis and occlusion may cause ischemic symptoms through both hemodynamic and embolic mechanisms. The purpose of this investigation was to determine the hemodynamic effects of these lesions. METHODS: Ten patients with angiographically confirmed symptomatic occlusion (n = 5) or stenosis (n = 5) of the M1 segment of the MCA were studied by clinical examination, arteriography, and positron emission tomography (PET). Arterial supply to the distal MCA territory was classified from a review of the angiogram as being through the stenosis or from

Cerebral revascularization using muscle free flap for ischemic cerebrovascular disease in adult patients.

The efficacy of encephalo-myo-synangiosis (EMS) using muscle free flap was evaluated for the treatment of ischemic cerebrovascular disease in adult patients. Three patients with adult moyamoya disease and three patients with atherosclerotic ischemic cerebrovascular disease were treated. EMS used four latissimus dorsi muscles and two serratus anterior muscles. Postoperative selective angiography showed collateral circulation from the transferred muscle to the brain in four of the six patients. The other two patients showed patent nutrient artery of the transferred muscle flap. Cerebral blood fl

Stroke risk factors and development of collateral flow in carotid occlusive disease.

BACKGROUND AND PURPOSE: Blood flow through collateral vessels compensates for reduced blood flow through stenotic or occluded extracranial carotid arteries. Previous studies have shown that extent of collateral flow influences likelihood of stroke and its outcome. Here we analyzed the relationship between stroke risk factors (hypertension, coronary artery disease, diabetes mellitus, tobacco smoking and hypercholesterolemia) and number of patent intracranial collaterals detected by transcranial Doppler ultrasonography. SUBJECTS AND METHODS: We studied 182 patients with various degrees of angio

Carotid disease in acute stroke.

BACKGROUND: the Oxfordshire Community Stroke Project (OCSP) devised a simple clinical classification for acute stroke which predicted mortality, functional recovery and patterns of recurrent stroke. We aimed to determine whether this could predict the presence of carotid disease and be used to select which patients with acute stroke should be referred for carotid imaging with a view to subsequent carotid endarterectomy. METHODS: we assessed patients with acute stroke admitted to seven hospitals over a 10-month period. Patients were classified according to the OCSP system and their carotid art

Ischemic optic neuropathy associated with internal carotid artery dissection.

BACKGROUND: Ischemic optic neuropathy (ION) is an infarction of the anterior or, less frequently, posterior part of the optic nerve, usually due to a disease of small arteries supplying the optic nerve. Carotid stenosis or occlusions are rare causes, and among them, carotid dissections have been so far reported in only 5 cases. METHODS: We describe 4 patients with ION (2 anterior and 2 posterior) due to internal carotid artery dissection of a consecutive series of 110 patients with internal carotid artery dissection (3.6%). RESULTS: None of the patients had signs of central retinal artery oc

Percutaneous transluminal angioplasty of intracranial artery stenosis: clinical results in 24 patients.

Circumscribed stenotic lesions of the intracranial arteries can cause cerebral ischemia by hemodynamic and/or thromboembolic mechanisms. Anticoagulation therapy, antiplatelet therapy, and bypass surgery are treatment strategies that have no direct impact on the underlying lesion. This study summarizes the experience of a single institution at which percutaneous transluminal angioplasty (PTA) of intracranial atherosclerotic stenoses was performed. The authors performed a retrospective analysis of 24 consecutive patients. Their medical histories (cardiovascular risk factors, current clinical sig

Ruptured distal anterior choroidal artery aneurysm presenting with casting intraventricular haemorrhage.

This report describes a rare case of a distal anterior choroidal artery aneurysm which developed intraventricular haemorrhage without subarachnoid haemorrhage as shown on computerized tomographic (CT) scan. A 69-year-old hypertensive man suddenly became unconscious. An emergency CT scan showed a severe intraventricular haemorrhage and a small round low-dense lesion within the haematoma at the right trigone. The haematoma with obstructive hydrocephalus made the lateral ventricles larger on the right than on the left. CT scan could not detect any subarachnoid haemorrhage. Right interal carotid a

[Movement and behavioral disorders in anterior cerebral artery territory infarction].

Anterior cerebral artery territory infarction, which affects the medial frontal lobe and anterior part of the corpus callosum, may cause disinhibitory movement and behavioral disorders. The pathophysiology of disinhibitory movement and behavioral disorders was discussed in relation to the role of the frontal lobe in motor control and hemispheric asymmetry of motor functions. Instinctive grasp reaction is the prototype of disjunctive motor disinhibition syndromes, and diagnostic dyspraxia and compulsive manipulation of tools are the most illustrative forms. Diagnostic dyspraxia can be defined a

[Cerebrovascular disorder and the language areas].

Recent progress of imaging techniques has achieved precise descriptions of aphasic syndromes associated with cerebrovascular disorders. Infarction of the left anterior cerebral artery brings about transcortical motor aphasia. Occlusion of each branches of the left middle cerebral artery produces characteristic language symptoms. Recently, cases with infarction of the areas of prefrontal and precentral arteries were reported to manifest fluent aphasia indistinguishable from the classical transcortical sensory aphasia. Rare cases with lesions restricted to the Broca's area (territory of the prec

[Simultaneous supratentorial multiple cerebral infarctions].

We assessed the frequency, clinical and etiological correlates of acute supratentorial multiple infarcts from a hospital-based prospective stroke registry. Among 383 consecutive patients admitted with acute supratentorial infarcts, 352 had single infarcts and 31 (8.1 p. 100) had multiple infarcts on computed tomography. Multiple infarcts were bilateral in 18 patients and unilateral in 13 patients. In only one third of cases, could the diagnosis of multiple infarcts be suspected clinically before neuroimaging. Compared with patients with acute supratentorial single infarcts, the 18 patients wit

[Transdural anastomosis in a juvenile form of moyamoya disease].

Moya-Moya disease has a low prevalence and world-wide distribution, with greater incidence in the first 5 years of life (juvenile form) or in the third decade (adult form). Stenosis of the intracranial portion of the internal carotid artery leads to secondary establishment of intracranial compensating anastamoses at different levels (leptomeninges, basal ganglia and transdural). We present the case of a 7 year old boy who presented clinically with choreiform movements and later had a cerebral infarct. On angio-MR and MR studies we observed the typical smoky image together with development of a

Power-based versus conventional transcranial color-coded duplex sonography in the assessment of the vertebrobasilar-posterior system.

BACKGROUND: Power Doppler (PD) is a new ultrasonic technique that allows improved visualization of vascular structures. The aim of our study was to compare the ability of power-based transcranial color-coded duplex sonography (p-TCCS) to conventional transcranial color-coded duplex sonography (TCCS) and contrast-enhanced transcranial color-coded duplex sonography (CE-TCCS) in the visualization of the vertebrobasilar system. METHODS: In 41 patients without cerebrovascular diseases, we evaluated and compared identification rates of major vessels of the vertebrobasilar system and branches using
