Imaging Review and Clinical Manifestations of Lacunar Stroke Syndromes.

Lacunar strokes are small subcortical infarcts that result from the occlusion of small perforating arteries, and they comprise about 25% of all strokes. Unlike other types of infarcts, lacunar strokes are primarily due to cerebral small vessel disease. Detection of acute lacunar infarcts may be challenging due to their small size, but it can be improved with clinical-radiologic correlation. Numerous mimics should also be kept in mind in the differential diagnosis. While lacunar infarcts are more often asymptomatic, they may manifest with distinct clinical syndromes. Five classic lacunar syndro

Pure Sensory Lacunar Infarction in the Thalamus Presented as Bilateral Hypogeusia: A Case Report.

PURPOSE: While the gustatory pathway of animals has been well-researched, that of humans is still a mystery. Several theories have been established, and some earlier reports hypothesized the relation to laterality. However, some cases could not be fully explained by the laterality theory (1). To clarify the gustatory pathway, we reported a case with bilateral hypogeusia after right thalamic infarction. CASE: This 55-year-old, right-handed man suffered from sudden decreased sensitivity of taste. He was unable to differentiate sweetness and saltiness at bilateral anterior parts of tongue. Addit

Three-year mortality after acute myocardial infarction in patients with different diabetic status.

INTRODUCTION: The prevalence of diabetes mellitus (DM) in patients with myocardial infarction (MI) is estimated at about 25%. Short- and long-term prognosis is worse in patients with DM and hyperglycemia compared with nondiabetics. OBJECTIVES: Our aim was to analyze the impact of DM and transient hyperglycemia on in-hospital complications and long-term outcomes in patients with MI. PATIENTS AND METHODS: We evaluated a prospective cohort of 58 394 patients with MI in the Polish Registry of Acute Coronary Syndromes (PL-ACS) in terms of in-hospital complications and 30-day, 12-month, and 36-mon

Focal Hypoperfusion in Acute Ischemic Stroke Perfusion CT: Clinical and Radiologic Predictors and Accuracy for Infarct Prediction.

BACKGROUND AND PURPOSE: Perfusion CT may improve the diagnostic performance of noncontrast CT in acute ischemic stroke. We assessed predictors of focal hypoperfusion in acute ischemic stroke and perfusion CT performance in predicting infarction on follow-up imaging. MATERIALS AND METHODS: Patients from the Acute STroke Registry and Analysis of Lausanne data base with acute ischemic stroke and perfusion CT were included. Clinical and radiologic data were collected. We identified predictors of focal hypoperfusion using multivariate analyses. RESULTS: From the 2216 patients with perfusion CT, 3

Unilateral Perioral, Thumb, and Thenar Numbness Secondary to Acute Thalamic Infarct.

Lacunar infarcts are small, deep infarcts that occur in subcortical regions of the brain and can result in pure sensory stroke syndromes, ataxia, and dysarthria. The most common predisposing etiology is small-vessel lipohyalinosis or microatheroma formation usually secondary to diabetes mellitus or systemic hypertension. We report a patient who presented with cheiro-oral syndrome (COS) with left sided perioral, thumb, and thenar numbness after an acute lacunar infarct of the right thalamus.

A case report: Numb Chin Syndrome due to thalamic infarction: a rare case.

BACKGROUND: Numb Chin Syndrome (NCS), which is also characterized as sensory neuropathy of the mental nerve, describes a mostly unilateral numbness of the chin and lower lip. Benign and malignant diseases are known to cause this circumscribed symptom, which can easily be overlooked or misdiagnosed. In this article we present the very rare case of a clinical NCS caused by thalamic lacunar infarction. As a pure sensory stroke it is a rare variant of the Cheiro-Oral Syndrome (COS). CASE PRESENTATION: A 63-year-old male patient received an emergency referral to our department after the patient ha

Remote ischemic postconditioning alleviates myocardial ischemia/reperfusion injury by up-regulating ALDH2.

A total of 17 patients with lacunar syndromes due to intracerebral hemorrhage or hemorrhagic lacunar stroke (pure motor hemiparesis 9, sensorimotor stroke 5, pure sensory stroke 3) are reported. Data from these patients were obtained from consecutive stroke patients included in the prospective Hospital Sagrat Cor-Aliança Stroke Registry. Hemorrhagic lacunar stroke accounted for 3.8% of all cases of lacunar syndrome (n = 439) and 7.4% of all cases of intracerebral hemorrhage (n = 229) entered in the database. Demographic, anamnestic, clinical and neuroimaging variables in patients with hemorrha

Diagnostic accuracy and risk factors of the different lacunar syndromes.

BACKGROUND: The lacunar syndrome is characterized by pure motor, pure sensory, or sensorimotor hemisymptoms without cortical deficits. It may be less predictable for a lacunar infarct (LI) than previously believed. The aims of the present study were to evaluate the diagnostic accuracy of the different lacunar syndromes and investigate factors associated with acute LI on diffusion-weighted imaging (DWI). METHODS: Consecutive patients presenting with an acute lacunar syndrome who were admitted to the stroke unit were enrolled. The patients were examined clinically and underwent magnetic resonan

Cheiro-Oral syndrome secondary to thalamic infarction: a case report and literature review.

INTRODUCTION: Small thalamic infarcts can present with a variety of sensory deficits that can be difficult to diagnose clinically because of their seemingly disconnected manifestations. CASE REPORT: Here we report a case of a 55-year-old man presenting with the clinical findings of Cheiro-Oral syndrome, a pure sensory thalamic lacunar syndrome that clinically presents with contralateral sensory deficits of the fingers and hemimouth. CONCLUSIONS: This report highlights both common and unusual clinical presentations of isolated thalamic infarctions, correlating clinical symptomatology with ana

Pure sensory syndromes and post-stroke pain secondary to bilateral thalamic lacunar infarcts: a case report.

INTRODUCTION: Patients often complain about sensory symptoms that appear to the doctor as harmless, and reassurances are often given. Sensory strokes may easily be ignored. CASE PRESENTATION: A 48-year-old Caucasian woman with insulin-dependent diabetes and hyperlipidemia experienced symptoms that progressed within hours to a complete left-sided hemisensory syndrome. This was caused by a lacunar infarct in the ventral posterior tier nuclei of the right thalamus. A few days later she gradually developed an almost identical, but incomplete hemisensory syndrome on the opposite side caused by a c

Pure hemi-chorea resulting from an acute phase of contralateral thalamic lacunar infarction: a case report.

BACKGROUND: Thalamic lesions give rise to a variety of clinical syndromes such as pure sensory stroke, ataxic hemiparesis, and rarely involuntary movements including chorea. Generally and classically, lacunar infarction in the subthalamic nucleus has been regarded as the lesion mainly responsible for hemi-chorea and hemi-ballismus, on the basis of previous anatomical studies. CASE PRESENTATION: This report describes the case of an 81-year-old man who developed sudden-onset pure hemi-chorea in the right limbs resulting from an acute phase of left thalamic lacunar infarction detected on a diffu

Hemiparesthesias in lacunar pontine ischemic stroke.

Isolated paresthesia, or paresthesia not accompanied by sensory and/or motor deficits, is an extremely rare manifestation of a cerebrovascular accident. Lacunar pure sensory stroke (PSS) confined to thalamus is characterized by persistent or transient numbness, tingling, pain, burning, or another unpleasant sensation on one side of the body. However, in this condition a sensory loss to all primary modalities in the contralateral face and body is very often encountered. Also previous reported cases of PSS due to lacunar stroke in regions other than thalamus are characterized by the presence of

Clinical predictors of lacunar syndrome not due to lacunar infarction.

BACKGROUND: Lacunar syndrome not due to lacunar infarct is poorly characterised. This single centre, retrospective study was conducted to describe the clinical characteristics of patients with lacunar syndrome not due to lacunar infarct and to identify clinical predictors of this variant of lacunar stroke. METHODS: A total of 146 patients with lacunar syndrome not due to lacunar infarction were included in the "Sagrat Cor Hospital of Barcelona Stroke Registry" during a period of 19 years (1986-2004). Data from stroke patients are entered in the stroke registry following a standardized protoco

Lacunar stroke.

Lacunar infarcts or small subcortical infarcts result from occlusion of a single penetrating artery and account for one quarter of cerebral infarctions. Patients with a lacunar infarct usually present with a classical lacunar syndrome (pure motor hemiparesis, pure sensory syndrome, sensorimotor stroke, ataxic hemiparesis or dysarthria-clumsy hand) and, less frequently, an atypical lacunar syndrome. Hypertension and diabetes mellitus are major risk factors for lacunar stroke. Lacunar infarcts show a paradoxical clinical course with a favorable prognosis in the short term, characterized by a low

Selective activation of human finger muscles after stroke or amputation.

Individuated finger movements of the human hand require selective activation of particular sets of muscles. Such selective activation is controlled primarily by the motor cortex via the corticospinal tract. Is this selectivity therefore lost when lesions damage the corticospinal tract? Or when the motor cortex reorganizes after amputation? We studied finger movements in normal human subjects and in patients who had recovered substantially from pure motor hemiparesis caused by lacunar strokes, which damage the corticospinal tract without affecting other pathways. Even after substantial recovery

Subcortical ischemic cerebrovascular dementia.

It has become increasingly apparent, especially with the advent of MRI brain scanning, that a large number of patients develop signal intensity changes in the subcortical white matter and periventricular region as they age. This appears to be accelerated by risk factors for small vessel cerebrovascular disease such as hypertension, smoking, diabetes mellitus and hyperlipidemia. The major question becomes when such changes become clinically significant. It is obvious that subcortical lacunar-type infarction can be identified by the clinical presentation. For example, typical examples of so-call
