Effects of CPAP withdrawal on sex hormone levels in patients with obstructive sleep apnoea: data from two randomised, controlled trials.

Sexual dysfunction (SD) is common in patients with obstructive sleep apnoea (OSA). Although continuous positive airway pressure (CPAP) therapy improves SD, it does not consistently modify testosterone or estradiol levels, suggesting involvement of other hormonal pathways. We analysed sex hormone levels in 70 patients with OSA before and after two weeks of CPAP withdrawal. No statistically significant treatment effects were observed for testosterone, androstenedione, gestagens, dehydroepiandrosterone sulfate, luteinizing hormone or follicle-stimulating hormone, whereas dehydroepiandrosterone (D

Surgical management of periorbital non-traumatic subcutaneous emphysema during continuous positive airway pressure.

A 47-year-old male requested treatment for recurrent, rapid left upper eyelid swelling due to subcutaneous emphysema (SE) with use of continuous positive airway pressure (CPAP) for treatment of obstructive sleep apnoea. There was no previous history of facial trauma or surgery. Computerised tomography revealed pneumatisation of the lacrimal sac fossa and nasolacrimal duct with a prominent agger nasi air cell medial to the lacrimal sac extending down the nasolacrimal duct. He underwent endoscopic surgery to reduce the pneumatisation superomedial to the lacrimal sac. The subcutaneous emphysema s

Altered Neurovascular Coupling in Obstructive Sleep Apnoea: Assessment via CBF-ALFF Spatial Coupling.

OBJECTIVES: To investigate alterations in CBF-ALFF spatial coupling in obstructive sleep apnoea (OSA). METHODS: Thirty-two patients with OSA and 32 healthy controls(HCs) matched for sex, age, and education background were recruited for this work. All participants received arterial spin labelling (ASL) and resting-state functional MRI (rs-fMRI) examinations. Cerebral blood flow (CBF) and amplitude of low-frequency fluctuation (ALFF) maps were derived from ASL and fMRI data, respectively. Between-group comparisons were performed to identify differences in ALFF, CBF, and CBF-ALFF coupling metric

Obstructive sleep apnoea severity and insulin resistance in children with Prader-Willi Syndrome: the SPISE index as a candidate metabolic marker.

UNLABELLED: We compared two surrogate markers of insulin sensitivity - the single-point insulin sensitivity estimator (SPISE) and HOMA-IR - and evaluated their associations with OSA severity in children with genetically confirmed PWS and a symptomatic non-syndromic referral group (NSR) with available polysomnography (PSG) and fasting biochemical data. In this single-centre retrospective cross-sectional study, OSA severity was indexed by the obstructive apnoea-hypopnoea index (oAHI), central AHI (cAHI) and oxygen desaturation index (ODI), and caregiver-reported symptoms by the PSQ-SRBD scale. ERS/ESRS clinical practice guideline for continuous positive airway pressure for the treatment of adult obstructive sleep apnoea.

BACKGROUND: Continuous positive airway pressure (CPAP) is the primary treatment for obstructive sleep apnoea (OSA), although its effectiveness may vary across OSA subtypes. This joint European Respiratory Society (ERS)/European Sleep Research Society (ESRS) clinical practice guideline evaluates CPAP's impact on patient-centred and prognostically relevant outcomes, comparing it to no or sham treatment in different OSA subgroups. METHODS: The joint ERS/ESRS Task Force consisted of experts in pulmonology, cardiology and respiratory sleep medicine, patient representatives and methodologists. Ques

Event-Weighted Desaturation Burden and Cardiovascular Mortality in Sleep Apnoea: Findings from a Large Community-Based Prospective Cohort.

BACKGROUND AND AIMS: Obstructive sleep apnoea's prognostic value is limited by reliance on the apnoea-hypopnoea index (AHI), which fails to capture physiological heterogeneity of respiratory events. We developed a composite metric, event-weighted desaturation burden (EWDB), integrating weighted respiratory event load, regularity of consecutive inter-event intervals, and cumulative desaturation area. We evaluated its association with cardiovascular mortality and its incremental prognostic value compared with AHI and established hypoxic metrics. METHODS: We analysed 4,904 adults from the Sleep

Obstructive sleep apnoea and Alzheimer's disease pathology: a review of plausible pathological mechanisms and potential modifying impact of hypertension.

Alzheimer's Disease (AD) is estimated to account for up to 75% of all dementia cases, of which there is a global prevalence of 50 million. There are no curative treatments available for AD dementia, therefore, early identification of risk factors is paramount. Up to 40% of all dementia cases may be attributable to modifiable risk factors. One such risk factor is poor sleep. Obstructive sleep apnoea (OSA) is a common sleep disorder characterised by repeated upper airway collapse, contributing to intermittent hypoxaemia and sleep fragmentation. Hypertension is frequently comorbid with OSA and is

Adults with congenital heart disease: the growing impact of acquired cardiovascular risk factors on late mortality.

BACKGROUND AND AIMS: With improved survival, adults with congenital heart disease (ACHD) increasingly face acquired cardiovascular risk factors; however, their impact on late mortality remains incompletely defined. This study aimed to evaluate the contribution of acquired cardiovascular risk factors to mortality in ACHD. METHODS: A total of 9826 ACHD patients (mean age 39.0 ± 16.4 years; 50.3% male) who attended our tertiary centre between 2000 and 2024 were retrospectively analysed. Clinical and demographic data were obtained from a prospectively maintained database. Cox proportional hazards

Sleep quality, disturbance and the prevalence of obstructive sleep apnoea in chronic spontaneous urticaria and associated risk factors.

BACKGROUND: Chronic spontaneous urticaria (CSU) is a common dermatological disorder impairing quality of life. The impact of CSU on sleep quality and obstructive sleep apnoea remains unclear. OBJECTIVE: To estimate the prevalence of sleep disturbance and obstructive sleep apnoea (OSA) in CSU patients and identify associated risk factors. METHODS: We conducted an analytical cross-sectional study involving adult CSU patients (n = 327) per eligibility criteria. We used the Pittsburgh Sleep Quality Index (PSQI) and STOP-Bang questionnaires to evaluate sleep disturbance and risk for OSA, respecti

Lesser Occipital Neuralgia Secondary to Continuous Positive Airway Pressure (CPAP) Mask Headgear: A Case Report

Occipital neuralgia is a well-characterised cause of unilateral head and neck pain arising from irritation of the greater or lesser occipital nerve (LON). External headwear (helmets, goggles, and tight headbands) is a recognised trigger of pericranial pain, formally classified by the International Classification of Headache Disorders (ICHD-3) as external-compression or external-traction headache. To our knowledge, lesser occipital neuralgia specifically attributed to continuous positive airway pressure (CPAP) mask headgear has not been previously reported. We report an 81-year-old man referred

Coblation intracapsular tonsillectomy outcomes in paediatric obstructive sleep disordered breathing: A Singapore study.

INTRODUCTION: Coblation intracapsular tonsillectomy (ICT) may offer a more favourable post-operative morbidity profile compared with extracapsular tonsillectomy for paediatric obstructive sleep-disordered breathing (oSDB). Data from the Singapore paediatric population remain limited. This study examines the outcomes of coblation ICT in children with oSDB at a tertiary centre in Singapore. METHOD: This was a single-centre prospective cohort study of 102 consecutive children aged 21 months to 15 years undergoing coblation ICT for oSDB (April-October 2023). Concurrent adenoidectomy was performed

Childhood Predictors of Disconnection for Young Men and Women: Evidence from the Future of Families and Child Wellbeing Study.

Although most young adults are working and/or in school, a significant share is disconnected - that is, neither employed nor enrolled in education or training. As of 2023, roughly 10% of America's young adults were disconnected, with wide-ranging implications for individuals, families, and society. A broad body of research has begun to identify factors associated with disconnection, yet much of this evidence relies on foreign samples, older U.S. cohorts, or data beginning in adolescence - limiting its relevance to contemporary young adults and its ability to detect factors that emerge early in

Reversible bilateral cerebellar ataxia resulting from hypercapnic encephalopathy in an undiagnosed patient with obstructive sleep apnoea.

A man in his 40s with morbid obesity and type 2 diabetes mellitus presented with early-morning drowsiness and respiratory distress after several days of nocturnal dyspnoea and unsupervised loop diuretic use. Initial assessment showed hypoglycaemia, acute-on-chronic hypercapnic respiratory acidosis with metabolic alkalosis and obstructive ventilatory mechanics, requiring intubation. Within 24 hours of extubation, he re-presented with recurrent drowsiness and striking bilateral cerebellar ataxia despite normal neuroimaging. Repeat arterial blood gases again demonstrated acute-on-chronic hypercap
