Deceptive Mimics of Trigeminal Schwannoma: Be Careful with Primary Radiosurgery.

Trigeminal schwannomas are rare lesions centered on the trigeminal ganglion at Meckel?s cave. The complexity and morbidity of surgery for these lesions have allowed stereotactic radiosurgery (SRS) to emerge as a safe and viable option for treatment. Various other lesions at this location must alert one to consider an alternative diagnosis before upfront SRS without histopathological correlation. We present three patients with trigeminal neuropathy with imaging suggesting trigeminal schwannoma. Primary clinicians recommended primary radiosurgery to these patients based on radiological diagnosis

Are dentists aware of post-traumatic trigeminal neuropathic pain? A web-based epidemiological survey.

BACKGROUND: Post-traumatic trigeminal neuropathic pain represents neuropathic pain in the distribution of the trigeminal nerve caused by trauma to the trigeminal nerve. Dental traumatic interventions, such as root-canal therapy and extractions, are reported to precede, in some cases, the onset of the disease. The current study aims to investigate how much dentists are trained to recognize, treat or properly address patients suffering from Post-traumatic trigeminal neuropathic pain. METHODS: Data were collected from a large sample of Italian dentists in 2021. The setting of this study relates

Transcutaneous occipital nerve stimulation alleviated migraine related pain by regulating synaptic plasticity and CGRP expression in the periaqueductal gray of male rats.

BACKGROUND: Clinical observations have shown that transcutaneous occipital nerve stimulation (tONS) is effective in treating migraine. However, the underlying mechanisms are poorly understood. This study employs a rodent model to investigate the therapeutic effects of tONS on migraine-related pain and to explore potential mechanisms. METHODS: The SD rats were used to establish the migraine model by repeated epidural infusions of inflammatory soup (IS). Modified bilateral electrodes were attached noninvasively for tONS treatments. Periorbital mechanical thresholds were assessed using von-Frey

Correlation Between Meckel's Cave Size and lohexol Dosage in Percutaneous Microballoon Compression for Trigeminal Neuralgia of Aged Patients.

BACKGROUND: Percutaneous microballoon compression (PMC) is a safe and effective method (1) for treating trigeminal neuralgia (TN). Though many surgeons have carried out this kind of surgery, practitioners who lack sufficient surgical experience may be unaware of this method of improving the clinical effectiveness of PMC for aged patients. OBJECTIVE: To investigate the relationship among the success of the PMC method, the size of a patient's Meckel's cave (MC), and the iohexol dosage in a pear-shaped balloon and form a mathematical model for the effectiveness of the treatment. STUDY DESIGN: R

The Relationship Between Orthodontic Abnormalities and Spinal Deformities: A Review of Literature.

Spinal deformities comprise abnormal curvatures of the spine which impact posture, musculoskeletal balance, as well as biomechanical function. Malocclusion, the improper contact between occlusal surfaces of opposing teeth, affects the occlusal relationship and functionality of the temporomandibular joint (TMJ). While both conditions have been extensively studied independently, their interconnection remains a subject of debate. The aim of this work is to comprehensively review the current literature concerning spinal curvature abnormalities and stomatognathic alterations, concomitantly investig

Various gases for the treatment of neuropathic pain: mechanisms, current status, and future perspectives.

In recent years, medical gas therapy has emerged as a promising approach for treating neuropathic pain. This review article aimed to investigate the therapeutic effects of medical gas therapy on neuropathic pain and its underlying mechanisms, thereby providing a theoretical foundation for clinical practice. A literature search was conducted using the Web of Science Core Collection database. Co-occurrence analysis of keywords revealed that terms including "neuropathic pain," "nitric oxide," "nitric oxide synthase," "pain," and "ozone" frequently appeared. Cluster analysis grouped these keywords

Revisiting Härtel's technique for percutaneous transoval glycerol injection.

PURPOSE: Percutaneous transoval glycerol injection (GI) has been widely used since 1981 in the treatment of patients with trigeminal neuralgia. However, outcomes have been more variable than with other percutaneous treatments. Although most authors state that they use Härtel's technique, the variations are numerous-which may explain procedural problems and most of the poor results. The aim of the present imaging-based study, therefore, was to revisit Härtel's technique and identify optimal landmarks for guiding the needle from the cheek to Meckel's cave. METHODS: Eleven patients referred for

Neuromodulation in Chronic Migraine: Evidence and Recommendations from the GRADE Framework.

Chronic migraine (CM) affects approximately 2% of the general population and is defined by the persistence of migraine symptoms for at least 15 days per month for at least 3 months. CM is often refractory to common drug treatments and is associated with a significant burden in functions of daily life during ictal phases, productivity loss, and direct costs. Modulation of pain is considered pivotal to reduce its impact and to improve the quality of life among patients with CM. In recent years, neuromodulation in CM has received growing attention; however, there remains no consensus regarding th

Endonasal endoscopic surgical approach for treating trigeminal schwannomas: A systematic review and meta-analysis.

INTRODUCTION: Trigeminal schwannomas (TSs) are the second most common type of intracranial schwannoma. Surgical approaches are chosen depending on the type of tumor extension. The middle fossa extradural approach, infratemporal extradural approach, transmaxillary approach, transmandibular approach, and transcervical approach have been used. However, these approaches are associated with a variety of complications including other cranial nerve dysfunction. Recently, with the wide application of endoscopic technology, the endoscopic endonasal approach (EEA) seems to be an alternative skull base s

Case series of CT-fusion and real-time US-guidance in percutaneous RF thermocoagulation for trigeminal rhizotomy.

This case series evaluates the feasibility, safety, and efficacy of real-time computed tomography fusion with ultrasound guidance (CTF-USG) in percutaneous radiofrequency thermocoagulation (RFT) for trigeminal rhizotomy. Traditional imaging modalities such as CT and fluoroscopy lack real-time visualization, increasing the risk of vascular injury and reducing procedural precision. The CTF-USG technique integrates CT imaging with real-time ultrasound via electromagnetic tracking, enabling accurate needle navigation and anatomical confirmation. Six patients with refractory trigeminal neuralgia af

Evaluation of Outgrowth Potential of Rat Pheochromocytoma Cells Supplied with Highly Purified Rapidly Expanding Clones and Potential Application to Trigeminal Nerve Regeneration.

BACKGROUND: Mesenchymal stem/stromal cells (MSCs) are non-hematopoietic, plastic-adherent, and self-renewing cells capable of in vitro trilineage differentiation into fat, bone, and cartilage tissue. Suggestively, MSCs have additional plasticity, as demonstrated by their ability to differentiate in vitro into myocytes, neuron-like cells, and hepatocytes. MSCs are ideal for therapeutic application owing to their numerous advantages; they exhibit limited growth and differentiation abilities, leading to heterogeneous cell populations with inconsistent functions. However, highly purified MSCs, nam

The Nasal-Brain Drug Delivery Route: Mechanisms and Applications to Central Nervous System Diseases.

The blood-brain barrier (BBB) is a highly selective and protective barrier that restricts the entry of most therapeutic agents into the central nervous system (CNS), posing a significant challenge for the treatment of CNS diseases. The nose-to-brain drug delivery (NBDD) route has emerged as a promising strategy to bypass the BBB, offering direct, noninvasive, and efficient transport of drugs to the brain. This review begins with a concise overview of the BBB structure and its biofunctions, followed by an in-depth discussion of the mechanisms underlying the nose-to-brain pathway, including the

Role of CSF flow and meningeal barriers in the development of inflammatory lesions at the CNS-PNS transition zone of cranial nerves in autoimmune demyelinating diseases.

Patients with autoimmune inflammatory demyelinating diseases have been shown to present with trigeminal and cochlear nerve lesions restricted at the root transition zone, which contrasts with the relatively extensive distribution of lesions in optic neuritis. To better understand the mechanism underlying the different distribution pattern for cranial nerve lesions in these autoimmune neuroinflammatory diseases, we focused on the CNS-PNS transition zone (TZ) of the trigeminal and cochlear nerves in a MOG-driven active EAE model. These nerves were found to exhibit unique arrangements of anatomic

Use of low-dose naltrexone in the management of posttraumatic trigeminal neuropathic pain: a retrospective case series.

OBJECTIVE: Dental practitioners provide treatment of anatomical structures innervated by the trigeminal system, such as the teeth and gingiva, which can be subject to injury even following routine and well-performed dental procedures. As a result, the dental clinician is often presented with patients with neuropathic pain or unusual sensory distortions. In addition, the dental clinician treats patients following facial and oral trauma which may result in chronic pain. Therefore, recognition of posttraumatic trigeminal neuropathic pain (PTTNP) and its management must be considered essential for

Recurrence of trigeminal neuralgia after microvascular decompression: a systematic histopathological evaluation of Teflon granuloma.

OBJECTIVE: Teflon granuloma is one of the possible causes of recurrence in patients with trigeminal neuralgia (TN) who undergo successful microvascular decompression (MVD). The incidence of Teflon granuloma has been variable and the pathophysiology and mechanisms for recurrence have not been well defined. The aim of this study was to characterize the histological features of Teflon granulomas and correlate their occurrence with clinical and intraoperative findings. METHODS: Histopathological studies were available for 14 specimens from 13 patients over a 15-year period. Histopathological data

Fatostatin delayed lip sensory recovery after inferior alveolar nerve transection by inhibiting sterol regulatory element-binding protein 1.

Lipid metabolism is essential for nerve repair in damaged nerves. Fatostatin, a selective inhibitor of sterol regulatory element-binding protein 1 (SREBP1), could reduce cholesterol synthesis and disturb lipid homeostasis. However, whether fatostatin would delay lip sensory recovery after inferior alveolar nerve transection remains unclear. In this preliminary study, we investigated the effects of fatostatin on lip sensory recovery in vivo and axon growth in vitro. Fatostatin significantly delayed lip sensory recovery of mice after inferior alveolar nerve transection as evidenced by quantitati

Minimally invasive microvascular decompression for trigeminal neuralgia: results and complication avoidance in a consecutive series of patients.

OBJECTIVE: Microvascular decompression (MVD) represents a milestone for the treatment of trigeminal neuralgia (TN). Nevertheless, several complications still occur and may negatively affect the outcome. The authors recently proposed a minimally invasive technique, including endoscopic assistance in instances of intraoperative hidden corners, with which they were able to achieve good results in terms of pain relief and minimize overall complication rates. The aim of this study was to verify the short- and long-term efficacy of the proposed refinement of the standard MVD technique in terms of pa

How to deal with the superior petrosal vein in microvascular decompression for trigeminal neuralgia?

OBJECTIVE: How to manage the superior petrosal vein (SPV) obstructing the operative field during microvascular decompression (MVD) for trigeminal neuralgia (TN) remains controversial. The authors aimed to evaluate the safety profile of a specific SPV division technique used during MVD for TN. METHODS: This retrospective analysis included patients who underwent first-time MVD for medically refractory TN from 2005 to 2025 at a single center. When the SPV obstructed the operative field, it was coagulated at its main trunk near the entry site into the superior petrosal sinus, maintaining venous c
