Predicting recovery in Bell's palsy: the impact of early rehabilitation and prognostic indicators.

PURPOSE: To evaluate functional outcomes and prognostic factors in Bell's palsy (BP) patients treated with medical therapy and facial neuromuscular retraining (fNMR), with a focus on the timing of rehabilitation. METHODS: Ninety-four patients with FP treated at our center were retrospectively analyzed. Patients were stratified by time to rehabilitation (TTR): Early Rehabilitation (≤ 60 days), who accessed our unit directly, and Late Rehabilitation (> 60 days), who had first received other treatments or rehabilitation not including fNMR elsewhere. Outcomes included House-Brackmann (HB) grade,

Associations of AI-Based Facial Metrics With Patient-Reported Outcomes in Idiopathic Facial Paralysis.

OBJECTIVE: Conventional grading scales and static image assessments may not capture dynamic facial movement in facial paralysis. We developed a video-based, dynamic, artificial intelligence (AI) application, DynaFace, to objectively quantify facial metrics and determine which measures correspond with patient-reported appearance, function, and psychosocial outcomes, providing insight into how objective dynamics relate to subjective patient experience. METHODS: DynaFace automatically extracted dynamic facial metrics (facial asymmetry index [FAI], bilateral palpebral fissure, and oral commissure

Predicting the Prognosis of Bell's Palsy in Infants and Young Children Using the Nerve Excitability Test.

OBJECTIVE: To evaluate whether the nerve excitability test using a 10 mA pulse current (NET10 mA) can serve as a simple and reliable method of predicting the prognosis of Bell's palsy in infants and young children, and to compare its diagnostic value with that of electroneurography (ENoG). METHODS: This retrospective study included 19 pediatric patients aged 0 to 5 years with Bell's palsy who underwent both a NET and ENoG 8 days to 3 weeks after paralysis onset. During the NET, the marginal mandibular branch of the facial nerve was stimulated with a 10-mA pulse current, and any movement in th

Mandibular Dislocation in a Patient Diagnosed with Bell's Palsy: A Case Report and Literature Review.

Mandibular dislocation is a musculoskeletal disorder often associated with trauma and anatomical predispositions, which can result in severe pain and difficulty moving the mandible. Some conditions, such as Bell's palsy, can mimic signs and symptoms of mandibular dislocation, confusing some professionals during diagnosis. For this reason, a detailed anamnesis and clinical examination, combined with complementary tests such as computed tomography (CT) and magnetic resonance imaging (MRI), are necessary for a reliable diagnosis. The aim of this case report was to explore the management of mandib

Review of Treatment Options for Facial Synkinesis: Working Toward a Standardized Algorithm.

BACKGROUND: Facial synkinesis describes a sequela of transient facial nerve injury that can occur following viral reactivation syndromes such as Bell's palsy or following traumatic facial nerve injury. Facial synkinesis is frequently characterized by muscle hypertonicity, involuntary co-contraction of facial muscles, and spasm and can have severe functional and esthetic consequences. Although this condition is not curable, several treatment options exist; however, there is currently a lack of consensus regarding the appropriate timeline and sequence of synkinesis treatment strategies. SUMMARY

Time-Continuous Prognostic Mapping of Bell's Palsy Using Yanagihara Scores and Electroneurography.

OBJECTIVE: Bell's palsy impairs social functioning. Conventional prognostic assessment relies on clinical grading and electroneurography (ENoG), but predictive accuracy remains limited. This study introduces a time-continuous probabilistic approach that integrates longitudinal Yanagihara scores and ENoG to enable dynamic, individualized prognostic estimation. METHODS: We conducted a retrospective longitudinal study of patients with Bell's palsy treated between April 2013 and March 2024. Eligible patients were aged ≥ 15 years, hospitalized within 1 week of onset, and received high-dose cortico

Bilateral idiopathic facial nerve palsy.

In this report, we describe a case of idiopathic bilateral facial nerve palsy in a previously healthy male in his 30s, with recent unprotected sexual activity and a possible genetic predisposition as the only identifiable risk factors. The case highlights both the diagnostic complexity of bilateral facial palsy and the importance of methodical exclusion of systemic causes and early initiation of corticosteroid therapy, which led to complete recovery.

Association between dietary inflammatory index and Bell's palsy: a retrospective case-control study.

OBJECTIVES: Bell's palsy (BP) is an acute facial nerve disorder characterized by rapid-onset unilateral paralysis, in which inflammatory and immune-mediated processes are implicated. The Dietary Inflammatory Index (DII) quantifies the inflammatory potential of the diet and reflects systemic inflammatory burden. However, its association with BP has not been previously investigated. METHODS: This retrospective case-control study included 30 patients with newly diagnosed BP and 30 age- and sex-matched controls without a history of facial paralysis. Dietary intake was assessed using a validated f

Diagnostic errors in peripheral facial palsy: A systematic review and meta-analysis.

This systematic review and meta-analysis was conducted in accordance with PRISMA guidelines to explore diagnostic errors in peripheral facial palsy in adults and their impact on medical and paramedical care. Four databases were used to collect 303 articles, 23 of which, published between 2015 and 2024, were analyzed using the CARE grid for case studies and the JBI grid for case series. Fifty cases of diagnostic error were identified. A tumor was found to be the cause in 56% of cases that were initially identified as Bell's palsy. The results of this meta-analysis show that symptoms that come o

Preoperative Neutrophil-to-Lymphocyte Ratio is Associated with Early Postoperative Pain and Opioid Consumption after Total Knee Arthroplasty: A Retrospective Study.

BACKGROUND: Improving quality and effectiveness of health care is one of the priorities of health policies. Hospital or physician volume represents a measurable variable with a relevant impact on effectiveness of health care. A recent Italian law, the "spending review", calls for the definition of "qualitative, structural, technological and quantitative standards of hospital care". There is a need for an accurate evaluation of the available scientific evidence in order to identify these standards, including the volume of care above or below which the public and private hospitals may be accredi
