INTRODUCTION: Refractory Peripheral Facial Palsy (RPFP) is defined as peripheral facial paralysis (PFP) that fails to achieve significant clinical recovery despite conventional therapeutic interventions. Due to severe facial nerve injury and a prolonged recovery course, RPFP often results in lagophthalmos, which in turn predisposes to ocular surface diseases (OSDs), most commonly exposure keratitis and dry eye disease. Currently, symptomatic and supportive therapies such as sodium hyaluronate eye drops are standard for managing OSDs secondary to RPFP, yet no other effective treatments have bee

History A 76-year-old man presented to the emergency department with a 1-month history of worsening binocular diplopia on left horizontal gaze, along with 1 week of progressive dysarthria and dizziness. Additionally, he had a chronic history of right-sided cervicobrachialgia related to degenerative disk disease, which had been improving with oral analgesia. The patient's past medical history included hypertension, dyslipidemia, insomnia, and hypothyroidism. He also had postpolio syndrome affecting the right lower limb. He reported no fever, chills, cough, shortness of breath, abdominal pain, o

BACKGROUND: The optimal surgical management of high-grade parotid carcinoma confined to the superficial lobe remains controversial, with limited evidence comparing total and superficial parotidectomy in this setting.
OBJECTIVE: To compare oncologic outcomes and complications between total parotidectomy with postoperative radiotherapy (TP + PORT) and superficial parotidectomy with postoperative radiotherapy (SP + PORT).
STUDY DESIGN: Multicentre retrospective cohort study.
METHODS: Adult patients (> 18 years) with high-grade parotid carcinoma confined to the superficial lobe treated between

Ramsay Hunt Syndrome (RHS) arises from reactivation of varicella-zoster virus (VZV) in the geniculate ganglion, clinically manifesting as facial palsy and an auricular rash. Involvement of cranial nerves (CN) beyond the facial nerve (CN VII) occurs in up to half of cases, complicating the clinical presentation. Current knowledge regarding polycranial involvement in RHS remains fragmented, largely drawn from isolated case reports and small series. This review synthesizes available evidence on the epidemiology, clinical spectrum, differential diagnosis, and mechanisms of cranial nerve involvemen

BACKGROUND: Despite reporting low levels of well-being, anorexia nervosa patients express temperament traits (e.g., extraversion and persistence) necessary for high levels of life satisfaction. Nevertheless, among individuals without eating disorders, a balanced organization of the flow of time, influences life satisfaction beyond temperamental dispositions. A balanced time perspective is defined as: high past positive, low past negative, high present hedonistic, low present fatalistic, and high future. We investigated differences in time perspective dimensions, personality traits, and life sa

Creutzfeldt-Jakob disease (CJD) is a rare, rapidly progressive, and invariably fatal prion disease whose early clinical features often overlap with more common neurological conditions, leading to diagnostic delay. The following case describes a 53-year-old man with a history of hyperlipidemia and hypothyroidism who presented with a two-day history of right-sided facial and extremity numbness, right lower extremity weakness, and myoclonus of the right upper extremity. Initial stroke workup, including computed tomography (CT) and magnetic resonance imaging (MRI) of the brain, was unremarkable. C

OBJECTIVE: To explore pathological findings on magnetic resonance imaging (MRI) and their diagnostic implications in early-stage neuroborreliosis (NB).
METHOD: Adult patients from the Danish neuroinfections cohort (DASGIB, 2015-2019) with confirmed NB, symptom duration < 6 months, and MRI performed within 14 days of diagnosis were included. MRIs were retrospectively reinterpreted by an unblinded neuroradiologist.
RESULTS: In 116 patients, 123 MRIs were performed (99 brain, 44 spine, 46 with contrast). White matter lesions (WML) were common, but non-specific and associated with increasing age

BACKGROUND: Peripheral neuropathy is a common complication of Lyme disease. Cranial mononeuropathy, particularly that affecting the facial nerve, can be a presenting symptom, and at times, it can be associated with polyradiculopathies or plexopathies. However, isolated femoral neuropathy has not yet been reported in Lyme disease; therefore, we felt the need to present this case.
CASE PRESENTATION: Laboratory investigations were performed on a 67-year-old man living in a region at high risk for Lyme disease after he developed erythema migrans on his chest, accompanied by the swelling of his le

BACKGROUND: Bilateral facial nerve palsy (BFNP) is a rare clinical entity in children and is more often associated with systemic or infectious diseases than unilateral facial palsy. Epstein-Barr virus (EBV) infection is an uncommon but recognized cause of facial nerve palsy and may present with bilateral involvement.
CASE PRESENTATION: We report the case of a 3-year-old boy who presented with progressive bilateral facial weakness following a febrile illness with pharyngitis and cervical lymphadenopathy. Neurological examination revealed complete bilateral facial paralysis (House-Brackmann gra

INTRODUCTION: Lyme neuroborreliosis (LNB) is a recognized cause of peripheral facial paralysis (PFP) in children, particularly in endemic regions. Differentiating LNB from idiopathic facial paralysis at initial presentation remains challenging, as early biological confirmation may be delayed. This study aimed to determine the proportion of LNB among children presenting with facial paralysis at two tertiary pediatric centers in Eastern France, to identify clinical, epidemiological, and biological characteristics associated with LNB, and to evaluate local diagnostic and therapeutic practices in

Lyme disease is a common tick-borne infection in the United States and Europe that may involve the nervous system during the disseminated stage. Guillain-Barré syndrome (GBS) is an acute immune-mediated polyneuropathy usually triggered by infection; however, its association with Borrelia burgdorferi is uncommon and can pose diagnostic challenges. We report the case of a previously healthy 61-year-old female patient who presented with progressive ascending weakness and areflexia suggestive of GBS. During hospitalisation, she developed bilateral facial nerve palsy, prompting further evaluation.

Overlapping clinical features between Lyme disease and Guillain-Barré Syndrome (GBS) can complicate diagnosis, and a definitive causal relationship has not been established. A 58-year-old woman experiencing unsheltered homelessness was referred to the emergency department by her street medicine physician with progressive symmetric weakness, unilateral facial nerve palsy, dysphagia, and dyspnea following tick bites obtained at her encampment in the woods. Workup showed elevated cerebrospinal fluid (CSF) protein with lymphocytic pleocytosis and positive serum Lyme serology tests, consistent with

Lyme disease is a spirochete infection acquired after inoculation of Borrelia burgdorferi with the bite of an Ixodes tick, commonly encountered in the Northeastern United States, usually characterized by erythema migrans, arthritis, and meningitis. Bannwarth syndrome is an uncommon manifestation of Lyme neuroborreliosis (LNB) in the United States. It can present with various clinical features, which often make the diagnostic process challenging. In our case, the subacute onset of flaccid leg weakness with diminished deep tendon reflexes and facial weakness mimicked Guillain-Barré syndrome (GBS

Acute isolated peripheral facial nerve palsy is mainly idiopathic. In a minority of cases, however, it may result from other disorders that complicate the diagnostic process. Atypical clinical features, temporal evolution, and associated signs or symptoms are crucial in guiding diagnostic suspicion. In this case, we discuss a 57-year-old woman with acute unilateral facial weakness. The clinical course, together with the combination of imaging and laboratory studies, ultimately led to the final diagnosis of neurosiphilis. In the antibiotic era, the number of patients with the classic form of ne

Simultaneous diagnosis of myasthenia gravis (MG) and early asymptomatic neurosyphilis is extremely rare. There have only been two cases described in the literature. To the best of our knowledge, this was the first case of MG resolving with concurrent treatment of neurosyphilis. Our patient was a 67-year-old male who presented to the hospital with a diffuse rash, bilateral ptosis, bilateral facial weakness, and bulbar symptoms (dysarthria, difficulty with mastication, and difficulty drinking fluids). After testing positive for syphilis in serum and cerebrospinal fluid (CSF), the patient was di

Vernal keratoconjunctivitis (VKC) is a chronic and potentially sight-threatening disease. Topical corticosteroids (Cs) seem to be the only effective treatment for this condition, although severe side effects may occur owing to their prolonged use. More recently, cyclosporine (Cyc) eye drops have been reported as a valid alternative, but so far such treatment has only been successfully experimented for a short time and in small numbers of patients. The aim of our study is to evaluate the long term safety and efficacy of topical cyclosporine eye drops in children suffering from VKC. Over a perio

BACKGROUND: Cranial nerve neuropathies represent a rare manifestation of cat scratch disease (CSD). Only a few case reports have been published, and the full clinical spectrum remains poorly characterized. We aimed to describe the clinical presentation, diagnostic approach, and prognosis of cat scratch disease (CSD)-associated cranial nerve neuropathies, a manifestation that is poorly characterized. METHODS: Using data from a national CSD surveillance study, we identified patients with CSD-associated cranial neuropathies confirmed by serology and/or PCR for Bartonella henselae. Clinical, epide
