We report a rare case of oral AA amyloidosis coexisting with granulomatous cheilitis. A 48-year-old woman presented with extensive, rubbery, nodular masses on the buccal mucosa accompanied by fluctuating granulomatous cheilitis. Laboratory findings showed elevated levels of C-reactive protein (CRP), rheumatoid factor (RF), serum free kappa light chain ( κ ), serum free lambda light chain ( λ ), and serum amyloid A (SAA). In the pathological examination, Congo-red staining showed a characteristic apple-green birefringence under polarized light. The final diagnosis is AA amyloidosis with granulo

PURPOSE: Trigeminal neuralgia (TN), a debilitating condition, is commonly treated with microvascular decompression (MVD). However, effective intraoperative neurophysiological monitoring remains challenging. This study introduces a novel electrophysiological technique using sensory-masseter response (SMR) to monitor trigeminal nerve compression during MVD.
METHODS: A total of 34 patients with TN underwent MVD. A concentric neurostimulator was employed to systematically deliver microcurrent stimulation to various intracranial segments of the trigeminal sensory root. We specifically probed the s

BACKGROUND: The trigeminal ganglion (TG) is a central hub for craniofacial injurious messaging, and its abnormal function is closely related to the pathogenesis of trigeminal neuralgia (TN). Bone morphogenetic protein 7 (BMP7), a pleiotropic cytokine with both neuroprotective and anti-inflammatory effects, has been shown to have therapeutic potential for neuropathic pain (NP) and neurodegenerative diseases. However, it remains to be elucidated whether BMP7 is involved in the pathological process of TN through the regulation of TG.
OBJECTIVE: This study aimed to investigate whether BMP7 allevi

Chronic neuropathic pain is increasingly recognized to impair cognitive function, yet its underlying mechanisms remain unclear. This study tested whether trigeminal neuropathic pain (TNP) modeled by distal infraorbital nerve chronic constriction injury (dION-CCI) provokes hippocampal myelin damage associated with cognitive dysfunction, and whether atorvastatin attenuates mechanical hypersensitivity and cognitive deficits. We also explored whether these effects relate to attenuation of the CD95/CD95L (Fas/Fas ligand)-NF-κB-interleukin-1β (IL-1β) axis and preservation of myelin integrity. Behavi

Teflon felt (TFF) is widely adopted for microvascular decompression surgeries (MVDS) for hyperactivity syndrome involving the trigeminal neuralgia (TGN) and hemifacial spasm (HFS) while bovine pericardial patch (BPP) used for dural graft and not MVDS. We speculated that BPP roll around the nerve like an eggroll and fixed with a closing suture may solve the complications associated with TFF dependent MVDS and reduce hospital cost. We conducted a comparative retrospective study on all MVDS for TGN and HFS performed at the department of Neurosurgery, West China Hospital from January 1, 2015, to J

Trigeminal neuralgia (TN) is a debilitating neuropathic pain disorder with profound impacts on quality of life. Although alterations in circulating inflammatory proteins have been reported, observational findings remain inconsistent and are vulnerable to confounding and reverse causation. Whether these immune changes are causal or merely downstream consequences of TN remains uncertain, underscoring the need for robust causal inference methods. We conducted a bidirectional 2-sample Mendelian randomization (MR) analysis to investigate the causal relationships between circulating inflammatory pro

Trigeminal neuralgia is a debilitating neuropathic pain disorder characterized by facial hypersensitivity, yet its underlying molecular mechanisms remain incompletely understood. Using a mouse model of partial infraorbital nerve transection (pT-ION), we investigated transcriptomic alterations in the trigeminal ganglion (TG) to identify molecular contributors to orofacial pain. Microarray analysis identified 200 differentially expressed genes, with functional enrichment highlighting immune-related processes, calcium signaling, and lysosomal pathways. Among these, Calb2 (calretinin) emerged as a

BACKGROUND AND OBJECTIVES: Although microvascular decompression (MVD) is the most effective long-term treatment for trigeminal neuralgia (TN), its risks make some neurosurgeons favor ablative procedures in elderly patients, despite their limited durability and higher risk of sensory deficits, sparking debate over the optimal approach. This retrospective analysis aims to evaluate the safety and efficacy of MVD in octogenarian patients with TN compared with younger patients.
METHODS: In this matched cohort analysis, a total of 280 patients were initially included, with 33 in the octogenarian gr
