Rationale: Both the site of upper airway collapse during drug-induced sleep endoscopy (DISE) and pathophysiological endotypic traits are associated with non-continuous positive airway pressure treatment outcomes for obstructive sleep apnea (OSA). Reduced hypoglossal nerve stimulation treatment efficacy has been associated with complete concentric collapse at the level of the palate (CCCp), lateral wall collapse, lower arousal threshold, and poor dilator muscle compensation. However, these predictors may not be independent. Objective: Assess the relationship between the site of upper airway col

BACKGROUND Obstructive sleep apnea (OSA) is a highly prevalent condition associated with several cardiovascular diseases, and positive airway pressure (PAP) therapy is considered the standard treatment. In patients with OSA and nocturnal bradyarrhythmias, treatment with PAP therapy improves nocturnal bradyarrhythmias and is the basis for recommending OSA screening in patients with asymptomatic nocturnal bradyarrhythmia. As many patients do not tolerate PAP therapy, alternative treatment options like hypoglossal nerve stimulation (HNS) have been developed. Less is known regarding whether HNS th

Hemilingual atrophy, often secondary to lesions (e.g. neoplasia) affecting the hypoglossal nerve ipsilaterally, is the only unilateral tongue muscle-related neurological disorder reported in dogs. Hemilingual spasm is a descriptive term used in the human medical literature that refers to a disorder of involuntary, unilateral contractions in tongue musculature. We report a 5-year-old Border collie with hemilingual tetany ("hemilingual spasm"). Clinical examination revealed a resting unilateral contraction of the tongue on the right side with a resultant curvature of the tongue of variable degre

OBJECTIVES: To validate a mouse nerve injury model of microaspiration pneumonia due to dysfunctional oropharyngeal swallowing.
METHODS: C57BL/6J mice were divided into nerve transection (Nx; in which the left hypoglossal and vagus nerves were transected) and nonsurgical control groups. The lungs were removed the day after nerve transection. Then, the expression of inflammation-related genes (TNFα, CCL2, and IL-6) in the lung tissue was measured via qPCR and lung tissues were evaluated histologically via HE staining (Ctl = 6, Nx = 4). Bronchoalveolar lavage fluid (BALF) was collected at these

BACKGROUND: Submandibular gland (SMG) excision is traditionally performed using a transcervical approach, which, although effective, carries risks such as visible neck scarring and nerve complications. The transoral approach presents a cosmetically favorable alternative, yet its adoption remains limited. This case series was done to evaluate the safety, efficacy, and complication profile of the transoral approach for SMG excision in patients with benign pathologies.
METHODS: This retrospective case series was conducted at a single tertiary-care institution and included 30 patients who underwe

Background: Treacher Collins Syndrome (TCS) is a rare, congenital craniofacial syndrome. Its most characteristic feature is mandibular and midface hypoplasia. Due to malformations of the facial skeleton, airway abnormalities can also be observed, predisposing individuals to obstructive sleep apnoea (OSA). OSA in TCS may contribute to significant morbidity, including developmental delays, cardiovascular disorders and reduced quality of life. Objectives: This narrative review aims to present the true prevalence of OSA and the treatment options for TCS patients. Additionally, the pathophysiology

PURPOSE: Evaluate patient phenotypes and longitudinal patterns of hypoglossal nerve stimulator (HGNS) use and identify predictors of long-term HGNS adherence.
METHODS: Patients who underwent HGNS implantation for obstructive sleep apnea (OSA) from 2017 to 2023 and had available data through 9 months post-device activation were included. Adherence rate was defined as percentage of patients using the device for at least 4 h for 70% of nights. Repeated measures ANOVA and Cochran's Q tests were used to analyze changes in HGNS use over time. A k-means clustering analysis was used to identify HGNS

OBJECTIVE: To compile and evaluate characteristics of patients with hypoglossal canal synovial cysts reported in the literature.
BACKGROUND: Synovial cysts are benign soft-tissue masses that are rarely located in the occipitocervical junction and surrounding anatomical location. They may lead to compression of the hypoglossal nerve. Typically, cysts in this region remain asymptomatic, and biopsies of the hypoglossal canal have high risks, so imaging is a very important diagnostic tool.
METHODS: A systematic search of PubMed, Medline, Embase, Scopus, and Web of Science was conducted in line w

BACKGROUND: Patient selection for hypoglossal nerve stimulation (HGNS) for obstructive sleep apnoea (OSA) requires assessment of pharyngeal site of collapse using drug-induced sleep endoscopy (DISE). The current study aims to address two key knowledge gaps. First, we prospectively confirm that, among HGNS candidates, reduced HGNS efficacy is associated with oropharyngeal lateral wall (OLW) collapse (Aim 1). Second, given DISE is a resource-intensive procedure and delays treatment, we evaluate whether a recently developed non-invasive method for identifying OLW collapse using airflow shapes is

Hypoglossal schwannomas are extremely rare benign tumors arising from Schwann cells of the 12th cranial nerve. Surgical management of hypoglossal schwannomas requires detailed knowledge of the anatomy around the lower cerebellopontine angle (CPA), the craniovertebral junction (CVJ), and high cervical regions. If a tumor extends through the hypoglossal canal to the extracranial space, the canal is exposed by drilling the posterior half of the occipital condyle (OC), maintaining craniovertebral stability. Based on our experience and a literature review, we proposed the following modified grading

INTRODUCTION: The implantation of breathing-dependent unilateral hypoglossal nerve stimulation (HNS) has traditionally required the preoperative exclusion of complete concentric velopharyngeal collapse (CCC) through drug-induced sleep endoscopy (DISE). This prerequisite poses logistical and operational challenges for implantation teams. To address these issues the Eppendorf DISE concept was developed to streamline the process, making it more patient-friendly, cost-effective, and time-efficient.
MATERIAL AND METHODS: We retrospectively examined HNS patients who were preoperatively examined for

Introduction: Side-to-end hypoglossal-facial nerve transfer (HFT) is an established facial reanimation technique. However, evidence regarding predictors of outcomes is limited. Objective: To evaluate predictors of facial and tongue outcomes after HFT in a large series of patients with facial palsy. Methods: A retrospective review of 64 patients undergoing side-to-end HFT was conducted. Additional masseteric nerve transfer and/or cross-facial nerve grafting were performed in selected cases. Facial outcomes were evaluated with the House-Brackmann (HB) scale, the Sunnybrook Facial Grading System,

BACKGROUND: This proof-of-concept case-control study examined the feasibility of tongue high-density surface electromyography (HDS-EMG) to detect hypoglossal neuropathy.
METHODS: We analyzed tongue HDS-EMG for N = 2 participants. Subjects were graded through clinical tongue functional measures and gold-standard needle EMG. We collected HDS-EMG in three tongue tasks: relaxation, protrusion, and isometric maximum effort. The HDS-EMG was decomposed into motor unit spike train estimates.
RESULTS: Muscle relaxation HDS-EMG contained spikes consistent with fasciculation potential morphology. Durin

BACKGROUND: Late radiation-associated dysphagia (late-RAD) commonly presents in patients with signs of hypoglossal neuropathy, with hallmark clinical features including lingual atrophy, deviation, and fasciculation. Gold-standard electromyography (EMG) has not been used to explore the frequency of hypoglossal neuropathy in patients with late-RAD.
METHODS: Exploratory post hoc secondary analysis of MANTLE trial (NCT03612531) was completed. The presence of cranial nerve XII (CN XII) neuropathy was classified by (1) features of clinical assessment as well as (2) intramuscular genioglossus EMG pr

Isolated hypoglossal nerve palsy is a clinical disorder affecting the hypoglossal nerve presenting with dysphagia, dysarthria, and tongue wasting. The common etiologies are tumor, trauma, vascular lesions, infections, autoimmune/demyelinating disorders, and idiopathic. Isolated idiopathic hypoglossal can occur in a substantial number of cases with variable outcome. This case series of 3 patients (58-year-old woman, 26-year-old woman, and 67-year-old man) presented with isolated idiopathic unilateral hypoglossal palsy of 1-2 months duration, which remained persistent even on 6-8 months of follo

BACKGROUND AND IMPORTANCE: Lesions confined to the hypoglossal canal are rare, but may lead to invalidating symptoms. Moreover, extensive approaches to this region frequently require complex dissections and high surgical risks. We propose a minimally invasive approach specifically tailored to purely intracanalicular lesions of the hypoglossal canal.
CLINICAL PRESENTATION: With the aid of operative images and anatomic dissection, we present a clinical case of such a lesion and describe a targeted surgical approach for treatment.
CONCLUSION: For lesions purely confined to the hypoglossal canal

A woman in her 60s reported 'cramping' of her tongue, and examination showed continuous undulating movements of the right tongue typical of myokymia. MRI demonstrated compression of the right hypoglossal nerve and the right internal carotid artery by a soft tissue mass involving the right carotid space and hypoglossal canal. A biopsy of the mass was considered too dangerous, but anti-MPO-ANCA (myeloperoxidase specific antineutrophil cytoplasmic) antibodies were found positive, and a tentative diagnosis of MPO-ANCA-associated vasculitis (i.e., microscopic polyangiitis-MPA) was made. She was tre

Obstructive sleep apnea (OSA) is one of the most common widespread diseases and is associated with significant health risks and a considerable reduction in quality of life. Treatment is multimodal. Risk assessment tools such as the STOP-BANG questionnaire are commonly used, while the Epworth Sleepiness Scale helps evaluate subjective daytime sleepiness. Sleep studies play a central role in diagnosis. While in-lab polysomnography is considered the gold standard, for many patients, home sleep apnea testing (HSAT) using polygraphy provides sufficient objective data to confirm the diagnosis. The p
