[Transient visual loss in morning glory syndrome].

The causes of monocular transient visual loss (TVL) are diverse, necessitating a careful approach to diagnosis and differential evaluation of multiple conditions. In rare cases, TVL may occur in association with optic disc anomalies such as morning glory syndrome (MGS), coloboma, or optic disc drusen. The purpose of this article is to present a clinical case of TVL in a patient with MGS. A 25-year-old patient reported experiencing TVL in the left eye over the past 3-4 years, triggered by physical exertion, straining, or visual tasks requiring concentration. Ophthalmoscopy revealed MGS in the l

A Non-spinal Neural Circuit for Transmitting Information of Bladder Conditions.

The bladder is essential for the body's fluid balance to ensure normal physiological conditions; thus, a non-pathological bladder-that is, one sustaining internal homeostasis-is critical for this function. However, the neuro-network maintaining the bladder's intrinsic homeostasis is much less well-known compared with that for urination. Here, we identified that vagal nodose ganglion-pseudounipolar neurons project down to the bladder, up to the nucleus of the solitary tract, and further to multiple brain regions. The components of this network and those revealed by direct tracing from the bladd

OPTIC DISK COLOBOMA MAY TRIGGER MULTIPLE EVANESCENT WHITE DOT SYNDROME: EXPANDING THE CLINICAL SPECTRUM.

PURPOSE: To report a case of secondary multiple evanescent white dot syndrome (MEWDS) that occurred in a patient with optic disk coloboma. METHODS: A case report. RESULTS: A 23-year-old otherwise healthy man experienced decreased vision and photopsia in the right eye for one month. He reported poor visual acuity in the left eye because of refractive amblyopia. His best-corrected visual acuity was 20/100 in the right eye and 20/80 in the left eye. An examination of the right eye showed that the patient had optic disk coloboma. The results of multimodal imaging indicated MEWDS in the right eye

Imaging Insights into Perineural Tumor Spread.

Perineural spread (PNS) is a clinically significant route of tumor extension in head and neck malignancies and is not uncommonly overlooked on imaging. Often clinically occult, PNS may first be suggested by radiologists, and its detection can have significant implications for prognosis and management. This article provides a comprehensive overview of PNS, including relevant cranial nerve anatomy, optimized imaging protocols, and characteristic radiologic features. Multiple illustrative cases are included to improve recognition and highlight the critical role of imaging in the early identificat

Pseudo Foster-Kennedy Syndrome in an amblyopic patient: a case report.

Pseudo-Foster-Kennedy syndrome presents with optic disc swelling in one eye and optic atrophy in the other eye. It differs from the true Foster-Kennedy syndrome due to the absence of an intracranial mass. One of the most common causes of pseudo-Foster-Kennedy syndrome is sequential bilateral NAION. We present the case of a male patient who came to the emergency room in our clinic complaining of sudden vision loss in his right eye, headache, and hearing loss. The patient also had a diagnosis of high amblyopia in his left eye, where we also discovered an optic atrophy. We established the positiv

Optic Nerve Atrophy Conditions Associated With 3D Unsegmented Optical Coherence Tomography Volumes Using Deep Learning.

IMPORTANCE: Accurate differentiation of optic nerve head (ONH) atrophy is vital for guiding diagnosis and treatment of conditions such as glaucoma, nonarteritic anterior ischemic optic neuropathy (NAION), and optic neuritis. Traditional 2-dimensional assessments may overlook subtle, volumetric changes. OBJECTIVE: To determine whether a 3-dimensional (3D) deep learning model trained on unsegmented ONH optical coherence tomography (OCT) scans can reliably distinguish optic atrophy in glaucoma, NAION, optic neuritis, and healthy eyes. DESIGN, SETTING, AND PARTICIPANTS: This cross-sectional stud

Moebius-plus: a severe congenital multiple cranial neuropathy syndrome.

We describe a child with a severe form of Moebius syndrome comprising VI, VII, VIII, IX, X, XI and XII cranial neuropathies with septo-optic dysplasia, severe hypopituitarism and behavioural features of autism, which we propose to describe as Moebius-plus syndrome. In addition, we illustrate the importance of a holistic and multidisciplinary approach to both diagnosis and management for patient outcomes when such complexity arises.

Metachronous tumour (DNET and haemorrhagic chiasmal tumour) in a patient with encephalocraniocutaneous syndrome (ECCL).

Encephalocraniocutaneous lipomatosis (ECCL) is a rare neurocutaneous multisystem disorder affecting ectodermal and mesodermal tissues, including the eyes, skin, adipose tissue and the brain. It is hypothesised to be a neural crest disorder. While ECCL presents with various neurological features, the occurrence of brain tumour is an extremely rare association. Here, we report a case of metachronous tumours comprising a dysembryoplastic neuroepithelial tumour (DNET) and a haemorrhagic chiasmal tumour in a patient with ECCL. To the best of our knowledge, this is the first documented case of ECCL

Atypical diabetic neuropathies.

Diabetes is one of the most common conditions in the world and is associated with a broad range of adverse effects in multiple organ systems. Roughly half of all patients with diabetes will develop a typical distal, symmetric polyneuropathy, but several other atypical peripheral nerve conditions can also occur. Treatment induced neuropathy of diabetes is an acute and severely painful small fiber neuropathy that occurs in association with a precipitous drop in glycated hemoglobin levels. Radiculoplexus neuropathies include lumbosacral, cervical, and thoracic forms in which pain and weight loss

Optic nerve sheath enhancement on orbital MRI in giant cell arteritis.

BACKGROUND: Differentiating arteritic anterior ischaemic optic neuropathy (A-AION) due to giant cell arteritis (GCA) from non-arteritic anterior ischaemic optic neuropathy (NA-AION) may pose a diagnostic challenge. Our study aimed to assess the use of standard orbital MRI in distinguishing ocular manifestations of GCA from NA-AION. METHODS: This study included 25 consecutive patients (11 GCA, 14 NA-AION) who underwent contrast-enhanced orbital MRIs within 3 months of symptom onset. Two radiologists blinded to clinical data independently evaluated MRIs for the enhancement of the optic nerve sh

Case Report: Successful treatment of severe Guillain-Barré syndrome with paralytic ileus as a presenting symptom by intensive immunotherapy.

Involvement of the intestinal autonomic nerves in Guillain-Barré syndrome (GBS) can lead to paralytic ileus, a condition commonly observed in severe cases during later stages of the disease. Cases with paralytic ileus as a presenting symptom are very rare. We report a case of a 35-year-old male patient who was admitted to the hospital with acute abdominal pain persisting for 12 hours. Abdominal CT suggested small bowel obstruction, for which routine conventional pharmacological treatment were ineffective. Subsequently, the patient presented with multiple sets of cranial nerve paralysis, bilate

Orbital apex-like syndrome with multiple cranial neuropathies secondary to chronic inflammatory demyelinating polyneuropathy.

Chronic inflammatory demyelinating polyneuropathy (CIDP) is a rare autoimmune disorder characterized by progressive demyelination of peripheral nerves, leading to motor and sensory deficits. Ophthalmologic involvement, though uncommon, may manifest as ophthalmoplegia, papilledema, optic neuropathy, or proptosis. This report presents a 49-year-old male with refractory CIDP and extensive orbital involvement, including bilateral proptosis and enlargement of multiple cranial nerves, resulting in an orbital apex-like syndrome and compressive optic neuropathy. The patient's condition was refractory

Peripheral nerve anomalies and associated musculoskeletal defects in anuran species: New anatomical records.

In this study, we analyzed peripheral nerve anomalies in 15 anuran species spanning multiple developmental stages. Anatomical examinations focused on both the central nervous system-including the olfactory bulbs, telencephalon, diencephalon, midbrain, cerebellum, and hindbrain-and the peripheral nervous system, with particular attention to cranial and spinal nerves. Spinal nerve anomalies were often associated with vertebral deformities such as scoliosis and commonly presented as nerve thinning, fusion, or asymmetrical branching along the curvature's concave side. Limb rotation primarily affec

Adherence to unilateral hypoglossal nerve stimulation and changes in Epworth Sleepiness Scale scores.

STUDY OBJECTIVES: Effective treatment of obstructive sleep apnea depends on efficacy and adherence. The objective of this analysis was to examine the association between adherence to unilateral hypoglossal nerve stimulation (Inspire upper airway stimulation) and changes in daytime sleepiness. METHODS: Participants were adults treated with unilateral hypoglossal nerve stimulation enrolled in the ADHERE Registry. All participants included in the analysis completed a final visit 1-2 years following implantation. Outcomes were the change in Epworth Sleepiness Scale (ESS) score and normalization o

Bilateral Facial Nerve Palsy Revealing Diffuse Large B-Cell Lymphoma With Leptomeningeal Involvement.

We present a diagnostically challenging case of a 54-year-old woman who developed progressive bilateral lower motor neuron (LMN) facial nerve palsy, later accompanied by lower limb weakness and impaired dexterity. The initial presentation was misattributed to idiopathic Bell's palsy, which delayed recognition of a more serious underlying condition. Neuroimaging revealed enhancement of multiple cranial nerves and diffuse leptomeningeal involvement. Cerebrospinal fluid (CSF) analysis demonstrated lymphocytic pleocytosis, elevated protein levels, low glucose, and increased lactate dehydrogenase (

A case of hypoglossal nerve palsy with evolving cranial nerve involvement in renal cell carcinoma: a case report.

BACKGROUND: Renal cell carcinoma is a rare pediatric solid tumor that typically presents with hematuria, abdominal mass, or flank pain. It is uncommon for renal cell carcinoma to manifest with headache and isolated extra-urogenital symptoms. We present, to our knowledge, the first case of renal cell carcinoma with bony metastases, presenting initially as isolated cranial nerve twelve palsy. Although bony metastases can occur in renal cell carcinoma, skull-based metastases and cranial neuropathies are exceedingly rare, especially in the pediatric population. CASE PRESENTATION: We describe the
