Molecular and genetic insights into vestibular schwannoma.

BACKGROUND: Vestibular Schwannomas (VS) are benign nerve sheath tumors arising from Schwann cells of the vestibulocochlear nerve, occurring sporadically or in association with neurofibromatosis type 2 (NF2). Clinical management of VS is challenging due to the tumor's location adjacent to critical neural and vascular structures. The primary morbidities are hearing loss and facial nerve dysfunction, which significantly affect quality of life. There are no FDA-approved medical therapies, and treatment typically consists of surgery or radiation, with treatment decisions centered on optimizing hear

Orbital pseudotumor and skull base infiltration as the sole manifestation of granulomatosis with polyangiitis.

Granulomatosis with polyangiitis (GPA) is a vasculitis affecting small and medium-sized blood vessels. It typically presents with upper respiratory tract, lung, and kidney involvement. However, the disease may involve multiple other organs, and not all cases present with these classic manifestations. Periocular lesions are described in the GPA and, especially in the case of isolated, single-site disease, require extensive differentiation from other diseases, such as IgG4-related disease or myofibroblastic tumor. However, periocular lesions and coexisting symptoms suggesting changes in the cent

Clinical Parameters and Radiomics of Vestibular Schwannomas in NF2-related Schwannomatosis.

BACKGROUND/AIM: NF2-related schwannomatosis is a rare hereditary tumor predisposition syndrome, formerly known as Neurofibromatosis type 2 (NF2), and is characterized by the development of multiple schwannomas. The hallmark manifestation is the occurrence of bilateral vestibular schwannomas (VSs). Disease progression and clinical outcomes vary widely among patients, and conventional magnetic resonance imaging (MRI) metrics-such as tumor size-do not fully account for these differences. Radiomic analysis offers a quantitative approach to extract advanced imaging biomarkers that may capture tumor

Orbital apex syndrome from middle meningeal artery embolization.

PURPOSE: To present two cases of orbital apex syndrome, a previously unreported complication of middle meningeal artery (MMA) embolization. BACKGROUND: MMA embolization is increasingly used to treat chronic subdural hematomas and is generally considered safe. However, anastomotic connections to the ophthalmic circulation may permit complications from embolic migration. METHODS: Retrospective case series of two patients who developed neuro-ophthalmic complications following MMA embolization for subdural hematoma. RESULTS: A 75-year-old male developed acute diplopia, a relative afferent pupil

End of a diagnostic odyssey: the added value of multi-tissue analysis in the identification of mosaicism in tumour predisposition syndromes.

Mosaicism refers to the presence of multiple cell clones with distinct genotypes arising from a single zygote. The phenotype of mosaic individuals depends on the extent of mosaicism, ranging from localised to almost generalised. We report three diagnostically challenging cases with previously unrecognised mosaicism in the tumour predisposition syndromes (TPS) von Hippel-Lindau syndrome, neurofibromatosis type 1 and neurofibromatosis type 2. In all three patients there was a strong clinical suspicion for a TPS; however, no pathogenic variant (PV) was detected in these patients in DNA extracted

Flame-shaped epiretinal membranes: expanding the spectrum beyond neurofibromatosis type 2.

OBJECTIVE: To characterize the spectral-domain optical coherence tomography (SD-OCT) features of flame-shaped epiretinal membranes (ERMs) in patients with and without neurofibromatosis type 2 (NF2). METHODS: This retrospective study included 9 patients (13 eyes) with flame-shaped ERMs from 3 tertiary centres. Multimodal imaging and clinical data were systematically analyzed. RESULTS: Six of 9 subjects with flame-shaped ERM were confirmed NF2 patients. Among the NF2 cohort of 12 eyes, SD-OCT revealed flame-shaped ERMs in 10 eyes (83%). Flame-shaped ERMs were bilateral in 4 patients (8 eyes) a

[89Zr]bevacizumab PET/CT imaging of vestibular schwannomas for the prediction of bevacizumab treatment effect in patients with symptomatic NF2-related schwannomatosis: a study protocol for a phase II single centre, prospective, feasibility trial.

INTRODUCTION: Treatment with bevacizumab achieves both tumour stabilisation or regression and preservation or improvement of hearing. However, the efficacy of bevacizumab varies between patients and within patients. Side effects due to bevacizumab treatment are also common. It would be of value to predict therapeutic response prior to initiating therapy to prevent unnecessary exposure in patients unlikely to benefit. METHODS AND ANALYSIS: We aim to recruit 25 patients with NF2-related schwannomatosis (NF2) with bilateral vestibular schwannomas. Patients will receive an intravenous injection o

Regression-Based Analysis of Vestibular Laboratory Tests for the Prediction of Unilateral Vestibular Schwannoma.

OBJECTIVE: The diagnostic work-up for vestibular pathologies involves a battery of tests designed to quantify the functioning of the otolith organs and semicircular canals. Clinical data from video head-impulse tests, vestibular-evoked myogenic potentials, subjective visual verticality, and caloric tests are usually collected. Our study applied regression analyses to predict the affected side of a patient group with vestibular schwannoma, learning from laboratory vestibular tests, to assess their relative predictive capacity in predicting the tumor side. Technology or Method: The dataset was p

Irbesartan Promotes Retinal Ganglion Cell Survival and Modulates Mitochondrial and Inflammatory Pathways in Mouse Retinal Explant and in Optic Nerve Crush Models.

Glaucoma is a neurodegenerative eye disease characterized by progressive loss of retinal ganglion cells (RGCs), resulting in irreversible vision loss. Previous work demonstrated that irbesartan, an angiotensin II type I receptor blocker, promotes RGC survival in rat retinal explants. This study extends these findings to ex vivo mouse explants, in vivo optic nerve crush model, and transcriptomic analysis to identify associated pathways. Retinal explants from C57BL/6 mice were cultured for 4 days with irbesartan (10 μmol/L), vehicle, or angiotensin II (2 μmol/L). RGC survival was assessed by βII

Multiple inflammatory cytokines correlate with the vestibular and oculomotor dysfunction in Fabry disease: a prospective, longitudinal study.

OBJECTIVES: Vestibular and oculomotor abnormalities have been widely identified in Fabry disease (FD), with inflammation potentially playing an important role. We aim to investigate the expression of inflammatory cytokines (ICs) in FD patients and their relationship with the vestibular/oculomotor dysfunctions. METHODS: This prospective observational study enrolled 40 FD patients. All 40 patients underwent the visuo-oculomotor examination, and 22 of them received the vestibulo-oculomotor examination. Plasma concentrations of 14 ICs were detected, including interferon-γ, interleukin (IL)-1β, IL

Duplication of Right Superior Cerebellar Artery Leading to Trigeminal Neuralgia: A Rare Neurovascular Conflict.

Trigeminal neuralgia (TN) is a chronic neuropathic pain condition characterized by brief, intense, and electrical shock-like facial pain. The most common causes of this are due to neurovascular compression of the superior cerebellar artery (SCA). However, as seen in this case report, duplication of the SCA resulting in TN is extremely uncommon. A 63-year-old woman presented with a 1-day history of paroxysmal, shock-like pain on the right side of her face affecting both the ophthalmic (V1) and maxillary (V2) branches of the trigeminal nerve. Each episode of pain would last 1-2 min, occur multip

Peripheral trigeminal nerve branch lesions: CT and MRI findings in a pathologically confirmed case series.

PURPOSE: The peripheral branches of the trigeminal nerve (V1, V2, and V3) follow an intricate and tortuous course through various skull base foramina and soft tissue spaces. Due to the rarity of lesions specifically involving these distal segments, radiological diagnosis remains a significant challenge. This study aims to summarize the characteristic imaging patterns of diverse pathologically confirmed lesions affecting the peripheral trigeminal branches using CT and MRI. METHODS: We retrospectively reviewed 18 patients (2008-2022) with pathologically proven peripheral trigeminal lesions. The

[Left eye optic neuropathy and right eye retinopathy after embolization of middle cerebral artery aneurysm: a case report].

A 45 year old patient presented with left upper visual field defect and decreased vision on the first postoperative day following "stent-assisted endovascular embolization for a ruptured left middle cerebral artery aneurysm with subarachnoid hemorrhage" performed in the neurology department. Lumbar puncture showed normal cerebrospinal fluid pressure initially, but repeated follow-up measurements during hospitalization indicated elevated intracranial pressure. The patient exhibited unequal pupil size, with a positive relative afferent pupillary defect (RAPD) in the left eye and slight pallor of

A review of targeted therapies for NF2-related vestibular schwannoma: molecular pathogenesis, emerging therapeutics, and future clinical horizons.

BACKGROUND: Neurofibromatosis type 2 (NF2) is a hereditary tumor syndrome driven by mutations in the NF2 gene. The mutation leads to aberrant proliferation of Schwann cells along the vestibular division of cranial nerve VIII, resulting in bilateral vestibular schwannomas (VS) that cause progressive hearing loss and neurological dysfunction. Loss of the tumor suppressor merlin results in dysregulation of multiple oncogenic pathways, including VEGF, MAPK/ERK, PI3K/AKT/mTOR, EGFR/ErbB, PDGFR, and Hippo-YAP. The inability of conventional management modalities to address the multifocal and progress

Botulinumtoxin Type-A (BoNTA) in the Management of Refractory Trigeminal Neuralgia: An Expert-Opinion, Practice-Oriented Narrative Review on Behalf of the GRASP Study Group.

Trigeminal neuralgia (TN) ranks among the most excruciating neuropathic pain syndromes, characterized clinically by multiple daily episodes of unilateral, paroxysmal, electric shock-like facial pain. The daily activities and quality of life of affected patients are profoundly diminished. First-line pharmacological agents, such as carbamazepine and oxcarbazepine, provide initial relief for many patients. However, a significant proportion eventually develops refractory symptoms or experience intolerable adverse effects, leading to the discontinuation of traditional oral medications. For these pa

How I do it: microvascular decompression for classical trigeminal neuralgia by arterial compression.

Microvascular decompression has been shown in multiple long-term series to provide durable pain freedom in 75-95% of patients with classical trigeminal neuralgia, defined as neuralgia related to an identified neurovascular conflict, frequently arterial. Surgical success relies on an effective decompression, achieved by displacing the offending arteries while avoiding neo-compression(s) by implanted material. The procedure is performed through a keyhole retrosigmoid (retromastoid) craniectomy, using an infratentorial, supracerebellar corridor to the trigeminal root that minimizes traction on th

Clinical, Electrophysiological, and Neuropathological Features of Peripheral Neuropathies in People With T-Cell Lymphoproliferative Disorder.

BACKGROUND AND OBJECTIVES: Peripheral neuropathies (PNs) associated with T-cell lymphoproliferative disorders (T-Ly) are exceptionally rare and poorly characterized. Whether their mechanisms, clinical features, and outcome differ from PN associated with B-cell lymphoproliferative disorders (B-Ly) remains unclear. We aimed to characterize the clinical, electrophysiologic, and pathologic spectrum of T-Ly-associated PN, evaluate treatment responses, and compare these findings with B-Ly-associated PN. METHODS: We conducted a retrospective multicentric cohort study across 17 PN reference centers i
