A thoracic aortic aneurysm (TAA) is characterized by dilation of the aortic root or ascending aorta; it is a heritable disease in 10% to 20% and of unknown origin in the remainder; it is an increasingly recognized condition that is often diagnosed incidentally. It could be potentially life-threatening if left untreated due to its catastrophic rupture. The pooled incidence and prevalence of TAAs is about 5.3 per 100 000 individuals/year and 0.16%, respectively. The pooled incidence of ruptured aneurysms was estimated at 1.6 per 100 000 individuals/year. Genetic studies indicate that the somatic

OBJECTIVE: The use of bioprosthetic valves in the Bentall procedure (bio-Bentall) has increased for its anticoagulant-free lifestyle benefits. Valve-sparing root replacement (VSRR) offers an alternative to the bio-Bentall with the preservation of native valves. However, comparative evidence between VSRR and bio-Bentall is scarce. This meta-analysis aims to address this knowledge gap. METHODS: A systematic review was conducted on databases from inception through December 2025. Studies reporting adjusted outcomes were included. The primary outcome was mid-term all-cause mortality and aortic valv

INTRODUCTION: Sepsis after aortic root replacement (ARR) is a severe complication that results in significant postoperative mortality. To date, risk factors associated with development of sepsis after ARR have not been thoroughly studied. The objective of this study was to identify risk factors of sepsis following elective ARR using the National Inpatient Sample (NIS) database.
METHODS: The NIS core files of the Health care Cost and Utilization Project database from 2017 to 2022 were used to identify adults (≥18 y) who underwent elective ARR. Demographic, comorbidity, and other health factors

Ischemic heart disease (IHD) remains a leading cause of death worldwide, with dietary risks being its most significant modifiable factor. Here, using the Global Burden of Diseases, Injuries and Risk Factors Study 2023, we estimated the mortality and disability-adjusted life years from diet-related IHD across 204 countries. In 2023, a suboptimal diet was responsible for 4.06 million (95% uncertainty interval (UI) 0.74-6.22) IHD deaths and 96.84 million (18.82-142.52) IHD disability-adjusted life years. The global age-standardized death rate of IHD attributable to suboptimal diet decreased by 43

BACKGROUND: Rheumatoid arthritis (RA)-related severe manifestations in the cervical spine have become less prevalent since the introduction of disease modifying anti-rheumatic medications. Nonetheless, the consequences of subaxial subluxations and potential for cord compression leading to myelopathy are significant and often demand complex surgical decompression and fixation strategies. We present a novel case of severe T1/2 hyperkyphotic deformity complicated by osteoporosis requiring significant intraoperative reduction and multi-level cervicothoracic fusion.

Aortic regurgitation (AR) in young adults (aged 18-44 years) is often under- or misdiagnosed and is not always benign. As in older populations, prognosis depends on the severity of regurgitation, the underlying cause, early left ventricular dysfunction, and the chosen intervention strategy. Prompt diagnosis and timely treatment improve outcomes, yet the optimal timing for surgery in asymptomatic chronic AR remains uncertain. Recent studies have proposed new thresholds for left ventricular structural and functional assessment to better evaluate the impact of AR on prognosis and guide interventi

Thoracic aortic aneurysms involving the root and ascending aorta progressively enlarge and can lead to life-threatening acute aortic dissections, collectively termed thoracic aortic disease (TAD). Oxidative stress due to an excess of reactive oxygen species (ROS) has been implicated in TAD pathogenesis by promoting vascular inflammation, extracellular matrix degradation, and smooth muscle cell apoptosis. Cobinamide (Cbi), a potent antioxidant and vitamin B12 analog, has been reported to slow aortic aneurysm growth in TAD mouse models. We evaluated its efficacy in mice administered β-aminopropi

BACKGROUND: Chronic axial spinal pain is one of the leading causes of disability. The literature demonstrates that expenditures for low back and neck pain continue to rise, not only in parallel with increasing disability, but also with escalating healthcare costs, accounting for the highest spending among multiple disease categories. Based on current evidence utilizing controlled diagnostic blocks, facet joints, nerve root dura, and sacroiliac joints have been identified as potential sources of spinal pain.
STUDY DESIGN: A systematic review of the diagnostic accuracy of spinal facet joint ner

INTRODUCTION: Syndromic hereditary thoracic aortic disease (sHTAD), including Loeys-Dietz syndrome (LDS), often lead to aneurysmal disease with diffuse arterial involvement. Although open repair remains the gold standard in sHTAD, it carries significant morbidity. Herein, a hybrid approach for subclavian-axillary arterial aneurysm repair in patients with LDS is described.
TECHNIQUE: Two patients with prior aortic repair extending from the aortic root to the abdominal aorta were managed for extensive aneurysms involving the subclavian and proximal axillary artery. Via ipsilateral upper limb ac

BACKGROUND: Double aortic arch (DAA) is a rare congenital aortic arch anomaly that is usually identified in infancy because of symptoms related to a vascular ring. Acute type A intramural hematoma (IMH) with a focal intimal tear in an adult with DAA is extremely rare, and no consensus has been established regarding emergency management.
CASE PRESENTATION: Seventy-three year-old woman presented with burning pain in the throat and suprasternal notch and was initially suspected of having acute coronary syndrome. Computed tomography angiography (CTA) of the whole aorta and supra-aortic vessels de

Marfan syndrome (MFS) is characterized by progressive aortic aneurysm formation resulting from mutations in the fibrillin-1 (Fbn1) gene. Although the thoracic aorta is the primary site of pathology, accumulating evidence indicates that vascular dysfunction in MFS extends beyond the aorta to involve multiple arterial beds. Statins have been shown to attenuate aneurysm progression in experimental models of MFS; however, their effects on systemic vascular remodeling and arterial stiffness outside the aorta remain poorly characterized. In this study, we investigated the impact of chronic atorvasta

BACKGROUND: Personalized external aortic root support (PEARS) is a prophylactic surgical technique designed to stabilize the aortic root in patients with heritable thoracic aortic disease (HTAD), including Marfan and Loeys-Dietz syndromes. Although early outcomes have been encouraging, distal aortic complications after PEARS remain poorly characterized. Type B aortic dissection (TBAD) following PEARS implantation has rarely been reported.
CASE PRESENTATION: We report two patients with HTAD who developed complicated TBAD several years after PEARS implantation. The first patient, a woman with L

BACKGROUND: Periscapular and interscapular pain are common musculoskeletal complaints; however, dorsal scapular nerve (DSN) entrapment neuropathy remains an under-recognized and frequently overlooked etiology. DSN pathology may mimic multiple cervical, shoulder, and upper-extremity disorders, resulting in delayed diagnosis and treatment. To systematically review the current literature regarding the anatomy, etiology, clinical manifestations, diagnostic evaluation, and treatment of DSN entrapment neuropathy.
METHODS: A systematic review was performed using PubMed/MEDLINE, Scopus, and Google Sc

BACKGROUND: The aim of this study was to quantify the prevalence of thoracic aortic aneurysms (TAAs) in patients with a known abdominal aortic aneurysm (AAA) within the Northwest of England and understand the characteristics of this subpopulation.
METHODS: A retrospective review of data from all patients diagnosed with an AAA from 2010 to 2023 and entered onto AAA surveillance within the National Health Service (NHS) Manchester University Foundation Trust. Of these, patients with a thoracic computed tomography (CT) available were reviewed for diagnosis of TAA.
RESULTS: Out of 250 patients wi

BACKGROUND: The optimal valve substitute for young and middle-aged patients with severe symptomatic aortic valve disease remains a subject of debate. In this patient group, prosthetic valves are associated with reduced survival, impaired hemodynamics, and lifelong risk of valve-related complications. The Ross procedure has re-emerged as the most optimal valve substitute following surgical refinements and improved perioperative care. The current study describes the feasibility in terms of safety and early outcomes of a newly initiated Ross program using the patient-tailored total root technique

Chronic obstructive pulmonary disease (COPD) is a progressive lung disorder that affects millions of people globally. Although the mechanisms of COPD pathogenesis are not completely known, oxidative stress and lung inflammation caused by chronic exposure to cigarette smoke, environmental or occupational pollutants, gas from burning biomass fuel are thought to contribute to development of COPD. Therefore, therapies aimed at reducing oxidative stress along with inflammation may be important in treating COPD. However, the current pharmacological therapies treat symptoms and reduce acute exacerbat

The management of complex aortic disease has evolved toward a "lifetime management" strategy, where the initial surgical arch repair critically influences the feasibility of future staged interventions. While the left subclavian artery has received significant attention, the strategic role of the innominate artery (IA) as a primary endovascular gateway remains underappreciated. This paper explores how IA reimplantation geometry determines the success of subsequent endovascular procedures. We analyzed the technical requirements for optimal antegrade endovascular access during complex downstream

Frozen elephant trunk (FET) is an established treatment modality for complex aortic arch and proximal descending aortic pathologies. While patients with limited life expectancy are more frequently considered for endovascular treatment options, the FET remains a sustainable platform, especially for younger post-dissection and heritable thoracic aortic disease (HTAD) patients. These patients frequently have undergone previous transsternal operations. A substantial portion require concomitant aortic root or other cardiac procedures. In this manuscript we describe our technique of redo valve spari
