Tarsal tunnel syndrome is an entrapment neuropathy of the posterior tibial nerve beneath the flexor retinaculum that can be precipitated by either intrinsic or extrinsic factors. We report a unique case of a posterior medial ankle joint capsular defect with localized fluid extravasation between the flexor digitorum longus and flexor hallucis longus leading to symptoms consistent with tarsal tunnel syndrome in a collegiate tennis player. This patient is a 19-year-old female with no past medical history who presented with symptoms consistent with tarsal tunnel syndrome. After confirmation with m

Lower limb entrapment neuropathies comprise a wide range of disorders, including less common conditions like tarsal tunnel syndrome, Morton neuroma, obturator nerve entrapment syndrome, superior gluteal nerve entrapment, and cluneal nerve entrapment syndrome. Despite being less prevalent, these syndromes are equally significant, presenting with symptoms such as pain, dysesthesia, muscular weakness, and distinct physical signs. Accurate diagnosis of these less common disorders is crucial for successful therapy and patient recovery, as they can sometimes be mistaken for lumbar plexopathies, radi

BACKGROUND: Tarsal tunnel syndrome (TTS) is a common entrapment neuropathy of the posterior tibial nerve. Surgery can be performed less invasively under local anesthesia. We adopted zig-zag skin incision to prevent postoperative wound complications.
METHODS: Between July 2022 and June 2023, we operated on 19 legs of 14 consecutive TTS patients (5 males, 11 females; average age 73.3 years). We made a 2- to 3-cm zig-zag skin incision on the tarsal tunnel. After posterior tibial nerve decompression by posterior tibial artery (PTA) transposition, the subcutaneous layer was tightly sutured with 4-

Peripheral neuropathies of the foot and ankle can be challenging to diagnose clinically due to concomitant traumatic and nontraumatic or degenerative orthopedic conditions. Although clinical history, physical examination, and electrodiagnostic testing comprised of nerve conduction velocities and electromyography are used primarily for the identification and classification of peripheral nerve disorders, MR neurography (MRN) can be used to visualize the peripheral nerves as well as the skeletal muscles of the foot and ankle for primary neurogenic pathology and skeletal muscle denervation effect.

BACKGROUND/OBJECTIVES: The tibial nerve, commonly misnamed the "posterior tibial nerve", branches into four key nerves: the medial plantar, lateral plantar, inferior calcaneal (Baxter's nerve), and medial calcaneal branches. These nerves are integral to both the sensory and motor functions of the foot. Approximately 15% of adults with foot issues experience heel pain, frequently stemming from neural origins, such as tarsal tunnel syndrome (TTS). TTS diagnosis remains challenging due to a high false negative rate in neurophysiological studies. This study aims to improve the understanding and di

Anterior tarsal tunnel syndrome, an infrequent entrapment neuropathy involving the deep peroneal nerve beneath the inferior extensor retinaculum in the anterior ankle, is often overlooked on medical images, leading to delayed diagnosis and treatment. We present the case of a 52-year-old male, an avid runner, who exhibited a sensation of burning and tingling in the dorsal region of both feet. Electrophysiologic studies suggested bilateral deep peroneal neuropathy. Subsequent magnetic resonance imaging revealed bilateral focal regions of signal alteration, consistent with scarring, encasing the

Necrotizing fasciitis is a severe and rapidly progressing soft tissue infection that requires immediate intervention. However, its manifestation as tarsal tunnel syndrome in a diabetic patient is an extremely rare occurrence, with no previous reports found in the existing literature. We present a case report of a patient in their late 50s with uncontrolled diabetes who had necrotizing fasciitis and presented initially to the emergency department with hypotension. The patient complained of symptoms consistent with acute tarsal tunnel syndrome and displayed systemic signs of infection, necessita

Tarsal tunnel syndrome (TTS) refers to compression of the posterior tibial nerve as it traverses the tarsal tunnel in the ankle. First described by Keck and Lam in 1962, TTS is an underdiagnosed cause of heel pain and foot dysfunction.1,2 The tarsal tunnel contains the tibial nerve, posterior tibial artery, and tendons of the tibialis posterior, flexor digitorum longus, and flexor hallucis longus muscles. Compression of the tibial nerve within this tunnel leads to pain, numbness, tingling, and weakness along its distribution. The clinical presentation of TTS can vary due to the numerous etiolo

Variations of the ankle anatomy are infrequent and exist as supernumerary muscles and tendons. Often understudied and overlooked, their presence can cause many complications of the lower extremity. These muscles, although often asymptomatic, can cause great pain and complications such as tenosynovitis, tarsal tunnel syndrome, lateral ankle instability, and ankle pain when they impinge on the normal anatomy of the patient. The goals of this study were to examine the prevalence of common variants such as the peroneus quartus, peroneus digiti quinti, accessory soleus, flexor accessorius digitorum

BACKGROUND: Although an association between peripheral nerve entrapment issues and rheumatoid arthritis (RA) has been found, research has generally focused solely on nerve entrapment in the upper or lower extremity individually rather than on the consideration of nerve entrapment simultaneously in the upper and lower extremities. In addition, most of these studies have used small sample sizes. The aim of this study was to evaluate the incidence of carpal tunnel syndrome (CTS) and tarsal tunnel syndrome (TTS) concurrently in patients with RA using a relatively large sample size.

Tarsal tunnel syndrome is a common entrapment neuropathy that is sometimes elicited by ganglia in the tarsal tunnel. The tarsal ganglion may penetrate through the flexor retinaculum and have an hourglass appearance with components deep and superficial to the retinaculum. The purpose of this Technical Note is to describe technical details of endoscopic management of hourglass ganglion of the tarsal tunnel. This comprises excision of the part of the ganglion superficial to the flexor retinaculum and internal drainage of the deep part of the ganglion to the tendon sheath of the posterior tibial t

BACKGROUND: Tarsal tunnel syndrome (TTS) describes a compression neuropathy of the tibial nerve or its branches as they pass under the flexor retinaculum of the ankle. TTS is a clinical diagnosis, but often involves needle electromyography, nerve conduction studies (NCS), and magnetic resonance imaging. To our knowledge, the use of ultrasound to diagnose tibial nerve compression from synovial cysts (SCs) in the tarsal tunnel has been sparsely reported upon.
CASE REPORT: A 50-year-old male patient suffering from TTS whose diagnosis of SC-related compression was confirmed via ultrasound. Patien

BACKGROUND: The symptoms of tarsal tunnel syndrome (TTS) have traditionally been considered the result of entrapment of the tibial nerve due to the laciniate ligament or compression of the nerve's distal branches as they course beneath the abductor hallucis (AH) muscle. Recent research shows that TTS may be due to compression of the tibial nerve proximal to these structures, at the level of the high ankle (termed high TTS). This study appears to be among the first to investigate the incidence of compression at either or both of these sites in persons with clinical findings of TTS.

BACKGROUND: Previous studies on magnetic resonance neurography (MRN) found different patterns of structural nerve damage in type 1 diabetes (T1D) and type 2 diabetes (T2D). Magnetization transfer ratio (MTR) is a quantitative technique to analyze the macromolecular tissue composition. We compared MTR values of the sciatic nerve in patients with T1D, T2D, and healthy controls (HC).
METHODS: 3-T MRN of the right sciatic nerve at thigh level was performed in 14 HC, 10 patients with T1D (3 with diabetic neuropathy), and 28 patients with T2D (10 with diabetic neuropathy). Results were subsequently

PURPOSE: To investigate the safety and accessibility of direct posterior medial and lateral portals into the knee.
METHODS: This study was a controlled laboratory study that comprised a sample of 95 formalin-embalmed cadaveric knees and 9 fresh-frozen knees. Cannulas were inserted into the knees, 16 mm from the vertical plane between the medial epicondyle of the femur and the medial condyle of the tibia, and 8 (females) and 14 mm (males) from the vertical plane connecting the lateral femoral epicondyle and lateral tibial condyle. Landmarks were identified in full extension, and cannula insert

The soleus H-reflex modulation pattern was investigated during stepping following transspinal stimulation over the thoracolumbar region at 15, 30, and 50 Hz with 10 kHz carry-over frequency above and below the paresthesia threshold. The soleus H-reflex was elicited by posterior tibial nerve stimulation with a single 1 ms pulse at an intensity that the M-wave amplitudes ranged from 0 to 15% of the maximal M-wave evoked 80 ms after the test stimulus, and the soleus H-reflex was half the size of the maximal H-reflex evoked on the ascending portion of the recruitment curve. During treadmill walkin

Intradural extramedullary metastases from systemic neoplasms are very rare, with an incidence ranging from 2% to 5% of all secondary spinal diseases. We present the case of a 53-year-old man diagnosed with lung adenocarcinoma with symptoms of severe back pain and tibial paresis. The magnetic resonance imaging (MRI) revealed an intradural lesion originating from the right S1 nerve root mimicking neurinoma. Total tumor removal was achieved via posterior midline approach. The histological examination was consistent with lung carcinoma metastasis. Due to the rarity of single nodular nerve root met

Tarsal tunnel syndrome (TTS) is a nerve entrapment of the posterior tibial nerve. This uncommon condition frequently goes undiagnosed or misdiagnosed even though it interferes with the daily activities of workers. Here we discuss the return to work status of a 37-year-old male patient who manages a manufacturing plant. He was identified as having Tarsal Tunnel Syndrome as a result of a foot abnormality and improper shoe wear. He had moderate pes planus and underwent tarsal tunnel release on his right foot. What are the determinant factors in defining a patient's status for returning to work af
