Common sports-related nerve injuries seen by the electrodiagnostic medical consultant.

The high physiologic demands of sports create dynamic stress on joints, soft tissues, and nerves which may lead to injuries in the athlete. Electrodiagnostic (EDx) assessment is essential to identify the correct diagnosis, localization, and prognosis, to guide management of sports-related neuropathies. A comprehensive review was performed to provide the EDx medical consultant with a practical approach to the common peripheral nerve disorders seen in athletes. Sports-related neuropathies reviewed include transient traumatic irritation of the brachial plexus and/or cervical nerve roots ("Burners

Multiple schwannomas of bilateral posterior tibial nerves - A case report and review of literature.

INTRODUCTION: Schwannoma is a type of slow growing benign nerve sheath tumor arising from Schwann cells. Peripherally, they are found in association with the ulnar, sciatic and posterior tibial nerves. Peripheral schwannomas, neurofibromas and malignant PNSTs are collectively grouped as peripheral nerve sheath tumors. PRESENTATION OF CASE: We report a case of a 35-year-old male, without clinical features of NF2, presenting with 2 painful lumps around the ankle, and was incidentally found to have multiple lumps in both lower limbs. MR imaging, surgical excision and histology revealed multiple

Using an acellular dermal matrix as a neuroprotective wrap-around for treating recurrent nerve entrapment syndromes: a proof of concept.

Recurrent nerve entrapment syndrome is a well-known complication in peripheral nerve surgery that often leads to multiple reoperations and increases the risk of unfavorable outcomes due to scarring. In our outpatient clinic, we found two patients with recurrent nerve entrapment syndrome with significant symptoms such as complete sensory loss and chronic pain who were willing to undergo re-exploration of their entrapped nerves and cover them with a human acellular dermal matrix (ADM) as a protective shield against recurrence. Both patients had complete recovery of the nerve entrapment syndrome

Dellon decompression of the tarsal tunnel: An effective approach to improving blood flow, promoting ulcer healing and recovery of sensibility in diabetic patients.

We hypothesized that tibial nerve decompression would improve sympathetic nerve function, leading to increased blood flow, enhanced plantar sensitivity, and reduced healing time for diabetic neuropathic ulcers. This prospective study involved 20 patients with non-healing diabetic neuropathic ulcers averaging 17.1 months in duration. Patients had good glycemic control and positive Tinel sign at the tarsal tunnel, with average ulcer size of 10.49 cm². Preoperatively, sensory symptoms were assessed using the Michigan Neuropathy Screening Instrument (MNSI) and two-point discrimination test, ulcer

Tarsal tunnel syndrome in runners: diagnosis and return to physical activity - a review.

OBJECTIVE: Aim: Tarsal Tunnel Syndrome (TTS) is a rare compressive neuropathy of the posterior tibial nerve, often undiagnosed but particularly relevant to runners due to the unique biomechanical demands of the sport. Running places repetitive stress on the foot and ankle, increasing susceptibility to nerve compression within the tarsal tunnel. PATIENTS AND METHODS: Materials and Methods: This review examines the anatomical structure of the tarsal tunnel, the etiology, and epidemiology of TTS, and highlights risk factors such as excessive pronation, high arches, and prior ankle injuries. Tarsal Tunnel Syndrome Following a Total Knee Replacement.

Tarsal tunnel syndrome (TTS) is defined as an entrapment neuropathy from compression of the tibial nerve and/or distal terminal nerves in the tarsal tunnel, the medial foot, and/or ankle. While the incidence of TTS following a total knee replacement (TKR) could not be found, we present the case of a 78-year-old white female who developed symptoms of TTS 2 weeks after a left knee replacement. The history, examination, imaging, and electrodiagnostic (EDX) studies were consistent with the diagnosis of left-sided TTS. After a course of conservative care failed to alleviate her symptoms, the patien

The Utility of the Tinel Sign in the Diagnosis of High Tarsal Tunnel Syndrome.

BACKGROUND: Tarsal tunnel syndrome (TTS) can be divided into distal and proximal components. The latter is referred to as high TTS (HTTS) in which the tibial nerve is compressed above the location of the laciniate ligament. Although diagnosis of TTS has been shown to be enhanced by elicitation of a positive Tinel sign, no research into the utility of this provocation test has been conducted in the diagnosis of HTTS. This study aims to investigate the usefulness of the Tinel sign in diagnosing HTTS. METHODS: Seventy patients with electrophysiologic confirmation of HTTS were evaluated for the p

Baxter's nerve: the hidden culprit of chronic heel pain.

Baxter's nerve entrapment, an underrecognized cause of medial plantar heel pain, is frequently misdiagnosed as plantar fasciitis, leading to inadequate treatment. This condition involves compression of the first branch of the lateral plantar nerve, often resulting in chronic, burning pain with neurological symptoms. Despite accounting for up to 20% of chronic heel pain cases, clinical awareness remains low, delaying diagnosis and management. Differential diagnosis from plantar fasciitis, tarsal tunnel syndrome, and calcaneal stress fractures is crucial. High-resolution ultrasound, electromyogr

An Incidental Discovery of the Tibiocalcaneus Internus Muscle of Testut: An Exceedingly Rare Variation of the Soleus Muscle.

This report describes an incredibly rare variant of the soleus, the tibiocalcaneus internus muscle of Testut, observed during routine cadaveric dissection. In our cadaveric dissection, we observed this tibiocalcaneus internus muscle of Testut as an accessory muscle in the posterior compartment of the leg, which originates at the soleal line of the tibia and travels deep to the flexor retinaculum, ending in a tendinous insertion on the medial surface of the calcaneus, medial to the tendon of Achilles. Due to the few existing reports of this anomalous finding, any occurrence warrants reporting t

The Unusual Suspect-Tarsal Tunnel Syndrome Due to Flexor Digitorum Brevis Hypertrophy.

A rare case of tarsal tunnel syndrome caused by flexor digitorum brevis hypertrophy. The patient presented with medial plantar pain and paresthesia but denies any injury. Magnetic resonance imaging revealed a marked hypertrophy of the flexor digitorum brevis in close proximity to the medial and lateral plantar nerves, with herniation into the porta pedis. Surgical excision of the herniated muscle was performed, along with a tarsal tunnel release. The combined treatment resulted in symptomatic relief.

The Association between Body Mass Index, HbA1c Levels, and Tarsal Tunnel Syndrome: A Clinical and Neurophysiological Study.

Tarsal Tunnel Syndrome (TTS) is a focal compressive entrapment neuropathy affecting the posterior tibial nerve. While obesity and diabetes mellitus are known as systemic risk factors, their specific impact on TTS severity remains insufficiently investigated. We aimed to determine the association between body mass index (BMI), glycemic control (HbA1c level), and TTS severity. A cross-sectional study included 34 patients presented with signs and symptoms of TTS. For these patients, BMI was calculated, HbA1c levels were recorded, and clinical severity was evaluated using the Takakura scale and Mo

Tarsal tunnel syndrome: clinical insights, vascular etiologies, and the role of ultrasonography in the diagnosis.

Tarsal tunnel syndrome results from compression or damage to the tibial nerve or its branches as they pass through the tarsal tunnel beneath the flexor retinaculum. Diagnosing tarsal tunnel syndrome is challenging because of nonspecific symptoms that overlap with those of other lower limb pathologies. Vascular disorders are common causes but are often overlooked. High-resolution ultrasonography has emerged as a valuable diagnostic tool, offering advantages over other imaging methods. This technique allows real-time assessment, identifying the factors that cause vascular compression and improvi
