Eosinophilic granulomatosis with polyangiitis (EGPA) is a rare disease characterized by necrotizing vasculitis of small and medium-sized systemic blood vessels. The three phases to this disease are: 1) prodromal phase, which includes asthma, 2) eosinophilic phase, and 3) vasculitic phase. There is currently no diagnostic criterion for the diagnosis of EGPA, and diagnosis relies more on clinical features. On laboratory testing, peripheral eosinophilia is a hallmark of diagnosis. Treatment for new-onset antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis with organ or life-threateni

A 71-year-old Japanese woman was hospitalized for heart failure with Loeffler endocarditis, bronchial asthma, fever, and mononeuritis multiplex after receiving the fifth dose of the coronavirus disease 2019 (COVID-19) vaccine. Laboratory tests revealed eosinophilia and increased antineutrophil cytoplasmic antibody levels. The patient's condition improved after anticoagulant therapy. Loeffler endocarditis with antineutrophil cytoplasmic antibody-associated vasculitis may be associated with the COVID-19 vaccination.

A 37-year-old woman with Blau syndrome was extensively investigated for a rapidly progressive ascending sensorimotor polyneuropathy initially thought to be Guillain-Barré syndrome. The patient's symptoms progressed despite intravenous immunoglobulin therapy, with increasingly asymmetric involvement suggestive of mononeuritis multiplex, a pattern confirmed on nerve conduction studies. As her symptoms failed to respond to treatment, the question arose whether or not granulomatous inflammation in Blau syndrome (congenital autoinflammatory granulomatous polyarthritis, dermatitis, and uveitis) migh

BACKGROUND AND AIMS: Peripheral neuropathy may present with a variety of phenotypes depending on the pattern of weakness and sensory loss, the neurophysiological characteristics (axonal or demyelinating) and additional features such as involvement of the autonomic nervous system or the cranial nerves. The most common phenotype is a symmetrical length-dependent sensory and motor neuropathy. Other phenotypes include non-length-dependent forms such as chronic inflammatory demyelinating polyradiculoneuropathy (CIDP) or a sensory neuronopathy or ganglionopathy. Asymmetric forms of neuropathy are mo

OBJECTIVE: Antiphospholipid syndrome (APS) is a disease characterized by recurrent thrombosis in the presence of antiphospholipid antibodies. The most uncommon events described in the literature have been peripheral neurological disorders. This paper aims to systematically review the cases of peripheral neuropathy (PN) in APS patients.
METHODS: We systematically searched articles on PN and APS with English abstracts in PubMed from 1966 to August 2022.
RESULTS: We found 10 articles on PN and APS with 100 patients. Age varied from 25 to 78 years; 86-100% of patients in these studies were femal

We present a case of a 78-year-old female who developed persistent neurological complications following a concurrent dengue and chikungunya viral infection. The patient experienced progressive weakness and ambulatory difficulties, culminating in left foot drop. This case highlights the potential long-term neurological sequelae of these tropical viral infections and emphasizes the importance of considering a post-viral vasculitis-like presentation in patients with delayed neurological symptoms.

Rheumatoid vasculitis (RV) is an uncommon extra-articular manifestation of Rheumatoid Arthritis (RA) that typically affects patients with a more aggressive form of the disease. The diagnosis of rheumatoid vasculitis is suggested based on clinical presentation, laboratory findings, and confirmed through biopsy. We describe the case of a 28-year-old man who initially presented with myalgia and weakness in all four limbs. Subsequently, he developed hypertension and testicular tenderness. Neurological examination revealed painful sensory-motor neuropathy involving the bilateral median and ulnar ne

Rheumatoid vasculitis (RV) is a rare but severe complication of long-standing, seropositive rheumatoid arthritis (RA). We present a case of a woman in her 70s with a 32-year history of RA who developed life-threatening vascular complications, including digital ulcers, mononeuritis multiplex, deep vein thrombosis, pulmonary embolism and haemorrhagic shock due to multiple visceral artery aneurysms. CT angiography revealed active mesenteric bleeding, and angiography confirmed aneurysms in the celiac territory. She underwent successful coil embolisation and stabilisation through multidisciplinary

Polyneuropathies are common, can have very different clinical presentations and numerous causes. This article is intended to provide a practice-oriented overview of possible symptoms, the most important causes and a targeted, step-by-step diagnostic approach. Based on red flags, patients should be identified who urgently require a timely specialist assessment. The most important treatment approaches should be briefly explained.

Anti-synthetase syndrome is an autoimmune disorder associated with anti-aminoacyl-tRNA synthetase antibodies, including anti-Jo-1. However, antibody positivity alone is insufficient for the diagnosis because overlapping features with other diseases may lead to a misdiagnosis. We herein report a 69-year-old woman initially suspected of having anti-synthetase syndrome due to anti-Jo-1 antibody positivity and muscle weakness. Additional findings, including mononeuritis multiplex, purpura, crescentic glomerulonephritis, and strongly positive myeloperoxidase-antineutrophil cytoplasmic antibodies, l

Eosinophilic granulomatosis with polyangiitis (EGPA), known as Churg-Strauss syndrome, is a rare multisystem vasculitis mainly affecting small- to medium-sized vessels. It is characterized by eosinophil-rich granulomatous inflammation and necrotizing vasculitis. EGPA is considered a part of the ANCA-associated vasculitis (AAVs), but it also shares features with hypereosinophilic syndrome (HES), reflecting the dual contributions of vascular inflammation and eosinophilic tissue infiltration to organ damage. This case report describes a 65-year-old male presenting with progressive bilateral foot

Diabetes mellitus is a metabolic condition leading to elevated blood glucose levels due to insulin deficiency, insulin resistance, or a combination of both. Chronically raised blood glucose levels can lead to a broad variety of microvascular and macrovascular complications. Neurological disorders are a common manifestation of diabetes mellitus, and poorly controlled diabetes mellitus frequently causes peripheral sensorimotor polyneuropathy and autonomic neuropathy. More rare manifestations include diabetic amyotrophy and mononeuritis multiplex. Hemichorea-hemiballismus (HCHB) syndrome is a mov

Antineutrophil cytoplasmic antibodies (ANCA) associated vasculitis (AAA) is a systemic disorder involving small vessels. Neuropathy may be associated with the involvement of other organs such as the kidneys, lungs, and heart. ANCA is detected in only 80%-90% of cases; hence, high degree of clinical suspicion is required. We report a case of 49-year-old male patient who had a known case of type-2 Diabetes mellitus and chronic kidney disease on regular peritoneal dialysis. He developed progressive tingling and burning paresthesia and weakness in all four limbs. Examination revealed quadriparesis

An antineutrophil cytoplasmic antibody-associated small artery vasculitis that primarily affects the skin, lungs, and heart is called eosinophilic granulomatosis with polyangiitis (EGPA). It typically begins with initial respiratory symptoms like asthma and chronic sinusitis, and later progresses to systemic vasculitic issues that are linked to increased levels of eosinophils in the blood and tissues. While both central nervous system (CNS) and peripheral nervous systems can be affected, CNS involvement is rare. Nerve damage from vasculitis in EGPA usually appears as mononeuritis multiplex and

A 63-year-old female patient presented to the acute medical unit with what appeared to be a case of progressive mobility decline, a common and often nonspecific presentation in acute medicine. She described upper and lower limb weakness and numbness affecting her ability to walk. Originally, her upper limb numbness was treated as ulnar nerve compression in the community. She also had multiple episodes of shortness of breath throughout the year, which were initially managed in the community as suspected asthma exacerbations; however, her symptoms were non-specific. Routine investigations reveal

Immunoglobulin G4-related disease (IgG4-RD) neurological involvement typically presents as pachymeningitis. However, there have been reports of neuromuscular manifestations. To review the involvement of the peripheral nerves, neuromuscular junction, and muscles in IgG4-RD, we conducted a systematic review of case reports and case series of patients with IgG4-related disease (IgG4-RD) presenting with neuromuscular manifestations, using the PubMed/MEDLINE, Embase, and Scopus databases. Articles were analyzed for demographic characteristics, neurological presentations, systemic involvement, and i

A 74-year-old gentleman with a background of early stage melanoma presented with subacute onset of asymmetric sensorimotor symptoms, involving all four limbs. This had progressed over the preceding three-months - he had become severely debilitated and wheelchair-bound over this time. At presentation, there was leg weakness with profound bilateral foot drop, left worse than right, and milder proximal weakness of both hip flexors. There was also very severe weakness in both hands, and patchy upper and lower limb sensory loss. Initial symptom onset was approximately two-weeks after commencing adj

A 58-year-old woman was referred to our institution for infiltration shadows in the left upper lobe and a positive polymerase chain reaction for Mycobacterium tuberculosis (Mtb). She had worsening pain and sensory abnormalities in both lower legs. Red skin rashes were observed on both lower legs, which had resolved at the time of referral to our institution. She received oral prednisolone (PSL) monotherapy (10 mg/day) for these symptoms from her previous physician. She had a history of asthma and chronic rhinosinusitis. The nerve conduction studies and blood tests revealed mononeuritis multipl
