Re-evaluation of different electrophysiological criteria for Guillain-Barré syndrome in a single cohort from China.

BACKGROUND AND AIMS: We aimed to compare electrophysiological subtypes of Guillain-Barré syndrome (GBS) in a single cohort from northern China using three criteria (Ho's, Hadden's, Rajabally's), to clarify whether GBS subtypes are affected by electrophysiological criteria and nerve conduction studies (NCS) examination timing, to explore the value of sural sparing in subtype classification, and analyze the association between conduction block (CB) and short-term prognosis of Acute Motor Axonal Neuropathy (AMAN). METHODS: We retrospectively collected GBS patients hospitalized in the Department

Protective Efficacy of a Modified Vaccinia Ankara-Based Vaccine Against Zika.

Background: Zika virus (ZIKV) is a mosquito-borne flavivirus associated with severe neurological disease, including congenital Zika syndrome (CZS) following utero infection and Guillain-Barré syndrome in adults. The 2015-2016 epidemic in the Americas highlighted the profound maternal and neonatal consequences of ZIKV infection. Although reported transmission has declined, ongoing circulation of competent vectors and population susceptibility sustain a substantial risk of future outbreaks, underscoring the need for effective vaccines. Methods: We developed a recombinant Modified Vaccinia Ankara

Recent advances in drug discovery for Zika virus therapeutics.

Zika virus (ZIKV) is a mosquito-borne Orthoflavivirus that became a major global health concern following the 2015-2016 outbreaks in South America, where infection was linked to severe neurological outcomes, including microcephaly, congenital Zika syndrome (CZS), and Guillain-Barré syndrome (GBS). Although substantial efforts are underway to develop effective preventive measures, including vaccines and vector-control strategies, ZIKV remains a priority pathogen under the World Health Organization (WHO) R&D Blueprint due to its pandemic potential. This concern is intensified by the expansion of

ActIIR inhibition improves motor outcome and preserves muscle fibers after experimental autoimmune neuritis.

Secondary muscle atrophy resulting from neuronal denervation is a key determinant of poor functional recovery in immune-mediated neuropathies, even after inflammation is resolved. While current immunotherapies effectively limit immune-mediated nerve injury, secondary muscle atrophy as a downstream consequence of denervation is not addressed by existing treatment strategies. Activin type II receptors (ActIIR) are central regulators of skeletal muscle atrophy, and pharmacological inhibition of ActIIR signaling has shown variable efficacy in primary myopathies. Here, we investigated the therapeut

Zika virus infection preceding Guillain-Barré syndrome: Two cases from Southern India.

Guillain-Barré syndrome (GBS) is a leading cause of acute flaccid paralysis and has been associated with several arboviral infections. It is an acute immune-mediated polyradiculo-neuropathy characterised by rapidly progressive limb weakness, areflexia, and variable cranial nerve involvement. Antecedent infections are reported in the majority of patients, most commonly gastro-intestinal or respiratory. In recent years, arboviruses such as Dengue, Chikungunya, and Zika have emerged as important infectious triggers of GBS, particularly in tropical regions. Zika virus infection is often asymptomat

The EGR1/ZFP36 axis governs glycosphingolipid metabolic reprogramming in monocyte-derived macrophages in guillain-barré syndrome.

Guillain-Barré syndrome (GBS) is an immune-mediated demyelinating disorder of peripheral nerves with an unclear pathogenesis. This study integrated GBS clinical single-cell data with EAN model transcriptome data, establishing in vivo and in vitro experimental systems to reveal, for the first time, a novel mechanism involving EGR1-ZFP36 and its mediated metabolic reprogramming in GBS pathogenesis. Findings indicated that the transcription factor EGR1 and its predicted target gene ZFP36 were downregulated in both GBS patients and EAN rats. Molecular interaction validation confirmed that EGR1 dir

Methylprednisolone as an adjunct to intravenous immunoglobulin in pediatric Guillain-Barré syndrome: a prospective comparative study.

Guillain–Barré syndrome (GBS) is the most common cause of acute flaccid paralysis in children. Intravenous immunoglobulin (IVIG) is considered the standard therapy; however, the potential benefit of adjunctive corticosteroids remains uncertain. This study aimed to explore whether the addition of short-course intravenous methylprednisolone to IVIG may accelerate recovery and alleviate neuropathic pain in pediatric patients with GBS. A prospective, comparative study was conducted at Al-Azhar University Hospitals, enrolling 28 pediatric patients with clinically and electrophysiologically confirmed

Neuropsychiatric Sequelae of COVID-19: A Systematic Review of Acute and Prolonged Effects.

INTRODUCTION: Following the global emergence of SARS-CoV-2, it has become evident that COVID-19 can produce persistent, multisystemic symptoms known as "long COVID." Neurocognitive and neuropsychiatric symptoms are reported in approximately 40% of patients, often resulting in significant difficulties with daily functioning, workplace challenges, increased healthcare consumption, and a substantial economic burden. This condition is further linked to serious neurological complications, such as encephalitis, stroke, seizures, and Guillain-Barré syndrome. METHODS: This study comprised a comprehen

Guillain-Barré Syndrome Presenting With Unilateral Facial Nerve Palsy: A Rare Presentation.

Guillain-Barré syndrome (GBS) is an acute immune-mediated polyradiculoneuropathy typically presenting with symmetrical ascending limb weakness and areflexia. Cranial nerve involvement is recognized, most commonly affecting the facial nerves bilaterally. Unilateral facial nerve palsy, particularly in association with severe radicular lower back pain, is an atypical presentation and may lead to diagnostic uncertainty. We report the case of a 40-year-old woman with a background of papillary thyroid cancer who presented with severe lower back pain, progressive bilateral lower limb weakness, and ac

Cerebrospinal fluid alpha-internexin concentrations measured in patients with Guillain-Barré syndrome: a possible prognostic biomarker for disability at 12 months.

BACKGROUND: There is an unmet need for predictive biomarkers in Guillain-Barré syndrome (GBS). We aimed to determine whether cerebrospinal fluid (CSF) alpha-internexin (AINX) concentrations are associated with disease severity and outcome in GBS. METHODS: This retrospective cohort included 100 GBS patients. AINX concentrations were measured at diagnosis using a sensitive Simoa assay. Disease severity at nadir was assessed with the GBS Disability Scale (GBSDS) and the need for mechanical ventilation. Poor outcome was defined as GBSDS > 2 at 12 months. Logistic regression with log-transformed A

Guillain-Barré Syndrome: Progress in Diagnosis, Biomarkers, Neuroimaging and Management.

Guillain-Barré syndrome (GBS) is the leading cause of acute flaccid paralysis worldwide and represents a heterogenous spectrum of acute autoimmune polyradiculoneuropathies. Over the past decade, large international cohorts and advances in neuroimmunology have improved understanding of its clinical subtypes, infectious triggers, and pathophysiology. This review summarises recent progress across key aspects of diagnosis, prognostication, and treatment. We first describe updated perspectives of clinical heterogeneity including GBS variants and geographical variability, and the recognition of auto

Pharyngeal-Cervical-Brachial Variant of Guillain-Barré Syndrome: A Clinical Case Report.

Guillain-Barré syndrome (GBS) is an acute, immune-mediated polyradiculoneuropathy with several atypical variants. The pharyngeal-cervical-brachial (PCB) variant is a rare subtype characterized by rapidly progressive weakness of the oropharyngeal, neck, and upper limb muscles, often with relative sparing of the lower limbs. We present a patient who developed progressive upper-limb weakness and neck involvement without significant lower-limb deficit, clinically consistent with the PCB variant of GBS. Daily neurological assessments showed no improvement until the 3 rd day of admission, after whic

Persistent Müllerian Duct Syndrome on 18 F-FDG PET/CT.

A 32-year-old man presenting with limb weakness was diagnosed with Guillain-Barré syndrome. Abdominal distension was noted, and imaging revealed a large, heterogeneously enhancing abdominopelvic mass with bilateral cryptorchidism and uterine-like tissue. The mass showed high FDG uptake on 18 F-FDG PET/CT, while the uterine-like tissue did not. Following surgical biopsy, histopathologic examination demonstrated seminoma and rudimentary uterine tissue, confirming persistent Müllerian duct syndrome (PMDS). PMDS is a rare autosomal recessive disorder characterized by male phenotype with cryptorchi

Longitudinal Extension of the Win Odds for Ordinal Repeated Measurements.

Initially proposed for analyzing composite endpoints, the win odds have recently received increasing interest for the analysis of ordinal outcomes. When comparing an ordinal outcome between two groups, the win odds are the odds that a randomly selected subject from the first group has a better outcome than a randomly selected subject from the second group. As such, the win odds are an effect size that is closely related to the Mann-Whitney U test. The win odds can be adjusted for covariates by the probabilistic index model. Here, we aim to extend this model for repeated measurements. We modify

Time is brain: an expanded concept, not only for ischemic stroke A viewpoint of the Italian Association for Emergency Neurology (ANEU).

BACKGROUND: The concept of time is crucial in managing neurological diseases, as it includes both the natural history of a disease and the urgency of treatment. This paper explores the aphorism "Time is Brain," emphasizing the importance of timely interventions, not only in ischemic stroke, and its applicability to many other neurological conditions. METHODS: The review synthesizes existing literature on the implications of time in various neurological conditions, including ischemic stroke, intracerebral hemorrhage, status epilepticus, and Guillain-Barré syndrome. It evaluates how timely treat

The eye as a diagnostic key: the ophthalmologist's role in an atypical case of Miller Fisher Syndrome.

Miller Fisher syndrome (MFS) is an uncommon variant of Guillain-Barré syndrome characterized by the triad of ophthalmoplegia, ataxia, and areflexia. Diagnosis can be particularly challenging when the clinical presentation is incomplete or atypical, especially in early stages. We report the case of a 54-year-old woman who presented to the emergency department with blurred vision and nonspecific dizziness, without diplopia or other relevant neurological findings. Hospital discharge was initially considered. However, ophthalmologic examination revealed severe bilateral ophthalmoplegia with supran
