Clinical and quality-of-life outcomes associated with efgartigimod in patients with generalized myasthenia gravis transitioning from intravenous immunoglobulin.

BACKGROUND: Myasthenia gravis (MG) requires long-term therapeutic strategies that control symptoms while minimizing exposure to oral corticosteroids (OCS). Fast-acting treatments (FT), such as intravenous immunoglobulin (IVIG), are commonly used during exacerbations, whereas efgartigimod (EFG) has recently emerged as an alternative outpatient therapy. This study aimed to compare the efficacy and quality of life (QOL) outcomes of IVIG and EFG in patients with MG. METHODS: A retrospective analysis was conducted in 67 patients with MG. Fifty-five patients continued IVIG during exacerbations (IVI

Pearls & Oy-sters: Use of Short-Acting B-Agonist in DOK7-Related Congenital Myasthenic Syndrome Treatment.

Three brothers presented in early childhood with nonspecific symptoms progressive weakness since birth and fatigue over the course of their lives; 2 of them also had ptosis. An initial diagnosis of congenital muscular dystrophy was made based on muscle biopsy findings, but clinical review in adulthood and genetic testing led to a diagnosis of DOK7-related congenital myasthenic syndrome. A low-cost treatment with oral albuterol provided subjective and objective improvements in function. Reevaluation of remote diagnoses based on muscle biopsy findings in the setting of readily available genetic

Minimal Symptom Expression in Generalized Myasthenia Gravis: A Valuable Patient-Centric Treatment Goal.

INTRODUCTION: Generalized myasthenia gravis (gMG) is a chronic autoimmune neuromuscular disorder characterized by fluctuating muscle weakness and exertional fatigue, often requiring long-term immunosuppressive or biologic treatment. Although international guidelines recommend achieving minimal manifestations as the primary treatment goal, the lack of a standardized measurement approach has limited its application in clinical settings and trials. Minimal Symptom Expression (MSE), defined as a Myasthenia Gravis Activities of Daily Living (MG-ADL) score of 0 or 1, has recently emerged as a practi

Burden of glucocorticoid use in commercially insured adults with generalized myasthenia gravis in the United States: A retrospective claims-based analysis.

Generalized myasthenia gravis (gMG) is a chronic autoimmune condition characterized by muscle weakness. If frontline treatments inadequately manage symptoms, patients may require use of glucocorticoids (GC); however, long-term use of GCs is associated with toxicities. Few studies have examined the incidence of GC-related toxicities and economic burden among patients with gMG. This study aims to describe GC toxicities, healthcare resource utilization, and costs among US adults with gMG, stratified by level of GC use. Adults with gMG were identified using IQVIA PharMetrics® Plus data and classif

Direct AMPK Activation Confers Mutation-Independent Therapeutic Benefit in Duchenne Muscular Dystrophy.

BACKGROUND: Duchenne muscular dystrophy (DMD) is a severe, life-limiting neuromuscular disorder (NMD) characterized by progressive muscle wasting and mitochondrial dysfunction. Although gene therapies offer promise, even those already approved by regulatory agencies, their use remains constrained by mutation specificity, delivery challenges and durability. Pharmacologically targeting AMPK has shown potential to ameliorate dystrophic pathology, but prior strategies have been hindered by inadequate efficacy and off-target effects. METHODS: Comparative transcriptomic analyses were conducted to a

From Limited Samples to Mechanistic Insights: Exploratory Identification and Functional Validation of a hsa_circ_0062400/hsa_circ_0002397-miR-338-3p-NRP1 Axis in Myasthenia Gravis.

INTRODUCTION: CircRNAs are implicated in various autoimmune diseases, such as Myasthenia Gravis (MG), yet their regulatory mechanisms remain poorly understood. This research investigated the regulatory network of circRNAs, miRNAs, and mRNAs in modulating MG progression. MATERIALS AND METHODS: Three MG patients (male: female = 1:2) and three healthy controls were enrolled for microarray analysis. The “Limma” package was employed to identify the differentially expressed circRNAs (DECs). Target miRNAs of DECs were predicted via the CircInteractome database, while target genes of miRNAs were pred

Pediatric Myasthenia Gravis.

Pediatric myasthenia gravis encompasses a heterogenous group of disorders that cause impaired neuromuscular junction transmission. Patients exhibit a characteristic fatigable weakness that can be life-threatening if respiration is affected. In children, myasthenia gravis can be grouped into 3 subtypes: transient neonatal myasthenia (TNM) arising from transplacental antibody transfer, congenital myasthenic syndromes (CMS) related to genetic defects, and juvenile myasthenia gravis (JMG) caused by antibodies to synaptic components. Diagnostic evaluation combines clinical evaluation with serologic

The amino acid transporter LAT1 coordinates proper motor function at the perinatal stage.

L-type amino acid transporter 1 (LAT1, encoded by Slc7a5) contributes to amino acid homeostasis and signaling in numerous cell types. Several lines of evidence implicate LAT1 in mammalian central nervous system development, but its functional significance in specific neuronal subtypes is largely unknown. Here, we demonstrate that LAT1/Slc7a5 expression in synapsin 1 (Syn1)-expressing neurons is essential for motor circuit development and motor coordination at the perinatal stage. Mice lacking Slc7a5 in Syn1-expressing neurons exhibited progressive motor coordination deficits and early postnata

Half a century of change: demographic trends and their clinical impact in acetylcholine receptor antibody-positive myasthenia gravis.

BACKGROUND: The impact of myasthenia gravis (MG) changing epidemiology on disease management remains unclear. We investigated how MG subtypes changing over the last 50 years have affected disease prognosis. METHODS: Adult patients with MG onset between 1970 and 2020, followed up at our center for ≥ 2 years, were included in this retrospective study. Demographics, MG subtypes, and antibody (Ab) status over the last 50 years were assessed. Treatment response and adverse events (AEs) were evaluated in acetylcholine receptor (AChR)-MG patients with onset between 2000 and 2020. Treatment goal was

Assessing respiratory status in myasthenia gravis: limited value of the MG-ADL as a standalone tool compared with spirometry in a Danish cohort.

BACKGROUND: Patients with generalized Myasthenia Gravis (gMG) may develop respiratory involvement, which can be life-threatening. In clinical practice, the Myasthenia Gravis Activities of Daily Living scale (MG-ADL) is often used to assess patient-reported respiratory impairment, as spirometry can be time-consuming and require specialized equipment and expertise. However, it is unclear whether the MG-ADL is a sufficient screening tool for identifying respiratory insufficiency. METHODS: In this cross-sectional study with a longitudinal subgroup, patients completed the MG-ADL, the Myasthenia Gr

Engineered Bi-Specific AChR CAAR T Cells for Selective Elimination of Myasthenia Gravis B Cells.

Myasthenia gravis (MG) is an autoimmune neuromuscular disorder caused by autoantibodies targeting the nicotinic acetylcholine receptor (nAChR), which can lead to severe disability and life-threatening crises. Current therapies rely on broad immunosuppression and fail to achieve sustained remission in the majority of patients. Here, we report the development of AChR chimeric autoantibody receptor (CAAR) T cells engineered to selectively eliminate autoreactive B cells producing anti-AChR autoantibodies. T cells were co-transduced with CAARs expressing extracellular domains of the AChR α1 or β1 s

Selective Depletion of Autoreactive Plasma Cells as a Novel Strategy to Treat Acetylcholine Receptor Antibody-Positive Myasthenia Gravis.

Myasthenia gravis (MG) is a chronic autoimmune disease mediated by autoantibodies targeting the neuromuscular junction and leading to muscle weakness. Although there are autoantibodies of different specificities, most MG patients have autoantibodies directed against the nicotinic acetylcholine receptor (AChR), in particular against the extracellular domain of the α1 subunit (αECD), containing the main immunogenic region (MIR). Here, we demonstrate an original approach to selectively deplete plasma cells secreting autoantibodies targeting αECD. An antibody-mediated cytotoxicity-engager (ACE) co
