"A rough and swirling sea": Voicing Amyotrophic Lateral Sclerosis through discourse analysis.

Amyotrophic Lateral Sclerosis (ALS) is a rare, incurable neurodegenerative disease that leads to death within 5 years of diagnosis and greatly impacts patients' and caregivers' quality of life (QoL). Typically leading to death within 5 years, ALS underscores the need for emotional and psychological support. This study analyzes 118 testimonies from patients (n = 67), informal caregivers (n = 41), and formal caregivers (n = 10), with a gender-balanced sample (n = 67 women), using discourse analysis. Narratives reveal a complex emotional landscape of suffering and resilience, showing ALS as a dis

From theory to practice: physicians' knowledge, attitudes, and practices regarding amyotrophic lateral sclerosis in Egypt - a cross-sectional study.

BACKGROUND: Amyotrophic lateral sclerosis (ALS) is a progressive neurodegenerative disorder with heterogeneous presentations that often mimic other conditions, leading to frequent misdiagnosis and delayed recognition. Physician awareness and diagnostic confidence are critical for early detection, yet little is known about these factors in the Middle East and North Africa (MENA) region, where ALS services and specialized centers remain limited. METHODS: We conducted a nationwide cross-sectional survey of 1,320 physicians in Egypt. A validated online questionnaire assessed demographics, knowled

Emerging strategies for interpreting variants of uncertain significance (VUS) in amyotrophic lateral sclerosis.

As the use of genetic testing for neurological diseases increases exponentially, interpreting variants of uncertain significance (VUS) has become a challenging problem. Nowhere has this become more evident than in amyotrophic lateral sclerosis (ALS) and the frontotemporal dementias (FTDs), both of which are complex heterogeneous disorders in which an increasing array of disease-causative and modifying genetic variants are observed. Moreover, while traditionally identified as distinct clinical syndromes, ALS and FTD are increasingly recognized to exist along a spectrum of clinical syndromes (te

Strengthening care and research in ALS in Southeast Asia: a call for action.

In Southeast Asia (SEA), healthcare resources, infrastructure, and access to therapies in amyotrophic lateral sclerosis (ALS) remain limited compared to other parts of the Asia-Pacific region. Many SEA countries continue to face delays in diagnosis, fragmented care pathways, and limited representation in international research. Beyond health system challenges, ALS in SEA may also differ biologically. Emerging studies suggest that genetic variants and clinical phenotypes in Asian populations are not fully mirrored in Western cohorts. In this report, the authors summarize their country-specific

Oligogenic variants in NEK1 and ATXN2 in amyotrophic lateral sclerosis: report of two cases and review of the literature.

Amyotrophic lateral sclerosis (ALS) is a neurodegenerative disorder that affects the upper and lower motor neurons and leads to progressive paralysis. More than 40 genes have been implicated in familial ALS, which represents about 10% of ALS cases. Some genes, including C9orf72, SOD1, FUS and TARDBP are undoubtedly considered causative, but many others have uncertain pathogenicity and low penetrance. Here, we described the cases of two siblings affected by ALS and carrying both an ATXN2 heterozygous 32 CAG trinucleotide repeat expansion and a novel NEK1 heterozygous c.1674_1677dup. The segrega

The role of the thalamic paraventricular nucleus-prelimbic cortex pathway in diabetes-associated mechanical allodynia and thermal hyperalgesia.

Diabetic peripheral neuropathy (DPN) is a chronic complication resulting from diabetes that leads to pain and sensory abnormalities, significantly impacting the quality of life for affected patients. This study aims to investigate the role of glutamatergic neurons in the paraventricular thalamus (PVT) and the prelimbic cortex (PrL) in diabetes-induced nociceptive hypersensitivity. We established a diabetic mouse model through a single intraperitoneal injection of streptozotocin (STZ) and employed fiber photometry, optogenetics, and chemogenetics to examine the responses of glutamatergic neuron

SGK1-mediated deficits in microglial phagocytosis drive pathological progression in amyotrophic lateral sclerosis.

Alterations in microglial function and transcriptomic profiles are major pathological hallmarks of amyotrophic lateral sclerosis (ALS). However, the dynamics and regulatory mechanisms underlying microglial phagocytic activity during disease progression remain unclear. In this study, we observed stage-dependent alterations in microglial phagocytic activity during disease progression in SOD1G93A mice. Single-cell RNA sequencing suggested that this change was associated with a reduced abundance of microglial subpopulations enriched for phagocytosis-related pathways. Transcriptomic analysis identi

Clinical and non-clinical formulations of paclitaxel produce qualitatively and quantitatively different peripheral neuropathy in rats and mice.

Paclitaxel-induced peripheral neuropathy (PIPN) is a common side effect after treatment with paclitaxel, a chemotherapy medication used for solid-tumor cancers. Preclinical studies of PIPN using rodents have focused on male rats and demonstrated a high variability in behavioral outcomes. Here we report a simple, robust, reproducible PIPN protocol that is fast-acting, effective over the course of 3 weeks, and efficacious in both male and female cohorts of inbred mouse and outbred rat strains. Mechanoreceptive and thermoreceptive behavioral outcomes were measured using standard von Frey, Hargrea

Unraveling the role of lipid metabolism in ALS risk: a Mendelian randomization analysis using GWAS data.

BACKGROUND: Amyotrophic lateral sclerosis (ALS) is a progressive neurodegenerative disease. Lipid metabolism is closely related to neuronal function and energy homeostasis, but the genetic association between specific lipid species and ALS risk remains unclear. OBJECTIVE: This study aimed to investigate the potential causal associations between genetically predicted lipid species and ALS risk using a two-sample Mendelian randomization (MR) approach. METHODS: Summary-level GWAS data for 179 lipid species were obtained from 7,174 Finnish participants in the GeneRISK cohort. ALS GWAS data inclu

A Blood-Derived Factor Rescues ALS: Platelet Factor 4 Activates OPTN-Dependent Autophagy to Clear SOD1 Aggregates Independently of PINK1.

Peripheral factors that systemically regulate amyotrophic lateral sclerosis (ALS) have remained elusive-until now. Here, by integrating population-scale epidemiology with mechanistic dissection, we identify platelet factor 4 (PF4) as the central driver of a circulating neuroprotective axis that restores proteostasis and rescues ALS. In a prospective cohort of >500 000 UK Biobank participants, platelet indices were strongly associated with ALS risk, and serum PF4 levels were significantly reduced in ALS patients. Systemic administration of recombinant PF4 in hSOD1G93A mice produced dramatic the

Toward a Behavioral Reserve Model in Amyotrophic Lateral Sclerosis.

OBJECTIVE: Behavioral impairment is common in amyotrophic lateral sclerosis (ALS) and strongly affects autonomy, caregiver burden, and outcomes, yet predictors of vulnerability remain unclear. We investigated whether premorbid regulatory traits and socio-educational exposures are associated with behavioral phenotypes in ALS within a behavioral reserve framework. METHODS: We analyzed 965 consecutively assessed patients with ALS from a prospective tertiary-center cohort. Behavioral impairment was measured using the Frontal Systems Behavior Scale (FrSBe; family-rated before and after onset) and,

Cardiac Autonomic Neuropathy in Children and Youth with Type 1 Diabetes: A Pilot Study.

This study compared the cardiac autonomic function in 30 children and youth with type 1 diabetes (T1D) with 30 age- and sex-matched healthy siblings using Cardiovascular Autonomic Reflex Tests (CARTs). The T1D group showed consistently poorer performance across all parameters, indicating reduced autonomic function. The 30:15 ratio was significantly lower in the T1D group (P = 0.027), suggesting impaired heart rate response to postural change. Cardiac autonomic neuropathy (CAN) was identified in 30% of cases. These findings underscore the importance of early CAN screening in young individuals w

Genotype-phenotype relationship in a cohort of 131 Chinese patients with myotonic dystrophy type 1.

INTRODUCTION: Myotonic dystrophy type 1 (DM1) is an autosomal dominant dynamic mutation disorder characterized by myotonia and muscle weakness. The genotype-phenotype relationship has not been clearly defined in Chinese patients. METHODS: The clinical and genetic characteristics of 131 patients from 105 pedigrees were retrospectively analyzed. Triplet-primed Polymerase Chain Reaction (PCR) and Flanking PCR were performed to analyze the CTG repeats in DMPK gene. RESULTS: Among 131 DM1 patients, 64.9% were male, indicating a higher male admission rate, possibly due to limited diagnostic awaren
