A Systematic Review of Management of Cramping Pain in Patients with Amyotrophic Lateral Sclerosis.

INTRODUCTION: Pain, particularly cramping, in people living with amyotrophic lateral sclerosis (ALS) is often underrecognized and under-treated. Despite affecting over 70% of people living with ALS (plwALS), cramping pain remains inadequately managed due to its complex nature and the difficulties plwALS face in communicating their symptoms as the disease progresses. This systematic review explores both pharmacological and non-pharmacological treatments for cramping pain in ALS, aiming to assess and compare their efficacy. METHODS: The systematic review was conducted following PRISMA guideline

New SMA era: A broad-range tiered assessment of function for the evolving SMA phenotype (EVOLVE-SMA).

BACKGROUND: The clinical course of Spinal Muscular Atrophy (SMA) has been transformed by new disease-modifying therapies (DMT). Before DMT, the clinical course of SMA was marked by an inflection point between developmental improvement and degenerative functional losses. The well-established SMA classification by "type" was based upon the age and functional level at which this inflection point manifested. Following DMT, the natural history of SMA has evolved. The evolution of the SMA clinical course following DMT thus necessitates an updated means to characterize individuals with SMA. OBJECTIV

Portuguese Consensus Guidelines for the Diagnosis and Treatment of Myasthenia Gravis.

Myasthenia gravis is an autoimmune disease that affects the neuromuscular junction, mainly through the action of pathogenic antibodies such as those directed against the nicotinic acetylcholine receptor or, more infrequently, against the muscle specific kinase. Other important components of the autoimmune process include the complement pathway and B and T cell populations. Diagnosis is based on the finding of fatigable muscle weakness, which can affect several muscle groups, either in isolation or in variable combinations. Neurophysiological techniques and/or the detection of pathogenic antibo

Perioperative Management of Minimally Invasive Thymectomy in a Pediatric Patient With Myasthenia Gravis After Respiratory Crisis: A Case Report.

We report the anesthetic management of a 10-year-old girl with treatment-refractory, acetylcholine receptor antibody-positive juvenile myasthenia gravis (JMG) undergoing minimally invasive thymectomy after a recent myasthenic crisis. Anesthetic planning focused on avoiding neuromuscular blockers, minimizing stress, and preserving respiratory function. Paravertebral blocks with intercostal cryoablation provided postoperative analgesia-an approach not previously reported in pediatric JMG. This case illustrates the complexity of anesthetic care in JMG and the value of individualized, multidiscipl

Novel muscle MRI features in Desmin related myasthenic myopathy.

Primary desminopathies often present as myofibrillar myopathy with predominant dominant inheritance. There are only few reports of recessive desminopathies. Distinct muscle MRI features with predominant involvement of gluteus maximus, semitendinosus, sartorius, gracilis and peroneal muscles have been described in dominant desminopathies. We report five patients with recessive desminopathies carrying novel homozygous DES variants (c.1023+5G>A and c.958delG). All presented with features of congenital myasthenic syndrome since early childhood. Novel MRI features with fatty infiltration of gluteus

Myosteatosis and Type 2 Diabetes Mellitus.

Myosteatosis refers to the infiltration of fat into skeletal muscle tissue, being influenced by factors such as advanced age and overweight, which increase the inability of adipocytes to store lipids. This condition not only alters the structure of the muscle but is also associated with endocrinological imbalances such as insulin resistance (IR) and hyperinsulinemia, increasing the risk of developing type 2 diabetes mellitus (T2DM) and cardiovascular diseases. Computed Tomography (CT) and Magnetic Resonance Imaging (MRI) are effective methods for measuring myosteatosis, identifying areas of fa

Physical activity and its effects in myasthenia gravis: a patient-reported study on habits and impact.

BACKGROUND: Physical activity and exercise have been classified as safe and effective in mild to moderate cases of myasthenia gravis (MG) across various studies. Subsequently, adequate physical activity is generally recommended. Nevertheless, individuals with MG remain less physically active than the general population, without a precise definition of the low-activity group so far. METHODS: In this prospective single-center study, individuals with MG completed a questionnaire assessing general mobility, weekly physical activity levels, and beliefs toward specific statements about physical exe

Estrogen-Related Receptor Alpha Promotes Skeletal Muscle Regeneration and Mitigates Muscular Dystrophy.

Skeletal muscle regeneration in chronic muscle diseases such as Duchenne Muscular Dystrophy (DMD) has remained clinically unsurmountable. Estrogen-related receptor alpha (ERRα) plays a critical role in adult skeletal muscle metabolism and exercise fitness. Whether ERRα activation can drive muscle regeneration and mitigate dystrophy in DMD is not known. We have investigated ERRα signaling in pre-clinical models of acute muscle injury and DMD. ERRα is induced in differentiating C2C12 myoblast and regenerating muscle. ERRα silencing suppressed proliferation and differentiation in C2C12 myoblasts.

Humoral correlates of clinical response to thymectomy in myasthenia gravis.

Mechanisms by which thymectomy confers its clinical benefit in patients with AChR-antibody (Ab) positive myasthenia gravis (MG) remain poorly understood. We used a systems-level approach combined with high-dimensional characterization of Ab-associated immune features to identify Ab-features that track with clinical response to thymectomy in 78 patients with AChR-Ab positive MG, recruited during the MGTX trial, an NIH-sponsored randomized, controlled study of thymectomy plus prednisone versus prednisone alone. Clinically meaningful improvement was defined as a change of at least 3 points on the

Eculizumab as a new option for perioperative treatment in thymoma-associated myasthenia gravis.

The efficacy of eculizumab as a perioperative treatment in patients with thymoma-associated myasthenia gravis (TAMG) has not been evaluated. This prospective study included patients with TAMG who received standard-of-care therapy (Soc group, n = 20) or combined with eculizumab (Ecu group, n = 13). At 1 week, the Ecu group showed a marked improvement compared to the Soc group, as evidenced by a reduction in Quantitative Myasthenia Gravis (QMG) (7.39 ± 3.47 vs 0.25 ± 5.13, P = 0.0001), Manual Muscle Test (MMT) (6.39 ± 4.66 vs 0.15 ± 3.96, P = 0.0003), and MG Activities of Daily Living (MG-ADL) (

In-Depth Profiling Highlights the Effect of Efgartigimod on Peripheral Innate and Adaptive Immune Cells in Myasthenia Gravis.

BACKGROUND: Myasthenia gravis (MG) is an autoimmune disorder characterized by antibody-mediated complement activation. Efgartigimod, a neonatal Fc receptor (FcRn) antagonist, is approved for treating generalized MG (gMG). However, its modulatory effects on upstream innate and adaptive immune cells remain largely unexplored. METHODS: We first quantified FcRn expression in peripheral immune subsets from acetylcholine receptor antibodies (AChR-Ab) positive gMG. Then we assessed efgartigimod's impact on immune cells via in-depth profiling in vitro and later validated these findings in a prospecti

Body weight distribution of US patients with myasthenia gravis.

OBJECTIVE: Several novel therapies have been approved for treatment of generalized myasthenia gravis (gMG). The dosing requirements of certain therapies (efgartigimod intravenous, nipocalimab, ravulizumab, rozanolixizumab, zilucoplan) are based on patient's body weight. To determine costs of weight-based gMG treatments to payers, accurate assessment of the body weight distribution of patients with MG is required. However, little is known about these patients' weight profile. The current study aims to provide robust weight data in multiple cohorts of patients with MG and elucidate the cost of g

Remote Monitoring in Myasthenia Gravis: Exploring Symptom Variability.

BACKGROUND: Myasthenia gravis (MG) is a rare, autoimmune disorder characterized by fluctuating muscle weakness and potential life-threatening crises. While continuous specialized care is essential, access barriers often delay timely interventions. To address this, we developed MyaLink, a telemedical platform for MG patients. This study evaluated whether frequently assessed clinical outcomes via MyaLink can capture symptom variability between clinical visits. METHODS: In this randomized, controlled, 12-week study, 45 MG patients received either standard care (control, N = 15) or standard care

Integrative Transcriptomic and Network-Based Analysis of Neuromuscular Diseases.

Neuromuscular diseases (NMDs) like Duchenne muscular dystrophy (DMD), limb-girdle muscular dystrophy (LGMD), and amyotrophic lateral sclerosis (ALS) are rare, progressive disorders with complex molecular mechanisms. Traditional transcriptomic analyses often struggle to capture systems-level dysregulation, especially given the small sample sizes typical of rare disease studies. Our differential expression analysis of eight public RNA-seq datasets from various cell types in DMD, LGMD, and ALS revealed not only disease-relevant pathways but also unexpected enrichments, such as renal development,

The Disparities in Myasthenia Gravis Clinical Trial Enrollment in the United States and Canada.

INTRODUCTION/AIMS: The demographic and geographic representation of participants in myasthenia gravis (MG) trials has yet to be systematically reviewed. The goal of this study was to explore potential disparities in MG interventional clinical trial enrollment. METHODS: We included completed interventional clinical trials from January 2002 to December 2021 that enrolled participants with MG within the United States and Canada. Twenty-eight trials meeting these criteria were identified at Clinicaltrials.gov, and 16 trials contributed data. Study sponsors provided data for age, sex/gender, race,

Exploring psychosocial experiences in clinically stable generalised Myasthenia Gravis: A thematic analysis.

Myasthenia Gravis (MG) is a chronic autoimmune neuromuscular condition that significantly impacts patients' lives. Whilst psychosocial challenges are increasingly recognised as important in understanding the lived experience of patients, insight into these experiences during periods of clinical stability remains underexplored. This qualitative study explores the psychological and social aspects of living with MG through thematic analysis of semi-structured interviews with eight adults from a specialist MG care clinic in London, all considered clinically stable for at least three months. Learnings from a registry-based cohort study for spinal muscular atrophy disease.

Spinal muscular atrophy is a rare recessive progressive neurodegenerative disorder. To better understand the progression of spinal muscular atrophy the European Medicines Agency launched a study based on registry data. This manuscript describes some lessons learnt and considerations how to improve future registry-based studies that aim to inform regulatory decision-making. The study started with a feasibility assessment to select appropriate registries from the TREAT-NMD network. The feasibility assessment is key to understand upfront the quality, capability and capacity of registries to addre
