BACKGROUND AND OBJECTIVES: Newborn screening (NBS) for spinal muscular atrophy (SMA) and reimbursement of disease-modifying therapies (DMTs) are managed provincially in Canada, creating inequities in diagnosis and treatment. We describe NBS methodology, reporting practices, and DMT reimbursement across Canada and report the first national birth prevalence of SMA.
METHODS: We conducted a cross-sectional survey of laboratory directors from each provincial NBS program, validated against publicly available government sources (censored December 31, 2025). Birth prevalence was calculated using a Po

PURPOSE OF REVIEW: Living donor liver transplantation (LDLT) increasingly depends on biologically related donors, particularly in Asia and the Middle East, where deceased donation remains limited. This review evaluates genetic risks in living donors and proposes a system-based approach to donor selection.
RECENT FINDINGS: Heterozygous carriers exhibit variable functional capacity determined by enzyme thresholds and epigenetic mechanisms such as X-chromosome lyonization. In ornithine transcarbamylase deficiency (OTCD), maternal donors may have insufficient enzymatic activity, resulting in dono

BACKGROUND: Neuromuscular diseases (NMDs) are frequently complicated by progressive cardiomyopathy leading to advanced heart failure. Ventricular assist device (VAD) implantation has emerged as a potential option in selected pediatric and young adult patients, but evidence on indications and outcomes is limited. We aimed to systematically review evidence on VAD implantation in children and young adults with NMDs, focusing on device strategies and clinical outcomes.
METHODS: Systematic review following Cochrane methodology and reported per the PRISMA guidelines. Six major biomedical databases

Cell-based regenerative therapies represent a promising platform for muscular dystrophies, but the role of histocompatibility for muscle engraftment remains poorly defined. Given their close genetic relationship to humans, nonhuman primate (NHP) models provide an ideal system to investigate this question. Here, we generated myogenic progenitors from induced pluripotent stem cell (iPSC) lines derived from five major histocompatibility complex (MHC)-typed Mauritian-origin cynomolgus macaques (MCMs). Following extensive characterization, one iPSC clone from each donor was selected for transplanta

INTRODUCTION: Facioscapulohumeral muscular dystrophy (FSHD) is a hereditary muscular dystrophy with a highly variable and slowly progressing disease course. This variable natural history complicates the identification of biomarkers of disease progression and treatment effects, while there is a growing need for measures which can capture changes within a trial timeframe.
METHODS AND ANALYSIS: This study presents the protocol of the MRI UltraSound Comparison Load Evaluation study, a prospective observational muscle imaging biomarker study where Dutch adult patients with FSHD type 1 or type 2 (n

Duchenne muscular dystrophy (DMD) involves chronic degeneration and regeneration, causing dystrophic muscle stem cells (MuSCs) to differ from uninjured wild-type tissue. Conventional disease-versus-resting-control comparisons may therefore conflate dysregulation with regeneration. We developed a physiological reference-mapping framework integrating normal regeneration trajectories with replicated dystrophic datasets to estimate disease-associated deviations relative to regenerative state and spatial context. We applied it across single-cell/single-nucleus, bulk, and spatial RNA-sequencing data

Pathogenic, bi-allelic variants in RMND1 typically manifest as an early-onset multisystemic mitochondrial disease, yet the consequences of RMND1 dysfunction in vivo remain poorly understood. To answer this question, we generated and characterized the first in vivo model of RMND1-related disease, carrying the recurrent p.(Asn238Ser) variant. Homozygous Rmnd1N238S mice exhibit growth defect, premature mortality, and a multisystem phenotype marked by widespread tissue-dependent defects in oxidative phosphorylation assembly and activity. Quantitative proteomic analysis of Rmnd1N238S heart revealed

How mechanical stimuli cross the nuclear envelope and are converted into nuclear structural and transcriptional responses is a central question in understanding cellular mechanical adaptation and its failure. The LINC complex is composed of inner nuclear membrane SUN proteins and outer nuclear membrane KASH/Nesprin proteins, and establishes a continuous transnuclear mechanical pathway through its connections with the nuclear lamina and the cytoskeleton. Current evidence indicates that the LINC complex is not a passive molecular bridge. Instead, through SUN-KASH pairing, Nesprin isoforms and pe

Sarcopenia, the age-related loss of skeletal muscle mass and function, is a major driver of frailty in older adults, yet its molecular basis remains incompletely defined. Ferroptosis, an iron-dependent regulated cell death, has emerged as a link between iron dysregulation and muscle wasting in aging. However, ferroptosis-related signatures and their interplay with immune remodeling in degenerating muscle remain uncharacterized. DMD shares selected molecular and pathological features with aging-related muscle degeneration, including iron overload, oxidative stress, and impaired regenerative cap

BACKGROUND: There are no approved exon skipping therapies to treat Duchenne muscular dystrophy due to DMD variants amenable to exon 44 skipping (DMD44). Delpacibart zotadirsen (del-zota, also known as AOC 1044) is a first-in-class antibody-oligonucleotide conjugate designed to deliver an exon 44-skipping phosphorodiamidate morpholino oligomer to muscle to restore dystrophin expression for individuals with DMD44. EXPLORE44 was a first-in-human trial to evaluate del-zota's safety, tolerability, pharmacokinetics, pharmacodynamics, and efficacy.
METHODS: EXPLORE44 was a two-part randomised, doubl

In vitro cell culture models of human skeletal muscle offer a valuable platform for studies of pathology and discovery of new therapeutics. However, traditional two-dimensional cell culture models suffer from limited durability and challenges with assessment of skeletal muscle contractile function. Furthermore, finite proliferation potential and scarce availability of patient-derived primary myoblasts limit experimental throughput when using these cells. Here, we describe a method for using human induced pluripotent stem cells (hiPSCs) to generate functional three-dimensional skeletal muscle t

Adult skeletal muscle can regenerate. After muscle injury, muscle stem cells (satellite cells) can generate new muscle fibers. Under normal conditions, satellite cells are mitotically quiescent and located beneath the basal lamina of muscle fibers. In response to regenerative stimuli, satellite cells activate and proliferate as myogenic precursor cells. After several rounds of cell division, these precursor cells differentiate and fuse to form new muscle fibers. A subset of proliferating myogenic precursor cells returns to a quiescent state to replenish the satellite cell pool through self-ren

Calpain-3 (CAPN3) is a Ca2+-dependent cysteine protease predominantly expressed in skeletal muscles. Mutations in CAPN3 are associated with limb girdle muscular dystrophy R1 (LGMDR1), an autosomal recessive muscular dystrophy. CAPN3 is uniquely activated by rapid autolytic processing; however, the mechanisms regulating this activation are poorly understood. Here we show that phosphorylation regulates CAPN3 autolytic processing in living cells. We found that ouabain, a cardiac glycoside, activated ERK, which induced the phosphorylation of CAPN3 at S19 located within its N-terminal region. This

Oculopharyngeal muscular dystrophy (OPMD) is a rare, late-onset, autosomal dominant muscular dystrophy caused by a trinucleotide GCN repeat expansion in the PABPN1 gene on chromosome 14q11.2. The disease is clinically characterised by progressive ptosis, dysphagia, and proximal limb weakness, with onset typically between the fifth and sixth decades. OPMD affects approximately 1:100,000 individuals in Europe, with substantially higher prevalence in specific founder populations, reaching 1:600 among Bukharan Jews and 1:1,000 in French Canadians. The expanded polyalanine tract in PABPN1 promotes

Duchenne muscular dystrophy (DMD), a rare X-linked disorder caused by dystrophin mutations, leads to progressive muscle degeneration and cardiomyopathy. We previously showed elevated DNA damage and mutagenesis in DMD pluripotent stem cells, due to nitric oxide synthase (NOS) dysregulation and reactive species. Here we examined how these disruptions influence cardiac tissue development. In developing cardiac organoids, the DMD cardiovascular progenitor (CP) population shows a premature, elevated activation of CP markers, followed by early maturation gene expression and reduced proliferation. In regions with high consanguinity, rare recessive disorders like Ullrich congenital muscular dystrophy (UCMD) may present at birth with hypotonia and respiratory distress. To enhance quality of life and prevent recurrence in future pregnancies, early recognition, genetic counseling, and care from a multidisciplinary team of specialists are essential.

Dysferlinopathy (Limb Girdle Muscular Dystrophy R2/LGMD2B) causes progressive weakness of limb-girdle muscles. Although gait abnormalities are common, the contribution of spinal muscles to disease symptoms remains poorly understood. Using cross-sectional MRI in patients and longitudinal analyses in a dysferlinopathic mouse model, we identify early-onset, progressive adipogenic degeneration of spinal muscles as a previously unrecognized hallmark of dysferlinopathy. Multiple spinal muscles, including the iliocostalis, longissimus, and spinalis, are affected, with degeneration preceding pathology
