Malignant hyperthermia (MH) is a rare, life-threatening inherited disorder triggered by volatile inhalational anesthetics and/or the depolarizing muscle relaxant suxamethonium. In susceptible individuals, calcium release from the sarcoplasmic reticulum in the skeletal muscle becomes abnormally accelerated, leading to a hypermetabolic state. Early signs of MH include unexplained hypercarbia (end-tidal carbon dioxide > 55 mm Hg), tachycardia, and muscle rigidity, particularly in the masseter. Rapid increases in core temperature (> 0.5 °C/15 min, with temperatures often exceeding 40 °C) are typic

Proteostasis maintains the balance between protein synthesis, folding, and degradation within the endoplasmic reticulum (ER). This quality-control system ensures that proteins undergo proper post-translational modifications-such as PDI-ERO1-mediated oxidative folding and STT3-dependent N-glycosylation-so that only correctly folded proteins proceed through the secretory pathway. Impairment of protein load, folding capacity, or degradation via the ER-associated degradation (ERAD) pathway leads to the accumulation of unfolded proteins, triggering ER stress and activating the unfolded protein resp

This update begins with a discussion of treatable lipid storage myopathies associated with the use of sertraline and ranolazine. We then turn to monoclonal gammopathy-associated myopathies including a more recently identified disorder that is also treatment-responsive, monoclonal gammopathy-associated glycogen storage myopathy. Several reports on statins and muscle symptoms, toxic myopathy, and immune-mediated necrotizing myopathy follow. Other autoimmune myopathy topics are highlighted. Then, muscular dystrophies are covered with a focus on dystrophinopathies, limb-girdle muscular dystrophy,

Muscle biopsy has long been regarded as the cornerstone for diagnosing pediatric muscular disorders; however, it is invasive and may be limited by sampling error and inconclusive histopathological findings. This study aimed to evaluate whether whole-exome sequencing (WES) can effectively replace muscle biopsy as a first-line diagnostic approach in children with suspected neuromuscular disorders. Between January 2018 and December 2025, we prospectively enrolled 47 pediatric patients presenting with clinical features suggestive of muscular disorders at a tertiary medical center in Taiwan. BACKGROUND AND OBJECTIVES: Congenital myopathies (CMYOs) and congenital muscular dystrophies (CMDs) are rare, clinically and genetically heterogeneous neuromuscular conditions characterized by muscle weakness, usually with onset at birth or in the first few months of life. Next-generation sequencing (NGS) has significantly enhanced diagnostic capabilities and transformed the diagnostic process for such rare conditions. The aim of this study was to describe the outcomes of NGS analysis and genotypic prevalence among patients with CMYO and CMD referred for diagnostic assessment to the National H

BACKGROUND: Myosin heavy chain (MyHC)-related congenital myopathies display variable age of onset and clinical manifestations depending on the mutated isoform. Cardiomyopathy, ophthalmoplegia and primarily proximal weakness may be part of the clinical picture. CASE PRESENTATION: A 57-year-old male patient with a history of arterial hypertension and hyperferritinemia (thalassemic trait) began to experience lower limb proximal weakness at the age of 23 years and he got progressively worse over the years when he also reported mild dyspnea, easy fatigability, fasciculations and a sort of discomfor

INTRODUCTION: Fetal akinesia deformation sequence (FADS), a severe prenatal phenotype associated with congenital myopathies (CMs), is linked to mutations in the RYR1 gene. Although whole exome sequencing (WES) is a standard diagnostic approach, certain pathogenic variants may remain undetected. In this study, we utilized whole genome sequencing (WGS) to investigate a recurrent case of FADS with negative WES results.
METHODS: A couple who experienced four consecutive pregnancy losses, including two fetuses affected by FADS, underwent trio-WGS following negative WES findings. Variants were anno

This review highlights the emerging biochemistry and biology of the metazoan tRNA-ligase complex (tRNA-LC). We begin with an overview of the cleavage-ligation pathways dependent on the tRNA-LC, epitomised by the essential process of pre-tRNA splicing, but also critical for the unfolded protein response and the struggle against transposons. The catalytic core of the tRNA-LC is the non-conventional RNA ligase RTCB, found in all domains of life. We start from the universal principles of its catalytic cycle, entailing GTP-dependent ligation of RNA molecules with 2’,3’-cyclic phosphate and 5’-hydro

INTRODUCTION/AIMS: Respiratory muscle dysfunction is prevalent in congenital myopathies and often leads to respiratory failure. Ultrasound shear wave elastography (SWE) measures tissue elasticity and has been proposed as an alternative to invasive measures of respiratory muscle contractility. This study aims to evaluate SWE to assess respiratory muscle function in healthy individuals and congenital myopathy patients, while also determining SWE reliability.
METHODS: Shear wave velocity in the diaphragm, parasternal intercostal, sternocleidomastoid, external oblique, internal oblique, and trans

INTRODUCTION/AIMS: Data on respiratory, feeding, ambulatory outcomes and prognostic factors for congenital myopathies (CM) and congenital muscular dystrophies (CMD) remain limited. Therefore, in this study, we report the characteristics of a large single-center cohort of patients with CM and CMD, focusing on long-term outcomes and aiming to identify factors associated with invasive ventilation, feeding tube, and wheelchair dependence.
METHODS: Patients with a diagnosis of CM or CMD evaluated at Children's Health Dallas between 2011 and 2024 were identified using ICD codes and their medical re

Venous thromboembolism (VTE), comprising deep vein thrombosis and pulmonary embolism, is an important but under-recognised complication in neuromuscular diseases. In adults, emerging epidemiological data suggests increased VTE occurrence in conditions such as Amyotrophic Lateral Sclerosis, myotonic dystrophy, myasthenia gravis, inflammatory neuropathies, inflammatory myopathies, and POEMS syndrome. This heightened risk reflects not only disease-related immobility but also disorder-specific biological mechanisms, including inflammation, endothelial dysfunction and cardiomyopathy-related stasis.

OBJECTIVE: To establish a normal reference interval for the mid-thigh lean mass (MTLM) and mid-thigh soft tissue (MTST) ratio (LMSTr) between 20 and 32 weeks of gestation in a Chinese cohort and to report the association between the abnormal LMSTr and congenital myopathies.
METHOD: Pregnant women with a singleton non anomalous pregnancy were prospectively recruited and underwent a normal scan at 20-32 weeks. We performed an inter- and intra-observer variability assessment in the measurement of MTLM and MTST in 50 patients. Reference ranges for LMSTr were calculated in the normal fetuses. The

Pathogenic RYR1 variants are associated with a set of rare neuromuscular disorders termed RYR1-related disorders (RYR1-RD). Clinical manifestations of RYR1-RD include proximal/axial muscle weakness, delayed motor milestones, impaired mobility, muscle pain, and fatigue. Muscle-specific microRNAs (miRNAs) are mostly expressed in muscle tissue and can be detected peripherally in plasma. Using a digital detection system, here we identified and quantified differential amounts of miRNAs in six adult (four monoallelic and two biallelic) RYR1-RD patient plasma samples compared to controls. Overall, 51

Missense mutations in the TPM2 gene encoding skeletal muscle tropomyosin Tpm2.2 cause congenital myopathies associated with hyper- and hypocontractile phenotypes. Mutation-dependent defects in thin filament stability and length maintenance may contribute to sarcomere dysfunction. To address this possibility, four disease-associated substitutions in Tpm2.2 were analyzed: hypercontractile D20H and E181K, and hypocontractile E41K and N202K. Recombinant proteins were examined in vitro for their effects on actin filament polymerization, stability, and cofilin-2-dependent filament length regulation

Arthrogryposis multiplex congenita (AMC) is a diverse group of conditions characterized by multiple joint contractures. Although individually rare, these disorders are estimated to affect 1 in 3000-5000 live births. Their common pathophysiological mechanism is fetal akinesia, a sustained reduction of fetal movement that may arise from intrinsic disturbances-such as central nervous system malformations, motor neuronopathies, neuropathies, neuromuscular junction defects, congenital myopathies, muscular dystrophies, or metabolic diseases-or from extrinsic factors including uterine constraint, mat
