Magnetic resonance of diseased skeletal muscle: combined T1 measurement and chemical shift imaging.

Magnetic resonance examinations of skeletal muscle with differential T1 relaxation time measurements were performed in 19 patients with muscular dystrophies and congenital myopathies, and in eight control subjects. A field echo chemical shift imaging technique was used. T1 values of muscular tissue were measured from the primary composite images, and differential T1 values were calculated separately from water and fat images. Longitudinal relaxation times of skeletal muscle were significantly increased in both dystrophies and myopathies. The results of differential relaxation time measurements

Fiber density in congenital muscle fiber type disproportion. I. Congenital myopathies.

In 15 cases with congenital muscle fiber type disproportion, concentric needle EMG (CNEMG) and single fiber EMG (SFEMG) with fiber density (FD) estimation was performed. The aim of the work was to establish the nature of congenital muscle fiber type disproportion. In 11 cases CNEMG revealed interference pattern on maximal effort and low, polyphasic motor unit action potentials (MUAPs) of short duration indicating a myopathic lesion. In four other cases electromyographic changes were less evident. The FD values were slightly increased in 9 cases. This finding could be explained by pronounced mu

Congenital myopathy and cardiomyopathy with identical ultrastructural changes.

A 7-day-old girl with congenital hypotonia and unexplained episodes of bradycardia had a broad spectrum of similar skeletal muscle and myocardial degenerative ultrastructural abnormalities. Ultrastructural studies showed obliteration of cross striations, myofilament disorganization, streaming, smearing, clumping, and zigzag Z-band deformities. A decrease in glycogen, mitochondria, and T-tubular system occurred in the regions showing Z-band abnormalities of both skeletal muscle and myocardium. Concurrent structural cardiomyopathy should be considered in patients with congenital myopathies, part

Magnetic resonance imaging of primary skeletal muscle diseases: patterns of distribution and severity of involvement.

Magnetic resonance imaging of the lower extremities was performed with a low field system in 51 patients representing three different categories of biopsy-proven primary skeletal muscle disease; muscular dystrophies, congenital myopathies and polymyositis. The intermuscular distribution of abnormal signal intensity and the grade of involvement of individual muscles were assessed. Large differences in the degree of pathological signal intensity between individual muscles were found in all categories. In the muscular dystrophy and polymyositis patients, the overall involvement was significantly

[Congenital myopathy associated with cutis laxa].

Congenital myopathies have been described in association with systemic and skin anomalies but not with cutis laxa. De Barsey syndrome relates cutis laxa with neurological alterations and muscular hypotonia, but electromyographic nor muscular pathological studies have been made in previously reported cases. We report a case associating two major features: cutis laxa and congenital myopathy, both confirmed by biopsy. We propose the diagnosis of De Barsey syndrome in an incomplete form.

[Muscular diseases in relation to drug consumption].

Toxic myopathies are an uncommon manifestation of chemotherapeutic agents. Most myopathies are characterized by a weakness affecting proximal limb-muscles more than distal ones. Steroids, chloroquine and ipecac syrup are the most common drugs inducing myopathies. Toxic myositis is rare and reported with D-penicillamine abuse. Myotoxicity of local agents injection such as anesthetics, steroids or antibiotics is also known. Congenital myopathies such as malignant hyperthermia, hypokalemic periodic paralysis or paroxysmal myoglobinuria may also be induced by drugs but are very rare.

Congenital myopathies.

A retrospective review of 55 children with the clinical (pre-biopsy) suspicion of a congenital or metabolic myopathy was undertaken. After investigations, 45% remained unclassified. The only statistically significant differences between this unclassified group and the others were that males with a history of progressive disease were more likely to have a definable myopathy, while 'neuropathic' features were more common in the unclassified group.

[Clinical characteristics and differential diagnosis of congenital myopathies].

Sixteen patients with congenital myopathies with the onset of the disease in early childhood were examined. All patients were subjected to morphological investigation of biopsy samples of muscle tissue, using histochemical methods for the determination of the activity of redox and glycolysis enzymes. A clinical polymorphism of these forms of myodystrophies and the presence of nondifferentiated variants were established. The distribution of the activities of oxidation and glycolysis enzymes was histochemically different.

Congenital myopathies.

The evaluation of the "floppy infant" is a common problem for neurologists who deal with infants. One major category of diseases to consider in the diagnosis of hypotonia in the infant is the congenital myopathies. The congenital myopathies are a group of nonprogressive primary diseases of muscle that present in infancy with hypotonia and weakness and that are subdivided on the basis of the morphologic characteristics seen on histochemical study of the muscle. These diseases share many clinical features despite demonstrating a wide range of variation. Through the study of the various congenita

Morphological observations in skeletal muscle from patients with a mitochondrial myopathy.

Mitochondrial metabolic dysfunction is considered to be the cause of certain congenital myopathies and a number of multisystem disorders in humans. The morphological hallmark of these diseases is the 'ragged red' fibre, which shows abnormally intensive oxidative enzyme reactions. Electron microscopy reveals that the numerically increased mitochondria in these fibres are often markedly enlarged and possess aberrant configurations of cristae. The mitochondrial matrix often contains lipid-like inclusions or shows vacuolation. The most characteristic mitochondrial abnormality is the occurrence of

Congenital myopathies with "diagnostic" pathological features.

This addition to the classification of myopathies is a result of the improved diagnostic techniques in muscle pathology in recent years, largely through the application of electronmicroscopy and histochemistry. After summarizing the clinical picture, the authors call for classification: (1) Congenital myopathies due to developmental arrest: myotubular myopathy, congenital fiber type disproportion. (2) Congenital myopathies due to persistent organellar regression: focal loss of cross striation, myopathy with lysis of myofibrils, nemaline body myopathy, zebra body myopathy, spheroid body myopath

[Multiminicore disease in a rigid spine syndrome].

The case of a 15 year-old girl, who had difficulties in walking from early childhood is reported. The clinical picture was that of the rigid spine syndrome, spine rigidity being associated with retractions and diffuse amyotrophy of the upper limbs. Histochemical and ultra-structural studies of the muscle showed typical multiminicores. The multiminicore myopathy has not particular clinical features. In the rigid spine syndrome various histological changes can be found. For these reasons, we think that the rigid spine syndrome should be classified within the group of congenital myopathies.

Unusual paraspinal muscle lesions in ankylosing spondylitis.

Minicore, multicore, core-targetoid and other ultrastructural lesions were found in the paraspinal muscles of patients with AS. The 10 patients studied, all men with AS, showed varying degrees of muscle fibre atrophy, Z band streaming, rod body formation, minicores, multicores and core-targetoid fibres. Central core disease, rod body myopathy, minicore and multicore diseases are recognized clinical entities within the congenital group of structural myopathies. Target fibres are believed to be a feature of reinnervation. It is also known that experimental tenotomy causes core-targetoid changes,

Quantitative histopathology in congenital myopathies.

A histo-morphometric analysis has been carried out in 23 muscle biopsies obtained from patients with congenital myopathies (6 nemaline m., 10 centronuclear m., 4 central core m., 3 multicore m.) in order to improve diagnosis and to confirm the data available in literature. No relationship has been found between the severity of the disease and the histo-pathological features as previously described. Nevertheless, the following pathological aspects diverge from previous reports: a) the absence of rods within the nuclei and of mitochondrial abnormalities in nemaline myopathy; b) the presence of t

On some myopathies with oculomotor involvement.

The conditions under discussion are the oculopharyngeal muscular dystrophy, the various forms of centronuclear myopathies and a variant of the multiminicore disease, namely the pleocore disease. For each of these disorders, the main clinical (including the involvement of the extraocular muscles) and histopathological features are illustrated using already reported or unpublished personal observations. The accent will essentially be laid on the lack of specificity of the clinical symptoms in congenital myopathies and on the difficulties to identify specific histological characteristics.

Cerebral dysgeneses and their influence on fetal muscle development.

Abnormal suprasegmental influences of the brainstem and cerebellum on the developing motor unit during the histochemical stage of muscle development (20-28 weeks gestation) may alter the rate of maturation of striated muscle or may cause abnormal proportions and relative sizes of histochemical fibre types. Such aberrations without primary myopathic or denervative changes are commonly found in children with cerebral malformations, particularly associated with cerebellar hypoplasia. Upper motor neuron disease during embryonic life may explain histochemical alterations in some nonprogressive 'con

Pleocore disease. Multi-minicore disease and focal loss of cross striations.

We report clinical and morphological data on seven patients with a congenital myopathy as well as data concerning five parents. Classical myopathies such as rod disease, centronuclear myopathy or central core disease could be ruled out. Structural abnormalities of intracellular organelles or particulate inclusions were rare and insignificant. The most prominent and constant features were minicores and focal loss of cross striations, associated with a prevalence of type 1 fibres, increasing with the age at time of biopsy. A carrier state could not be defined in the five examined parents neither
