Skeletal muscle samples from the upper and lower extremities of 38 human fetuses (6 to 18 weeks' gestation) of both sexes were studied by histologic, histochemical, and electron microscopic methods. Ultrastructural morphometry was applied. In the different stages of normal development are found regressive changes, ranging from reversible dystrophic to irreversible necrotic alterations, which are characteristic of either primary myogenic myopathies or primary neurogenic muscle atrophies in older age. Several pathologic patterns of so-called congenital myopathies also presented. On the basis of

Biopsies from various skeletal muscles were taken post mortem from infants with conceptional ages between 25 and 40 weeks. Cryostat sections were analysed quantitatively. The growth of the mean fibre diameter follows an exponential curve, which is mainly due to an initial predominance of type II-fibres. Before the 34th week of gestation only isolated large "Wohlfart"-fibres show type I staining characteristics. A great number of smaller type I-fibres appear suddenly around the 34th week. Their percentage of the total fibre population increases linearly to 40% at term. The development of the "t

Clinical investigations of infants hospitalized with botulism demonstrate a remarkable uniformity of complaints and physical findings. Constipation precedes a course of progressive weakness and cranial nerve dysfunction. Examination reveals hypotonia, hyporeflexia, and a variable pattern of involvement of the motor cranial nerves. Initial laboratory investigations should include electrodiagnostic tests, because findings of an incremental response to rapid, repetitive nerve stimulation and of brief, small-amplitude motor units on electromyography are virtually pathognomonic of botulism in the i

Five cases with type I fiber hypotrophy myopathy from two families are reported. The first two cases belong to the type I fiber hypotrophy and central nuclei. Six cases have already been published. In addition to hypotonia, obvious thoracic anomalies are associated. The three other cases are members of the second sibship and belong to the type I fiber hypotrophy without central nuclei nor myothony and show the typical phenotypic characters: elongated face and adynamic appearance. Out of these three brothers, two show no progress and the other is getting worse slowly. The pathological findings

Muscle biopsy specimens of 20 full-term neonates (13 surgical and seven necropsy specimens) with clinical evidence of neuromuscular disease were studied to determine the diagnostic usefulness of this procedure in the newborn. Characteristic pathologic alterations were identified in specific diseases. Some findings were similar to those seen later in life, but others differed from those expected in the same diseases at older ages. Persistence of fetal muscle cells was a characteristic common to several congenital myopathies and neuropathies. Lymphocytic infiltrates, muscle fiber necrosis, and a

Nonprogressive congenital myopathies are usually classified according to structural or ultrastructural changes in the muscle fibres. In our material cases without specific structural characteristics but with an abnormal distribution and size of one fibre type, and dystrophic changes with or without fibre type changes are far more numerous. 15 cases are reviewed: Congenital dystrophy (4 cases); congenital dystrophy with complete lack of type II fibres [1]; fibre type disproportion with predominance of abnormally small type I fibres [5]; fibre type disproportion with predominance of small and la

Modern morphological diagnostic methods for neuromuscular diseases are based on enzyme histological and electron microscopic techniques. By enzyme histochemical differentiation of fiber types, early morphological changes such as Type I atrophy in myotonic dystrophy, or particular disease forms like Type II atrophy or congenital fiber type disproportion and other congenital myopathies associated with a Type I atrophy may be detected. The ultrastructural characteristics and the fine structure of autofluorescent lipopigments rich in phosphatases in the skeletal muscle fiber permit appropriate dia

Two patients with clinically mild congenital myopathies presented with chronic respiratory failure. Muscle weakness alone could not account for the respiratory insufficiency since static respiratory pressures were not markedly impaired, ventilation during exercise was normal, and daytime ventilation was normal if ventilatory assistance was provided at night. The ventilatory responses to inhaled carbon dioxide were very low, suggesting that impairment of the central nervous respiratory chemoreceptor contributed to hypoventilation. These patients and others described in the literature suggest th

Changes in motor innervation were compared with histologic and histochemical pattern of muscle fibers in three biopsies of central core disease, four biopsies of nemaline myopathy, one biopsy of myotubular myopathy, and three biopsies of mitochondrial myopathy. Evidence of collateral reinnervation was obtained only in one biopsy from central core disease. In other biopsies, no structural or ultrastructural abnormality of axis cylinders, myelin, or myoneural junction suggesting denervation were observed. The only relevant change found in centronuclear myopathy and to a lesser extent in nemaline

A case is reported of a 28-year-old woman with central core disease who prior to muscle biopsy was diagnosed to have muscular dystrophy. Histologic evaluation confirmed a diagnosis of central core disease, a nonprogressive or slowly progressive disorder of voluntary muscle belonging to a group of muscle diseases called benign congenital myopathies. The clinical and pathologic features fo this benign disorder are reviewed. Two sons were examined and found to be normal.

In two sisters with a neo-natal hypotonia, muscle biopsies demonstrated as main pathological feature a disproportion in size between the two types of muscle fibers defined according to their myofibrillar ATPase activity. Type I fiber mean diameter was at the lower limit of the normal values, and type II fibers were larger than normal. Their father's biopsy also showed an abnormal smallness of the type I fibers, with a bimodal distribution. By electron microscopy, the small type I fibers did not reveal any significant abnormality in children's biopsies. In father's biopsy, there was an abnormal

Two sisters presenting with benign congenital hypotonia are reported. In both cases the muscle biopsies demonstrated the same pathological pattern, consisting in an abnormal size disproportion between the two main cytoenzymological types of muscle fibers. Their father, exhibiting a slight and diffuse muscle weakness, showed a closely related histological aspect. These three cases bring the first evidence of a familial transmission of this new entity. Its relationship with the other types of "congenital myopathies" is discussed.
