Combined qualitative and quantitative MRI reveals short-term progression in facioscapulohumeral muscular dystrophy.

Facioscapulohumeral muscular dystrophy (FSHD) is characterized by slowly progressive and heterogeneous muscle involvement; therefore, clinical measures show limited sensitivity to short-term change. Magnetic resonance imaging (MRI) provides useful biomarkers of muscle degeneration and disease activity. To investigate one-year longitudinal changes in lower leg muscles of patients with FSHD using qualitative (T1-weighted [T1w], STIR) and quantitative MRI (fat fraction [FF], water T2), and to assess their relationship with clinical function. Thirty-one patients with FSHD (21 men; age 44.2±17 year

A Case of Eosinophilic Granulomatosis With Polyangiitis Presenting With Conjunctival Lesions During Treatment With IL-5 Inhibitors.

Eosinophilic granulomatosis with polyangiitis (EGPA) is a systemic rheumatic disease characterized by necrotizing vasculitis in the small vessels with eosinophilic inflammation. EGPA involves multiple organs, such as the heart, lungs, kidneys, gastrointestinal tract, nerves, and ophthalmic organs. Ophthalmic involvement includes conjunctival nodules, orbital myositis, orbital inflammatory syndrome, dacryoadenitis, and cranial nerve palsy. Notably, conjunctival nodules in patients with EGPA are very rare lesions but may present with atypical eyelid swelling. Herein, we present a case of EGPA wi

Differentiated skeletal myotubes coupled with bimodal biosensing for dynamic functional evaluation of inflammatory myotube dysfunction.

Inflammatory skeletal muscle atrophy is a growing clinical challenge, driven by chronic inflammatory conditions and life-threatening illnesses that cause progressive muscle loss. This condition leads to premature muscle fatigue and a decline in muscle function, highlighting the necessity for highly sensitive diagnostic methods to detect impairment prior to the occurrence of structural damage. Conventional diagnostic methods, including electromyography, serum biomarker assays, and medical imaging, often fail to detect early and subtle alterations in myotube electrophysiological responsiveness b

Innervation of the subscapularis muscle and implications for treatment of spasticity.

BACKGROUND: Spasticity of the muscles in the shoulder girdle, frequently observed in upper motor neuron syndromes, leads to an internal rotation deformity with associated functional limitation. Selective neurectomy (SN) of motor branches can offer targeted relief whilst preserving glenohumeral joint stability. OBJECTIVE: To provide detailed anatomical mapping of motor branches to the subscapularis muscle and assess the feasibility of SN in the management of shoulder spasticity. METHODS: Twenty shoulders from ten formalin-fixed cadavers were dissected via an infraclavicular approach. The numb

Ebselen restores mitochondrial function through metabolic rewiring in Barth syndrome and related mitochondrial diseases.

Despite major advances in the genetic diagnosis of mitochondrial disorders, effective disease-modifying therapies remain scarce. Here, we identify ebselen (EBS) as a promising therapeutic candidate through a phenotypic drug screen in a yeast model of Barth syndrome, a cardiomyopathy caused by defective cardiolipin maturation. EBS improved oxidative phosphorylation-dependent growth across diverse fungal models of mitochondrial diseases, including defects in complex I, complex IV, mitochondrial DNA maintenance, mitochondrial translation, and ATP synthase. Therapeutic efficacy was further validat

Case report: Atypical anti-GlyR antibody-positive pediatric progressive encephalomyelitis with rigidity and myoclonus presenting as chronic joint pain.

Progressive encephalomyelitis with rigidity and myoclonus (PERM) represents the most severe subtype of stiff-person spectrum disorders and is exceedingly rare in children. We report a 14-year-old female patient who presented with chronic joint pain and axial rigidity as the initial manifestations, with a disease course of two years, during which she had been diagnosed with juvenile idiopathic arthritis but lacked typical peripheral arthritis signs and showed poor response to therapy. The patient exhibited paroxysmal axial rigidity, autonomic dysfunction, and sleep disturbances. Although typica

ZBP1 contributes to IFN-β-induced PANoptosis-related skeletal muscle cell injury in dermatomyositis.

BACKGROUND: Dermatomyositis (DM) is a systemic autoimmune disease characterized by cutaneous manifestations and inflammatory skeletal muscle injury. Dysregulated type I interferon signaling and regulated cell death pathways may converge to promote myofiber damage. PANoptosis integrates core components of pyroptosis, apoptosis, and necroptosis, but its contribution to DM remains unclear. METHODS: Transcriptomic datasets from DM skin and skeletal muscle were integrated to identify differentially expressed genes overlapping with a predefined PANoptosis-related gene set. Candidate core genes were

AAVcc47-μDys Gene Therapy Ameliorates Respiratory Pathology in mdx Mouse Model of Duchenne Muscular Dystrophy.

Duchenne muscular dystrophy (DMD) is a progressive X-linked neuromuscular disorder caused by deficiency of dystrophin - a protein essential for muscle integrity. Loss of dystrophin results in global muscle weakness, including in the muscles of respiration. As a result, respiratory failure is a leading cause of morbidity and mortality in patients with DMD. Gene therapy using adeno-associated virus (AAV) is ideal for monogenetic diseases. However, the dystrophin gene is too large to be packaged into AAV; therefore, microdystrophin (μDys) containing important elements of the dystrophin gene is us

Clinical profile of pulmonary involvement in rheumatological disorders: a cross-sectional study in a rheumatology-associated lung disease clinic.

Rheumatological diseases (RDs) are characterized by autoimmune-mediated organ damage, with the lungs being an important target organ. Almost all compartments of the pulmonary system may be involved, with considerable heterogeneity in the frequency, severity, and spectrum of pulmonary manifestations. This heterogeneity may result in delayed diagnosis and treatment, leading to adverse outcomes. The objective of this study was to characterize the clinical, demographic, serological, and radiological features of pulmonary manifestations across a spectrum of rheumatic diseases. This cross-sectional

[Ultrasound-Guided PRP for Tendinopathy: a Practical, Multimodal Framework from Biology to Rehabilitation].

Tendinopathies are common and hete- rogeneous, and recovery depends on both the local biological milieu and appropriately progressed mechanical loading. Platelet-rich plasma (PRP) is a biologically plausible adjunct, yet reported clinical effects are inconsistent, likely due to variation in phenotype, PRP formulation and dosing, delivery technique, and rehabilitation protocols. Ultrasound guidance improves injection accuracy and safety, but its incremental clinical value may be context dependent. This review outlines tendon-healing biology, summarizes evidence for conservative management, and

Comparison of two repetitive peripheral magnetic stimulation protocols for upper trapezius myofascial pain syndrome: A randomized trial.

BACKGROUND: Upper trapezius myofascial pain syndrome (MPS) is a common contributor to chronic neck pain and functional limitations. Repetitive peripheral magnetic stimulation (rPMS) has emerged as a non-invasive neuromodulatory intervention; however, the optimal stimulation duration remains uncertain. Clarifying duration-dependent effects is important for informing parameter standardization in rehabilitation practice. METHODS: This single-centre, assessor-blinded randomized trial enrolled adults aged 18-55 years with chronic (≥ 3 months) upper trapezius MPS and baseline Visual Analog Scale (V
