Calcium Pyrophosphate Deposition Disease of the Temporomandibular Joint Extending into the External Auditory Canal and the Middle Ear, Coexisting with Otosclerosis: A Case Report.

Calcium pyrophosphate deposition disease (CPPD) is caused by CPP crystals and rarely forms tumors in joints. The authors describe a case of tumoral CPPD in the temporomandibular joint (TMJ), extending into the external auditory canal (EAC) and the middle ear. A 71-year-old man was referred to the department with a tentative diagnosis of CPPD in the right TMJ. Computed tomography showed the mass extending into the EAC, tympanic cavity, and bony Eustachian tube, with opacification of the mastoid cells. The audiogram revealed bilateral mixed hearing loss, suggesting the coexistence of bilateral o

Exploring the Role of Hyperfocus in the Psychophysiology of Probable Bruxism and Painful Temporomandibular Disorders.

BACKGROUND: Bruxism and temporomandibular disorders (TMD) are multifactorial conditions in which cognitive and behavioural traits may play an underrecognised role. Hyperfocus, defined as intense and sustained attentional engagement, has been proposed as a potential driver of heightened sensorimotor activity and pain vulnerability, yet its relevance to bruxism and painful TMD remains poorly understood. OBJECTIVES: To examine whether hyperfocus is associated with probable bruxism and painful TMD (pTMD), and to determine whether probable bruxism mediates the relationship between hyperfocus and p

Muscle cramp susceptibility is associated with muscle-specific modulation of human motoneuron discharge patterns.

Muscle cramps are painful, involuntary contractions thought to arise from altered spinal motoneuron excitability, potentially involving persistent inward currents (PICs). We examined whether cramp susceptibility is associated with altered PIC-related motoneuron behaviour and whether electrically induced cramp modifies these estimates. Twenty-eight healthy, active adults performed triangular plantar-flexor contractions before and after a cramp threshold frequency test designed to evoke cramp in gastrocnemius medialis. Participants were classified as crampers or controls according to cramp test

Feasibility and Patient-Reported Outcomes of Full-Body Red and Near-Infrared Photobiomodulation for Musculoskeletal Pain in a Predominantly Hispanic/Latino Population: A Single-Arm Pilot Study.

OBJECTIVE: To characterize feasibility, tolerability, and patient-reported outcomes after a 10-session full-body red and near-infrared photobiomodulation (PBM) protocol for musculoskeletal pain in a predominantly Hispanic/Latino community sample. BACKGROUND: PBM is a non-pharmacological intervention with potential analgesic and anti-inflammatory effects. Evidence from community-based and Hispanic/Latino populations remains limited, and full-body LED protocols are incompletely standardized. METHODS: This single-arm observational pilot study included 40 adults with self-reported acute or chron

Understanding Diagnostic Failure in Temporomandibular Disorders: A Narrative Synthesis of Evidence and Five-Domain Framework for Iatrogenic Mismanagement.

BACKGROUND: Diagnostic errors in temporomandibular disorders (TMDs) contribute to unnecessary procedures and avoidable patient mismanagement despite advances in pain science, diagnostic classification systems, and evidence-based care. OBJECTIVE: To conduct a narrative review and present a dynamic, five-domain framework explaining how diagnostic uncertainty interacts with educational, cognitive, systemic, and ethical factors to produce iatrogenic overtreatment. METHODS: Searches of PubMed, Scopus, Web of Science, and Google Scholar (inception to April 2026) identified literature on TMD diagno

Diagnostic Utility of USP6 RNA Chromogenic In Situ Hybridization in the Differential Diagnosis of Nodular Fasciitis and Aneurysmal Bone Cyst.

Recurrent USP6 gene rearrangements underlie a subset of mesenchymal neoplasms, including nodular fasciitis and aneurysmal bone cyst. These rearrangements place USP6 under the regulatory control of another gene's constitutively active promoter, resulting in USP6 overexpression. Recently, it has been demonstrated in a limited number of cases that detection of USP6 overexpression by RNA chromogenic in situ hybridization (CISH) can serve as a surrogate marker for USP6-rearranged tumors. To further assess the diagnostic utility of USP6 RNA CISH, we evaluated its application in 1210 bone and soft ti

Nontraumatic Myositis Ossificans Adjacent to the Femoral Nerve: A Ubiquitin-Specific Peptidase 6 (USP6)-Rearranged Lesion Mimicking Malignancy.

Myositis ossificans (MO) is a benign, self-limiting lesion characterized by heterotopic ossification that can mimic malignancy both radiographically and histologically. Non-traumatic MO is particularly challenging to diagnose, especially when arising adjacent to major neurovascular structures. Recent studies have identified ubiquitin-specific peptidase 6 (USP6) gene rearrangements in a subset of MO, linking it to a family of transient fibroblastic/myofibroblastic neoplasms. We present a 21-year-old female who presented with progressive right lower extremity pain and weakness without a history

What is the current state of evidence on passive exoskeletons designed to support the lumbar region? A systematic review and meta-analysis of physiological, biomechanical, and user-related outcomes.

INTRODUCTION: Work-related musculoskeletal disorders (WMSDs) are the most commonly reported health problem among workers in Europe. Manual handling of loads and awkward postures are cited as important risk factors for WMSDs. Exoskeletons, wearable devices that support and stabilize the locomotor system, are emerging as a lifting aid that could reduce muscular effort and potentially lessen the impact and the prevalence of WMSDs among manual workers. This systematic review and meta-analysis aims to summarize the current scientific evidence on passive exoskeletons designed to support the lower ba

Ctcf deficiency in myofibers induces pathological genome reprogramming toward the spontaneous development of myopathy.

How perennial, postmitotic multinucleated tissues, such as skeletal myofibers, maintain their identity and transcriptional adaptation to homeostatic perturbations through adult life is an outstanding question. To address this issue, we investigated the consequences of loss of 3D-genome architecture in skeletal muscles by generating myofiber-specific Ctcf- deficient ( Ctcf mKO ) mice. Ctcf mKO mice did not exhibit muscular phenotype at birth but spontaneously developed a severe myopathy. Integrated analysis of snRNAseq, ATACseq and promoter-capture Hi-C revealed both common and fiber-type speci

FAP adipogenesis drives early spinal muscle loss in dysferlinopathy and reveals a therapeutic target.

Dysferlinopathy (Limb Girdle Muscular Dystrophy R2/LGMD2B) causes progressive weakness of limb-girdle muscles. Although gait abnormalities are common, the contribution of spinal muscles to disease symptoms remains poorly understood. Using cross-sectional MRI in patients and longitudinal analyses in a dysferlinopathic mouse model, we identify early-onset, progressive adipogenic degeneration of spinal muscles as a previously unrecognized hallmark of dysferlinopathy. Multiple spinal muscles, including the iliocostalis, longissimus, and spinalis, are affected, with degeneration preceding pathology

Interlaboratory comparison of immunoprecipitation-based testing for myositis-specific autoantibodies.

INTRODUCTION: Myositis-specific autoantibodies (MSAs) are clinically important biomarkers that help classify idiopathic inflammatory myopathies and guide clinical management, but reliable detection remains challenging. Immunoprecipitation (IP) is widely regarded as a reference method for MSA detection in specialist laboratories. However, IP comprises a range of related techniques rather than a single standardised assay, and variability between expert laboratories using different IP-based workflows is not well characterised. METHODS: Serum samples previously submitted to the Bath Diagnostic Se

A systematic review of psychological variables associated with juvenile primary fibromyalgia syndrome: Comparison between individuals with and without this condition.

Juvenile primary fibromyalgia syndrome (JPFS) is a pediatric rheumatologic condition characterized by (1) persistent, widespread musculoskeletal pain in multiple locations and (2) other symptoms such as fatigue, headaches, poor sleep or cognitive symptoms, among others. These alterations may be associated with the development of psychological distress, poorer mental health and lower quality of life, although no clear consensus has been reached. Thus, this study aimed to examine the psychological symptoms associated with JPFS, compared with a healthy comparison group of children and adolescents
