Laboratory factors and diagnostic aids for compartment syndrome in extracorporeal membrane oxygenation: An exploratory single-center study.

PurposeExtracorporeal membrane oxygenation (ECMO) patients have increased risk for limb acute compartment syndrome (ACS), but diagnosis remains difficult given the lack of subjective and pulsatility assessments in this patient population. This exploratory study characterized ECMO patients who developed ACS and investigated laboratory measures that can aid diagnosis.MethodsWe reviewed records of adult patients placed on ECMO between 2016 and 2022 at our single center, collecting clinical, laboratory, and survival data upon cannulation. We performed comparisons of baseline and laboratory data be

Identifying post-exertional malaise subtypes: Differentiating physical and mental PEM manifestations.

Myalgic Encephalomyelitis/Chronic Fatigue Syndrome (ME/CFS) is a chronic illness with post-exertional malaise (PEM) as a key symptom. This study categorized participants with ME/CFS who met PEM criteria into four groups based on severity of physical and mental PEM: severe physical PEM (Physical group), severe mental PEM (Mental group), both severe (Both group), or neither severe (Neither group). A control group was also included. The Both group exhibited the highest symptom severity, while the Neither group displayed lower scores. The Neither group experienced less disability than other ME/CFS

Central Sensitization as a Risk Factor in Work-related Musculoskeletal Pain - New Aspects of Functional Disorders.

When analyzing work-related complaints, it is important to consider not only external and psychosocial factors but also conditions that have their origin in neurological processes, which lead to sensitization. This results in a lowering of receptor thresholds, leading to increased sensitivity to often inadequate stimuli. This hypersensitivity manifests not only as pain but also in the form of various discomforts. The basis of many workplace problems may therefore not be isolated causes, but rather an underlying sensitization process as an expression of dysfunctional regulatory feedback loops.

Radiofrequency Versus Myofascial Therapy in Women With Myofascial Chronic Pelvic Pain: A Randomized Clinical Trial.

INTRODUCTION AND HYPOTHESIS: Chronic pelvic pain (CPP) affects 5.7-26.6% of women and can severely affect quality of life (QoL). When combined with myofascial syndrome, CPP is often more severe and challenging to manage. Myofascial manual therapy (MMT) is the standard conservative treatment, whereas capacitive-resistive monopolar radiofrequency (RF) may be an effective alternative based on benefits seen in other musculoskeletal disorders. However, its standalone efficacy in women with CPP and a myofascial syndrome has not been established. METHODS: In this randomized controlled trial at a ter

FLNC Complex Structural Variant Causing Distal Myopathy Identified by Family-Based Genome Sequencing.

Distal myopathies (DM) are clinically and genetically heterogeneous neuromuscular disorders, and identifying a molecular genetic cause may remain challenging in a subset of cases. Moreover, DM may be misdiagnosed as hereditary neuropathies due to overlapping clinical features. Here, we report a novel structural variant in FLNC associated with DM identified through genome sequencing (GS). Two affected relatives initially presented independently with referral diagnoses of Charcot-Marie-Tooth disease and amyotrophic lateral sclerosis. Clinical re-evaluation led to a change of the diagnosis to DM.

Biphasic Brachial Plexus Blockade Following Interscalene Administration of Liposomal Bupivacaine (Exparel): A Case Report.

This case report describes a 70-year-old male who experienced a biphasic blockade following an interscalene block with 10 mL of liposomal bupivacaine (Exparel) mixed with 5 mL of 0.25% bupivacaine for shoulder arthroscopy. The patient exhibited complete motor and almost complete sensory recovery by postoperative day (POD) 1, followed by a recurrence of motor and sensory deficits on POD2, which resolved spontaneously by POD3. To our knowledge, this represents one of the earliest documented recurrences of dense motor blockade after brachial plexus block with Exparel. Potential mechanisms, includ

Australian registry reports poor health and wellbeing in people living with ME/CFS.

INTRODUCTION: Myalgic Encephalomyelitis/Chronic Fatigue Syndrome (ME/CFS) is a debilitating condition hallmarked by post-exertional malaise accompanied by an array of multisystemic symptoms. Consequently, people living with ME/CFS experience significant health impacts and report poor health-related quality of life (HRQoL). Living with a condition of an area of unmet need, there is a call for a greater understanding of the health impacts of ME/CFS in Australia. Despite growing recognition and awareness, large-scale national comparative investigations using ME/CFS cohorts have not been reported.

Improving stem cell-derived extracellular vesicles for better tendon and tendon-to-bone junction regeneration: strategies and future directions.

Chronic tendinopathy is a debilitating tendon overuse disorder characterized by localized tenderness, swelling, and pain, significantly impairing physical function. It is particularly common among the overuse and aging populations. Traditional treatment options, including conservative therapies and surgical interventions, often yield limited success. Recent studies indicate that extracellular vesicles (EV) derived from stem cells provide a promising therapeutic avenue for healing of tendon and tendon-to-bone junction (TBJ) injuries associated with chronic tendinopathy. Their unique properties,

Exploring the Role of Hyperfocus in the Psychophysiology of Probable Bruxism and Painful Temporomandibular Disorders.

BACKGROUND: Bruxism and temporomandibular disorders (TMD) are multifactorial conditions in which cognitive and behavioural traits may play an underrecognised role. Hyperfocus, defined as intense and sustained attentional engagement, has been proposed as a potential driver of heightened sensorimotor activity and pain vulnerability, yet its relevance to bruxism and painful TMD remains poorly understood. OBJECTIVES: To examine whether hyperfocus is associated with probable bruxism and painful TMD (pTMD), and to determine whether probable bruxism mediates the relationship between hyperfocus and p

Muscle cramp susceptibility is associated with muscle-specific modulation of human motoneuron discharge patterns.

Muscle cramps are painful, involuntary contractions thought to arise from altered spinal motoneuron excitability, potentially involving persistent inward currents (PICs). We examined whether cramp susceptibility is associated with altered PIC-related motoneuron behaviour and whether electrically induced cramp modifies these estimates. Twenty-eight healthy, active adults performed triangular plantar-flexor contractions before and after a cramp threshold frequency test designed to evoke cramp in gastrocnemius medialis. Participants were classified as crampers or controls according to cramp test

Feasibility and Patient-Reported Outcomes of Full-Body Red and Near-Infrared Photobiomodulation for Musculoskeletal Pain in a Predominantly Hispanic/Latino Population: A Single-Arm Pilot Study.

OBJECTIVE: To characterize feasibility, tolerability, and patient-reported outcomes after a 10-session full-body red and near-infrared photobiomodulation (PBM) protocol for musculoskeletal pain in a predominantly Hispanic/Latino community sample. BACKGROUND: PBM is a non-pharmacological intervention with potential analgesic and anti-inflammatory effects. Evidence from community-based and Hispanic/Latino populations remains limited, and full-body LED protocols are incompletely standardized. METHODS: This single-arm observational pilot study included 40 adults with self-reported acute or chron

What is the current state of evidence on passive exoskeletons designed to support the lumbar region? A systematic review and meta-analysis of physiological, biomechanical, and user-related outcomes.

INTRODUCTION: Work-related musculoskeletal disorders (WMSDs) are the most commonly reported health problem among workers in Europe. Manual handling of loads and awkward postures are cited as important risk factors for WMSDs. Exoskeletons, wearable devices that support and stabilize the locomotor system, are emerging as a lifting aid that could reduce muscular effort and potentially lessen the impact and the prevalence of WMSDs among manual workers. This systematic review and meta-analysis aims to summarize the current scientific evidence on passive exoskeletons designed to support the lower ba
