Exploring motor variability in adults with severe athetoid or ataxic cerebral palsy: Impact on error-based and reward-based learning.

Individuals with cerebral palsy (CP) are characterized by elevated movement variability. Movement variability has been proposed as a mechanism that facilitates the exploration of diverse movement strategies. However, in the case of CP, this variability is often excessive, potentially impeding effective error correction and motor learning processes. This study aims to investigate motor variability in adults with severe athetoid and ataxic CP profiles, focusing on its impact on error-based and reward-based learning mechanisms. The study included 13 adults with severe CP and a control group of 18

Cryoneurolysis for Spasticity in a Pediatric Patient With Cold Urticaria Managed With Targeted Skin Warming: A Case Report.

Spasticity affects a significant portion of individuals with cerebral palsy, often progressing to contractures and neuro-orthopedic deformities if left untreated. Cryoneurolysis is an emerging, mini-invasive ultrasound-guided technique that induces secondary axonotmesis of peripheral nerves through the formation of an ice ball, shown to result in months of improved range of motion. However, cold urticaria is listed as a contraindication for this procedure because of the risk of localized or systemic anaphylaxis. We present a case of a 10-year-old girl with ataxic cerebral palsy who successfull

PHACE syndrome: a systematic literature review and illustrative case report of a patient with severe cerebrovascular and neurodevelopmental sequelae.

BACKGROUND: PHACE syndrome is a rare neurocutaneous disorder defined by the association of large segmental infantile hemangiomas of the head and neck with malformations of the posterior fossa, cerebral and cervical arteries, heart, eyes, and ventral midline structures. Although facial hemangiomas are often the presenting feature, the cerebrovascular, neurodevelopmental, and airway manifestations are responsible for the greatest long-term morbidity. METHODS: A systematic literature review was conducted following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) gu

Neuroimaging to Genetics: Unraveling the Etiology of Cerebral Palsy in Children From Southern Brazil.

This retrospective observational study investigates the clinical and neuroimaging profiles of children with cerebral palsy and explores the contribution of genetic factors to its etiology. We reviewed 302 pediatric cases diagnosed with cerebral palsy in Southern Brazil during 2023. Neuroimaging abnormalities were present in 92.1% of cases, with leukomalacia being most frequent. Neonatal encephalopathy emerged as the leading etiology, followed by prematurity and genetic conditions. Genetic testing was performed in 68 patients, identifying 29 distinct genes, notably in cases with preserved imagi

Elevated cerebrospinal fluid 2-Hydroxybutyric acid in two siblings with aspartate-glutamate carrier 1 deficiency.

The mitochondrial aspartate-glutamate carrier 1 (AGC1 or aralar) is a key component of the malate-aspartate shuttle (MAS), which transfers NADH-derived reducing equivalents from the cytosol into mitochondria to support oxidative phosphorylation. Disruption of MAS leads to cytosolic NADH accumulation, NAD+ depletion, and a reduced NAD+/NADH ratio, impairing redox-sensitive enzymes. AGC1 is primarily expressed in the central nervous system. AGC1 deficiency is a rare autosomal recessive disorder characterized by seizures, intellectual disability, and hypomyelination. The disorder impairs mitochon

Expanding the Landscape of Spinocerebellar Ataxia Type 5.

Spinocerebellar ataxia type 5 (SCA5) is a rare subtype of SCA that usually affects adults. It has been recently reported in children in Europe, North America, and China. This study aims to describe clinical, radiological, and genetic data of a child presenting with SCA5, caused by a heterozygous likely pathogenic missense variant in SPTBN2 (NM_006946.3: c.1052G > C, p.Arg351Pro). According to databases and a review of the literature, this is one of few cases of SCA5 from Latin America. Expanding the landscape of SCA5 is relevant to the differential diagnosis of ataxic cerebral palsy and the au

The Effects of Over-Ground Robot-Assisted Gait Training for Children with Ataxic Cerebral Palsy: A Case Report.

Poor balance and ataxic gait are major impediments to independent living in ataxic cerebral palsy (CP). Robot assisted-gait training (RAGT) has been shown to improve the postural balance and gait function in children with CP. However, there is no report on the application of RAGT for children with ataxic CP. Here, we report two cases of children with ataxic CP who underwent over-ground RAGT along with conventional therapy for 4 weeks. Outcome measures including the gross motor function measure (GMFM), pediatric balance scale, pediatric reach scale, one-minute walk test, and Timed Up and Go tes

Infantile Onset of Spinocerebellar Ataxia Type 5 (SCA-5) in a 6 Month Old with Ataxic Cerebral Palsy.

Spinocerebellar ataxia type 5 (SCA-5) is a predominantly slowly progressive adult onset ataxia. We describe a child with a presentation of ataxic cerebral palsy (CP) and developmental delay at 6 months of age. Genetic testing confirmed a c.812C>T p.(Thr271Ile) mutation within the SPTBN2 gene. Seven previous cases of infantile onset SCA-5 are reported in the literature, four of which had a CP presentation. Early onset of SCA-5 presents with ataxic CP and is a rare cause of cerebral palsy.
