Genotype-phenotype correlations in 18 European patients with heterozygous KIF1A variants: key considerations for assessing KIF1A variant causality.

BACKGROUND: Variants in the KIF1A have been associated with a wide range of phenotypes. Most variants are found in the protein's motor domain. The clinical phenotype of KIF1A-associated disorders (KAND) correlates with the position and the type of variant. Missense variants in the motor domain are predominantly associated with severe phenotypes and often occur de novo. METHODS: Patients from the Czech Republic and Switzerland were identified during DNA diagnostics for neuromuscular or neurodevelopmental disorders. Clinical and genetic data were analyzed retrospectively. RESULTS: A total of 1

Diagnostic yield of genome sequencing in children with progressive movement disorders.

Childhood-onset movement disorders are clinically and genetically heterogeneous, with over 500 implicated genes. Standard clinical genetic testing, including exome sequencing, has limited sensitivity for certain variants, including repeat expansions, structural variants (SVs), copy number variants (CNVs), and deep intronic changes. We evaluated the diagnostic utility of short-read whole genome sequencing (srWGS) and, in selected cases, long-read genome sequencing (lrWGS) in a real-world cohort of children and young adults with early-onset progressive movement disorders and prior nondiagnostic

Expanding the repertoire of loss-of-function variants in HACE1 causing complex spastic paraplegia: literature review and recommendations on clinical management.

BACKGROUND: HACE1 encodes a HECT domain and ankyrin repeat containing protein regulating several small GTPases. This protein is involved in several important functions, such as cell division, protein ubiquitination, and localization. Biallelic variants in HACE1 have been implicated in spastic paraplegia and psychomotor retardation with or without seizures (MIM: 616756). Previously, 32 patients of various ethnicities have been reported with different types of variants; loss-of-function (LoF), missense, and indels. RESULTS: Here, we studied five unrelated families of diverse ethnicities with ei

Mammalian lipophagy: process and function.

Lipophagy, the selective autophagic degradation of lipid droplets (LDs), is a key mechanism for lipid homeostasis and cellular adaptation to metabolic and stress conditions. In mammals, lipophagy is governed by signaling pathways, LD-associated receptors (e.g. SQSTM1/p62, NBR1, OPTN, SPART, OSBPL8, DDHD2, VPS4A, ATG14, and TP53INP2), and transcription factors (TFEB, TFE3, FOXO1, PPARA, PPARG, and SREBF1/SREBP1) that coordinate LD recognition, sequestration, and lysosomal degradation. Dysregulated lipophagy contributes to the pathogenesis of metabolic and age-related diseases, including metabol

Very late-onset Krabbe disease with concomitant dementia: case description and a critical review of the literature.

BACKGROUND: Krabbe disease (KD) is a rare autosomal recessive lysosomal storage disorder caused by pathogenic variants in GALC. Despite accounting only for 5% of forms, reports of adult-onset KD cases are increasingly described. METHODS: A female patient manifesting KD after the age of 60 years, presenting with spastic paraplegia and cognitive decline, is described. The scientific literature of KD with onset > 10 years has been extensively reviewed to refine the spectrum of later-onset KD manifestations. RESULTS: Including ours, we identified 84 KD adolescent/adult-onset patients (mean age at

Loss-of-function variants in the CAPN1 activator CD99L2 cause X-linked spastic ataxia.

Most patients with a rare movement disorder (MD) do not receive a molecular diagnosis, and the underlying genetic variants and mediating genes remain elusive. Here, we evaluate the diagnostic accuracy of conventional and next-generation sequencing-based genetic testing strategies in a cohort of 2,811 individuals with ataxia, spastic paraplegia and dystonia. Exome sequencing establishes genetic diagnoses in 19.3% of cases, and specificity of phenotypic features and age at testing are positive predictors. Genome analysis 'beyond the exome' increases the diagnostic yield by 7.5%, mostly due to th

Neuroradiological Phenotype Expansion of the Siddiqi Syndrome: A Case Report.

Siddiqi syndrome is a rare autosomal recessive deafness-dystonia disorder caused by pathogenic variants in the FITM2 gene. To date, only 5 unrelated families have been reported in the literature carrying loss-of-function variants in FIMT2 gene. In this report, we describe a 29-year-old woman with compound heterozygous novel variants identified by trio-based exome sequencing. She carries the paternally inherited delins variant c.158_161delinsTCAT, p.(Arg53_Asn54delinsLeuIle) and the maternally inherited frameshift variant c.567del, p.(Thr190ProfsTer9) in FITM2 gene. [A female case of phospholipase A2 group VI-associated neurodegeneration with childhood onset and long-term follow-up until 49 years of age].

The patient was a 39-year-old woman. At the age of 7, she developed cognitive delay and gait disturbance due to flexion deformities of both toes. Her motor function progressively declined over the years, and she became bedridden at the age of 38. At 39, she was referred to our department and was clinically diagnosed with complicated spastic paraplegia. Genetic testing revealed compound heterozygous missense mutations in PLA2G6 (c.662C>T; p.L221P / c.991G>T; p.D331Y), leading to a diagnosis of phospholipase A2 group VI-associated neurodegeneration (PLAN). Despite the progressive nature of her c

Electronic Device Chargers in the Hospital: A Case Study.

This case report follows M.K., a 31-year-old man with spastic paraplegia who sustained a second-degree skin burn after his mobile phone charger cord was trapped beneath his back while hospitalized. This led to complications that necessitated three hospitalizations, multiple surgical interventions, and treatment for a hospital-acquired infection caused by multidrug-resistant bacteria. Nearly every patient brings a mobile phone (and, often, a charger) into the hospital, and the use of these and other electronic devices carries risks. Although numerous media and medical publications have reported

Movement Disorders in Aicardi-Goutières Syndrome and Response to Immunomodulation.

This study characterizes movement disorders and treatment responses in seven children with Aicardi-Goutières syndrome (AGS). We retrospectively evaluated motor phenotypes, neuroimaging, and interferon signatures in patients treated with baricitinib or anifrolumab. Spasticity affected all patients, while dystonia was present in 4/7. GMFCS levels ranged from I to V. Following immunomodulation, interferon signatures normalized in 6/7 of patients, and 6/7 showed clinical stabilization or improvement, with no further regression events. These findings indicate that targeted therapy was associated wi

Dystonia as an early and prominent feature in a patient with CYP2U1 gene mutation: expanding the phenotype of SPG56-a case report.

Hereditary Spastic Paraplegias (HSPs) are a heterogeneous group of neurodegenerative disorders primarily characterized by progressive lower limb spasticity and weakness. Among them, Spastic Paraplegia Type 56 (SPG56) is a rare autosomal recessive form caused by CYP2U1 mutations. While spasticity is the hallmark of SPG56, recent reports have shown expanding phenotypic variability, including dystonia. We report an 8-month-old Arab male who presented with generalized tonic seizures and abnormal head movements, later identified as dystonic in nature. MRI revealed cerebral atrophy and delayed myeli
