Cauda Equina Syndrome Associated with Dural Ectasia in Ankylosing Spondylitis: Case Report.

Cauda equina syndrome (CES) associated with dural ectasia is a rare manifestation in patients with ankylosing spondylitis (AS). We report a case of a 53-year-old female with a 10-year history of AS who developed CES in the past 1 year. The CT and MRI findings revealed the unique appearances of dural ectasia, multiple ventral dural diverticula, erosion of lumbar vertebral bodies, adhesion of the nerve roots of the cauda equine, and the conus medullaris to the wall of the dural sac. Due to progressive clinical deterioration, surgical decompression was performed via laminoplasty with filum termin

Intradural Capillary Hemangioma: Case Report of a Rare Tumor of Cauda Equina.

Intradural extramedullary capillary hemangiomas of the cauda equina are very rare. Radiographically, they may mimic other intradural tumors like meningioma or nerve sheath tumors. Here, we present a case of a young woman who presented with low back and left leg radicular pain. Magnetic resonance imaging showed an enhancing lesion in the intradural space at the L4/L5 level. Initially, a diagnosis of meningioma with differential diagnosis of schwannoma was suspected; however, histopathology revealed it to be a capillary hemangioma. Although rare, because of its similar clinico-radiographical fea

Case of ossification of the ligamentum flavum causing cauda equina compression.

BACKGROUND: Ossification of the ligamentum flavum (OLF) is characterized by heterotopic bone formation within the ligamentum flavum. It typically causes progressive, chronic spinal canal stenosis, but only rarely acute cauda equina syndromes. CASE DESCRIPTION: A 29-year-old patient presented with cauda equina syndrome characterized by saddle numbness, urinary incontinence, and bilateral foot drop. She underwent an urgent decompression for a disc herniation, at which time the surgeons encountered severe OLF, causing cauda equina compression. After performing a computed tomography scan to asses

Sacral Ewing Sarcoma Presenting With Cauda Equina Syndrome: A Case Report and Radiological Review.

Ewing sarcoma is an aggressive neoplasm that primarily affects children and young adults. Due to the complex anatomy of the sacral region and the proximity to critical neurovascular structures, these tumors often present a significant diagnostic and therapeutic challenge. In this case, the patient initially presented with lower back pain before worsening and presenting with cauda equina syndrome. This is a rather rare case of sacral Ewing sarcoma presenting with cauda equina syndrome in a pediatric patient. A high index of clinical suspicion and timely imaging play a vital role in diagnosis an

A patient with lumbar disc herniation complicates cauda equina syndrome after epidural steroid injection: A case report.

RATIONALE: While epidural steroid injections (ESI) are a routine component of lumbar disc herniation management, the procedure carries inherent risks. Certain clinical presentations may signal an elevated risk for catastrophic complications, such as cauda equina syndrome. This report examines the significance of severe positional pain as a pre-procedural warning sign. PATIENT CONCERNS: We present the case of a 65-year-old male who exhibited a critical physical sign: severe low back pain prohibiting a supine position. Despite this red flag, an interlaminar ESI was administered at the L2-3 leve

A narrative review of the role of AI and machine learning in enhancing cauda equina syndrome diagnosis.

INTRODUCTION: Cauda Equina Syndrome (CES) is a neurological emergency requiring rapid diagnosis. Traditional diagnostic methods face challenges due to variable presentations. This review evaluates the clinical performance, limitations, and validation needs of artificial intelligence and machine learning (AI/ML) compared to traditional CES diagnostic approaches. METHODS: A systematic PRISMA 2020 search of PubMed and ScienceDirect (2019-2025) using "cauda equina" yielded 520 articles. After removing 200 duplicates and 284 non-diagnostic papers, 36 diagnostic studies were identified. Ultimately,

An Unusual Etiology of Cauda Equina Syndrome in an AIDS Patient: A Case Report.

Patients with advanced HIV/AIDS are at high risk for Primary central nervous system lymphoma (PCNSL) due to impaired immune surveillance and HIV-driven B-cell proliferation. While intracranial lesions are well recognized, spinal involvement remains underrecognized due to the scarcity of well-documented reported cases in the current literature. In this report, we describe the case of a 35-year-old male with a new HIV diagnosis who experienced acute bilateral lower extremity weakness and urinary retention during hospitalization for fever, perianal abscess, and urinary tract infection. Initial CD

Neurological Phenotype and MRI Severity as Predictors of Duloxetine Response in Lumbar Spinal Stenosis: A Retrospective Cohort Study.

Background/Objectives: Lumbar spinal canal stenosis (LSS) is a major cause of neurological disability and is frequently accompanied by neuropathic pain (NeP). Duloxetine is widely used for NeP, but its clinical role in LSS and the determinants of treatment response remain unclear. This study aimed to identify predictors of duloxetine efficacy in LSS. Methods: We retrospectively analyzed 145 patients with LSS who received duloxetine for ≥3 months (median dose 40 mg) with at least 1-year follow-up. Patients were classified into those with radicular pain or cauda equina syndrome. Treatment respon

Dural Sac Cross-Sectional Area Measurement as an Indicator of Cauda Equina Syndrome Risk in Patients with Lumbar Disc Herniation.

Background/Objectives: Cauda equina syndrome (CES) is a rare but severe complication of lumbar disc herniation (LDH). This study aimed to assess the diagnostic value of the dural sac cross-sectional area (DSCSA) in predicting CES in affected patients. Methods: In this retrospective observational study, we analyzed 99 patients who underwent surgery for LDH, including cases with CES, between 2014 and 2023. The DSCSA was measured at the narrowest level of the dural sac using axial T2-weighted magnetic resonance imaging. Univariable and multivariable logistic regression were performed on DSCSA and

Rising suspicion, stable disease: an 8-year analysis of suspected and confirmed cauda equina syndrome.

BACKGROUND CONTEXT: Suspected cauda equina syndrome (CES) referral pathways are designed to maximise sensitivity, often prompting urgent MRI based on single red-flag features. Rising referral volumes have raised concern that pathway-driven behaviour may not reflect true disease incidence. Longitudinal analyses anchored to robust diagnostic adjudication remain limited. PURPOSE: To evaluate temporal trends in suspected CES referrals over 8 years within a regional tertiary spinal service, using multidisciplinary team (MDT) clinico-radiological consensus as the diagnostic gold standard. Cauda Equina Syndrome Following Interbody Cage Migration after Lumbar Fusion: A Rare Case Report.

INTRODUCTION: Transforaminal lumbar interbody fusion (TLIF) is a widely accepted surgical technique for the management of degenerative lumbar spondylolisthesis. The procedure provides circumferential decompression, restoration of disc height, and segmental stabilization. Despite favorable outcomes, complications such as interbody cage migration or posterior cage back-out may occur compromise surgical success, necessitating revision surgery. CASE REPORT: We present the case of a 45-year-old male with symptomatic degenerative L4-L5 spondylolisthesis who underwent minimally invasive TLIF (MIS-TL

Cauda Equina Syndrome: A Narrative Review Exploring the Pathophysiology, Clinical Spectrum, Diagnostic Strategies, and Time-Critical Management.

Cauda equina syndrome (CES) is an uncommon but severe neurological condition resulting from compression of the lumbosacral nerve roots, leading to motor, sensory, and autonomic dysfunction. Despite its low incidence, CES carries a high risk of permanent disability and medico-legal consequences due to the narrow therapeutic window for intervention. This narrative review synthesizes current evidence on the anatomical basis, pathophysiology, etiology, clinical presentation, diagnostic pathways, classification systems, and management strategies for CES. Particular emphasis is placed on the prognos

Simultaneous saddle pulmonary embolism and right atrial thrombus managed with ICE-guided dual suction Thrombectomy.

Saddle pulmonary embolism (PE) complicated by right atrial thrombus in transit is an infrequent but highly lethal condition. Standard treatments such as systemic thrombolysis or surgical embolectomy carry significant risks, particularly in elderly or comorbid patients. We describe the case of a 76-year-old man with spina bifida, cauda equina syndrome, chronic immobility, and multiple comorbidities who presented in shock. Imaging revealed a large saddle PE with mobile right atrial thrombus and right ventricular strain. Given prohibitive surgical risk and contraindications to thrombolysis due to
