Evaluating home-delivered meal service programs for obesity: a randomized controlled approach.

BACKGROUND: Though a healthy diet is the most effective solution for obesity, it is hindered by two main barriers: difficulty in accessing or preparing healthy meals and lack of portion control knowledge. OBJECTIVE: This study compared two different medical nutrition therapies (home-delivered diet meal service [HDMS] combined with dietary counseling and dietary counseling solely) in terms of anthropometric measurements, biochemical parameters and eating behaviors in women with overweight or obesity who participated in a weight loss program. DESIGN: A non-randomized, controlled, parallel-grou

Efficacy and Safety of Peripheral Nerve Blocks After Arthroscopic Shoulder Surgery: A Systematic Review and Network Meta-Analysis.

BACKGROUND: Nerve blocks are commonly used for analgesia following shoulder arthroscopy, but the optimal technique remains uncertain. This network meta-analysis (NMA) evaluates the effect of peripheral nerve blocks in adults undergoing shoulder arthroscopy. METHODS: We searched databases including PubMed, Embase, Web of Science, and the Cochrane Library from their inception to July 2025. Randomized controlled trials (RCTs) to evaluate the effect of peripheral nerve blocks after arthroscopic shoulder surgery were included. The primary outcome was the pain scores at 6 hours postoperatively. RE

Neuroblastoma in neonates: a case report and literature review.

Neuroblastoma in neonates is a rare solid tumor. This report presents two rare cases of cervical neuroblastoma, both characterized by stridor and coughing during feeding due to tumor compression. The first case of neuroblastoma presented with an intact capsule, allowing for complete surgical resection with preservation of the cervical vasculature and nerves. In contrast, although preoperative MRI indicated an intact capsule in the second case, the intraoperative findings revealed no discernible capsule formation and involvement of both the internal and external carotid arteries. None of the tw

Horner's Syndrome: An Unusual Complication of Transcervical Zenker's Diverticulectomy.

Horner's syndrome is a clinical entity encountered by head and neck surgeons in patients with penetrating neck injury, post-neck dissections, and cervical sympathetic chain schwannomas. However, Horner's syndrome after transcervical resection of Zenker's diverticulum has not been previously described in the published literature. A patient who had failed two previous attempts at endoscopic repair of a large Zenker's diverticulum underwent transcervical diverticulectomy with cricopharyngeal myotomy. A previous history of anterior cervical fusion at C5 and C6 was also present. Postoperatively, he

Repetitive Transcranial Magnetic Stimulation for Refractory, Widespread Complex Regional Pain Syndrome Type II: A Case Report.

Complex regional pain syndrome (CRPS) is a poorly understood pain disorder often resistant to conventional therapies. Although typically localized, CRPS can spread to other regions. Repetitive transcranial magnetic stimulation (rTMS) is a non-invasive treatment showing promise for neuropathic pain associated with CRPS. A 33-year-old active-duty service member developed widespread, refractory CRPS following a severe ankle sprain, with persistent pain and functional limitations despite medications, ketamine infusions, and sympathetic nerve blocks. The patient received 21 rTMS sessions over four

Autonomic Dysfunction After Pediatric Concussion: A Scoping Review.

PurposeA concussion is a common injury in children and adolescents. Despite how common these brain injuries are, relatively little is known about the underlying pathophysiology of pediatric concussion. This scoping review aims to collect and synthesize the literature on autonomic dysfunction (AD) in pediatric concussion, with a secondary aim to identify the procedures and measures testing autonomic function (AF).MethodsWe completed a systematic search using six databases of articles pertaining to AD after concussion. The resulting articles were exported, screened, and reviewed based on Preferr

Effect of preoperative ultrasound-guided stellate ganglion block on pain thresholds and postoperative nausea and vomiting in gynecological laparoscopy: a randomised clinical trial.

BACKGROUND AND OBJECTIVES: Despite advancements in laparoscopic techniques, postoperative pain and nausea/vomiting (PONV) remain primary barriers to optimal Enhanced Recovery After Surgery (ERAS) outcomes in gynecology. While stellate ganglion block (SGB) modulates autonomic function, its efficacy in elevating pain thresholds, reducing rescue medication use, and accelerating functional recovery requires robust evidence. This randomized trial evaluated whether preoperative SGB could refine these ERAS metrics. METHODS: This single-center randomized controlled trial included 84 patients undergoin

Misdiagnosis of superior sulcus tumours: a scoping review.

BACKGROUND: Superior sulcus tumours (SSTs) are a rare but serious non-small cell lung cancer that requires aggressive multi-modal intervention. Existing literature suggests patient outcomes are adversely affected by diagnosis delay, often from misdiagnosis as a musculoskeletal condition. As such, the aims of this review were to identify the key clinical features of SSTs documented in the literature and differentiate them from commonly misdiagnosed musculoskeletal pathologies. METHODS: In accordance with Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines for scoping

Horner's syndrome due to postoperative hematoma following anterior cervical discectomy and fusion.

BACKGROUND: We report a symptomatic retropharyngeal hematoma resulting in a transient Horner's syndrome in a 51-year-old female following an anterior cervical discectomy and fusion (ACDF). CASE DESCRIPTION: A 51-year-old female underwent an ACDF at C5-6 for cervical spondylotic radiculopathy. On postoperative day 4, she developed a retropharyngeal hematoma and Horner's syndrome. A review of the preoperative magnetic resonance imaging and intraoperative findings in this patient uniquely indicated that her middle sympathetic ganglion and nerve ran inside the common carotid artery. CONCLUSION:

Beyond Parkinson's: The Challenge of Diagnosing Multiple System Atrophy-C.

Multiple system atrophy (MSA) is a rare, rapidly progressive neurodegenerative disorder characterized by varying combinations of Parkinsonian features, cerebellar ataxia, and autonomic dysfunction. The disease is classified into two major subtypes based on predominant symptoms: The Parkinsonian variant (MSA-P) and the cerebellar variant (MSA-C). MSA-C, in particular, presents significant diagnostic challenges due to its overlapping clinical features with other neurodegenerative disorders, such as cerebellar ataxias and atypical Parkinsonian syndromes. Early stages of the disease may not show c

Clinical implication of regional osteopenia in complex regional pain syndrome: a retrospective comparative study.

BACKGROUND: Despite the significant role of impaired bone metabolism in complex regional pain syndrome (CRPS), the clinical implications of osteopenia remain a focal point of investigation. We examined the prevalence and risk factors of affected limb osteopenia in CRPS and its association with the response to sympathetic blockade. METHODS: 167 patients with CRPS who underwent CT of their affected limbs were retrospectively reviewed. After conducting univariable analyses where regional osteopenia determined by CT was dependent and other clinical factors were independent variables, the first mu

Pure autonomic failure: a natural history study of the Queen Square cohort.

The current research challenge in pure autonomic failure (PAF) lies in identifying specific biomarkers that can differentiate it from the other Lewy body disorders (Parkinson's disease, Parkinson's disease dementia, dementia with Lewy bodies) and multiple system atrophy in the early stages and predict phenoconversion trajectories to more widespread impairment. In this study, we described the natural history of our cohort of patients with PAF over five decades and validated a cluster of clinical, autonomic, and neuroimaging biomarkers that help identify clinical profiles susceptible to further

Autonomic correlates of large muscle group movements during non-REM sleep in restless legs syndrome: a comparative analysis with periodic and non-periodic leg movements.

STUDY OBJECTIVES: Large muscle group movements during sleep (LMMS) have recently been recognized as a prevalent feature in patients with restless legs syndrome (RLS), yet their autonomic profile remains insufficiently characterized. This study aimed to compare heart rate (HR) changes associated with LMMS to those accompanying short-interval (SILMS), periodic (PLMS), and isolated leg movements (ISOLMS) during non-REM sleep in RLS. METHODS: Thirty drug-free RLS patients (20 women, mean age 57.6 ± 12.73 years) underwent full-night polysomnography. For each subject, five arousal-associated events

Bilateral Stellate Ganglion Block After Ventricular Arrhythmia May Hide Clinical Signs of the Cushing Triad: A Case Study and Review of the Literature.

BACKGROUND: Stellate ganglion blockade (SGB) has been described as a rescue therapy in recurrent electrical storm, with very few reported complications, except local hematomas. Yet the proximity of major blood vessels and the risk of a sympathetic reaction could expose patients to additional adverse events. CASE REPORT: We reported the case of a 34-year-old patient, in whom a bilateral SGB was performed after in-hospital cardiac arrest due to recurrent polymorphic ventricular tachycardia. She developed bilateral areflexic mydriasis, diffuse cerebral edema, pneumocephalus, and brainstem hernia

IgG4-related disease in correlation with left main coronary artery aneurysm as a rare cause of heart transplantation.

BACKGROUND: Eagle syndrome is caused by an elongated styloid process affecting carotid arteries and cranial nerves. Pain, dysphagia, tinnitus, paresthesia (classic subtype), and neurovascular events (vascular subtype) may be triggered by head movements or arise spontaneously. However, Eagle syndrome remains underappreciated in the neurological community. We aimed to determine the most common neurological and non-neurological clinical presentations in patients with Eagle syndrome and to assess the clinical outcome post-surgical resection in comparison to non-surgical therapies. METHODOLOGY: We
