Association of Orthostatic Hypotension with Patient-Reported Outcomes and Dopaminergic Medication in Parkinson Disease: A Pragmatic Cross-Sectional Study.

BACKGROUND: Orthostatic hypotension (OH) is common in Parkinson disease and contributes to injury, fatigue and reduced quality of life. More efficient approaches to identify higher-risk patients are needed. METHODS: A cross-sectional, single-center study of consecutive people with Parkinson disease was conducted over three months. Linear models adjusted for age, sex and disease duration assessed associations of OH to patient-reported outcomes (autonomic symptoms, quality of life), medications and other clinical features. Receiver operating characteristic curves evaluated patient-reported outc

Skin-Based α-Synuclein Deposits Detection Across the Prodromal Continuum of Synucleinopathies: Updated Evidence and Perspectives.

Parkinson's disease (PD) and associated synucleinopathies are preceded by a prolonged prodromal phase during which neurodegenerative processes evolve years before the onset of motor or cognitive symptoms. Identifying biologically specific and accessible biomarkers during this window is critical for early diagnosis, risk stratification, and the development of disease-modifying therapies. Increasing evidence supports the skin as a key peripheral tissue involved in synucleinopathy, offering a minimally invasive source for in vivo detection of pathological α-synuclein. This review summarizes curre

The unique efficacy of clozapine is attributable to muscarinic receptor agonism.

Clozapine is superior to all other antipsychotic drugs for treatment-resistant schizophrenia, but the molecular underpinnings for this have remained elusive for decades. Recently, we proposed a cholinergic mechanism, based on the distinct pharmacology of clozapine in the parasympathetic branch of the autonomic nervous system, a hypothesis strengthened by the translation of a new cholinergic-based antipsychotic drug into clinical practice. A recent article has challenged the cholinergic hypothesis and instead proposed a mechanism involving the action of clozapine at noradrenaline receptors of t

Favorable Effects of Renal Denervation in Hypertension-Related Comorbidities. A Systematic Review and Meta-Analysis.

BACKGROUND: Renal denervation (RDN) according to the latest ESH guidelines can be considered as a treatment option for patients with resistant hypertension. However, excess sympathetic nervous system activity constitutes an underlying pathophysiological mechanism in many disorders other than arterial hypertension. AIMS: This systematic review and meta-analysis investigated the therapeutic effects of RDN beyond arterial hypertension. METHODS: A systematic review and meta-analysis were conducted following PRISMA guidelines. Electronic databases (PubMed, Embase, Cochrane) were searched for stud

Adenosine A2a receptors offset cholinergic control of blood pressure, autonomic neuropathy, and brainstem neuroinflammation in sepsis.

The cholinergic antiinflammatory pathway functions to counterbalance the innate immune response to inflammatory disorders including sepsis. Considering the reported contradictory role of adenosine in inflammation, the present study investigated whether central adenosine A2a receptors (A2aARs) arbitrate the cholinergic modulation of cardiovascular and autonomic dysfunction induced by sepsis. Rats were instrumented with femoral and intracisternal (i.c.) catheters for hemodynamic monitoring and central drug administration, respectively, and sepsis was induced by cecal ligation and puncture (CLP).

Up-regulation of tRNA-derived miR-1274b in tears of Parkinson's disease patients.

BACKGROUND: Parkinson's disease (PD) is a complex neurodegenerative disorder characterized by motor and non-motor symptoms. Early diagnosis remains challenging due to the lack of reliable, non-invasive biomarkers. Here, we explored the potential of tear fluid (TF) as a diagnostic source by profiling its microRNA (miRNA) content. METHODS: The study included a discovery (8 PD, 7 healthy controls (HC)) and a replication cohort (9 PD, 9 HC). miRNA expression was assessed using TaqMan Human MicroRNA arrays. Independent in-silico validation was conducted using a qPCR-based miRNA array dataset compr

Metabolomic Cerebrospinal Fluid Biomarkers for the Diagnosis of Atypical Parkinsonian Syndromes.

Diagnosis of atypical parkinsonian syndromes (APS), including progressive supranuclear palsy (PSP) and multiple system atrophy (MSA), rely on clinical criteria that often result in misclassification or delayed confirmation. Cerebrospinal fluid (CSF) metabolomics offers the potential to identify disease-specific biochemical "fingerprints". The aim of the study is to identify CSF metabolomic biomarkers that distinguish PSP and MSA from each other and from non-neurodegenerative controls. Targeted mass spectrometry-based metabolomics was performed on CSF samples from 30 patients with MSA, 41 with

The role of the choroidal nervous system in central serous chorioretinopathy and associated diseases.

Central serous chorioretinopathy (CSCR) and associated diseases, are linked to abnormal remodeling of the choroidal vasculature, venous congestion and impaired regulation of choroidal blood flow. Although anatomical, genetic and hormonal factors have been identified, a unifying pathophysiological mechanism remains unclear. In this review, we propose that peripheral choroidal nervous system dysfunction may represent a central driver of this choroidal phenotype. First, we provide an updated overview of the organization and functions of the autonomous nervous system, its interaction with the hypo

Drug repurposing for disease-modifying effects in multiple system atrophy.

Multiple system atrophy (MSA) is a rapidly progressive neurodegenerative disorder lacking any proven disease-modifying therapy. Drug repurposing offers a strategy to accelerate the development of treatments by utilizing agents originally approved for other indications. This review summarizes repurposed drugs investigated as disease-modifying therapies for MSA, spanning preclinical in vitro and animal studies and clinical trials. We focus on agents targeting key pathogenic mechanisms in MSA-including α-synuclein aggregation (e.g., sirolimus/rapamycin, rifampicin, lithium, nilotinib, epigallocat

Measuring autonomic influence on gastrointestinal contractility: development and preliminary validation of novel capsule-based indices.

The migrating motor complex (MMC) is a key feature of fasting gastrointestinal (GI) motility, but its disruption in neuropathic conditions remains poorly characterized. Wireless motility capsules (WMCs) offer a noninvasive means of collecting motility data, facilitating study of larger cohorts. We aimed to develop WMC-derived metrics to identify neuropathic dysmotility and its associations with autonomic nervous system (ANS) function. We analyzed WMC data from 98 controls and 71 people living with human immunodeficiency virus (HIV; PWH) in whom autonomic neuropathy (AN) and delayed small bowel

Long-term NSAID use after CRPS diagnosis associated with increased risk of peripheral neuropathy and paresthesia.

PURPOSE: Long-term use of nonsteroidal anti-inflammatory drugs (NSAIDs) is common in complex regional pain syndrome (CRPS) management, but its impact on neuropathic sequelae remains unclear. This study evaluates the association between prolonged NSAID use post-CRPS diagnosis and peripheral neuropathy and paresthesia. METHODS: This retrospective cohort study utilized the TriNetX Research Network to identify patients who were diagnosed with CRPS type I between January 1, 2000 and January 1, 2014. Patients were stratified by long-term NSAID use until 1 year after diagnosis. Propensity score matc

A hypothesis connecting dysgeusia due to defects in ATP-P2X3 signaling and fatigue in myalgic encephalomyelitis/chronic fatigue syndrome: lessons learned from long-COVID.

Myalgic encephalomyelitis (ME)/chronic fatigue syndrome (CFS) is a neuroimmune disease characterized by debilitating post-exertional malaise (PEM), brain-fog/cognitive problems, and dysregulation of the autonomic nervous system. Currently, there are no objective biomarkers for ME/CFS despite decades of research. Here, we compile evidence from literature that supports taste dysfunction, particularly alterations of taste perception mediated by Type II taste receptor cells, may be a critical underrecognized feature of ME/CFS. The impetus is drawn from the emerging evidence of clinicopathological

[Rapidly progressive cognitive impairment as a single clinical manifestation of anti-NMDA receptor encephalitis].

Autoimmune encephalitides (AE) comprise a group of autoimmune diseases of the nervous system that primarily affect the brain's gray matter. The pathogenesis of AE is largely driven by the production of autoantibodies targeting intracellular and extracellular structures within the nervous system. The most extensively studied forms are those associated with antibodies targeting the NMDAR, AMPAR-, GABAβR-, LGI1, and Caspr2 extracellular antigens. Evidence suggests that anti-NMDA receptor encephalitis occurs more frequently than viral encephalitis in individuals under 30 years of age. Anti-NMDA re
