Oxidative stress drives disuse-induced chronic pain through sex-divergent immune-neural mechanisms.

Physical disuse is a major risk factor for complex regional pain syndrome type I. Using a rat chronic post-cast pain model, we investigated how oxidative stress modulates pain processing across the neuraxis and evaluated sex as a biological variable. Following immobilization, both sexes developed widespread mechanical hyperalgesia, which was markedly attenuated by the antioxidant Tempol. However, we uncovered a striking phenotypic dissociation: whereas male rats also developed tactile and cold allodynia, female rats completely lacked tactile allodynia. Peripherally, Tempol reduced neurogenic i

Preventive Effects of Avocado/Soybean Unsaponifiables on Complex Regional Pain Syndrome Type I in a Rat Model.

Background and Object: Complex Regional Pain Syndrome Type I (CRPS-I) is a debilitating condition often triggered by trauma, with early pathophysiology driven by neuroinflammation and oxidative stress. Avocado/soybean unsaponifiables (ASU) possess potent anti-inflammatory and antioxidant properties but have never been tested for CRPS-I prevention. This study investigated the preventive effects of early systemic administration of ASU on the development of CRPS-I-like features in a validated rat model of tibial fracture and cast immobilization. Methods: Twenty adult male Wistar rats were randomi

"Google It": Experiences of Children and Parents During the Assessment and Diagnosis of Complex Regional Pain Syndrome Type I.

Complex Regional Pain Syndrome Type I (CRPS-I) is a chronic pain disorder that can affect children most commonly in early adolescence. In comparison to adult literature, there is a sparsity of evidence regarding the effect of CRPS-I on children. This evidence gap contributes to diagnostic delays, affecting children and parents physically, psychologically, and socially. The experiences of children and their parents when receiving a CRPS-I diagnosis have yet to be explored. The current study explored the experiences of children and parents during the assessment and diagnosis of CRPS-I. This qual

Effect of high-intensity laser therapy and mirror therapy on complex regional pain syndrome type I in the hand area: A randomized controlled trial.

BACKGROUND: Complex regional pain syndrome type I (CRPS-I) is a painful condition with peripheral and central nervous system dysfunction, disproportionate inflammation, and the resultant muscle atrophy and restriction of motion. The use of high-intensity laser therapy (HILT) is being considered to reduce inflammation and neural and musculoskeletal pain. As maladaptive neuroplasticity occurs, peripheral treatment may not be enough and a combination of peripheral and centrally-focused interventions may be required. PURPOSE: To explore the impact of HILT combined with mirror therapy (MT) on pain

Usefulness of digital infrared thermography video using FLIR T560 in detecting hypothermia associated with complex regional pain syndrome type I: A CARE-compliant case report.

RATIONALE: Because the diagnosis of complex regional pain syndrome (CRPS) majorly relies on patients' subjective clinical complaints, CRPS can be overdiagnosed, underdiagnosed, or missed entirely in clinical practice. CRPS is related to dysfunction of the autonomic nervous system, which causes temperature changes in patients' skin. This case report evaluates the effectiveness of the FLIR T560 portable thermal imaging camera in detecting hypothermia associated with CRPS type I, potentially enhancing diagnostic accuracy. PATIENT CONCERNS: We present a 25-year-old female with pain and limited pa

Neuronal Reg3β/macrophage TNF-α-mediated positive feedback signaling contributes to pain chronicity in a rat model of CRPS-I.

Complex regional pain syndrome type I (CRPS-I) develops after an initial injury. It causes prolonged pain that persists beyond the usual expected time for tissue healing. Mechanisms underlying pain chronicity of CRPS-I remain unknown. Here, we identified the presence of long-lasting infiltration of macrophages in local dorsal root ganglia (DRG) of a rat model of CRPS-I. We demonstrate that regenerating islet-derived 3β (Reg3β) is specifically produced by DRG neurons upon model establishment and functions as an important signaling molecule to drive proinflammatory macrophage infiltration in loc

Diagnostic Guidance for Chronic Complex Regional Pain Syndrome Type I and Type II from The American Society of Interventional Physicians (ASIPP).

BACKGROUND: Complex Regional Pain Syndrome (CRPS) is a challenging and often disabling condition marked by persistent pain, most commonly in a limb following injury or surgery. It presents with a wide array of symptoms, including intense pain, swelling, alterations in skin color and temperature, motor dysfunction, and trophic changes such as skin and tissue atrophy. While the precise cause of CRPS is not fully understood, it is thought to stem from abnormal nervous system activity, leading to heightened pain sensitivity and inflammatory responses. A thorough understanding of CRPS is essential

TRPV4 Channel Participates in the Nociception and Spinal Neuroinflammation Involved in a Model of Complex Regional Pain Syndrome Type I (CPIP) in Mice.

Complex regional pain syndrome type I (CRPS-I) is a disabling pain condition that typically arises after ischemia/reperfusion injury. Managing its acute and chronic pain remains challenging. The transient receptor potential vanilloid 4 (TRPV4) channel is involved in neuropathic and inflammatory pain, making it a potential target for CRPS-I treatment. Thus, we evaluate the participation of the TRPV4 channel in nociception and spinal neuroinflammation induced by a preclinical model of CRPS-I. Male mice (C57BL/6) were used for chronic post-ischemia pain (CPIP) induction. Mechanical allodynia, the

Bone marrow mesenchymal stem cells attenuate pain and modulate peripheral sodium channel activity in a rat model of complex regional pain syndrome type I.

Complex Regional Pain Syndrome Type I (CRPS-I) is a chronic neuropathic pain disorder characterized by peripheral nerve hyperexcitability and altered nociceptive signaling. Voltage-gated sodium channels (Nav1.7, Nav1.8, Nav1.9) in dorsal root ganglia (DRG) are key contributors to pain hypersensitivity. This study investigated the analgesic effects and underlying mechanisms of bone marrow mesenchymal stem cell (BMSC) transplantation in a CRPS-I rat model. The model was induced by hind limb ischemia-reperfusion, followed by intrathecal administration of BMSCs. Pain behaviors were assessed using
