Laser therapy: palliative care for the Harlequin syndrome?

Harlequin syndrome is a rare condition characterized by facial dysautonomia with anhidrosis, hemifacial blush, and other nonfacial manifestations without conventional treatment. This report presents a case study of the potential treatment of Harlequin syndrome using laser therapy. A single-case AB design was used, allowing for a systematic comparison between the baseline and intervention phases. The baseline phase involved data collection during a period without intervention (control condition), that is before laser therap. As evaluation criteria, a clinical assessment of skin turgor, sweating

OPA1 Deficiency Impairs NGF Signaling and Drives Sympathetic Neurodegeneration.

Peripheral sympathetic neurodegeneration drives cardiac dysfunction in dominant optic atrophy, revealing a critical neuro-cardiac link. Optic atrophy factor-1 haploinsufficiency disrupts mitochondrial dynamics and neurotrophic signaling, causing targeted sympathetic denervation and arrhythmias. Restoring nerve growth factor transport and mitochondrial health in sympathetic neurons represents a promising therapeutic avenue for cardiac autonomic disorders. Future research must unravel mechanisms of neurocardiac crosstalk to develop precise interventions against neurogenic cardiac disease progres

Protocol for Safety, Feasibility, and Efficacy of Using Targeted Transcutaneous Spinal Cord Stimulation to Treat Hypotension During Acute Inpatient Rehabilitation in Individuals With SCI.

BACKGROUND: Transcutaneous spinal cord stimulation (tSCS) has shown evidence of leveraging intact neural circuits to increase blood pressure (BP) in hypotensive people with chronic spinal cord injury (SCI). OBJECTIVES: The primary aim is to determine the safety and feasibility of using tSCS to promote seated systolic BP (SBP) within a target range of 110-130 mm Hg and to mitigate the fall in BP with transition to an upright position in hypotensive participants during acute inpatient rehabilitation (AIR) following SCI. DESIGN/METHODS: Cathode placement on the spine will be randomized among 4

Cycling exercise for autonomic symptoms in early Parkinson's disease.

This exploratory analysis of a pilot randomized controlled trial investigated the effects of cycling exercise on autonomic symptoms in early Parkinson's disease (PD). Thirty patients were assigned to a cycling group, which performed three 40-60-min sessions per week for 24 weeks, or to a control group. Autonomic symptoms were assessed using the SCOPA-AUT scale. After 24 weeks, urinary symptoms increased significantly less in the cycling group compared with controls, whereas no differences were observed in other autonomic domains. Among urinary subitems, only urinary frequency improved signific

Use of a thermal comfort wearable improves temperature intolerance in patients with postural tachycardia syndrome.

PURPOSE: Temperature intolerance is a common and often debilitating symptom of postural tachycardia syndrome (POTS), however treatment options are limited. In this open-label pilot study, we examined the use of a novel wearable thermal watch (Embr Wave2™) on thermoregulatory and other autonomic symptoms in POTS. METHODS: Participants with POTS and temperature intolerance were recruited from the Stanford autonomic disorders clinic. All patients completed an online battery of autonomic and sleep questionnaires including the composite autonomic symptom score-31 (COMPASS-31), the Delphi interocep

Metabolomics of Multiple System Atrophy Patient-Derived Striatal Medium Spiny Neurons.

In multiple system atrophy (MSA), the fatal movement disorder, cell populations of the striatum and other subcortical brain regions degenerate, leading to a rapidly progressive, atypical Parkinsonian syndrome. The pathophysiology of neurons and glial cells shows misfolding, aggregation, and increased release of the protein α-synuclein. In addition, neuronal hypoexcitability, a reduction in the activity of the mitochondrial respiratory chain, and a dysregulation of the enzymes involved in the biosynthesis of coenzyme Q10 were observed in human stem-cell models. In this study, untargeted and tar

An alternative "Port"? Rationale and development process for POts Reprocessing Therapy (PORT), a brain-body therapy for postural tachycardic syndrome (POTS).

PURPOSE: Postural tachycardia syndrome (POTS) is a chronic disorder marked by excessive orthostatic tachycardia, without clear structural/organic (e.g., cardiovascular) etiology. Recent evidence suggests that persistent autonomic symptoms may be driven by threat-induced and central nervous system-maintained dysregulation, similar to centralized chronic pain disorders. This study describes the rationale and development process of a brain-body intervention-POts Reprocessing Therapy (PORT)-aimed at reducing orthostatic symptomatology by taking advantage of the brain's plasticity. METHODS: PORT w

Tremor in multiple system atrophy: a systematic literature review.

Up to 80% of patients with multiple system atrophy (MSA) exhibit tremor or tremor-like phenomena, yet the clinical features of these manifestations remain insufficiently characterized. Clearer phenotypic delineation may support differential diagnosis from other tremor-associated disorders, particularly Parkinson's disease (PD). We aimed to determine characteristics and prevalence of tremor, distribution across MSA phenotypes and body regions, and its treatment responsiveness. We systematically searched PUBMED for articles reporting tremor in MSA, following PRISMA guidelines. Eligible studies w

Optical coherence tomography findings in multiple system atrophy: insights into disease diagnosis, clinical correlations, and dopaminergic degeneration.

BACKGROUND: Differentiating multiple system atrophy (MSA) from Parkinson’s disease (PD) and progressive supranuclear palsy (PSP) remains challenging. Retinal optical coherence tomography (OCT) offers noninvasive insights into neurodegeneration but remains underexplored in MSA. OBJECTIVES: To compare OCT-derived retinal parameters in MSA, PD, PSP, and healthy controls (HCs), evaluate associations with clinical features and dopaminergic degeneration. METHODS: We enrolled 21 MSA, 22 PD, and 23 PSP patients, plus 23 HCs. OCT measured macular and retinal nerve fiber layer thicknesses. Clinical asse

What Are the Patient-reported Outcomes After Elective Transtibial Amputation? A Comparison of CRPS, Neuropathic Pain, and Other Conditions That Lead to Late Amputation After Limb Salvage.

BACKGROUND: In the United States, approximately 2.3 million people live with limb loss, of whom about 91% have undergone lower extremity amputation. Despite these numbers, relatively few studies have evaluated outcomes after elective transtibial amputation. In particular, we do not know whether the outcomes after amputation for intractable neurogenic pain such as complex regional pain syndrome (CRPS) are comparable to outcomes after late amputations (6 weeks or more after limb salvage) performed for other reasons such as recurrent infection, soft tissue problems, recalcitrant nonunion, poor fu

Sudomotor dysfunction in type 1 diabetes: A case report.

Sudomotor dysfunction is a frequent and early sign of diabetic neuropathy. It is a disorder of the autonomic nervous system that involves small fibers and is usually underdiagnosed despite the impact it has on the patients' quality of life. Its study is recommended as a strategy for early detection as an indicator of dysautonomic dysfunction and management of later complications. Although it can be highly disabling, it often goes unnoticed, as it is considered less alarming than other dysautonomic symptoms. We present the case of a diabetic patient with sudomotor disfunction who underwent a co

Generation and characterization of human induced pluripotent stem cells from neuropathologically confirmed multiple system atrophy patient-derived fibroblasts.

BACKGROUND: Multiple system atrophy (MSA) is an adult-onset, fatal neurodegenerative disease, classified as a synucleinopathy along with Parkinson's disease and dementia with Lewy bodies. The etiology of MSA is unknown and the treatment remains symptomatic with limited efficacy. To date most studies have been conducted on genetic mice models, engineered to overexpress α-synuclein (aSyn); and transmission mice models, treated with intracerebral injections of MSA brain extracts or fibrillar aSyn which can be assembled de novo or using MSA seeds. An in vitro human model is urgently needed to iden

Cardiovagal Tone in Generalised Anxiety Disorder: A Case Control Study.

Introduction Generalised anxiety disorder (GAD) is a prevalent psychiatric condition increasingly associated with autonomic nervous system dysfunction. Specifically, impaired cardiovagal tone, a marker of reduced parasympathetic modulation of cardiac function, has been linked to emotional dysregulation, behavioural rigidity, and elevated cardiovascular morbidity. The standard Ewing's battery of parasympathetic autonomic function tests provides a reproducible and non-invasive means of assessing such dysfunction. This study aimed to comprehensively evaluate cardiovagal tone in newly diagnosed, t

Herpesvirus genome integration in whole-genome sequences of dementia and control cohorts.

INTRODUCTION: The infectious hypothesis suggests that microbes like herpesviruses may play a role in the pathogenesis of Alzheimer's disease (AD) and other related dementias through methods that may include viral genome integration. The occurrence of herpesvirus genome integration in dementia patients has not been thoroughly characterized. METHODS: Over 7500 total whole-genome sequences from control, frontotemporal dementia/amyotrophic lateral sclerosis spectrum, Lewy body dementia (LBD), multiple system atrophy (MSA), and AD cohorts were screened for the integration of pathogen genomes using

Long-term relief of CRPS-associated limb dystonia with ketamine infusion.

Complex regional pain syndrome (CRPS) with associated dystonia is a severely debilitating condition often refractory to conventional management. We present a case report of three patients with chronic, treatment-refractory CRPS-related limb dystonia. Patients were treated with multiday intravenous ketamine infusions (target dose 30 mg/hour) every 3-6 months, supplemented with oral ketamine troches. This treatment protocol resulted in the complete resolution of dystonia in two patients and significant functional improvement in the third. The treatment was well-tolerated, with transient and mana

Symptom-based indicators of autonomic dysfunction and their bidirectional associations with Parkinson's disease incidence and adverse outcomes.

BACKGROUND: Symptom-based indicators suggestive of autonomic dysfunction are common but underrecognized in Parkinson's disease (PD), with potential implications as a biomarker of aging for early detection and prognosis. We aimed to examine the associations between autonomic dysfunction and PD in a large, population-based cohort. METHODS: We analyzed 374 657 UK Biobank participants who were free of PD at baseline. Autonomic symptoms-including orthostatic hypotension, constipation, urinary and sexual dysfunction, hyperhidrosis, and other autonomic disorders-were identified via hospital records.

Heart Rate Variability and Cognitive Function as Potential Endophenotypes in Schizophrenia: A Cross-Sectional Observational Study Using First-Degree Relatives.

Background Heart rate variability (HRV) represents beat-to-beat fluctuations in heart rate arising from the dynamic balance between sympathetic and parasympathetic nervous system activity. Altered HRV reflects autonomic dysregulation and has been reported across several psychiatric disorders, including schizophrenia, where it may contribute to cardiovascular risk and cognitive dysfunction. Aim The aim of this study was to compare the time-domain and frequency-domain heart rate variability parameters between patients with schizophrenia and their first-degree relatives and to analyze the correla
