Observation of the progression of tension in intracranial arachnoid cyst: a case report and review of literature.

The intracranial arachnoid cyst (IAC) are common benign intracranial space-occupying lesions in children, which can be categorized into tension and non-tension IAC based on imaging features. As observed cases of tension progression in IAC are extremely rare, the relationship between the two is currently unclear. Here, we present a rare case that demonstrates the transition from a non-tension IAC to a tension cyst by complete imaging evidence. Magnetic Resonance Imaging (MRI) of the brain performed 6 days after birth revealed the presence of a small non-tension IAC at the left cerebellopontine

Endoscopic Third Ventriculostomy for Adjunctive Management of Hydrocephalus Associated With Previously Embolized Vein of Galen Malformations.

BACKGROUND AND OBJECTIVES: Hydrocephalus is a comorbid condition in patients with vein of Galen malformations (VOGMs), and its management is complicated by venous hypertension, elevating surgical risk. Endoscopic third ventriculostomy (ETV) is commonly used for cerebrospinal fluid (CSF) diversion in children, but there are limited available data on its efficacy and safety for patients with VOGM. We aimed to review our experiences with ETV across 2 high-volume centers for VOGM patients with hydrocephalus. METHODS: A two-center retrospective review identified all patients with VOGM who underwen

Cryptococcal Meningitis in an Immunocompetent Patient With an Initially False-Negative Cerebrospinal Fluid Analysis: A Case Report.

Cryptococcal meningitis (CM) is traditionally associated with immunocompromised states, such as HIV/AIDS. However, its presentation in immunocompetent hosts is increasingly recognized and frequently poses a diagnostic dilemma. A 56-year-old immunocompetent male presented with obstructive hydrocephalus following a month of worsening headaches. Initial cerebrospinal fluid (CSF) analysis, including India ink, cryptococcal antigen, and cultures obtained during ventriculoperitoneal shunt placement, was unremarkable. Subsequent brain MRI demonstrated evolving leptomeningeal enhancement and small cer

Endoscopic management of cranial screw migration to cerebral aqueduct causing hydrocephalus.

BACKGROUND AND IMPORTANCE: Intracranial migration of cranial fixation hardware is a rare complication of neurosurgical procedures, and migration into the ventricular system is exceedingly uncommon. When such migration leads to obstruction of cerebrospinal fluid (CSF) pathways, it can result in acute hydrocephalus requiring urgent intervention. CLINICAL PRESENTATION: A 10-year-old girl with a history of left frontotemporal ganglioglioma underwent synthetic Medpor cranioplasty for radiation necrosis-associated calvarial erosion. Intraoperatively, a fixation screw inadvertently plunged through a

Insights into Accelerated MRI Protocols for Pediatric Brain Assessment in Emergency Cases.

Two accelerated magnetic resonance imaging (MRI) protocols for pediatric brain imaging, GOBrain and Deep Resolve Swift Brain, developed by Siemens Healthineers (Erlangen, Germany), were evaluated in a series of clinically relevant pediatric cases at 3 Tesla. Pediatric patients are particularly prone to motion, may be uncooperative, and often require sedation, especially in emergency settings. Consequently, there is a persistent clinical demand for fast brain MRI protocols that provide diagnostically sufficient image quality while minimizing examination time. Contemporary turbo spin-echo (TSE)-

Atypical Teratoid/Rhabdoid Tumor of the Posterior Fossa Mimicking Medulloblastoma in an Infant: A Case Report.

Atypical teratoid/rhabdoid tumor (AT/RT) is a rare and highly aggressive central nervous system (CNS) neoplasm that predominantly affects children under three years of age. In the posterior fossa, AT/RT frequently mimics medulloblastoma (MB) on both radiological and histopathological evaluation. Owing to significant morphological overlap, a definitive diagnosis cannot be established on histology alone and requires immunohistochemical (IHC) confirmation, particularly demonstration of loss of nuclear integrase interactor 1 (INI1) expression. We report the case of a seven-month-old male infant wh

Pilocytic astrocytoma surgery via the pineal region in a 9-year-old boy via the infratentorial-supracerebellar approach.

Pineal region tumors (PRTs) are rare neoplasms located in the epithalamus and account for 2.8-11% of pediatric brain tumors. These patients present symptoms such as headaches, nausea, vomiting, and cognitive decline due to obstructive hydrocephalus. Diagnosis involves advanced imaging (magnetic resonance imaging (MRI) and cerebral tomography (CT)) and biomarkers, with definitive diagnosis often requiring biopsy. Management includes neuroendoscopic biopsy, endoscopic third ventriculostomy (ETV), ventriculoperitoneal shunt (VPS), surgical resection, and adjuvant therapies such as chemotherapy an

Obstructive hydrocephalus secondary to an anterior mesencephalic cavernous malformation with familial cerebral cavernous malformation syndrome: A case report.

Cerebral Cavernous malformations (CCM) are the second most common vascular malformation of the central nervous system accounting (CNS) for 5%-10% of vascular malformations in the CNS with a prevalence of 0.5%. Most CCM are supratentorial with fewer than 20% located in the brainstem, mostly in the pons but very rarely in the anterior mesencephalon. The optimal treatment strategy for anterior mesencephalic cerebral cavernous malformations (AMCCM) is still debatable. Up to 20% of CCM are familial with patients presenting with multiple CCM. We report the case of a 62 year-old female who presented

Comparative study on imaging characteristics of pilocytic astrocytomas in children and adolescents.

This study investigated imaging differences of intracranial pilocytic astrocytomas (PA) between children and adolescents. We retrospectively analyzed imaging data from 19 children and 19 adolescents admitted to People's Hospital of Rizhao between January 2018 and December 2024. The mean age was 5.95±2.82 years in the children group and 21.00±3.61 years in the adolescent group. There were no significant differences in sex, extent of resection, tumor extension, or supratentorial/infratentorial location between the two groups. Tumors were significantly larger in the children group compared with t

Monitoring the Response of a Pineal Parenchymal Tumor of Intermediate Differentiation With Cerebrospinal Fluid Dissemination Using a Circulating Tumor Cell Assay.

Pineal parenchymal tumors of intermediate differentiation (PPTID) are aggressive, rare central nervous system tumors. The management of PPTID remains challenging, given the limited number of reported cases and the logistical difficulties in conducting studies on rare tumors such as these. Leptomeningeal disease (LMD) is a relatively frequent pattern of recurrence, and accurate monitoring is essential to optimize potentially efficacious therapies. We present the case of a 27-year-old woman who initially presented with positional headaches and mild blurry vision. Imaging revealed an enhancing th

Reverse brain herniation following CSF diversion in a 2-year-old child-case report and review of literature.

Reverse brain herniation (RBH) is a rare but catastrophic complication following cerebrospinal fluid (CSF) diversion in patients with posterior fossa tumours associated with obstructive hydrocephalus. It carries high mortality and remains underreported. The role of preoperative CSF diversion in such cases continues to be controversial. We report a case of a 2-year-old female with a fourth ventricular tumour presenting with acute decompensated obstructive hydrocephalus who underwent emergency ventriculoperitoneal (VP) shunt placement. Postoperatively, the patient failed to show neurological imp

Intracranial pressure management in severe intraventricular hemorrhage: A minireview.

Severe intraventricular hemorrhage (IVH) is a life-threatening neurological emergency that poses significant risks of morbidity and mortality, particularly due to the associated elevated intracranial pressure (ICP). Studies have shown that the incidence of IVH in very preterm infants is high, with management strategies playing a significant role in its development. Moreover, predictive models have been developed to forecast early mortality and severe IVH in very-low birth weight preterm infants, indicating the complexity and multifactorial nature of this condition. This mini-review synthesizes

Endoscopic Colloid Cyst Resection: A Case Series and Systematic Review.

BACKGROUND AND OBJECTIVES: Endoscopic surgery offers a minimally invasive alternative to microsurgical resection for intraventricular lesions such as colloid cysts, with potential advantages in morbidity and recovery. Although microsurgery remains a common procedure, endoscopic techniques are increasingly used, despite limited long-term outcomes data. This study presents a single institution case series assessing clinical outcomes after endoscopic resection with a comparison with open and endoscopic series through systematic review. METHODS: A retrospective analysis from 2014 to 2025 was cond

Congenital Chylous Ascites in a Neonate with Isolated Aqueductal Stenosis: A Case Report and Literature Review.

Background and Clinical Significance: Ventriculoperitoneal (VP) shunting remains the standard definitive treatment for progressive neonatal obstructive hydrocephalus. Congenital chylous ascites is an uncommon neonatal condition, most often related to developmental lymphatic abnormalities. The concurrence of hydrocephalus requiring VP diversion with congenital chylous ascites is exceptionally rare and may first become apparent during abdominal access for shunt placement. Awareness of this possibility is clinically important because milky peritoneal fluid at shunt surgery can mimic gastrointesti

Spontaneous atraumatic pediatric basal ganglia hemorrhage in the setting of COLGALT1-related collagenopathy: illustrative case.

BACKGROUND: Spontaneous intracranial hemorrhage (ICH) in children is rare and is most commonly attributed to vascular malformations or coagulopathies. When standard vascular and hematological evaluation is unrevealing, genetic small-vessel disease should be considered. Biallelic COLGALT1 variants represent an exceptionally uncommon cause of cerebral microangiopathy, with limited cases reported and minimal discussion of neurosurgical management. OBSERVATIONS: A 14-year-old female with a known biallelic COLGALT1 variant presented with an acute basal ganglia hemorrhage with intraventricular exte

Successfully treated delayed upward trans-tentorial herniation (UTTH) following ventriculoperitoneal shunt placement for an acute obstructive hydrocephalus secondary to midline posterior fossa mass in a low-resource setting: case report.

INTRODUCTION: UTTH is an exceedingly rare, deadly complication of ventriculoperitoneal shunt (VPS) placement for hydrocephalus secondary to a posterior fossa mass. PRESENTATION OF CASE: VPS was inserted for a 4-year-old patient with hydrocephalus secondary to a posterior fossa mass before tumor resection. Initially, the child had clinical improvement, but after 9 days, he developed deterioration. Brain computed tomography (CT) revealed a UTTH; therefore, ligation of the VPS, intubation, mechanical ventilator support, dexamethasone, mannitol, and urgent resection of the mass were performed. Reversible cognitive and gait impairment by a giant quadrigeminal cistern arachnoid cyst: A case report of marked reversal following neuroendoscopic fenestration.

BACKGROUND: Quadrigeminal cistern arachnoid cysts are rare in adults, but can cause reversible gait and cognitive impairment secondary to obstructive hydrocephalus. Differentiating these structural lesions from idiopathic normal-pressure hydrocephalus (iNPH) or neurodegenerative diseases is essential because they are surgically treatable. CASE DESCRIPTION: A 71-year-old woman presented with progressive gait disturbance and mild cognitive impairment. Neuropsychological tests showed a Montreal Cognitive Assessment-Japanese version (MoCA-J) score of 21/30 and a frontal assessment battery (FAB) s
