Cerebellar Tracts Microstructural Characteristics and Severity of Language Impairment in Post-stroke Patients with Aphasia: A DTI Study.

Aphasia is a language impairment resulting from stroke. While numerous studies have explored the association between supratentorial white matter damage and post-stroke aphasia using diffusion tensor imaging (DTI), the role of infratentorial structures, particularly the cerebellum, remains underinvestigated. Evidence suggests that the cerebellum may play a supportive role in language processing, but its contribution across different aphasia types is not well understood. This study aimed to explore cerebellar white matter involvement in individuals with Broca's, conduction, and anomic aphasia us

Behavioural and psychological symptom determinants of carer burden across clinical syndromes associated with frontotemporal lobar degeneration and Alzheimer's disease: a transdiagnostic analysis.

BACKGROUND: This study characterised overall transdiagnostic associations between psychological and behavioural symptoms and carer burden, and examined syndrome-specific contributions across clinically probable FTLD and AD groups. METHODS: A total of 432 individuals with clinically probable FTLD and AD were included. The primary analytic cohort comprised 358 participants with complete carer burden data, including 230 and 128 individuals in the FTLD and AD diagnostic groups, respectively. Behavioural and psychological symptoms were assessed using the Cambridge Behavioural Inventory-Revised (CB

Neuroinflammation-associated thalamocortical functional connectivity and cognition in older adults.

Neuroinflammation has detrimental neurocognitive effects in patients with systemic and neuropsychiatric diseases. Yet, outside of these conditions, it is unclear to what extent neuroinflammation modulates brain network-level function in older adults in the general population and, what, if any cognitive performance issues arise as a result. To evaluate these questions, we compared cerebrospinal fluid (CSF) inflammatory cytokine levels, resting-state functional magnetic resonance imaging, and cognitive performance data in 96 community-dwelling older adults (age ≥ 60 yrs). Principal component ana

Cerebrospinal fluid shunting or dural venous sinus stenting to preserve vision in idiopathic intracranial hypertension (IIH Intervention): protocol for an open-label, multicentre, randomised controlled phase IIb trial.

INTRODUCTION: Idiopathic intracranial hypertension (IIH) is characterised by raised intracranial pressure (ICP) and typically affects young women with obesity. Patients are at risk of permanent visual loss due to papilloedema. Some require emergency intervention to rapidly reduce papilloedema and preserve vision. The international standard of care for patients with sight-threatening IIH is cerebrospinal fluid (CSF) shunting. However, dural venous sinus stenting (DVSS) is an emerging procedure that is offered at many neuroscience centres internationally as the primary intervention. Currently, t

Temporal order of clinical, imaging, and biomarker changes in frontotemporal lobar degeneration-associated syndromes.

BACKGROUND: The temporal sequence of clinical, imaging, and biological changes in sporadic frontotemporal lobar degeneration (FTLD)-associated syndromes remains poorly characterized, and a comprehensive biomarker cascade model is lacking. METHODS: We developed a data-driven biomarker cascade model in 489 patients across the FTLD spectrum (211 behaviorial variant frontotemporal dementia [bvFTD], 129 primary progressive aphasia [PPA], 71 corticobasal syndrome [CBS], 66 progressive supranuclear palsy [PSP], and 12 FTD associated with amyotrophic lateral sclerosis [FTD-ALS]; 1904 patient-visit ob

Eye movements and ocular biomarkers in mild traumatic brain injury: from oculomotor pathophysiology to diagnosis and prognosis.

Mild traumatic brain injury (mTBI) is among the most common neurological presentations worldwide, yet its diagnosis remains largely clinical and its prognosis difficult to predict. Structural imaging is typically normal and self-reported symptom scales are insensitive and confounded by effort and comorbidity. Because the generation and control of eye movements recruit an unusually large and anatomically distributed proportion of the brain-from brainstem and cerebellar circuits to parietal, frontal and prefrontal cortices linked by long white-matter tracts-ocular-motor behaviour is sensitive to

TogoPhosTAC as a delivery-ready platform for targeted protein dephosphorylation.

Phosphorylation-targeting chimeras (PhosTACs) enable targeted protein dephosphorylation by recruiting phosphatases through induced proximity. However, the direct recruitment of phosphatase subunits or holoenzymes with small molecules remains challenging, as suitable ligands are scarce and often compromise enzymatic activity or cellular function. Here, we present togoPhosTAC, a hybrid modality that integrates a small-molecule PhosTAC, an engineered FKBP12F36V-phosphatase, and a lipid nanoparticle delivery system. This strategy allows delivery of preassembled PhosTAC-FKBP12F36V-phosphatase compl

Associations between serum estradiol and estrone and Alzheimer's disease biomarkers: An analysis in female participants from the European Prevention of Alzheimer's Dementia Longitudinal Cohort Study (EPAD LCS).

INTRODUCTION: Postmenopausal estrogen decline may contribute to Alzheimer's disease (AD), but evidence linking circulating estrogens to cerebrospinal fluid (CSF) AD biomarkers remains limited. METHODS: We analyzed 854 female participants from the European Prevention of Alzheimer's Dementia Longitudinal Cohort Study with baseline serum estradiol and estrone measured by liquid chromatography-tandem mass spectrometry and repeated CSF amyloid-beta 42 (Aβ42), phosphorylated tau 181 (pTau181), and total tau (tTau). Targeting transthyretin tetramer with a novel blood-brain barrier-penetrating small-molecule stabilizer for the treatment of Alzheimer's disease.

The stability of the transthyretin (TTR) tetramer is critical for regulating cerebral amyloid-β (Aβ) homeostasis in Alzheimer's disease (AD). Stable TTR tetramer can directly bind Aβ and promote its clearance from the brain. However, current TTR tetramer stabilizers lack blood-brain barrier (BBB) penetration, limiting their clinical translation. Therefore, to fully realize the therapeutic potential of TTR in AD, novel TTR tetramer stabilizers with superior BBB penetration must be discovered. Here, we aim to identify TTR tetramer stabilizers with BBB penetration via a multidimensional virtual s

Cerebral oxygen extraction fraction as a physiological biomarker in vascular cognitive impairment: A study in older adults enriched for small vessel diseases.

INTRODUCTION: Vascular cognitive impairment and dementia (VCID) is a common cause of cognitive impairment, yet validated biomarkers, particularly those reflecting vascular function, remain lacking. Oxygen extraction fraction (OEF), which reflects the functional status of cerebral small vessels, is a potential physiological biomarker for VCID. We tested this hypothesis in older adults enriched for small vessel diseases, controlling for Alzheimer's disease (AD) co-pathology. METHODS: Comprehensive cognitive, plasma, and imaging data were collected from 81 older adults with at least one vascular

The "Double-Hit" Model in Experimental Parkinsonism: Effects of Combined Exposure to Lactacystin and LPS.

The present study aimed to investigate a "double-hit" model of Parkinson's disease, implying intranigral administration of lactacystin and intraperitoneal injection of LPS. The model group rats exhibited significant deterioration in several parameters of the motor function (p ⩽ 0.04) and emotional impairment (p ⩽ 0.04) compared to the control. Morphological signs of neurodegeneration (p ⩽ 0.001) and microglial activation (p ⩽ 0.02) were observed in the substantia nigra. These findings support the utility of this "double-hit" model for elucidating the mechanisms underlying parkinsonism and for

Elevating cerebellar theta oscillations boosts noninvasively induced motor plasticity.

Matching brain stimulation to the brain's natural rhythms can drive plasticity, yet this principle has rarely been tested in humans. We targeted the cerebellum, a key hub for motor coordination and learning, using a rhythm-tuned protocol that pairs theta-frequency transcranial alternating current stimulation with intermittent theta-burst stimulation to engage plasticity of cerebello-cortical circuits. In young healthy adults, this pairing enhanced fine motor control and hand dexterity, with gains closely tracking physiological markers of cerebellar-driven plasticity. Applying the same approach

Aerobic exercise regulates the cGAS/STING signaling pathway and inhibits ferroptosis to alleviate ischemic stroke.

Aerobic exercise at the appropriate intensity alleviates motor dysfunction after ischemic stroke (IS); however, its regulatory mechanisms remain to be elucidated. In this study, the neuroprotective effects of exercise and the related mechanisms were investigated using a rat middle cerebral artery occlusion (MCAO) model and an HT22/PC12 cell oxygen-glucose deprivation/reperfusion (OGD/R) model. The rats were subjected to daily exercise training for durations of 3, 7 or 14 days post-surgery. The results revealed that exercise, particularly the 14-day regimen, significantly reduced the mNSS and i

Unable to walk or eat without assistance in pregnancy-Wilson's disease in a resource-limited and conflict-affected setting: a case report.

BACKGROUND: Wilson's disease, an autosomal recessive disorder, affects body copper metabolism. It can be fatal if left untreated as the copper accumulation ultimately leads to liver failure, brain damage, or both. In women discontinuation of menstruation, miscarriage and impaired conception can be symptoms of Wilson's disease. Successful pregnancies in untreated patients with major symptoms are rare. CASE PRESENTATION: A 30-year-old, gravidity 5, parity 4, Karen woman presented, after travelling for more than 5 h to a clinic on the Thailand-Myanmar border. Over the course of a year she progre

An Unusual Case Report of Pseudothalamic Pattern of Sensory Loss in Lateral Medullary Syndrome.

Lateral medullary syndrome is a collection of different neurological symptoms after ischemic/hemorrhagic insult to either the posterior inferior cerebellar artery/vertebral artery or rarely the anterior inferior cerebellar artery, causing infarction of ipsilateral cerebellum and posterolateral medulla (Tiedt and Weidauer, 2013). Dizziness, nausea, vertigo, vomiting, nystagmus, ataxia, dysphagia, hoarseness of voice, ptosis, and sensory impairment of face and body are typical. Here we report an unusual presentation of a 47-year-old female who was hypertensive and diabetic and who complained of

Health-related factors and their impact on blood-based biomarkers of Alzheimer's disease.

BACKGROUND: Health-related factors may influence blood-based biomarkers (BBBM) of Alzheimer's disease (AD). In this analysis, associations between modifiable factors and plasma biomarkers of neurodegeneration were investigated across the Alzheimer's disease spectrum and in cognitively healthy controls in a cerebrospinal fluid-confirmed (CSF) cohort. METHODS: Plasma biomarkers included the Aβ1-42/1-40 ratio, pTau181, GFAP, NFL and ApoE4. Multiple linear regression was used to test associations with lifestyle factors (physical activity and sleep), physiological factors (including renal and lipi

Design, Synthesis and Molecular Docking Studies of Some Novel Hydrazone Derivatives Based on Trimetazidine as Selective and Potent AChE Inhibitors.

Alzheimer's disease (AD) is a progressive neurodegenerative disease, and cholinergic dysfunction is considered one of the fundamental pathological factors of this disease. Guided by the clinically used acetylcholinesterase (AChE) inhibitor donepezil, we explored trimetazidine dihydrochloride as an accessible starting scaffold to generate donepezil-like features and to develop new anti-AD candidates. Accordingly, 20 novel hydrazone derivatives were designed and synthesized based on trimetazidine dihydrochloride, and their structures were confirmed by spectroscopic methods. The compounds were ev

A systematic assessment of safety, tolerability and blinding effectiveness of kilohertz transcranial magnetic perturbation (kTMP).

Objective.Non-invasive brain stimulation (NIBS) techniques are increasingly used to modulate brain activity in basic and translational research. Kilohertz transcranial magnetic perturbation (kTMP) is a recently developed NIBS approach that uses magnetic induction to generate subthreshold electric fields in the brain. kTMP has been shown to modulate cortical excitability while producing no perceptible sensation at the stimulation site. However, its safety and tolerability have not yet been systematically evaluated-a gap this study aims to address.Approach.We conducted sham-controlled experiment

A Case of Schizophrenia with Diagnostic Challenges Between Non-convulsive Status Epilepticus and Malignant Catatonia: Therapeutic Implications based on the GABA-A Dysfunction Hypothesis.

Catatonia is a psychomotor syndrome that occurs in various psychiatric, neurological, and medical conditions. Differentiating malignant catatonia from neuroleptic malignant syndrome (NMS) and non-convulsive status epilepticus (NCSE) is particularly challenging in complex cases involving multiple contributing factors. Both catatonia and NCSE have been associated with dysfunction of the gamma-aminobutyric acid (GABA)-A receptor system, which may underlie overlapping clinical features. We report the case of a 43-year-old man with a long-standing history of schizophrenia who developed malignant ca
