Recalibration of the Frontal Behavioural Inventory Cut-Off for Behavioural Screening in Amyotrophic Lateral Sclerosis.

OBJECTIVE: The Frontal Behavioural Inventory (FBI) was validated in behavioural-variant frontotemporal dementia (bvFTD) with a cut-off of 25. We tested whether this threshold is appropriate in amyotrophic lateral sclerosis (ALS), where behavioural change is milder and partly confounded by motor pathology. METHODS: We analysed 506 ALS patients with a valid FBI assessment. Behavioural impairment was defined using two complementary reference standards, to minimise dependence on any single behavioural instrument: a consensus rule (pathology on ≥ 2 of three independent instruments: Edinburgh Cogni

Effects of Felbamate on Tissue Caspase-3 Levels, Histopathology, and Neurological Function in Rats with Early Spinal Cord Ischemia-Reperfusion Injury.

AIM: To evaluate the potential neuroprotective effect of felbamate in a rat model of spinal cord ischemia-reperfusion injury. MATERIAL and METHODS: Twenty-one male Wistar albino rats were randomly assigned to three groups (n=7 each): control, ischemia, and felbamate-treated. Transient spinal cord ischemia was induced by abdominal aortic occlusion for 20 minutes, followed by reperfusion. Felbamate was administered via a nasogastric tube immediately after reperfusion at a dose of 20 mg/kg. Twenty-four hours after surgery, tissue caspase-3 levels were measured using an enzyme-linked immunosorbent

SOD1 co-pathologies in the brain: insights from neuropathological evidence and implications.

Superoxide dismutase 1 (SOD1), originally regarded as an antioxidant enzyme, is now recognized as a multifunctional protein whose conformational instability and aggregation contribute to neurodegeneration. The emerging evidence suggests that SOD1 pathology extends beyond amyotrophic lateral sclerosis, supporting a role in multiple neurodegenerative diseases. This review summarizes current neuropathological evidence of SOD1 co-pathologies in the human brain, with particular emphasis on its associations with TAR DNA-binding protein 43 (TDP-43), tau, alpha-synuclein, amyloid-β, and other aggregat

TDP-43 and ALS - what's wrong with my membranes?

In recent years, multiple pathologies of the mRNA binding protein TDP-43 have been linked to the neurodegenerative disease Amyotrophic Lateral Sclerosis (ALS), in which almost 97% of all ALS patients exhibit TDP-43 abnormalities in affected neurons. Typically, neuronal degeneration first affects alpha motor neurons of the spinal cord and the lower brainstem, before the disease progresses to Betz cells, giant neurons of the primary motor cortex, and subsequently spreads into further brain regions. It is just the Betz cells though, which do not show cytosolic TDP-43 aggregation upon loss of nucl

Drp1 in M1 layer V GABAergic neurons orchestrates rTMS-mediated motor restoration and analgesia after spinal cord injury.

Spinal cord injury (SCI) leads to chronic motor and sensory deficits, with progressive secondary neurodegeneration posing a major therapeutic challenge. Although high-frequency repetitive transcranial magnetic stimulation (HF-rTMS) over the primary motor cortex (M1) shows therapeutic potential, its underlying cellular mechanisms remain poorly understood. This translational study first demonstrated that HF-rTMS concurrently improved motor function and alleviated neuropathic pain in retrospective clinical cohorts and a validated murine SCI model. To decipher the supraspinal mechanism, we used an

A non-canonical function of glutathione to suppress ferroptosis via FSP1.

It is well established that glutathione (GSH) is mainly utilized by glutathione peroxidase 4 (GPX4) to diminish peroxidized phospholipids and defend against ferroptosis. However, we found that GSH robustly protects a variety of GPX4 knockout (KO) cells from ferroptosis, thus indicating that there is an alternative pathway for GSH to suppress ferroptosis. By carrying out genome-wide CRISPR-Cas9 screening, we identified that ferroptosis suppressor protein 1 (FSP1) is the key factor mediating the GPX4-independent protective effect of GSH. Mechanistically, FSP1 uses GSH to generate reduced ubiquin

Pediatric Spinal Cord Astrocytoma With Granular Cell-Like Morphology and KIAA1549::BRAF Fusion.

We report a pediatric spinal cord tumor with prominent granular cell-like features that posed a significant diagnostic challenge. Intraoperative findings and the initial histopathological evaluation revealed marked granular cell proliferation, leading to a provisional diagnosis of granular cell astrocytoma. Notably, the classical histological features of pilocytic astrocytoma (PA) were not evident. However, molecular profiling identified a KIAA1549::BRAF fusion, prompting diagnostic reconsideration. Re-evaluation of the histopathological findings in conjunction with the molecular results suppo

The Application Prospects and Challenges of Mesenchymal Stem Cell-Derived Exosomes in Spinal Cord Injury Repair.

Spinal cord injury (SCI) is a severe, disabling condition for which current treatments are largely insufficient in restoring neurological function. Despite advances in surgical and pharmacological interventions, no effective treatment currently exists to reverse neurological deficits caused by SCI. Mesenchymal stem cells (MSCs), especially human umbilical cord-derived MSCs (hucMSCs), have shown promise in tissue regeneration due to their multipotency and low immunogenicity. However, challenges such as low engraftment rates, tumorigenicity, and potential immune responses limit their clinical ap

A Case of Primary Intramedullary Infantile Fibrosarcoma With an RBPMS::NTRK3 Fusion and Response to Larotrectinib.

We report a case of primary intramedullary infantile fibrosarcoma (IFS) in a 4-week-old infant with rapidly progressive cervical myelopathy. Histology showed a spindle cell tumour with diffuse S100 and pan-TRK expression. RNA sequencing identified an RBPMS::NTRK3 fusion. RNA-based clustering and DNA methylation profiling classified the tumour as IFS. Treatment with the TRK inhibitor, larotrectinib, achieved marked clinical and radiological recovery within 1 month, expanding the spectrum of NTRK-rearranged sarcomas and confirming TRK inhibition's efficacy in primary central nervous system IFS.

Intramedullary Spinal Cord Metastasis (SCM) From Prostate Carcinoma Detected on [68Ga] Ga-PSMA-11 PET/CT.

Prostate carcinoma metastasizing to the spinal cord is an extremely rare entity. We present a 70-year-old man with prostate carcinoma with a Gleason's score of 10 and an elevated serum prostate-specific antigen (PSA) of 25.99 ng/mL. He underwent [68Ga] Ga-prostate-specific membrane antigen (PSMA)-11 positron emission tomography (PET)/computed tomography (CT) imaging for initial staging. The PET/CT revealed a high-grade [68Ga] Ga-PSMA-11 expressing primary prostatic lesion and metastatic abdomino-pelvic lymph nodes and skeletal lesions. Focal moderate grade [68Ga] Ga-PSMA-11 uptake was noted in

PU.1 restores microglial dysfunction caused by C9ORF72 repeat expansions in neural organoids.

Amyotrophic lateral sclerosis (ALS) is an adult-onset neurodegenerative disease characterized by loss of upper and lower motor neurons and progressive muscle wasting. Accumulating evidence indicates a role for non-neuronal cells in ALS pathogenesis, but their exact role and mechanism-of-action remain incompletely understood. A hexanucleotide (GGGGCC) repeat expansion (HRE) in C9ORF72 is the most common genetic cause of ALS (C9-ALS) and a frequent cause of frontotemporal dementia (FTD). Several lines of experimental evidence support a role for the immune system and microglia in C9-ALS/FTD, and
