Direct probabilistic IMPT treatment planning with setup and range errors for neuro-oncological patients.

Objective. To show computational feasibility of a previously proposed probabilistic planning approach on a neuro-oncological patient group, we compared probabilistic plans with (automated) robust plans for one patient (group A) that could achieve sufficient clinical target coverage and for four patients (group B) where target coverage had to be compromised due to organ-at-risk (OAR) dose constraints.Approach. The probabilistic approach is percentile-based and uses the fact that a (dose) percentile can be approximated as a linear combination of its expected value and standard deviation. This al

Biomechanical assessment of repetitive brain injury in pedestrian collisions based on wavelet packet energy analysis.

PURPOSE: Existing brain injury criteria mainly focus on assessing injuries in a single collision and lack of assessment of injuries in real-world multiple collisions. METHODS: Based on the head kinematics signals of rats in repeated collisions and pedestrian accident data, the frequency domain characteristics of linear and rotational motion signals were analyzed by wavelet packet transform. Finally, the ROC curve was used to evaluate the effectiveness of injury index prediction. RESULTS: The results of animal experiments show that as the impact intensity increases, the energy in the frequenc

Preterm Risk Factors, Epigenetic Age, and Neurodevelopmental Outcomes.

IMPORTANCE: Studies examining biological factors associated with neurodevelopment in the preterm population are limited. It was previously demonstrated that epigenetic age acceleration was associated with better neurodevelopmental outcomes among children born preterm. It is unclear whether this association is modified by clinical and environmental preterm risk factors. OBJECTIVE: To examine whether the association between epigenetic aging and neurodevelopmental outcomes is modified by preterm brain injury and socioeconomic status. DESIGN, SETTING, AND PARTICIPANTS: This was a prospective coh

Interventions to reduce cognitive decline post-stroke: an umbrella review.

INTRODUCTION: Post-stroke cognitive impairment (PSCI) remains a common problem that can lead to significant disability and dependency. Previous work suggested that methodologically robust trials were lacking and that there is continued uncertainty on the benefits and limitations of pharmacological and non-pharmacological interventions. This umbrella review aims to update and synthesise evidence on pharmacological and non-pharmacological interventions for PSCI. PATIENTS AND METHODS: This review was pre-registered on PROSPERO (CRD42024611909) with searches from 2019 to 2024. Eligible studies we

[Arachnoid web presenting as tetraparesis].

Arachnoid webs are rare intradural lesions which disrupt cerebrospinal fluid flow and may cause spinal cord compression. This is a case report of a previously healthy 19-year-old woman who presented with acute back pain and rapidly progressive tetraparesis. MRI revealed cervical spinal cord ischaemia and an arachnoid web at the thoracic level. Surgical removal led to partial neurological recovery. Early recognition is essential, as timely surgery may prevent irreversible injury.

From simulation to stimulation: a predictive synergy-based control strategy for functional electrical stimulation cycling in a person with spinal cord injury.

BACKGROUND: Functional electrical stimulation (FES) cycling stimulation strategies are often derived from heuristics, or able-bodied kinematics and electromyography, which may be unsafe and non-representative for people with complete spinal cord injury. Optimally controlled predictive musculoskeletal simulations with synergy implementation offer a way to generate individualized, physiologically plausible muscle trajectories without prior experimental data. Building on a previous proof of concept, this study developed and experimentally validated a simulation-derived stimulation strategy for FE

Connexins are essential for the contribution of latent progenitors to healing after spinal cord injury.

Gap junctions are important regulators of the biology of neural stem cells. Both in vertebrates with regenerative abilities and neonatal rodents, ependymal cells communicate via connexin (Cx) 43 and Cx26. Gap junction coupling and Cx26 are down-regulated as the ependyma becomes quiescent in adulthood, but injury overrules this developmental down-regulation suggesting a role for Cx signaling in the reactivation of ependymal cells. However, direct evidence of the involvement of these Cxs in the reactivation of the ependymal stem cell niche is lacking. Here, we aim to explore the role of Cx26 and

Comparison of non-invasive neuromodulation for motor function rehabilitation in incomplete spinal cord injury: a systematic review and network meta-analysis.

BACKGROUND: Spinal cord injury (SCI) disrupts structural and functional connectivity between the brain and spinal cord, resulting in severe motor dysfunction. Various neuromodulation techniques have been applied in SCI rehabilitation and have promoted motor recovery to varying degrees. A Bayesian network meta-analysis was therefore undertaken to compare and rank the relative effects of non-invasive neuromodulation techniques on motor-related outcomes in adults with incomplete SCI. METHODS: PubMed, Web of Science, Embase, and The Cochrane Library were searched for randomized controlled trials

Pro- and anti-inflammatory cytokine dysregulation in the saliva of patients with acromegaly: a cross-sectional study.

BACKGROUND: There is evidence to suggest that hormonal dysregulation induced by acromegaly may affect serum cytokine levels. However, to our knowledge, no previous studies have examined the salivary cytokine profile in adults with acromegaly. The aim of this study was to identify the salivary profile of pro- and anti-inflammatory cytokines associated with acromegaly in adults. METHODS: A cross-sectional study was conducted with 25 patients with acromegaly and age-, 25 age- and sex-matched controls. Demographic and general health data were collected. Saliva samples were obtained to quantify th

Timing and duration of diabetes and incident dementia in older adults: a prospective cohort study.

BACKGROUND: Diabetes is associated with dementia, but how this association varies with attained age and cumulative exposure remains unclear. We integrated time-updated exposure histories with repeated age-specific landmark cohorts, enabling complementary evaluation of overall and age-conditional dementia risk. METHODS: We analysed 18,028 dementia-free participants aged 50 years or older from the Health and Retirement Study. Incident dementia was classified using the Langa-Weir algorithm. Time-updated Cox models evaluated risk according to current diabetes status. Age-anchored landmark models

Associations of genetically predicted Helicobacter pylori antibodies with aneurysmal subarachnoid hemorrhage and unruptured intracranial aneurysm: A two-sample mendelian randomization study.

The causal relationship between Helicobacter pylori antibodies and the risk of cerebral aneurysms remains unclear. We used Mendelian randomization analysis to assess the causal relationship between Helicobacter pylori antibodies and the risk of cerebral aneurysms. A genome-wide association study (GWAS) dataset from the IEU Open GWAS project was utilized for antibodies to 7 different Helicobacter pylori-specific proteins, and from the Finnish database for cerebral aneurysms that included unruptured aneurysm (uIA) and aneurysmal subarachnoid hemorrhage (aSAH). A FAERS pharmacovigilance analysis of dementia-related adverse event reports.

Dementia-related adverse event (AE) reports represent an important area of concern in pharmacovigilance, particularly considering the clinical relevance of cognitive impairment and the complexity of medication use among older adults. This study aims to systematically characterize the reporting patterns of dementia-related AEs and detect disproportionality signals. Dementia-related AE reports in the Food and Drug Administration Adverse Event Reporting System database from the first quarter of 2004 to the first quarter of 2024 were analyzed using the reporting odds ratio and proportional reporti

World Stroke Organization mechanical thrombectomy workshops combined with the Global Stroke Alliance builds regional capacity.

INTRODUCTION: Access to mechanical thrombectomy (MT) remains limited in many low- and middle-income or resource-limited regions because of gaps in trained personnel, procedural experience, public awareness, and stroke-system readiness. The World Stroke Organization paired simulation-based MT workshops with the Global Stroke Alliance to combine hands-on neurointerventional education with regional advocacy, Ministry of Health engagement, public awareness activities, and stakeholder collaboration. This study evaluated the feasibility, scalability, and self-reported educational impact of MT worksh
