Application of Convolutional Neural Network Denoising to Improve Conebeam CT Myelographic Images.

Conebeam CT (CBCT) is an imaging technique that provides high-resolution, cross-sectional imaging in the fluoroscopy suite. In neuroradiology, CBCT has been used for various applications including temporal bone imaging and during spinal and cerebral angiography. Furthermore, CBCT has been shown to improve imaging of spinal CSF leaks during myelography. One drawback of CBCT is that images have a relatively high noise level. In this technical report, we describe the first application of a high-resolution convolutional neural network to denoise conebeam CT myelographic images. We show examples of

Enhancing the Characterization of Dural Tears on Photon-Counting CT Myelography: An Analysis of Reconstruction Techniques.

Photon-counting detector CT myelography is an effective technique for the localization of spinal CSF leaks. The initial studies describing this technique used a relatively smooth Br56 kernel. However, subsequent studies have demonstrated that the use of the sharpest quantitative kernel on photon-counting CT (Qr89), particularly when denoised with techniques such as quantum iterative reconstruction or convolutional neural networks, enhances the detection of CSF-venous fistulas. In this clinical report, we sought to determine whether the Qr89 kernel has utility in patients with dural tears, the

Spontaneous Intracranial Hypotension and CSF-Venous Fistulae: from Diagnostic Imaging to Interventional Management.

Over the past decade, major advances have transformed the understanding and management of cerebrospinal fluid (CSF) leaks leading to spontaneous intracranial hypotension (SIH). Among the most impactful developments has been the identification and characterization of CSF-venous fistulae (CVF), a distinct and still underrecognized cause of SIH. This discovery has prompted a paradigm shift in both diagnostic and therapeutic strategies. Novel imaging techniques-including lateral decubitus CT myelography and digital subtraction myelography-have markedly improved detection rates, while targeted inte

Disparities in Access to Specialized Care for Spontaneous Intracranial Hypotension: A Comparative Cohort Study.

BACKGROUND AND PURPOSE: Patients with spontaneous intracranial hypotension (SIH) often face substantial delays in diagnosis and treatment. Barriers to accessing specialized care for SIH remain poorly understood, particularly regarding potential demographic and/or socioeconomic disparities. Our aim was to evaluate whether patients seen in a specialized CSF leak clinic differ demographically or socioeconomically from those presenting to the emergency department (ED) with headache who undergo neuroimaging. MATERIALS AND METHODS: We retrospectively compared all patients evaluated in the Universit

Locating Spinal Leaks in Spontaneous Intracranial Hypotension: How Many Dynamic Myelographies Does It Take?

BACKGROUND AND PURPOSE: Localizing a CSF leak in a patient with spontaneous intracranial hypotension (SIH) is of utmost importance when pursuing a targeted therapy. The study aimed to evaluate the accuracy of dynamic myelography techniques in localizing spinal CSF leaks and report the number of examinations required, stratified by leak type and spinal level. MATERIALS AND METHODS: Consecutive patients with SIH with a spinal longitudinal extradural CSF collection (SLEC) investigated at our department from January 2013 to February 2025 were screened. All included patients underwent a dynamic my

Temporal Characteristics of Type 2 Lateral Spinal CSF Leaks on Digital Subtraction Myelography: Fast, Medium, or Slow Leaks?

BACKGROUND AND PURPOSE: Type 2 leaks occur in up to 20% of spontaneous intracranial hypotension (SIH) due to a spinal lateral dural tear, typically accompanied by an arachnoid hernia. Their CSF-outflow dynamics are unclear, but could have implications for performing myelography for the best possible detection. This cross-sectional study analyzed temporal characteristics of type 2 leaks using digital subtraction myelography (DSM). MATERIALS AND METHODS: Between February 2020 and April 2025, 63 consecutive patients with type 2 leaks were retrospectively identified. Patients undergoing sufficien

Atypical Imaging Manifestations of Lateral Dural Tears with Arachnoid Herniations: A Multi-Institutional Study.

Lateral dural tears are a common cause of spontaneous intracranial hypotension. Frequently, these leaks are associated with an arachnoid diverticulum that herniates through the dural defect. CSF egress into the epidural space occurs from the margins of the dural tear rather than from the lateral portion of the herniated arachnoid diverticulum. Nonetheless, arachnoid diverticula can have secondary effects that alter the physiology and imaging appearance of lateral leaks. Specifically, we have anecdotally observed 3 unique variations of lateral dural tears, including 1) lateral dural tears in wh

Factors Associated With Nocturnal Hypoglycemia After Exercise in the Type 1 Diabetes and Exercise Initiative (T1DEXI).

INTRODUCTION: The respiratory tract is one of the main points of entry of foreign substances into the body. Because of its location, the respiratory tract is heavily exposed to harmful agents, such as gases, vapors, or aerosols. AIM: Our objective was to evaluate the symptoms of occupational rhinitis in workers exposed to cotton dust. METHOD: The prospective study population consisted of workers from the "Nova Esperança" Cooperative of Nova Odessa (Sao Paulo), who were studied between September and December 2008. Data were collected through an individually and privately answered questionnair

The Association of Spinal Meningeal Diverticula Number and Size with the Detection of CSF-Venous Fistulas: Evidence from a Contemporary Myelography Cohort.

BACKGROUND AND PURPOSE: Meningeal diverticula are commonly seen on spine imaging, but their relationship to CSF leak remains debated. A prior study found no association with spontaneous intracranial hypotension (SIH), but this predated recognition of CSF-venous fistulas (CVFs). We reexamined this question, hypothesizing that greater diverticula burden is associated with a higher rate of CVF on myelography. Secondary aims included exploring associations with age, sex, and connective tissue disease (CTD). MATERIALS AND METHODS: Spine MRIs of 42 consecutive patients who underwent dynamic myelogr

Umbilical cerebrospinal fluid fistula following ventriculoperitoneal shunt placement: a case report and literature review.

Umbilical cerebrospinal fluid fistula is a rare complication associated with ventriculoperitoneal (VP) shunt. A 5-year-old girl with a history of myelomeningocele repair and VP shunt placement had persistent umbilical discharge for 2 years. Despite multiple medical consultations, the underlying cause remained undiagnosed. Investigations confirmed cerebrospinal fluid leakage, and an initial shunt revision was unsuccessful. Surgical exploration revealed a fistulous tract connecting the peritoneal cavity to the umbilicus, which was excised. The patient recovered without complications. This case u

Delayed cerebrospinal fluid leak (CSFL) occurring 21 days post-lumbar unilateral biportal endoscopic surgery: A case report.

RATIONALE: While minimally invasive endoscopic surgery has emerged as the primary approach for managing lumbar degeneration, cerebrospinal fluid leakage (CSFL) remains a common complication in traditional open spinal procedures. However, the literature documenting CSFL occurrence in minimally invasive endoscopic interventions is scarce, with delayed-onset cases (beyond 20 days postoperatively) representing an exceptionally rare clinical entity. PATIENT CONCERNS: We describe the case of a 60-year-old man who underwent unilateral transforaminal endoscopic spine surgery. On the 21st postoperativ

Multi-Layer Endoscopic Repair of Anterior Skull Base Defects.

Endoscopic endonasal approaches have revolutionized anterior skull base surgery, with multilayer vascularized reconstruction emerging as the gold standard for preventing postoperative cerebrospinal fluid leaks. Compared with free graft or single-layer techniques, multilayer repairs provide watertight closure, address high-flow leaks, and reduce long-term morbidity. The nasoseptal flap remains the primary vascularized option, though alternative flaps and grafts are vital in revision or salvage cases. Success depends on meticulous defect preparation, graft orientation, and material selection tai

CSF-venous fistulas Reconsidered: Pressure paradox and the Volume-Elastance Relationship.

BACKGROUND AND PURPOSE: Spontaneous intracranial hypotension (SIH) is a debilitating condition most often caused by spontaneous cerebrospinal fluid (CSF) leaks, with CSF-venous fistulas (CVF) representing an increasing number of cases. Pathophysiological understandings of CVFs, particularly those concerning pressure dynamics between the CSF and venous systems, remain elusive. This study aimed to mathematically model CVFs using first principles physics and to explore pressure dynamics and their implications for treatment and complications. MATERIALS AND METHODS: CSF-venous pressure dynamics we

Detection of Cerebrospinal Fluid-venous Fistulas Using Digital Subtraction Myelography in Refractory Spontaneous Intracranial Hypotension: A Single-center Study in an Asian Population.

Cerebrospinal fluid-venous fistulas are increasingly recognized as a cause of spontaneous intracranial hypotension, particularly in patients who do not respond to conventional therapies. Although widely reported in Western populations, their prevalence and diagnosis in Asian cohorts remain underexplored. This study aimed to evaluate the utility of digital subtraction myelography in diagnosing cerebrospinal fluid-venous fistulas in Japanese patients with refractory spontaneous intracranial hypotension. We retrospectively analyzed adult patients with spontaneous intracranial hypotension who unde

Early Epidural Cerebrospinal Fluid Leak After Anterior Cervical Discectomy and Fusion: A Case Report of Postoperative Intracranial Hypotension.

BACKGROUND Anterior cervical discectomy and fusion (ACDF) is a commonly performed and effective procedure for treating cervical spondylosis. Although cerebrospinal fluid (CSF) leakage is an uncommon complication, occurring in 0.2% to 1.7% of cases, it typically presents as an extraspinal leak. This report describes the case of a 51-year-old woman with postoperative headache due to an early epidural (intraspinal) CSF leak following ACDF - a complication that has not been previously reported. CASE REPORT A 51-year-old woman presented with chronic neck pain, cervicogenic headaches, and upper-limb
