Posttraumatic cerebrospinal fluid leak with cognitive dysfunction, especially visual memory disturbance: illustrative case.

BACKGROUND: Cerebrospinal fluid (CSF) leaks are associated with various clinical and neurological manifestations. Although cognitive dysfunction has been reported in patients with a CSF leak, its detailed characteristics and cognitive profile remain unclear. OBSERVATIONS: A 17-year-old male patient developed progressive cognitive decline after minor head trauma. The initial neuroimaging findings were unremarkable; however, over several months, he exhibited memory impairment, academic decline, and personality changes. Neuropsychological testing has demonstrated significant deficits in visual m

Hanky-in-the-Pocket Inlay Technique for Managing Incidental Dural Tears in Lumbar Unilateral Biportal Endoscopy Using Collagen Matrix-Technical Note with Case Series.

INTRODUCTION: Unilateral biportal endoscopy (UBE) is a novel technique for spine surgeries gaining popularity in recent times. Being minimally invasive, certain complications, such as dural tears can be difficult to manage due to restricted access. Incidental dural tears are common during endoscopic spine surgery and various techniques, such as application of fibrin sealant, non-penetrating dural clips, pressure with gelfoam and conversion to minimally invasive or open surgery have been described for the management. This study aims to describe the "Hanky in the Pocket" Technique with collagen

Symptomatic Spontaneous Intracranial Hypotension Caused by a Cerebrospinal Fluid Leak at the C1-C2 Level of the Spine Sealed With an Epidural Blood Patch-Case Series and Review of Literature.

BACKGROUND: Spontaneous intracranial hypotension (SIH) is a disorder caused by low cerebrospinal fluid (CSF) pressure due to a CSF leak located anywhere within the neuroaxis. Risk factors include trauma, degenerative disc disease, and especially connective tissue disorders. In most cases, SIH presents with orthostatic headache, worsening when upright and characteristic radiological features as diffuse pachymeningeal enhancement on MRI [4, 6]. There are a lot of publications concerning treatment of CSF leaks located in the lumbar and thoracic spine, but only scant information regarding treatmen

The "O-Ring" technique is effective in reducing cerebrospinal fluid leak in posterior fossa surgery.

BACKGROUND: Posterior fossa approaches are commonly used during routine neurosurgery. Cerebrospinal fluid (CSF) leakage is one of the most important posterior fossa surgery-related complications. CSF leakage can manifest as both pseudomeningocele and external leakage. If craniotomy or craniectomy is performed, the skull defect can be repaired using heterologous polymethylmethacrylate (PMMA) cranioplasty. The aim of this study was to evaluate the clinical results of the O-ring technique in terms of postoperative complications. METHODS: Patients treated using the retrosigmoid (RS), key-hole RS,

Post-market safety patterns of hydrogel dural sealants: a comparative manufacturer and user facility device experience analysis.

BACKGROUND: Polyethylene glycol (PEG) hydrogel dural sealants (DuraSeal, Adherus) reinforce dural closure, but case reports describe cerebrospinal fluid (CSF) leak, infection, and swelling-related neurologic injury. We characterized post-market safety in the Food and Drug Administration (FDA) Manufacturer and User Facility Device Experience (MAUDE) database, comparing structured problem codes with negation-aware narrative review. METHODS: MAUDE reports from January 1, 2015, to June 30, 2024, mentioning either device were extracted and deduplicated by unique identifiers (MDR_REPORT_KEY, REPORT

Sinonasal Mucosal Epithelioid Melanoma with Rapid Skull-Base and Orbital Progression.

Background: Sinonasal mucosal melanoma is a rare and aggressive malignancy arising from the nasal cavity and paranasal sinuses, characterized by high local recurrence rates and poor survival. Skull-base and orbital progression can occur rapidly, particularly when preoperative imaging underestimates local extension. This paper reports a case of sinonasal mucosal epithelioid melanoma with fulminant postoperative skull-base breach and orbital invasion, highlighting its clinical course, management challenges, and histopathological features. Methods: A 60-year-old woman with progressive unilateral

Vascularized Flaps for Anterior Skull Base Defects.

A variety of techniques exist for the reconstruction of skull base defects. However, the gold standard for skull base reconstruction is vascularized flaps, especially when there is a cerebrospinal fluid leak. The most frequently used flap is the nasoseptal flap, which has numerous applications, has relatively low morbidity, and is straightforward to harvest. When this flap is unavailable, other options include the inferior turbinate flap, middle turbinate flap, lateral wall flap, pericranial flap, and temporoparietal fascia flap.

Skull base CSF leak repair in the outpatient Arena: Pushing the boundaries of modern endoscopic surgery.

BACKGROUND: Skull base defects with cerebrospinal fluid (CSF) leak have been a significant clinical challenge due to the risk of life-threatening complications such as meningitis, pneumocephalus and intracranial abscess formation. Traditionally, these defects have been managed in the operating room under general anesthesia, using either open craniotomy or endoscopic endonasal approaches. However, with advances in high-definition endoscopy, specialized instruments and biomaterials for reconstruction, there is a growing chance of performing selected repairs in an outpatient setting under local a

Acellular Dermal Substitutes for Lining the Exenterated Orbit-A Systematic Review and Meta-Analysis of Clinical Outcomes.

Orbital exenteration reconstruction remains a major reconstructive challenge because of the orbit's complex anatomy and its close relationship with the paranasal sinuses, which predispose patients to delayed healing. Traditional approaches, such as secondary intention healing or split-thickness skin grafting, may result in prolonged epithelialization and can compromise timely prosthetic rehabilitation. Acellular dermal matrices (ADM) have emerged as biological scaffolds that provide immediate coverage of the defect while preserving an open orbital cavity suitable for prosthetic fitting. A syst

Technique of performing an epidural blood patch in spontaneous intracranial hypotension due to cerebrospinal fluid leakage from the nerve root sheath: Case series.

INTRODUCTION: Spontaneous intracranial hypotension (SIH) is frequently caused by cerebrospinal fluid leak throughout dural tear in spinal nerve sheath which effects in disruption of intracranial homeostasis. We present a method of sealing that defect with epidural blood patch which is administrated via intervertebral foramen using computer tomography (CT) guidance to maximize the procedure accuracy. METHODS: The method involves sealing CSF fistulae in spinal nerve sheaths using route via intervertebral foramen under constant CT guidance with patient's own venous blood. RESULTS: Technique has

Targeted repair of a refractory ventral cerebrospinal fluid leak identified by dynamic computed tomography myelography: illustrative case.

BACKGROUND: Spontaneous spinal CSF leaks can be difficult to diagnose, particularly with large or multiple dural tears. Although conventional CT myelography (CTM) is the standard imaging modality, it may fail to localize elusive leaks. Dynamic CTM (dCTM) enables real-time contrast tracking and improves localization, particularly for type 1 (ventral dural tear) and type 2 (lateral dural tear) leaks, to support targeted intervention. OBSERVATIONS: A 36-year-old woman with Birt-Hogg-Dubé syndrome and recurrent spontaneous CSF leaks previously treated with multiple epidural blood patches presente

Posterior occupancy ratio-guided anterior cervical discectomy and fusion combined with unilateral biportal endoscopic decompression for pincer-type cervical spondylotic myelopathy: a case report and literature review.

BACKGROUND: Pincer-type cervical spondylotic myelopathy (CSM) represents a severe clinical phenotype characterized by simultaneous spinal cord compression from both ventral and dorsal structures, frequently necessitating circumferential decompression. Although traditional single-stage combined anterior-posterior (A-P) open surgery provides adequate decompression, it is often associated with extensive surgical trauma and an elevated risk of perioperative complications. A focused literature review was conducted in PubMed, Embase, and Web of Science (inception to December 31, 2025) using keywords

A novel device for sealing dural perforation via an epidural needle: an in vitro evaluation of effectiveness.

BACKGROUND: Postdural puncture headache is a debilitating complication of unintended dural puncture. There is no currently available solution for sealing the perforation at the time of the puncture. METHODS: An in vitro model using fresh porcine dura secured to a vertical tube was established. Dural perforations were created with 16G, 17G or 18G Tuohy needles. Untreated perforations served as controls. In the intervention groups, a compressed collagen sponge plug was deployed through the Tuohy needle immediately after puncture. Leakage was assessed by measuring the time for the saline pressur
