Surgical Management of Temporal Bone Cerebrospinal Fluid Leaks from the Posterior Cranial Fossa.

Although spontaneous cerebrospinal fluid (CSF) leaks from the posterior cranial fossa are uncommon, postoperative CSF leak after surgical treatment of cerebellopontine angle pathology is one of the most common complications of these procedures. In this article, we review the current literature, introduce the host of autologous and synthetic agents available to the surgeon for reconstruction of skull base defects, and provide detailed explanations of one reliable technique, including common variations for closure after retrosigmoid and translabyrinthine approaches.

Giant Thoracic Meningioma: Missed Diagnosis and Challenging Management in a Resource-Limited Setting.

Intradural extramedullary spinal tumors are surgically treatable lesions, but delayed diagnosis, particularly in low-resource settings, can result in irreversible neurological deficits. We present a case of a 37-year-old woman with a five-year history of progressive paraplegia due to a thoracic T4-T5 intradural extramedullary tumor, whose diagnosis and management were significantly delayed due to system-level limitations. This case illustrates the severe consequences of missed or delayed diagnosis of surgically treatable spinal tumors in resource-limited settings. Early MRI interpretation by t

Occult Fronto-Orbital Fracture Presenting as Traumatic Pseudomeningocele in a 4-Month-Old Infant: Successful Surgical Management in a Resource-Limited Setting.

Traumatic fronto-orbital pseudomeningocele is an exceptionally rare complication of paediatric head trauma and may be overlooked because of subtle or delayed radiological findings. It results from cerebrospinal fluid (CSF) leakage through a dural defect associated with skull fractures. We report the case of a four-month-old male infant who presented with progressive right frontal and supraorbital swelling ten days after a minor fall. Cranial computed tomography revealed an occult fracture of the right frontal bone extending into the superior orbital wall with a communicating CSF collection cau

Minimally Invasive Versus Traditional Repair of Spinal Cerebral Spinal Fluid Leaks: A Systematic Review and Meta-Analysis.

Spontaneous spinal cerebrospinal fluid leaks are a major cause of spontaneous intracranial hypotension. Minimally invasive surgical (MIS) approaches have been developed to reduce perioperative morbidity while preserving repair durability, yet their comparative effectiveness remains incompletely defined. We performed a systematic review and meta-analysis of contemporary surgical series following Preferred Reporting Items for Systematic Reviews and Meta-Analyses 2020 guidelines. Twenty studies met criteria for qualitative synthesis; 9 contributed to the quantitative synthesis. Primary closure su

Electroconvulsive Therapy Following Transmastoid Repair of a Skull-Base Defect and Cerebrospinal Fluid Leak: A Case Report.

Electroconvulsive therapy (ECT) after skull-base reconstruction poses safety concerns related to potential elevations in intracranial pressure (ICP) and the risk of graft disruption. We report the case of a 61-year-old man with bipolar I depression who successfully underwent a course of right-unilateral brief-pulse ECT 7 weeks after transmastoid hydroxyapatite cement repair of a 1.5-cm tegmen mastoideum defect associated with cerebrospinal fluid (CSF) otorrhea. After initiation of ECT, the patient demonstrated marked clinical improvement [Montgomery-Åsberg Depression Rating Scale (MADRS) score

Under Pressure: Advancing the Diagnosis of Pediatric CSF Leaks with Digital Subtraction Myelography.

BACKGROUND AND PURPOSE: Intracranial hypotension-arising from CSF leaks that are iatrogenic, traumatic, or spontaneous-is a potentially debilitating condition resulting in orthostatic headaches and other neurologic symptoms. While pediatric CSF leaks are challenging to manage due to limited data, digital subtraction myelography (DSM) has shown promise as a highly sensitive diagnostic tool for visualizing these leaks. This study reviews the use of DSM in pediatric patients for diagnosing and localizing CSF leaks at a quaternary center for intracranial hypotension. MATERIALS AND METHODS: Using

Eustachian Tube Obliteration for Temporal Bone Cerebrospinal Fluid Leaks.

Temporal bone cerebrospinal fluid (CSF) leaks most commonly present as unilateral clear rhinorrhea or otorrhea and are often exacerbated by maneuvers that increase intracranial pressure. While a high index of clinical suspicion is essential, beta-2 transferrin remains the gold standard for confirming CSF in otorrhea or nasal secretions. When the Eustachian tube functions as the primary conduit for CSF egress, Eustachian tube obliteration (ETO) serves as an important adjunctive strategy for the management of high-flow or refractory leaks that fail conservative therapy. The transmastoid approach

[First clinical application of autologous fibrin sealant Vivostat for neurosurgery in Kazakhstan and Central Asia].

BACKGROUND: Watertight dural closure and skull base reconstruction remain key challenges in neurosurgery. Autologous fibrin sealant Vivostat, produced from the patient's own blood, provides high biocompatibility, elasticity, and reliable adhesion. The absence of clinical data on its use in Central Asian countries highlights the relevance of the present study. OBJECTIVE: To evaluate the clinical efficacy and safety of autologous fibrin sealant Vivostat for dural sealing and skull base reconstruction. MATERIAL AND METHODS: This retrospective-prospective study included 110 patients operated on

Glucagon-like peptide-1 receptor agonist use and clinical outcomes after posterior cervical spinal fusion for degenerative pathologies: A large cohort retrospective analysis.

BACKGROUND: Glucagon-like peptide-1 receptor agonists (GLP-1RAs) have emerged as potentially impactful agents in improving spinal fusion outcomes. While several recent studies have investigated the effect of these agents on lumbar spinal procedures, the current evidence of the impact of these agents on cervical spinal fusion is limited. This study aims to investigate the impact of GLP-1RA use on perioperative clinical outcomes of posterior cervical spinal fusion. METHODS: This analysis queried the TriNetX Research Network to identify patients receiving posterior cervical spinal fusion for deg

Spontaneous Cerebrospinal Fluid Rhinorrhea in an Otherwise Healthy 42-Year-Old Female Presenting to a Resident Clinic.

Rhinorrhea, a common symptom documented in patients presenting to primary care clinics, urgent care facilities, and emergency departments, is often attributed to viral, bacterial, or allergic causes when no other obvious etiology, such as a foreign body, is found. However, in the absence of other infectious signs or symptoms, cerebrospinal fluid (CSF) rhinorrhea should be considered more prominently in a physician's differential diagnosis than it often is. This process can be either traumatic or non-traumatic, so even in the absence of recent head trauma, it should be evaluated, as this also l

Cerebrospinal Fluid Leak and Pneumocephalus as Consequences of Sneeze Suppression: A Case Report and Review of the Literature.

BACKGROUND Sneeze suppression creates a sudden pressure change that forces air or fluid through the weak points in the sinonasal walls - either the skull base or orbit - potentially leading to a cerebrospinal fluid (CSF) leak and pneumocephalus, both of which can resolve with conservative care. Spontaneous CSF leaks are often overlooked in patients without a history of trauma or prior skull base surgery. CASE REPORT A 45-year-old woman with no history of trauma or surgery presented with a 3-month history of recurrent headaches and clear nasal discharge from the left side. Upon further question
