When coronaries speak last; atypical Kawasaki disease beyond the classical criteria - A four-case series.

Coronary artery aneurysms (CAAs) are the most serious complication of Kawasaki disease (KD). Delayed diagnosis is associated with an increased risk of CAA, particularly in atypical presentations lacking classic mucocutaneous features. We report four children diagnosed with atypical KD only after coronary abnormalities were detected on echocardiography. Initial presentations included acalculous cholecystitis with marked transaminase elevation, aseptic meningitis, and an inflammatory arthritis. None of the cases had fulfilled complete diagnostic criteria at admission. Coronary imaging showed sin

Clinical Characteristics, Diagnostic Performance, and Treatment Outcomes of Extrapulmonary Tuberculosis at a Tertiary Care Center in Oman: A 10-year Retrospective Cohort.

BACKGROUND: Extrapulmonary tuberculosis (EPTB) accounts for a substantial proportion of the global tuberculosis (TB) burden and continues to pose major diagnostic and therapeutic challenges, particularly in low-incidence settings. Comprehensive descriptions of the clinical spectrum, diagnostic yield of available laboratory modalities, and treatment-related toxicity of EPTB in the Arabian Gulf are limited. We aimed to describe the clinical characteristics, diagnostic performance of microbiological and histopathological tests, and safety profile of first-line anti-TB therapy in adult patients wi

Add-on Anifrolumab for Residual Mucocutaneous and Systemic Manifestations During Cyclophosphamide Therapy in Severe Neuropsychiatric Lupus: A Case Report.

A 29-year-old woman presented with fever, malar rash, and alopecia. Laboratory evaluation demonstrated a high-titre antinuclear antibody of 1:5120, hypocomplementaemia, and positivity for anti-double-stranded DNA and anti-Smith antibodies, leading to a diagnosis of systemic lupus erythematosus. During the clinical course, she developed progressive peripheral neuropathy with inability to walk. Intravenous methylprednisolone pulse therapy and intravenous immunoglobulin were administered, but neurological improvement was limited, and intravenous cyclophosphamide therapy was initiated. Subsequentl

Inhibition of PDGF signal suppresses the fibrotic scar formation and promotes axonal regeneration in a mouse nigrostriatal lesion model.

After traumatic injury to the central nervous system (CNS), meningeal fibroblasts migrate into the lesion site and synthesize extracellular matrix (ECM) molecules, leading to the formation of a fibrotic scar (FS). The FS prevents axonal regeneration by releasing inhibitory ECMs, including chondroitin sulfate proteoglycans, Slits, and semaphorin 3A. Platelet-derived growth factor (PDGF) has been implicated in fibrotic diseases, but its specific contribution to FS formation has remained unclear. In this study, we investigated the role of PDGF signaling in scar formation after CNS injury. In the

Emergence of short-lived meningococci causing focal epidemics can be associated with gene transfer from carriage-associated Neisseria.

UNLABELLED: In March 2026, an unusually large outbreak of invasive meningococcal disease (IMD) in Kent, UK, was linked to attendance at one nightclub over a single weekend. The outbreak organism was a Neisseria meningitidis variant belonging to the longstanding hyperinvasive genotype, cc41/44. Using genome analysis of six isolates from patients, alongside >48,000 meningococcal genomes, we investigated whether the outbreak variant had acquired traits potentially contributing to the highly invasive phenotype. The six isolates were capsular group B, sequence type (ST-)485, and essentially indisti

Fatal bacterial CNS infections: a systematic review and a forensic case report.

Central nervous system (CNS) infections may cause sudden unexpected death when diagnosis is delayed or missed. We report a fatal case of Streptococcus pyogenes meningoencephalitis and systematically review postmortem investigations of fatal bacterial CNS infections. A 57-year-old woman was found dead at home after untreated otalgia, hearing loss, purulent otorrhea, low-grade fever, asthenia, and vomiting. Autopsy showed diffuse cerebral edema, subdural empyema involving the posterior fossa, and cerebellar tonsillar herniation. Histology demonstrated acute suppurative meningoencephalitis with v

Cannabinoid actions in a nonsurgical mouse model of sensitized dural nociceptors.

Cannabinoid 1 and 2 receptor (CB1R and CB2R) agonists alleviate inflammatory and neuropathic pain symptoms. However, central nervous system (CNS) side effects mediated by CB1Rs limit their widespread use. We developed peripherally restricted cannabinoids (PRCBs), which exhibit analgesic efficacy with minimal CNS side effects. Here, we examined PRCB actions in a mouse model of dural acid-induced hypersensitivity. Supradural injections of pH-6.0 (but not pH-7.4) saline (4.5 µL) in five-week-old female C57BL/6J mice induced periorbital allodynia (assessed with von Frey filaments) with recovery by

Familial Aplasia Cutis Congenita With Exposure of the Meningeal Membrane: A Case Report.

Aplasia cutis congenita (ACC) is a rare congenital malformation characterized by the localized absence of skin and occasionally subcutaneous and bone tissues. Its etiology is multifactorial, and management ranges from conservative wound care to surgical intervention. We report the case of a male newborn presenting with a 4-5 cm ulcerated lesion at the vertex of the scalp, with complete absence of bone tissue and dural exposure. Physical examination revealed syndromic features (facial asymmetry, hypertelorism, low-set ears, and cutaneous syndactyly) and an atrial septal defect. A strong autosom

Leflunomide-associated fatal cryptococcal meningoencephalitis presenting as rapidly progressive dementia: a case report.

BACKGROUND: Cryptococcal meningoencephalitis is a serious opportunistic infection, classically associated with advanced immunosuppression. The increasing use of immunomodulatory drugs, such as leflunomide, has broadened the at-risk population. CASE REPORT: A 66-year-old woman with rheumatoid arthritis, on long-term leflunomide therapy, presented with progressive cognitive decline, behavioral changes, and myoclonus over a six-week period. The initial presentation mimicked rapidly progressive dementia. Cerebrospinal fluid analysis showed elevated opening pressure, hypoglycorrhachia, and hyperpr

Comprehensive metabolomic profiling of Angiostrongylus cantonensis across its life cycle reveals developmental and host-associated metabolic signatures.

BACKGROUND: Angiostrongylus cantonensis is a zoonotic nematode that causes eosinophilic meningitis in humans. Although its life cycle encompasses multiple developmental stages and hosts, the metabolic changes underlying its development and host interactions remain poorly understood. This study aimed to characterize the dynamic metabolomic profiles of A. cantonensis across its entire life cycle and to identify metabolic signatures associated with development and different host environments. METHODS: Untargeted metabolomics using liquid chromatography-mass spectrometry was performed on five key

Operationalizing perioperative clearance reserve: a reserve-load interaction model for neurocognitive outcomes in older surgical patients.

Older patients who undergo similar operations and receive comparable anesthetic care do not always recover cognitively in the same way. Inflammation explains an important part of perioperative brain injury, yet it does not fully account for why the response settles quickly in one patient and persists in another. We suggest that this difference may reflect an interaction between the patient's preoperative capacity for brain fluid and solute clearance and the load imposed by surgery and anesthesia. In the proposed model, baseline clearance reserve is a continuous latent phenotype informed primar

The Role of Innate Lymphoid Cells in Autoimmune, Allergic and Infectious Diseases: A Systematic Review.

Innate lymphoid cells (ILCs) are tissue-resident immune cells that functionally mirror CD4+ T helper (Th)1, Th2 and Th17 cells, and are accordingly classified as ILC1, ILC2 and ILC3. Additional members of the ILC family include lymphoid tissue inducer (LTi) cells and natural killer (NK) cells. Abundant at barrier surfaces, ILCs are among the first responders to pathogens, contributing to tissue homeostasis and adaptive immunity. This systematic review, conducted following PRISMA guidelines, synthesises human clinical evidence on ILCs in autoimmune, allergic and infectious diseases. Relevant pe

Osteoclasts intercept meningeal lymphatic paths for dural metastasis.

Dural metastasis (DM)-a devastating complication of advanced malignancies such as breast and lung cancers-is defined by tumour cell infiltration into the dura mater and presents unique clinical challenges distinct from brain metastasis1,2. Our study unveils a, to our knowledge, previously unrecognized role of meningeal lymphatic dysfunction in DM pathophysiology, providing a new conceptual framework for this unique form of metastatic disease. Distinctive cranial erosion patterns, identified through clinical imaging, set DM apart from other metastases. Using a new mouse DM model, we discovered

Fluid-structure interaction dynamics of cervical lymphatic vessel pumping and valvular function.

A substantial portion of cerebrospinal fluid (CSF) drains through cervical lymphatic vessels (CLVs), a pathway mediated by basal and dorsal meningeal lymphatics. Impaired drainage along this route has been implicated in ageing, Alzheimer's disease and traumatic brain injury. Despite considerable experimental investigation of CLV structure and function, computational modelling of this pathway remains limited. Here, we present a fully coupled two-dimensional fluid-structure interaction (FSI) model of a murine CLV constructed using the lattice Boltzmann method for fluid dynamics and the immersed

Investigating CD8 T Cell Exhaustion in Chronic LCMV Infection by Flow Cytometry.

CD8 T cells and the process of T cell exhaustion are central topics in both basic immunology and translational research, including clinical applications. Here, we provide a comprehensive, step-by-step guide to investigate CD8 T cells in murine in vivo lymphocytic choriomeningitis virus (LCMV) infection models. We outline viral stock preparation, infection, and viral titer evaluation. We put an emphasis on flow cytometry as a method to investigate phenotypical and functional characteristics of CD8 T cell exhaustion, but the protocols provided will also facilitate imaging or transcriptomic analy

Mast Cells in Neuroimmune Interactions: Mechanisms, Pathophysiological Roles, and Therapeutic Implications.

Mast cells (MCs) are versatile immune cells that play a key role in neuroimmune interactions due to their strategic localization in barrier tissues and perineuronal sites, including the meninges, choroid plexus, thalamus, hypothalamus, and hippocampus within the central nervous system (CNS), as well as peripheral sites such as the skin, gastrointestinal tract, and dorsal root ganglia. They rapidly release stored and newly synthesized mediators, including histamine, tryptase, tumor necrosis factor-α (TNF-α), interleukin-1β (IL-1β), substance P, and prostaglandins, which regulate nerve signaling
