Depleted before the dose: the mitochondrial vulnerability threshold hypothesis for fluoroquinolone-associated multisystem toxicity.

Fluoroquinolone (FQ) antibiotics have an established capacity for mitochondrial injury, yet only a subset of exposed patients develop severe or persistent toxicity. We propose the Mitochondrial Vulnerability Threshold (MVT) hypothesis: that whether a standard therapeutic FQ dose produces clinically significant toxicity may depend substantially on the patient's pre-existing mitochondrial reserve capacity rather than on fixed pharmacological properties alone. Although this toxicity was first recognized through musculoskeletal injury-particularly tendinopathy and tendon rupture-the reported syndr

From reference epigenomes to response epigenomes: a public-health case built on the developing brain.

Reference epigenome projects, such as the NIH roadmap, ENCODE, and the international human epigenome consortium, have started mapping the baseline regulatory states of human cell types with extraordinary detail. However, these projects were not designed to address the urgent questions posed by exposome science and child-health policy: how these states change after environmental exposure, in whom, during which developmental window, and whether they can recover. I argue that the missing object is a human response epigenome: the cell-type-specific change in epigenomic state over time following a

A controllable human spinal cord model with full dorsoventral patterning.

Dorsoventral (DV) patterning of the spinal cord (SC) is orchestrated by morphogen gradients that specify distinct neural progenitor domains, the dorsal roof plate (RP), ventral floor plate (FP), and delaminating neural crest cells (NCCs). While foundational insights into SC patterning have been gained from animal models, key aspects - such as the role of retinoic acid (RA), dynamics of NCC lineage development, and human-specific features - remain poorly understood due to limitations in existing in vivo and in vitro models. Here, we present a human pluripotent stem cell (hPSC)-derived SC model,

Transcranial ultrasound neuromodulation: A registry-based analysis of global clinical trials.

BACKGROUND: Transcranial ultrasound stimulation (TUS) enables non-invasive neuromodulation of cortical and subcortical brain regions. Technological advances have facilitated rapid expansion of the field in recent years. However, the overall scope, methodological trends, and reporting practices of human TUS research remain unclear. OBJECTIVE: To characterize the global landscape of registered clinical trials using TUS for neuromodulation, including their design features, indications, targets, device platforms, and dissemination. METHODS: A systematic search of major international registries (

Trends in net survival for patients with brain tumors in Japan, 2000-14 (CONCORD-3).

BACKGROUND: Brain tumors are a serious health issue, and survival is an effective indicator of how the healthcare system manages them. We examined net survival trends for patients diagnosed with a primary brain tumor during 2000-2014 in Japan, using data from 16 regional population-based cancer registries participating in the CONCORD-3 study. METHODS: We included patients aged 15-99 years, with follow-up completed either 5 years after diagnosis or up to 31 December, 2014. We estimated 5-year net survival by age group and morphological subtype using the Pohar Perme estimator, and age-standardi

Locus coeruleus modulation of neurophysiological sensory selectivity differs in autism and other mental health conditions.

Sensory symptoms are common in autism and may result from differences in sensory processing. The locus coeruleus-norepinephrine (LC-NE) system modulates sensory processing by increasing selectivity to salience. To specify this mechanism, we manipulated LC-NE activity and explored sensory selectivity in autistic and non-autistic adolescents. We assessed pupillometry and electroencephalography (EEG) in 52 autistic adolescents (ASD), 55 healthy controls (CON), and 43 adolescents with other mental health conditions (MHC) during a passive auditory oddball task. A handgrip exercise manipulated LC-NE

rAAV9 vector biodistribution in nonhuman primate brain and spinal cord following lumbar intrathecal infusion.

INTRODUCTION: Delivery of recombinant adeno-associated virus serotype 9 (rAAV9) into cerebrospinal fluid bypasses the blood-brain barrier and can permit efficient brain tissue transduction. Whereas intracisterna magna injection (ICM) and other routes have been considered, lumbar intrathecal (IT) delivery offers a minimally invasive approach for treating genetic disorders of the central nervous system. To evaluate central nervous system biodistribution, we quantified vector genome (vg) copies of five rAAV9 vectors in cynomolgus macaques following ICM or lumbar IT delivery. METHODS: A total of

Delayed 18F-FDG PET for the evaluation of suspected recurrent disease in brain metastases treated with radiation.

BACKGROUND AND PURPOSE: Differentiating progressive disease (PD) from radiation necrosis (RN) after treatment of brain metastases remains challenging. We evaluated the diagnostic utility of dual-timepoint 18F-FDG PET in distinguishing PD from RN in patients with treated brain metastases showing suspicious MRI findings. MATERIALS AND METHODS: Thirty-four patients with suspicious MRI enhancement following radiation therapy were retrospectively analyzed. Seventeen had PD and 17 had RN, confirmed by a composite reference standard of clinical and radiologic follow-up (all 34 patients) or histopath

A first realization of reinforcement learning-based closed-loop EEG-TMS.

Transcranial magnetic stimulation (TMS) is a powerful tool to investigate neurophysiology of the human brain and treat brain disorders. Traditionally, therapeutic TMS has been applied in a one-size-fits-all approach, disregarding inter- and intraindividual differences. Brain state-dependent EEG-TMS, such as coupling TMS with a pre-specified phase of the sensorimotor μ-rhythm, enables the induction of differential neuroplastic effects depending on the targeted phase. But this approach is still user-dependent as it requires defining an a-priori target phase. Here, we present a first realization

Functional inputs to the subgenual cingulate cortex differentiate levels of depressive symptom severity in neurotypical young adults: granger causality and clustering analyses.

INTRODUCTION: Extant research has implicated functional connectivity of the subgenual anterior cingulate cortex (sgACC) in major depressive disorders or depressive traits in neurotypical populations. However, prior studies have not distinguished the inputs and outputs of the sgACC, and the "diagnostic" accuracy of these connectivity metrics remains elusive. METHODS: Here, we analyzed data of 890 neurotypical young adults (459 women, age 22-35) from the Human Connectome Project using Granger causality analyses (GCA) with the sgACC as the seed and 268 regions of interest from the Shen's atlas a

The analysis of the prefrontal cortex and its facilitator role of violence: conclusions of a systematic review and meta-analysis based on neuroimaging results.

The prefrontal cortex (PFC) is a key brain region for behavioral regulation; therefore, PFC abnormalities might partly be responsible for violence proneness. However, less is known about whether these alterations in PFC subregions are present in various types of individuals with a history of violent behaviors, including those who have committed violent crimes and those engaging in violent attacks against others, with or without mental or personality disorders. Therefore, the main objective of this review is to summarize the key findings from neuroimaging research that measures whether differen

Brain aging in bipolar disorder using a neuroimaging and machine learning-derived metric: Findings from the ENIGMA BD Working Group.

BACKGROUND: Bipolar disorder (BD) is associated with clinical and biological markers of premature aging. In this largest study of brain age in BD to date, with 2919 participants, we compared brain-predicted age difference (brain-PAD) in individuals with BD and healthy comparison (HC) participants. Brain-PAD is a machine learning-estimated metric that quantifies the difference between an individual's predicted brain age and their chronological age, a potential clinical bio-signature of premature brain aging. Within individuals with BD, we also examined how medication and clinical characteristic

Hemispheric Asymmetries in Bipolar Disorder: A Systematic Review.

Background and Objectives: The joint study of cerebral asymmetries and bipolar disorder (BD) has long attracted the interest of researchers and clinicians. Nevertheless, despite the increasing awareness of hemispheric asymmetries in BD, the combined investigation of these two constructs constitutes a relatively recent area of inquiry. The main objective of the present systematic review is to systematically examine the existing literature in order to identify, integrate and critically discuss evidence of hemispheric asymmetry in BD patients in terms of brain anatomy, physiology and neuropsychol

Brain Morphology Mediators of the Association of Childhood Trauma With Bipolar Disorder: An International ENIGMA Bipolar Disorder Working Group Study.

IMPORTANCE: Childhood trauma is associated with increased risk for bipolar disorder, but the biological mechanisms of this association remain incompletely defined. Gray matter differences observed after trauma exposure overlap with those reported in bipolar disorder, suggesting that the association of childhood trauma with bipolar disorder might be mediated through brain morphology. OBJECTIVE: To determine whether cortical thickness, cortical surface, or subcortical volume mediate the association of childhood trauma with bipolar disorder. DESIGN, SETTING, AND PARTICIPANTS: This case-control

Detection of neural autoantibodies in dogs with non-infectious inflammatory central nervous system disorders.

BACKGROUND: Easily obtainable biomarkers for non-infectious, inflammatory central nervous system (CNS) disorders in dogs are lacking. HYPOTHESIS/OBJECTIVES: Dogs with non-infectious inflammatory CNS disorders might have specific neural autoantibodies in serum or cerebrospinal fluid (CSF), or both, discoverable by commercially available indirect immunofluorescence (IIF) assays. ANIMALS: Dogs (n = 23) with non-infectious inflammatory CNS disorders, including meningoencephalomyelitis of unknown origin, were recruited from the University Hospital for Companion Animals (Copenhagen, Denmark). Primary central nervous system T cell lymphoma involving multiple brain and spinal cord lesions with SETD2 mutation.

Primary central nervous system lymphoma (PCNSL) is a rare extranodal lymphoma, with diffuse large B cell lymphoma accounting for most cases. T cell variants are uncommon and associated with poor outcomes. We report a case of primary CNS T cell lymphoma with simultaneous brain and spinal cord involvement. A 37-year-old Japanese man presented with impaired consciousness, and brain magnetic resonance imaging revealed multiple contrast-enhancing parenchymal lesions. Histopathological examination of a brain biopsy specimen demonstrated medium- to large-sized atypical lymphoid cells, leading to a di

HGBL-NOS presenting as widespread extranodal disease without lymphadenopathy: a case report.

High-grade B-cell lymphoma, not otherwise specified (HGBL-NOS), is a rare and highly aggressive B-cell lymphoma with overlapping morphologic and immunophenotypic features that complicate distinction from other aggressive B-cell lymphomas. Disseminated extranodal involvement, absence of lymphadenopathy, and lack of standardized treatment guidelines pose significant diagnostic and therapeutic challenges in HGBL-NOS. We report a 27-year-old female who presented with a six-month history of progressive left-sided chest pain, dyspnea, hemoptysis, and dry cough without constitutional B symptoms. Labo

Classification accuracy of a hierarchical molecular inference-based deep-learning system for CNS tumour diagnosis: a multi-institutional, retrospective study.

BACKGROUND: Recent advances in artificial intelligence (AI) and computer vision empower deep-learning models to infer molecular features from histopathological images to classify CNS tumours. The aim of this study was to test the classification accuracy of a molecular inference-based AI assistant for CNS tumour diagnosis. METHODS: In this multi-institutional, retrospective study, we used data from whole slide images of samples from patients aged 0-95 years, diagnosed with primary or recurrent CNS tumours. Reference diagnostic labels were determined by DNA methylation-based tumour classificati

Low-Intensity Pulsed Ultrasound Promotes Spinal Cord Injury Recovery by Regulating Microglia-Mediated Neuroinflammation.

BACKGROUND: Spinal cord injury is a severe central nervous system disorder with high morbidity and mortality, whose pathological process is divided into primary and secondary injury. The regulation of neuroinflammation is a critical link in secondary injury, forming a positive feedback loop involving microglia and peripheral immune cells. Low-intensity pulsed ultrasound, a noninvasive physical therapy, exhibits significant potential in regulating inflammation and promoting microangiogenesis in orthopedics and nerve repair, providing new insights for intervening in the secondary pathological ca
